Very disappointed with the Brookdale experience, and would NOT recommend Brookdale to anyone. We were sold because the Director assure us they could take on our aunt who another local assisted living residence could not assure us the quality of care she needed. We took on the highest level package for our aunt and did not expect anything she did not pay for...I asked a number of times for things we payed for and were not provided. We would have hired additional outside staff if our aunts needs were beyond what we should have expected of the Brookdale staff. The residence provides 60 rooms and only 40 were filled - this should have been a sign right away there was going to be issues! We loved the size and thought our aunt would have gotten lost in a large facility and we believed that our visits would be great for other residents that did not have visitors. Brookdale was very sadly understaffed and the staff has way too many tasks for in a day to accommodate the residents in a manner that they promote in their sales pitch. Our aunt resided there 2 months before moving to a hospice care center. I was there every day but 1 and most days I was there twice. My husband was there every day and many days twice. My sister-in-law was there almost every day. So my review is not on a once a week or less visit. The management on many days in the short stay of our aunt was running around doing jobs of staff they were lacking. I could share dining experiences for the residents that were very poor- cold food, poor service, due to under staff. The care sold to us was not fulfilled! I could write an entire page on quality day to day fulfillment of needs. The only answer to us was your aunt does not have any soars therefore she is being cared for! The nursing and healthcare aspect was terrible. The only positive I can share - is that many of the staff are great! Front desk, marketing and many of the care givers are over the top and some may experience tremendous burn out but try really hard. I believe the director works well with what she has but cannot keep staff! Most likely due to pay scale or the huge amount of work required of a care giver (they are required to serve food, clean-up dining room and fix meal issues in addition to delivering meals to those who do not use dining room) in addition to transporting those who require it to and from dining and in Incontinence issues during meals that need to be addressed. This makes for a very long and often poor dining experience for most involved.

No kitchette. [Name removed] said she would add a cabinet but there still would be no sink. I did not care for the floor plan of rooms.

I would not give the Assisted Living more than one star for this facility. Please do your research before you place your loved one in the Assited Living area. It is a glorified nursing home. They are short staff often that tells me something is not right if their employees are not happy. This refelects on our family members quality of life. No matter weekday weekend residents are just lined up along the wall waiting for the next meal. Activities try their best with little support they are given from Leisure Park. Don't bother voicing your concerns to Administration it goes in one ear out the other. Nursing does the best they can, but some of the aides need to be retrained for care. Again review all options do not rely on a simple tour. When we toured they had a beautiful courtyard avaialbe, now used as construction storage really???? Pay for some outside storage units, now the residents who have mobility issues cant even enjoy the outside. does Leisure Park's corporation approve of this lifestyle I guess so.

