Lincoln Square has friendly staff, decent food and a nice facility. My dad stayed there a little over 3 months and declined to the point where the on site care facility could not accommodate his needs so I had to move him to an assisted living facility. My low rating for Lincoln Square is a result of how we were treated by their corporate headquarters upon the mutually agreed termination of stay. I gave them a 30 day notice and had my dad moved out within 2 weeks. A month after his move out they debited our account for another month. When questioned they said he hadn't stayed for the mandatory 90 days so they had to bill an extra month. When I pointed out he had been there over 100 days they said their contract said they can still do that. I carefully read the contract and pointed out according to the verbiage we fulfilled our obligation of notification and they billed us for occupancy far in excess of the end of 30 days of notification. They conceded and agreed to pro-rate a credit to us. That was 3 weeks ago and still nothing. The on site staff is working to resolve the issue but from what I've experienced the corporate headquarters could care less and has not told the truth - ever. It's hard enough trying to deal with the heartache of the failing health of a loved one without struggling to right a wrong from an uncaring headquarters.

If your loved one is capable of independent living, then this is a fine choice. However, if there is any significant need for in-depth care, memory care or other significant health issues then I would not recommend. The facility is not secure and residents have wondered. They have also been left in less then clean environments and it was confirmed that medication was not being given properly.

Manager does very little to solve problems and has her favorites. I have seen and heard conflicts and little or nothing is done. Your manager seems not to have the experience or dedication to improve working conditions or moral. In my opinion a management change is needed.

Nice enough place but the senior staff leaves alot to be desired. Very nosy, almost rude, worried only about the bottom line not the well being of their guests. Will randomly check rooms without first prior notice. The food I wouldn't feed to my pet dog. Serious changes need to be made from the top down or they will be empty.

This facility has gone down hill very fast. The management is not involved and the staff changes before you can learn their names. They are gone before their training ends. Morale seems very low. Most times there is nobody at the front desk and leaving the secured area takes anywhere from 3 to 6 rings on the buzzer before someone can be found to let the visitors out. Orders upon release from the hospital were disregarded and nobody seems to take responsibility or be in charge. The call light takes 25 to 35 minutes to be answered even when the family is there. The "redirection" of the unruly or violent residents is sometimes alarming. One of the workers offered that they thought a total meltdown of one of the residents was "rather funny". Any when a question is asked, the most common response is "um, I don't know" with the other one being, "I have no idea" and walking off. The food is okay but the back dining area is packed while the front, larger dining area has very few residents in it. These few would be considered the ones that need less care and help and supervision. Many times there is no one to be found once the meal is served. These residents are our loved ones and still should be treated with dignity and consideration. That is lacking here to a large extent, although there are a few that are dedicated and compassionate. Overall, I would not recommend this facility.

Needs lots of work to straighten out problems. No follow through, management ignores concerns. Could be a great place, but it remains an absolute unorganized mess. Employee pay seems to be very low ( below industry standards) causing high turnover.

I really couldn’t envision my Mother living here. If the Administrator could get anyone to live here, he would get the salesman of the year award. Don’t get me wrong, he was very nice, but the facility needs a lot of help. The place just looked really run down and it’s not the cleanest place I’ve seen. It was just so depressing. I wouldn’t even live here. I couldn’t even imagine putting my mom here. They actually allow smoking in the facility and I’m still dumbfounded by that. There are elderly people in this facility that are on oxygen and that is a huge safety hazard. The apartment they showed us was recently renovated and it reeked of smoke. I couldn’t even believe it. The only thing I could say that was decent is the meal we had there. It was adequate. I couldn’t leave fast enough. I do not recommend this facility.

Patients in the skilled living are heavily sedated for the convenience of the staff. Some of the nurses aides are so unpleasant that I fear for my loved one's life.

Actually my mom does not want to be here anymore. She is having a rough time with the transition. She said that she hates the place and that she just wants to go home. The staff has tried to get her involved and mom did play some Bingo. We think that they are good people.

1. review exactly what "Home for the aged" licensure means. 2. Review the credentials, licenses and certificates of care staff...and turnover. 3. Food is the absolute lowest cheapest quality probably same supplier to the State's prisons. 4. Activities look fantastic printed out on a fancy sheet of paper...

The apartment, upon entering, was clean. However, after one day, the care partners had made a mess and sanitation was ignored. The nurse had not read the information about the resident's medication and was not going to give the needed medication. The nurse lost her temper which made communication difficult. When medication was finally administered, the nurse did not know how to administer it so the family had to inform her.

