Not enough staff to care for the number of residents. Poorly paid staff so good workers leave and a lot of turnover in staff. Maintenance is poor (HVAC unit issues, plugged up toilets, carpets and furniture smell of urine). Medical staff does not administer meds appropriately. On staff nurses do not communicate with the residents family when there are medical issues. Saw many residents fall and I don't believe those incidents are reported. Saw so many seemingly healthy, active residents deteriorate before our eyes. Was so glad to get our loved one out before anything major happened.

The worst... my mom was only sent here for rehab and she never got a chance to come back home. They hardly ever answer to the call light. The charge nurse at the front desk is extremely rude. The staff is negligent, They lie to cover each other’s backs and now that my mom is gone all I can get is a “I’m sorry for your loss!”

This facility does not care about their residents. The turn over in staff has been significant including management. The memory care facility could not handle my Grandfather when he transitioned. This facility has been very disappointing and they definitely do not care about their residents. The new management has been awful. If your resident doesn’t fit the mold this will not be a good fit.

They have Bed Bugs. Staff is horrible starting with the Executive Director [name removed] all the way Down to housekeeping. They lie about everything. The residents bully other residents. Always working short staff and over working the good staff. Staff don’t answer call pendants within 7 minutes more like 27 minutes. One staff member told my love one that the water was cold so she couldn’t give a shower. That went on for a week. They double brief my love one. You have to hire a sitter to make sure things are getting done properly. I called [name removed] (RSD) never calls back. [name removed] is rude as heck.

Activity Directors very rude and unprofessional.

I would advise against sending a loved one here. This facility has alot of issues however staffing is the biggest with little to no staff especially on weekends. Ive experienced 1 cna and 1 nurse multiple times. With so many residents and the level of care needed especially in the back that is horrible. Admins only steps in if there is a tour or state is in the building or a family member complains. In most cases you will see housekeeping and activity aids acting as cnas due to lack of staff. The facility has been without a DON for quite a while leaving the business admin to attempt to run the nursing dept. The quality of care is dependant on who is working. Speak with the families and do research before looking into this facility. Its a beautiful building but thats where it ends. The level of care a loved one needs should be top priority not the appearance of the building.

My brother, sister-in-law, husband and I moved my mother into independent living here a few years ago. At that time, we all were really satisfied and my mother was happy which of course was the most important. Since that time, the building has changed hands management keeps changing employees come and go there never seem to be enough employees and those that are there are stretched really really thin. My mother shares an apartment and her first roommate really took more than half of the apartment but my mother has difficulty with confrontation so we just accepted that she was mostly confined to just her bedroom and wasn't able to have use of the living / dining area. Since her roommate moved out and she acquired a new one I thought things would improve. Not so at all. Now she has one cabinet in the entire kitchen and there are two closets in the main part of the living room what she used to share half and half with the old roommate. Just before the new roommate moved in, they took all of my mother's belongings from the less than half side of the decent closet and shoved everything in where a washer and dryer would be if they had one. Not really a closet just some space where a washer dryer would be. I complained and complained loudly and of course was poo-pooed and no problem we'll put some shelving in there for her. Since then my mother complain and they put in one Shelf. Whoop-dee-doo. Today I am infuriated again because my mother was in the chapel and of course I did not want to interrupt and I was trying to drop off a sack of groceries a case of soda and a sandwich for her dinner and was told due to liability we will no longer accept things for residents here. Now, of course this is where all deliveries are flowers packages excetera and now they're not going to accept??!! Fairly sure this is not legal and my next course of action is to look into the legality of it and try to complain again which never gets me anywhere. I hate to move my mother because of course it is very traumatic for her but I feel we have no choice because it is just so crazy there and so unfair with the division of the apartment with this we won't accept things for residents at the front desk. What I found the most galling today is that no one thought to say hey I'm so sorry we started a new policy but we can see you haven't heard about it so we will give her these things that you've brought for her but in future things have to be done differently. Even though I'm pretty sure it is it illegal to not accept things for residents on an apartment building it would have been less frustrating because we really don't get our money's worth there as I take my mother to all her doctor appointments versus using their bus I pick up all of my mother's groceries and personal items versus her using their bus and they couldn't go out of their way in this small way of keeping the bags there?! Really a sad situation and frustrating because again I don't want to discombobulate my mother but very frustrated with them and disappointed and angry and saddened. If you are considering this place, look elsewhere.

