Not worth the money

I would not recommend this ‘5 star’ community. Highly disappointed for the incredible amount of money my father spends every month. My father has been at Forwood for 3 months, he moved here after my mother died. He was very lonely and we thought this would be a great way for him to meet people. Covid is still going on and this facility does nothing to help the seniors stay active and provides NOTHING for to do to interact safely on a daily basis. After a storm over a month ago, the front entrance is still in disarray and no work has been completed to fix the entire front entrance and adjacent meeting area for guests ‘who are not allowed to visit’. My father wants to move and we are looking. You pay a 4000 dollar one time fee....you get nothing for this! Do not place your loved ones here...it is all a lie to get your money!

Understaffed and declining services

This used to be a nice facility, but unfortunately, it has gone downhill with care since last year. They are constantly understaffed and do not have time to clean and change beds according the the regular schedule. Meals are delivered cold (and without choice) to their rooms as they are no longer allowed to eat in the main dining area. Very minimal activities and the memory care residents are locked down most of the time. It is sad and depressing that a poorly run facility can affect the emotional well-being of the residents. Some of these changes seem to have started with covid-19, but after being locked down for 6 months, services have not improved.

Disappointed

After a month in this facility, my family and I have become very disappointed in the nursing care provided and the administrative staff for not listing and providing information as to decisions they make. Upon admitting him the staff welcomes you with open arms, and assures that this is a great place to be. We our family visit him every day, several times a day. and as weeks passed we noticed the lack of care being offered to him and other senior citizens. Lunch staff complaining about there pay in front of visiting family members. Patients in distress, (respiratory) and waiting 15 minutes up to an hour for nursing staff to respond after the nurse call button was pushed. On many occasions they stop speaking English so you can not under stand them. My father unfortunately can not afford a private room and can not afford to pay for extra amenities. I urge every one to pay close attention to what is going on around your loved one, and I hope the standard of care provided is not based on a monthly check. I have no choice but to look elsewhere for better care. Please note: that there are and have been some caring staff, but unfortunately were not involved with every day nursing care.

Not a good first choice

My relative lives at the Forum in assisted living. The Forum is not a clean facility and I do not feel that the care is adequate. The apartment unit always smells badly. We try to maintain cleanliness in the apartment and we ask the staff to assist as well but they do not. The unit continues to be dirty and smelly. The staff launders clothes that often we never get back. Instead of actually cleaning the bathroom, they will wipe here and there with a clorox wipe for one minute and leave the room. I do not recommend this facility for your family member if he or she requires a lot of help. We are investigating other living arrangements. The independent living area is probably fine as a resident can do more for themselves and are more aware. As a positive, I do think that the activities presented to residents are fun and engaging. As a whole, from what I see, the staff the friendly.

The facility and apts are nice enough. My tour guide was unable to give me dimensions of the rooms or even the overall square footage of any of the apts. The corporate website also didn't have that info. Missing important information was a turnoff for me.

Poorly Managed/Extremely short staffed

My sister was there for 5 months. Unlike other familys that tend to drop off their loved one & only come every once in a while, we are a very big loving family that was represented most all day every day so we saw everything first hand. At the beginning it wasn't too bad, but there were certainly issues as I am certain they weren't used to our level of involvement. We resolved those issues, and a new General Manager was brought in. [name removed] is a true phony, She was sweet as she could be to our face, but I tend to believe that she more than stretched the truth. The staffing was seriously depleted, and she had every excuse in the book. Naturally, it was everyone but her fault. She started firing her best most caring personnel for the most foolish reasons. None of which could possibly been verified. I truly believe she is a corporate woman who's only concern is the bottom line. This is not the type of facility that I would ever want to be a resident of or take one of my loved ones too.