My mother lived at Independence Village of Plymouth for 3 1/2 years until her death. While she enjoyed the social activities and the staff at the front desk, the pull cord response time was slow and the dining room workers were immature, dismissive, and constantly understaffed. No provisions were made available for high blood pressure, low-sodium, or diabetic diets with the response "we don't add salt" as an excuse when high sodium commercial bases, sauces, and broths are used in the kitchen. Countless times my mother, a CHF patient, would lose edema while in a skilled nursing center then quickly balloon back up when eating at Independence Village -- again with her breathing compromised. This always lead to another hospitalization. This cycle could have been averted with a low sodium diet. Following her death the family was shocked at being billed for meals and services on site when in fact Mom was in hospital and not even at the facility. It makes us question the validity of previous charges that were paid. If we knew then what we know now, I think our mother would have experienced less hospitalizations and had a better quality of life in a different, more health conscious facility.

The people at Spring Meadows were very nice, but the sales person was unorganized and it was very difficult to get final pricing. We set the appointment giving them full knowledge of our needs and after 7 days of trying to get an answer they were not able to help us. We lost critical time and it added a lot of extra stress.

Let me begin by saying this is a business and it appears everything is about cost cutting. I realize they are there to make a profit but really......there are two washers and dryers on each end of every floor. However, they are off limit between 9-4 M-F for house keeping. One of the washers was broken for 2 plus months. For the price you pay it should have been taken care of immediately. Need more washers and dryers for the number of people living there. It appears the people who tip get their rooms cleaned thoroughly. The others just get the drive by treatment. In their defense they are understaffed and overworked. Just not enough help. Management staff changes too often. I do believe Bob and Doreen do a great job. However, it appears everything just gets cheaper and cheaper. I am quite sure this is due to corporate guidelines. They are kind to the residents which is so appreciated. I do not think the food is up to par. They deserve a higher quality of food and choices. Again, it is all about staying within a budget. The elderly deserve the best care. The facility is not the same quality as it was when my parent moved in a few years ago. My parent is fortunate because we can supplement food, cleaning and laundry help as needed. However, there are many who do not have that support.

We were very dissapointed with Memory care. They talk a good game, but dont practice it. They line residents up in frontof tv while staff stands by talking with each other. On several occasions We found our mother soaked in urine and We had to toliet her our selves. Her hair was often dirty so We paid extra for a visiting beautician. We were rarely greeted by anyone other then the receptionist. The food was not very appealing. We were told by other visitors that They witnessed abusive behavior from staff. For the amount of money They charge, We expected 5 Star treatment not 2 Star.

We found this community somewhat lacking based on a recent tour. One of the units we were shown had ceiling water damage that maintenance had made only a weak attempt to paint over. The website had led us to believe that the food was very good. Instead, we got a dinner of what appeared to be a boboli-type pizza dough with some generic pepperoni, canned pizza sauce, and a slab of mozzarella slapped on it -- a meal any teenager with an IQ above 90 could have made, and served in a city that has many Italian Americans and offers as fine a selection of pizza and southern Italian food as you'd get in much larger urban areas. Also, there are only six washers and dryers in the entire building -- not nearly enough to accommodate the number of residents living there.

Terrible experience. They should be reported to health department if Ohio. The facility is going thru a transition. Bought out in [Date Removed] from private owned to corporate. Under staffed across all areas. Non responsiveness to residents needs. Mixed up meds. No leadership presence. No accountability. I would not recommend until they figure out their business model. We were sad that our decision to move mom here met with such dissatisfaction.

Yes, the building that the Moceri family designed and built with their love ones in mind is beautiful, now that it is corporately owned, the inner beauty is gone! The administration cares more about the bottom line than the human condition and their residents. They care more about finding ways to increase their revenue through trumped up care level assessments, a Wellness Director that lacks skills in leadership, medication administration, end of life care, etc. They also have an Executive Director and Regional Director that cares more about the optics of written communication than actual follow-through on what they pretend they state to be improving. Sad situation for so many families that now feel stuck in this facility! The OG staff remains committed to their residents, so the scores requested reflect the friendliness of the original staff members.

I have no idea where to even start with this place... TO start off dining is terrible they do not get there food for an hour PLUS.. Room orders dont get delivered on time they do not receive breakfast until 11-12 This is the start of lunch time. Half of the amenities that are offered and services that were offered to us never happened or existed. Activities are terrible... No residents care to show up. Ive seen a resident lead an excersize because no one showed up to host it like they were supposed to. Theres alot of broken promises. I bring a lot of things to the directors attention. Have yet to get done. The caregiver to resident ratio is BAD they have 2 caregivers to assigned to three floor on their assisted living unit and one nurse. This is a reoccurance not just a one day thing. No one in there seems to know what they're doing. Repeated meals, theres been days when they had no milk or juice. We (family members) have called state multiple times. Not to mention 4 DONs quit within 6m!

They are understaffed, it would be impossible for them to assist patients to the bathroom and meals, the patient sits behind a locked door, without a call button. They do not follow care plans, it would be very wise to show up at different times to observe the care of your family member.
