Special Conditions for dad.

This community is great for my father. The staff was nice, but they needed more training when it came to the care for my father. They do have activities to keep active. The food is appealing. Family members should check out all aspects of the community and the training that they offer for special conditions.

"Memory Care" Facility

Reviewers need to indicate which part of the facility to which their review applies. My mother is in the "Memory Care" part of this facility. I looked at these reviews and toured the facility after discovering the Saline location did not have openings for Memory Care placement. My mother still has some cognitive awareness and good mobility. Despite completing the paperwork listing her likes, dislikes and redirection techniques I've used for nearly 3 years, it appears the aides do not have training for residents with dementia or Alzheimer's. To my knowledge, only 1 aide read the "story" of Mom's life. As a result she's peppered with questions she's unable to answer. Open ended questions are the norm (would you like soup today? Coffee - when she drinks tea, etc). Within a week my mother described her situation as "being in jail". This is because they aren't permitted to even enter the courtyard w/o an aide opening the door to let them in/out. One afternoon I visited her and experience the aides sitting in their room laughing and talking among themselves while the residents sat around by themselves. Within 2 weeks of her placement, they wanted to administer psychotropic medication which was shocking. It's clear the residents are expected to adjust to the routine and to do so quickly. Mom is have a hard time "getting with the program" so it's clear this is not a good fit for her. If your parent requires more focus or attention this probably isn't the place for them.

They shouldn't have opened up yet!

The community is clean and the food is good. They are understaffed making it very difficult for them to give my mom the attention she needs. They don't have a full time activity coordinator. My mother has no privacy as residents are constantly walking into her room. I will be moving her out very soon.

Visited with Tim. Welcoming clean filled out paperwork Tim had nursing assessment....then after an hour realized that IDPH regs said they were not licensed for his level of care coming in with a colostomy. Was very disappointed as this was the place I picked for him.

Seem very understaffed

Over experience with StoryPoint was not a good one overall. Although the facility itself was clean and nice looking, and the staff were very friendly, they just did not have enough people on staff to take care of the residents. Our loved one had a few situations come up where it was obvious he was not being checked on enough. I also felt like they promised a lot during the sales process that they did not deliver on once we moved our loved one in.

Not all what they promised.

This assisted living looks nice. They talked a good game, but aren't delivering what was promised. The buddy system they said that was in place is not. The two hour checks around the clock don't happen. Some days my mom doesn't even see a tech. She's been there two weeks and they haven't washer her clothes yet. I've had to do them. Things just are as promised. However mom says the food is good.

I had my mother here for less than a few months. I felt that management did not care about my concerns but instead only worried about recieving thier monthly payment. They made many empty promises when we toured but failed to fufill them once she moved in. They are understaffed the care givers have an insane amount of people to care for, there are less nurses than care givers so you can imagine how understaffed they are. I felt like I could hire help to really be able to care for my mother for same price as having her live at Senior star. Ask specific questions about how many patients each care partner and nurse has. Or how many activities they have per day and the times. If your parent doesn't need much help this is the place to keep them but if they need much help I say look into hiring help in thier home.

This community was not giving my loved one the care that she needed. Told me that they did not have enough staff to keep an eye on her 24/7. I wanted to know why they waited so long to tell me that my loved one was ill, and it just turned into a bad experience. I would not recommend putting your loved one here unless they can take care of themselves 75% of the time. I had to get my loved one out of here.

Staff needs an attitude adjustment

West Court: the staff isrude except for the caregivers themselves, my father's lock is messed up, very difficult to get into and TP was hard to come by even before coronavirus. The public restroom facilities have backed up toilets and had no toilet paper in them on 03_15/2020.

Disapointment and dismay

Provision Living at Forest Hills is not as it appears aesthetically. Provision Living is managed by a Missouri company, and all decisions are made out of the corporate office. The in-house, management “team” is inexperienced, 9-5 workers with a corporate mentality. All seems to be done for appearance sake, and not for the good of the residents. There are so many inconsistencies, and very little, if any, follow-through. The activities are sometimes held, sometimes not, with lack of communication to the residents. They promised outside activities on the bus, but not all who sign up can fit on the bus, so they cannot go. This community is best fit for those who are independent. If your loved one needs help, do not look at this community for your loved one or yourself. As your loved ones age, and need more assistance, this is not the place for that. The management is reactive, and dont have established systems in place. They seem to be reacting to events as they occur, then they cover their tracks. The head of the nursing is an LPN, who is not competent in her position. The care staff is good, but they are overworked, and understaffed. The building is too big and the care staff has to go from one side of the building, and up and down. It is a bad configuration. Medication management is a problem. There are frequent med errors. Pills are missing, over/under medicating your loved one. There is no sense of urgency nor follow-through. There is favoritism toward certain residents, who basically run the activity times, the dining room eating groups, the outings, and who gets the most attention. The television is DirectTV, and it goes on and off frequently. If you are watching a football game etc, it can cut out for hours. The food is not good. Pork and beans, and soup and bread, and stuffed cheese hotdogs on a croissant, for example in one week, are not heart-healthy choices for the older population. Many basic food items run out- for instance, no lettuce for a week, no chicken breast, not many fresh fruits or vegatables. The wait in the dining room can be up to 45 for the food to be served. The dining room is extremely cold, as is the food many of the times. Residents are tired of complaining, and have waited so long for their food, that they are hungry. The management is the problem. They are condescending, and act as though you are a bother to them. They are trying to make a presence in the community, but are forgetting that the paying residents, who have physical and mental needs are their main income source, and responsibility. This place has only been open for going on 7-months, and you should be aware that it is not as it appears. They put on a great facade for the community. Be very thorough in your search for your loved-ones. They are great at “selling”, but what they are selling is not the reality. The residents are afraid to speak up because of fear of retaliation. Most bring things up to the staff, but nothing changes, so they keep quiet. They will kick your loved one out for any reason. One such beautiful 91-year-old person was kicked out after four-months of being at Provision Livind. It was heartbreaking. Looking for a place for your loved one is not an easy task. Even though we asked the right questions, the answers given by the Provision Living sales people were very well scripted. Please take this review with some merit, and be cautious. Your loved one’s care is of utmost importance, and tread lightly if you are thinking about moving them into Provision Living Forest Hills. Things aren’t always as they seem.