Stay away it is all about the money. Communication is bad no return phone calls.

Very poor management, my mom was here for awhile and at first it was good, but after your loved one is there awhile the care goes down hill. I've heard workers complain non stop about some of the care they had to do, and the gossiping is never ending, worker's on their phones constantly, they stand around and talk about their personal life non stop. I would not recommend this place to anyone you Love. ALS side and the MW side are not where you want to put your Loved one. Please do your research, this place needs new management.

I agree with the negative review about The HC Assisted Living completely. We were fortunate enough to move our family member out but it took us too long to see through the smiles. Although there were a few good people on the staff, the majority of the staff were awful and lacked the ability to give proper care to the residents. We hired outside care because of the poor staff. We are grateful that we moved our family member out to a compassionate Assisted Living facility where our loved was treated with the respect and dignity seniors deserve. I shudder each time I think about the lack of decency we subjected our loved one to before moving him out of HC. The review regarding the director of Assisted Living is accurate as she looked us in our eyes and never handled the concerns properly. The lies and lack of appropriate care became a nightmare.

Do not put your loved one here. My Mom was in the memory unit skilled care.They make care plans they do not follow. There was no way my Mom was toileted every 4 hours let alone every 2 hours like the care plan stated and she did not receive fluids every hour as a care plan stated yet staff signed off daily in a log book they did these things. The only time she got sent to the hospital for an issue was when we pressured them to do so and every visit to the emergency ended up in a hospitalization. Reported these issues to the department of health after our Mother died. Don't even bother with the Dept. of Health they just go in and look at the log books that are all lies filled with initials of staff stating they did things they didn't and so the Dept. of Health states the nursing home followed protocol.Always short staffed. High staff turnover.

Incompetent unprofessional does not follow procedures Rude director

My father moved into this facilty a few months ago. At the time it was marketed as a assisted living facility, geared toward people who needed extra attention and stimulation. The facility is now under new management, and they are now called a Senior Livng Facility. They have taken away most of the activities, which leave residents with little or nothing to do but sit in their room with idle time most of the day. They have also cut the caregiving staff in half, some days there is only on RA on staff to care for all of the residents!!! When my loved one moved in, one of the selling points was thay there was a RN on staff, not anymore, they have also let her go. This is not a assisted living faciltiy or a nursing home. It is a apartment complex that hires outside help. It is not licensed by the state, therefore they are not regulated by the state. They answer to no one but their investors!!!

I put my loved one there about 7 months ago and I started regretting it about 6 months ago. No dates are on these reviews so I'm going to tell you that it was in [Removed]. Keep in mind that I am there at least 3 times a week so I'll see a lot of what goes on during the day the night and on the weekends. You name the problem and we had it. Stopped up toilets, a small flood in the facility, medication management was disregarded from the nursing staff and the doctor that comes in once a week. I took my loved one to the regular Dr, not the primary care physician that shows up once a week, and the PCP would not follow the orders from The Specialists even when I brought in paperwork from the specialists. Laundry has also been lost. The food they serve them is horrible. I call it chicken McNuggets meets a can of veg all. Sometimes the need is undercooked along with the vegetables. This is very unhealthy for the meat to be undercooked. They Market that their residents go on outings, but there has only been one outing to get ice cream in 7 months. I've taking my loved one to the doctor twice and to the hospital once because the staff kept saying "we haven't noticed anything" one example is they said they could not get a urine sample because when they went in, there was no urine left by the resident after two days. Give me a break! This place is full of Alzheimer's people that cannot remember 5 minutes ago. What was wrong with stand in there and saying hey please give me a urine sample. I could write a book, but it appears that these people have absolutely no training in Alzheimer's care and it is 100% a Memory Care Facility. So if you put your loved one in there thinking they are going to be looked after for a cough or because the have been laying in bed for 2 days, forget it. You still have to go and check on them. When you ask for a plate to be sent to the residents room, it is a major hassle and sometimes they do not even send it when they are supposed to. Or in one instance, I asked for a plate and they warmed up a cold sandwich that had fresh fruit in it. Nobody seems to be accountable to anybody and if you ask for something they tell you to go run around the facility and look for such-and-such person. I do not think this is my job. HORRIBLE PLACE.

