Our mom, who had dementia, was assessed, and approved, for admission to the memory care unit. We thought her reluctance to be there would pass, but it didn't. She was on temporary respite care, and so it was easy to take her back home after 2-weeks. During her stay, she remained on the fringe of activities, but mostly was a participant observer, at least during the times I was there to visit. Meal times were challenging for the staff because our mom mostly refused to eat more than a couple bites of food each meal. Since she mostly refused to eat, the staff were unequipped to offer further persuasion. The 2nd day she was there, she reportedly fell out of her chair, which led to a trip to a local ER. She suffered many bruises, but nothing broken, according to the ER nurse. Two days after we brought her home she died. Medical examiner said it was a heart attack. I pray our mom forgives me for placing her somewhere she did not want to be. I have no other words.

Storypoint Cordova is having a very difficult time finding and keeping staff. As of late February, 2025, they are very short staffed in the maintenance, housekeeping departments, and in the kitchen. Very short staffed.

Great place on the AL side for more independent people but the memory care unit leaves much to be desired. They don't keep all the folks together. They let them sit in their apartments and become isolated. Dementia patients aren't the best at making decisions for themselves-so when left to decide whether they want to go do an activity they don't. My Father fell MULTIPLE times because he was ALWAYS sitting in his apartment alone.

That the facility takes your concerns seriously. To fill out forms from 3rd parties without being told a number of times and to call people back. The room size and lay-out is nice. [name removed] would like a microwave, that some place have.

The facility is clean, maintenance is spotty, very little emergency lighting during power outages, and the dinning facility serves below expected quality food. Under new ownership and rates appear to be going up. Good activity for the residents. Staff shortages and can't keep a Chef.

They all knew mom was coming, knew her name, knew about her issues against alcohol and made mom very welcome. The facility is nice and clean. Mom likes the food and her apartment. She is happy and making friends. Mom did not make this move willingly, she became resolved with it. They are lax in some areas. But mom does not see the business part they lack in, not huge issue. They did not debit her rent. I had to bug them. They do not return calls or emails quickly. Mom’s arrangement is that she takes care of her breakfast and lunch but goes to supper at the dining room. Staff does not seem to know her contract. They invite her to lunch and things like that. Right and left hands not in sync. Not big deal. Because mom is happy we are satisfied

I feel the care of the resident can be better. A lot of caregivers are on their phones sitting around. I feel the rooms r not cleaned very well, bathroom looks disgusting when you check your loved ones. They don’t deep clean what’s so ever. It would be nice if they helped the residents more. I’m just not happy at all with the care my loved one is getting.

Due to frequent staff changes, the food and food service quality varies greatly day by day. They have had ongoing maintenance/elevator issues that have taken a lot longer than they probably should have. They were bought out last spring and they have had multiple manager changes since then. They have had a hard time keeping staff in the food service and cleaning. And when I say elevator issue, they had been having issues, had the elevator rebuilt basically in November with ongoing issues until some final Parts came in last week. And they only have one elevator in the building.

We signed my father up for enhanced care which is a step above assisted living. It includes tidying up the room once a day and a full cleaning once a week with a change of sheets. However, they did not do the tidying up once a day until they were called on it more than two weeks after moving in. The side door shuts too quickly after scanning my dad’s badge. By the time he uses his walker to get the door at lacks again. My dad paid extra for a medical alert pin. He wore it for a week before another resident asked him if it worked. He hit the button and it didn’t work, so he was wearing it for a week without working. He had to go to the manager to get a new one. Things seem to be running smoother now, but it was very irritating that it wasn’t that way right from the start.

I felt they were understaffed. My mom was falling frequently and sitting on the bathroom floor for who knows how long before she was found. My mom was in memory care. She declined rapidly while in Danbury which was only 2 months before she died at 99 years old. She was put on hospice care during that time.

We were told there would be someone to take her around and kind of show her around. That never happened. We were told that they had to send workers to bring her down for meals, and that would cost us an additional $[fee removed] each day. We had to request that the bugs in her room be killed, tranportation¨ is included in the base pay; however, I don't know if they found a van driver yet. Still trying to get her medication correct. She has been there since the day after Thanksgiving . It seems like every time they have to do something for her. The price goes up. I have recently hired a caregiver to go in and work with my sister. I am reconsidering keeping her there. I must say the caregivers are very good

They do a good job selling the place. They promise much but do not keep those promises. They said he would have all new cabinets which did not happen. We gave them a long list of repairs which has not happened. The person upstairs had a leak which flooded his apt which they did nothing about except give him a small fan. Nice group of people, needs many building repairs which were promised and yet to be delivered.

The biggest negative I have is they were supposed to take the monthly cost out on the first of each month, and they never did. For elderly folks on a fixed budget this was unacceptable, we tried reaching out several times and the executive director, never returned our calls. When we toured the facility we were given one cost, and when we signed the contracts the cost had jumped [fee removed]. I asked for a copy of the assessment for level of care for my father and never received it. My dad enjoyed his time there. From a patient care perspective I would recommend it. From a cost and financial perspective I wouldn't

I have moved from there because of the maintenance . The elevators kept going out. Pipes kept bursting in many apartments, my air conditioning was broken for over 3 weeks in the middle of summer before it was fixed. The quality of the food had really gone down. The people they were renting to that needed to be in assisted living. Overall my experience with Lakeview at Josey Ranch was not a positive one.

Place is clean, staff is pleasant. My mother complains about food daily! She also says no one participated in activities so she does not go anymore usually only one or two people show up.

We weren't thrilled with Provisional. What they described before she came was just what we were looking for. What she got was less than she needed considering she was in memory care. Would not recommend it to friends.

Way too expensive for the quality of care we are going to move her. We dislike Having to call all the time because my mom is not clean nor is her room.

Not a good experience. My mom's room always smelled like urine. They lost a special comforter my mom had when she moved in. The staff was not nice to her or me.

The staff turnover at StoryPoint was ridiculous. The woman who supervised the caregivers was not competent. She promised to solve problems, big and small, but did not follow through. At times it took 3 follow up phone calls to get issues resolved. For that reason I wouldn't recommend the StoryPoint in Chesterfield. I worked in Mental Health for 22 years and I can honestly say that there is room for a lot of improvement there.

My mom lived there for eight years and it served her. Room was sizable and had outdoor patio. Rates were reasonable. Willow is in a very quiet location. The facility had good onsite (add-on) services such as physical therapy. Pet friendly. But place was getting tried and neglected overall. Lots of turnover in staff and of ownership. Food was okay but choices were very limited and service slow. Transportation bus had limited hours & outings to special events were unusual. Housekeeping provided for rooms was sub-par. Place only has one elevator so slow... get a room on the main floor. FYI much of the parking is via rear lower level entrance. No staff were onsite after 7:30pm...yes it is independent vs assisted living, but no staff was worrisome given condition of the residents. Outside walkways on east side are not ADA compliant.