Review of StoryPoint Saline

This is not a good facility because the food is stingy and less than (good). Too much sauce (especially white sauce) and not enough meat, poultry and fish for nutritional balance. Not enough activities for members or classes (like painting...book discussion groups...need more presentations for groups...or advance notice when activity is to be held. Too many "official doctors!" Not enough opportunities to present ideas from non-advisors.

Agree with prior review. My family is looking to get out...run out. The ED is a big problem....chuckles at every word, no action, zip on accountability. He is not qualified!!!!! Food needs improvement and care is marginal. Lots of turnover and management needs overhaul.

High turnover in mgmt and staff. Staff not well trained. Lots of promises, little follow up. It looks very nice, lip stick on a pig. Very expensive for Less than avg level of care

Independent Living?

I Have A real problem with the food.Granted it is not home cooking.It is highly salty, to much starchy food, meats are overcooked. Meals delivered to apartments are often incomplete and/ or changed from what what was in writing. Care services provided by another supplier are not delivered and they overcharge

Not living up to expectations/sales pitch

My family member is resident in memory care. They are understaffed. Memory care director is lovely and works extremely hard, as do the caregivers. However, the needs for memory care residents are really not being met. Have had a few conversations with executive director about the problems, and nothing has changed. The caregivers try very hard to keep up and are very kind. Unfortunately they don't have the management to back them up. Food has been awful since the day my family member moved there, no improvement. It's like eating the worse meals in middle school. Not healthy for residents who need proper nourishment. One day pizza was served with tater tots. Residents sit around watching tv because there isn't enough staff to help them. Don't see residents with water other than at meals, and when a smoothie snack is prepared. If you move your family member here, be prepared to add a private caregiver. The building itself has heating/cooling issues, and is poorly laid out. No laundry facility in memory care, so staff has to leave the area to do laundry. No technology to detect when a resident falls in their rooms, and there have been several incidents that I am aware of. They should put their marketing budget towards improving the promised services instead of advertising special meals and programs for the public. They certainly shouldn't be accepting new residents until better staffed.

Maintenance and Air Quality Issues

My father-in-law moved here after my MIL passed away and was apprehensive about the move. It was more confusing than it needed to be and there were maintenance issues with the air quality with his apartment requiring him to stay out of it throughout the day. This stressed him out and worried us. The issue took longer than anticipated to resolve and he had health issues through out the process. I'm sad to say he passed away and he never got a chance to really become part of this community the way we had hoped.

Buyer Beware

A very nice facility but not worth the price. I would agree that many of the staff are nice but they are not well trained. The dining rooms are beautiful but the food is average and the service is terrible. As for competency of administration, very disappointing. Nothing but the typical buck passing if you have a problem with either the corporate representative or the representative for the contracted care provider. If you really want quality care for your loved one look else ware. We have now experienced good care in a very nice facility and are very happy be done with Storypoint and [removed] the contracted care provider.

Avoid memory care unit

Memory care for mom is bad. I have found her bed saturated in several days of urine and feces. Used urine soked depends piled in the sink. Theft of her personal belonging is rampant. Mom did something on a Tuesday that bruised her face into one huge discolored bruise. I wasnt contacted. I found out when my son stopped by. It was Saturday and six hours of driving to take her to the doctor because they wouldnt. They refused to release the incident report to us. [Name removed] the manager told us she doesnt walk the facility and hadnt in months. And it is obvious. The distrixt manager promised to look into it and get back to me. That was 4 months ago. He dis call my sister. No one from the facility will call back. They will tell me anything they think I want to hear. But i have learned it is meaningless They are chronically short staffed. One evening it took 10 minutes just to find a memory care staff member. Five more for her to find a key to mom's room. When i asked her about the three people screaming for help the staff member just said that they would quiet down soon enough. I wouldn't bring anyone here again even jf they paid me.