A bit about my personal experiences. At one point, did not keep track of my relative and left them outside for multiple hours, in the beginning of July, while they were wearing long sleeve clothing. That relative became lethargic and had to go to the hospital. My family and I would visit frequently. On several occasions we discovered our relative wearing the same clothes multiple days in a row. For instance, we'd come on Sunday, and then when we came again on Wednesday, the relative was wearing the same exact thing.

Please do not take someone you love to this place. I give this place No Stars...The rooms do not get cleaned and roaches, The bathrooms should be cleaned daily, due to urine on the floor and feces on the toilet and grab bars.They make up the beds leaving the soiled sheets. Items go missing..vanish in thin air, as for CARE...there isn't any. Caregivers are not certified., nor are they compassionate. Memory care residents get corraled near the TV, where you may find 1 caregiver. The elderly need naps, but that would mean having to check on them individually... The most caregivers seen on the floor at any given time, other than during dinner, is 1-2. Don't bother to ask for help....they will ignore you or tell you they will find someone. During your initial tour, they tell you that there is a nurse Always on the premises...Not true. NO ACTIVITIES...unless corralling is an activity. Only bathing is done with a wet rag on their face. The sign says Assistant Memory Care...there is No Assistance. Your Love one is better Off alone in their Home. When you hear your love one tell a staff member that She is sorry and she will never do it again...That's a Red Flag!!! The Elderly with dementia who can't hear, are placed in Memory Care with Alztheimer patients, who are very lost. Therefore there is No stimulation for the resident who has dementia. They live in a world of silence and can't understand why they are being mistreated. Mental [Removed] and I would be concern about physical as well. There are No Cameras....no way of knowing which staff members are going in or out of the room. When you discover bruises and swelling....no one knows what happened. Of course, the fact that they are in Memory Care, your love one can't tell you what happened...they don't know. Do You believe the staff??? As for the time when you see More staff....breakfast, lunch and dinner. Take note...the extra staff is kitchen staff and sometimes 1-2 caregivers from the other side of the locked doors. How often are the residents taken to the toilet....0-1 How often are they changed 1 time per shift change. Each shift waits for the next to take care of it. No shift knows what happened on the shift before., and no one knows the reason why some patients can't walk or how to handle them when lifting. So....What can I tell you that will make your decision easy. Find another place.

We were given bad information. We were told there was a physiatrist on site and there isn't.This was one of the main reasons we chose Autumn Leaves now The Village. The community was also sold so my mother's had multiple nurses change and the billing has been messed up.

They need to get staff that has been employed longer than a month. They knew nothing about caring for a dementia patient. The first night my husband was there he fell, cracked his head open and had to be taken to the hospital. They let him run around in his diaper, one day they allowed him to go outside and roll around in the dirt in his diaper. They continually told me once the medication kicked in he would improve but no one ever knew what medication he was on. It was the worst two weeks of my life and I finally got him out of there. I wouldn't say anything good about it.

Since my mom moved in in August, there have been many problems. They have cut staff, made huge billing errors, tried to charge for services my mother has not received, seriously lacked in cleanliness, and obviously are only concerned about corporate profits.

I had the worst experience during my tour of this community. I wouldn't even send my dog here. Everything about this community needs improvement. From the staff to the up keep of the community, major improvement is needed.
