My dad couldn’t return to his assisted living after a fall because he needed more help and had to be pushed in his wheelchair. We were sad, but it turned out to be a blessing in disguise. He’s been at provisions for 2 months and is doing better than ever.

My advice to other families, no matter where they are going, is to always remember that they are advocates for their loved ones. It is important to establish clear communication early and often in the beginning to make sure your loved one is comfortable, and to make sure the facility knows your loved one and understands their needs.

My husband's brother feels comfortable living there. He likes the food and the facility in general. It's close to us so convenient to visit. Glad they are working on improving the buildings. The surrounding grounds are very welcoming. We are greeted every time we walk in the front door. It's overall a good fit for him.

Even though the facility is understaffed, as many places are, the workers all genuinely care and respect the residents at the facility. They all work very, very hard!

Some problems at first---limited meal options and common area cleanliness concerns. New management has been showing gradual improvement; our apartment is great as is location. We were not expecting so many residents needing assisted living services. Sales people should clarify that Story Point Wyoming is not Independent Living but a combination of Asst. Living and Assisted Living.

The young lady in the office is extremely helpful. I think her name is [name removed]. She had lots of helpful information. The apartment size is good for 1 person but the bedroom ilhas weird angles which makes arranging the furniture a little challenging. There are activities available if the resident is interested. My husband takes a while to get comfortable with new people. The staff checks on him regularly and encourages him to get out of his room.

Outstanding fun time. Staff was engaged. Enjoyed the band and information. Cause marketing for the Alzheimer’s walk was very nice. Won the wine. Just want to say that this is an amazing place. I am working with my mom to accept that this is the best option moving forward.

The young man, [name removed], in marketing was very helpful very nice! Activities director was A+!Caregivers were a C- .Director of nursing and executive director, a C. First couple of weeks there they lost my husband's walker and never found it the months that he continued to be there. They gave him other walkers but they were different sizes different shapes, different heights and during that time he fell a lot. They also lost quite a few pieces of his clothing even though I had them labeled. Food is a B+. Communication with staff C+ to B-.

My dad is adjusting well and the staff has been very supportive of his needs. It was tough on him after being independent for his whole life. Provision has worked well to encourage and support him and his needs.

Very responsive to questions and needs. Accommodate needs specific to each person. Liked that you don't have to move if you need more assistance at some point. Everything you need is in house ie- physician, physical therapy etc

All in all we really liked the facility and felt like she was well cared for. Hospice was there everyday too so it was sometimes tricky to differentiate who was doing the care but I did see some staff going above and beyond, putting lotion on her and fixing her hair before taking her out to dinner. The move in was great, because it was so quick, they were able to provide a lot of the furniture until we would get our own. Unfortunately, she went downhill pretty fast so she was only there about three weeks before passing and didn't participate in much, but when she was in the hospital, I was constantly on edge and advocating for her, and when she moved to Danbury, I felt like I could finally breathe a sigh of relief because at least there were people there who seem to care. The only critique I have is, it was a bit of a pain getting in touch with someone in billing and getting everything wrapped up after she passed but nothing too crazy and once I did talk to them, it was done, no back and forth or unfair charges.

I would give Story Point Chesterfield 5 stars based solely on executive director [name removed]. She’s been amazing. [name removed] is responsive to the needs of my parents and has been extremely accommodating. The caregiving side of Story Point hasn’t been as easy to deal with but I do have to say that the issues we’ve raised have been dealt with in reasonable manner. Overall, I would highly recommend Story Point Chesterfield.

Uncle lives here and is clean and friendly. Food could be better but isn't bad. Lots of friendly neighbors. He seems to feel quite safe there. Independent living so no help other than light house keeping and 3 meals a day.

All in all, the service, care and facility are all very good. Just a couple of suggestions: When it comes to conversations about loved ones, with staff members, it would help to not speak as though they aren’t in the room. Include them in the plan and ask their preferences. They need respect, and when they are capable, to be included in the “why” around decisions regarding their care or policies or procedures, (we can still make the bigger decisions in their best interests). Conversely, when working directly with a loved one and they are afraid to ask for toilet paper or don’t have their dirty laundry ready, take a moment to help them. Be more direct….with respect.

Every move that a patient with dementia makes reduces their life by a significant amount. Hospice was invaluable. They have far more knowledge regarding dementia patients than the staff....use them. They need more activities to keep people engaged. They had just hired someone and she was improving the process. Food was just not very good, often cold & dry. Frequently the choices were hard for people to chew.

Quality of care is lacking somewhat but it's a problem everywhere being short of help but they do respond when you complain so that is good. All are very friendly and want to help when can.

The facility is amazing. It is very clean & the staff is very nice. They treat my mom so good. There has been a lot of staff turnover since my mom moved in one year ago which gets confusing & frustrating l. However when we have questions we get a response asap and the executive is working hard to get the communication running smoother. The aids that help my mom are wonderful. They treat her with kindness & care. My mom isn't a huge fan of the food but there has also been about 4 chefs since she moved in so her opinion changes. All in all Independence Village has a wonderful place for my mom with plenty of activities & people around to help her & she's very happy there.

We feel the staff at Independence Village really cares about our dad. Additionally, they work very well with hospice. We are very glad we chose Independence Village.

The facility is really nice and large and open so that the residents have plenty of room to go for walks. The facility could use some updates such as carpet for a re-fresh. The staff is always super nice and very helpful and caring. By far the biggest weakness for Springvale is the quality of the food service.

Very friendly I enjoy the activities and the freedom to come and go as I please. Great place for mom and dad or just mom or Dad. On the negative the Kitchen needs organized a bit better. But the food is pretty good