My Dad was a resident there for 2 months because he was mis diagnosed as having too many memory problems for assisted living. We moved him to another facility where he could be in an assisted living environment. There were very few residents that he could have conversations with. Other residents were loud and would enter his room if it was not locked. Staff were kind but he had to wait longer than 10 minutes for them to answer his call. Communication from administrators and staff were below what was promised. It was confusing trying to find who to contact to get things done. Activities were listed with fancy names and amounted to watching tv in a group. There were no food choices at meal times. Quality of food ran from average to poor. The place was always cold without a way for residents to control heat or AC in there rooms.

It sucks getting old!

Short staff in all areas from Administrator on down. No guarantee they will show up in timely manner once call button is pushed, 5 minutes to over an hour, very dangerous. Meals never on time, cold and since Chef fired not really good, lots of hamburgers and hotdogs. The Aids and Med Techs have to serve food, you don’t know where they have been, cleaning up after Residents in any and all manners and then serving food, kind of gross. No hydration cart twice a day like they touted. NO SECURITY AT ALL, parking lot lights out!! Can’t get maintenance to come when needed, waited over 2 months for light bulbs in bathroom, family could have bought but paying for all of this. I could go on and on, After 7 months I believe I have given them more than enough time to get their stuff together. Feel foolish for believing they would do all they said and feel taken advantage of.

Strongly urge checking with VA Dept of Social Services

I’m being prompted by SeniorAdvisor to submit a review so I am doing so. I must first say that dad stayed at the Gardens during early 2018 and the Gardens had hired new staff to oversee needed changes at the time. I’d recommend you check the VA Dept of Social Services website for complaints filed to check status and whether care has improved this year. I checked the site too late, only after I had realized my first impression was egregiously wrong, and had to file complaints myself. There were open complaints being addressed and had been for some time. My main complaint was with the care and dispensing of medicine, which are crucial. Bathing, therapy, and medicine dispensing was nothing like they’d promised. Even with our daily presence and reminders, countless oversight and errors occurred. Communication among staff was poor with an inadequate system in place. The most heartbreaking were the errors and delays in med dispensing, and the running out of meds to the point where doses were skipped. They lost track of one med and it completely dropped off dad’s list. Dad ended up choking on meds bec they weren’t dispensed correctly and ended up in the hospital. It wasn’t long before he passed. This facility is probably great for a resident not needing much medical care. We paid extra for advanced level care but this place wasn’t prepared or equipped for it.

Consistently disappointing

My relative has lived at this facility over a year, and I visit her at least once a month. There is usually dried feces on her toilet seat, and I often see the same scraps of paper on the floor that were there during my previous visit, meaning the housekeeping is pretty bad. I visited last week, and there was no toilet paper in the bathroom nor any towels. At lunch, the help just put the food in front of the residents with a plop, then picked up the plates when they were done. The food wan't cut up for those needing it, so they ate with their fingers, and coffee wasn't poured until the residents yelled. Otherwise, the staff just stood around.

Would go someplace else if I had it to do over

I would not place my loved one here if I had it to do over again. All was fine while he was paying full private pay. As soon as the money runs out, treatment of the resident goes downhill and they want you out.

Quite frankly we were not impressed with this visit. We did not get a tour of the facility but in all fairness, I guess guess because I had a friend who lived there it was assumed that I was familiar with the facility. It would have helped to at least see floor plans of some of the apartments.

We thought that the rooms were small and did not give off the at home feeling. We will look at other places. The person that showed us around was very nice though and answered all of our questions.

Toured 4/2/18. Pleasant building, but available room smelled terrible -- like months of neglected urine in carpet. I would not want a loved one to live in that without complete replacement. And the smell made me wonder how often rooms are cleaned with residents in them. The administrator stressed that residents are strongly encouraged to leave their rooms during the day (easier to see from the main desk, she pointed out) and insisted on this point even when I noted that my mother is bed bound, in late stage of dementia. She doesn't need to be propped up in a hallway in her last months of life!

Warm, friendly but dangerous if depend on meds

Just left 3 days ago after one month respite. Facility cozy and rooms bright with large windows. Friendly staff. Do not recommend for respite unless things improve under direction of new Nursing Director. Significant cautions, particularly if medication is important for your loved one and if there is no one regularly there to advocate for them. Moved in at time (one month ago) when there was severe staffing shortage and disorganization and poor communication among staff and with residents and family. Despite our 6 hour daily presence and involvement, it took 13 days for dad to get his first scheduled bath. Rise time for the day ranged between 5 a.m. and 10 a.m. though resident requested 7 a.m. New Nursing Director recently arrived and is trying to overhaul. Scheduled bathing remedied. Rise time range improved a bit. Medication continued to be a problem until the end. Ran out of meds four times. Three times ran out before trying to refill. Fourth never refilled and dispensing accidentally stopped completely. Up until final days, frequent incorrect dosing of at least one of the meds and absence of one entirely. Dad was on special diet including thickened liquids but meds were dispensed with unthickened liquids for weeks until I caught it. While there, dad struggled severely with congestive heart failure and pneumonia. He aspirated meds when given multiple to swallow at one time. Had great trouble getting the lasix needed to help with his congestive heart failure. Had we not pushed, staff would have delayed for a week until traveling nurse came. We had similar challenges with treatment of his pneumonia. All diagnostics had to be requested by family. No indication that staff could/would have recognized problem and taken appropriate action on their own. The accidentally discontinued med and the misdosed one were crucial to his heart and swallowing conditions. Seems to explain why Dad suddenly developed uncontrollable congestive heart failure and pneumonia. His health was stable even before and after his fracture, surgery, and rehab. Health declined significantly only after arriving at the Gardens. By discharge, he had congestive heart failure and pneumonia in both lungs and we were debating whether to take him to ER instead of home as planned. Since he’s been home for three days, he’s begun to improve. Discharge note extremely riddled with errors: meds left off, misnamed, incorrect condition, incorrect and incomplete history (reported LEFT leg fracture when it was his RIGHT leg, other significant history omitted, current diagnoses omitted). This was important for dad who left for home therapies (PT/OT/ST). Errant report has caused a lot of confusion with home care admitting nurse and therapists. While at Gardens, we chose in-house therapy but would not recommend on-site PT/OT/ST unless there is no urgency nor need for predictability. Dad was there for recovery from fracture and was coming from a rehab facility. We’d expected no significant interruption in therapy but therapy was slow on starting with OT wanting to delay eval until a week post arrival. Additionally, we were unable to get the home aid prescription filled so when Gardens staff wasn’t available to give scheduled baths for two weeks, neither could we obtain outside help that was covered under insurance. Using an outside home therapy provider means more reliable and predictable visits, along with home aid for bathing. In-house therapy offers no such aid but has PT, OT, and ST, and makes available therapy room that is otherwise not available to visiting therapists. In-house therapists were very warm and kind. Nursing Director is very responsive and responsible and is in the process of implementing some significant and much-needed changes, but I’d recommend you do your own investigation. Ask residents and any family present about their current experiences. If your loved one’s health is stable and medication isn’t important to your loved one’s well-being, and if what your loved one needs is just warmth and affection, this is a sweet place.

Understaffed

As a frequent visitor (once or twice a week for the past year), I have found Elmcroft of Braeswood to be severely understaffed for long periods of time, leaving obvious gaps in housekeeping and personal hygiene assistance for scores of residents. There is a higher than usual turnover rate for staff and executive staff. At times, I wonder how this facility is passing state inspections or retaining such glowing ratings from [Removed] and possibly other senior living marketers. It has taken me awhile to write this review because I like everything else about Elmcroft. It is a friendly and cordial environment but falls short of giving each resident the quality of individual care and attention they deserve and need and for which they are paying. On many occasions, I have visited my elderly relative (who requires assistance with bathing) and returned a week later only to find him lying filthy on the same bedsheets and in the same clothes and underwear he had on the week before.