[Date removed] To the CMAs, RAs and CNAs I wanted to thank the hard working and dedicated caregivers who have cared for my Mom while she resided there this past almost 4 years. The issues I had with Winterberry Heights were not generally with you, but instead with the administration there. By in large, Mum was safe and well fed, until the past few weeks. There were past issues, but none as bad as what happened last week. When I found my Mom in a state of neglect on [date removed], it would been easy to quickly blame the RA on duty that day, but that simply is not the case. You all have impossible assignments. The facility accepts seniors and disabled folks who have needs far beyond what an assisted living facility can meet. It is a priority to keep those units filled in order to meet profit goals. Then they understaff the place. They assign one RA to an entire level of residents, maybe 35 to 40,, many of them needing lift assists. You are told to be in and out of a room in 5 minutes. This is a set up. This is a recipe for neglect and harm. This is exactly what happened to my Mom. She never wanted to go there. She forgot about a lot of things, but she never forgot her home and she missed it every single day. And she did not want to live with me. So, I looked to Winterberry to be the solution to her needs because of dementia. Most days it was ok. And then when her health started to decline, it was absolutely not ok. I kept a close watch on Mum but I trusted people that I should not have trusted. I wanted so badly to believe all of the promises that were made to me and Mum in that place. I truly believe that Winterberry has potential, but until the balance of only accepting the people who you can take care of, and having the essential level of staffing and skills in place happens, I believe that people will continue to be neglected and/or harmed. This is not your fault. This is the fault of your director, the Hawthorn Corporation and the State licensing and regulatory oversight. Rules need to be made, regulations need to be in place and enforced that will ensure the safety and well being of our seniors and disabled. Our beloved seniors deserve this at the very least. My mother is at [removed] recovering from pneumonia. She is ok and well cared for now. We are hoping that she can get back on her feet and be well soon. However, she will never return to Winterberry Heights. Thank you all from the bottom of my heart, for doing your level best, in impossible working conditions, to care for my Mom. When looking for an elderly care facility for your loved one do your homework, which I did not. Seeing a five star rating does not always mean you are getting five star service, especially when it comes to Leisure Park. The only thing that was five star was the nursing staff was five star. Some of the issues were due to the understaffing of aides and dining room staff which is my biggest issue with this facility. Due to lack of staff when it came to personal hygiene and dining assistance the waits were unreasonable. I addressed this with [name removed], administrator (no stars for him)! We were advised he would look into the matter, a month later and no change. When trying to contact him by phone on many occasions NO return calls which led to another visit. At this time his answer was, I was meaning to call you back. When questioned on staffing vs. residents he was unsure and advised he would have to look into it (really???). I also inquired about his interaction with the residents and their needs because he was never seen while visiting (which I did on an almost daily basis). I was told that the conversation was over and that he felt I was verbally attacking him. The meeting was less than 10 minutes and I thought he must have been joking, but he was not. Now this was an administrator and professional (LMAO). In my opinion his back was to the wall and because he did not have answers he ended the meeting. Being I was not getting anywhere I was forced to contact the corporate offices. I spoke to [name removed] who listened to all my concerns, but I never heard from her again! Leisure Park facilities were not well kept, my mothers room and bathroom were not cleaned to my standards. Part two: My father was in the skilled nursing unit DON'T EVEN THINK ABOUT IT if you need to place a loved one in this kind of care. His room was not clean, I found him laying in his own feces and he was always dressed in a hospital gown even though his personal clothing was available. When I confronted the administration on this again the same answer, not enough help. While paying top dollar for a five star facility and treatment for my beloved parents but in my opinion they didn't even receive one star treatment. After my fathers passing my mother was hospitalized and as a family we made the decision to move her out of Leisure Park. To date the new facility has opened our eyes even more to the disgraceful treatment that my parents received. MY ADVISE LOOK ELSEWHERE FOR THOSE YOU LOVE!

Within the last month, I discovered that my loved one's toenails had not been trimmed for so long that they had started growing sideways. When I addressed this with the nurse, it was apparent that she had no intention of correcting the problem. When I informed her that I had taken a picture of her foot and told her I considered this issue neglect, she contacted with podiatrist to come in. The last three weeks (three separate visits), I have informed the aides, Executive Director, nurses and Memory Care Director that my loved ones bed had bath towels placed on her bottom sheet and no top sheet. The aides offered various excuses or blamed someone else. The Executive Director, nurses and Memory Care Director all said they would look into and resolve. Nothing happened until my fourth visit this weekend when I got loud, angry and threatened legal action. At that time, the employees on duty assessed the situation and informed me that I needed to supply disposable mattress pads which I immediately did. Shame on the aides, Executor Director, nurses and Memory Care Director for inaction involving a 93 year old woman who can no longer speak for herself.

I am so unhappy that I had a “ I thought wonderful place “ for my mother but after months of lies from the director about getting a credit for things my mother was not getting starting to really push us in the move forward ! They ran the rcc to the ground after working her so much and putting so much on her ! The medtechs were overwelemed and unhappy and showed that in their work ethic . The marketer was great but had some untold truth about the cumunity ! They went months without a on the grounds rn and when they hired one was no help ! The co rcc from the memory care was a business office manager and was so unprofessional, she was so rude and made it clear that the rcc from assisted living can only take care of me so my mom would have to wait for her days off to speak about a issues !!!!! We appreciate the caring staff that cared for my mother !!!!!! [Name removed] - [Name removed]- [Name removed]-[Name removed] - [Name removed]and [Name removed].Thank you for all that you do !!! Farmington square is not running correctly there was only a few holding this place together !

Not worthy of one star!! Filthy, smells, the nursing staff not attentive to the residents needs. So sad!! Do not send your loved one to this place.

Visited Maine so we could see a loved one living at Birchwood Of Canco. Glad we visited because boy has that place gone down hill the past few years. Try calling and asking for a manager and find that no one is there during the day. Want to speak to a nurse, good luck. Where is the staff? Rang our loved ones bell because they needed help. Waited in their room for far too long until someone showed up out of breath and frustrated. Got chatting and flat out asked her what kind of training they receive and she responded with an honest laugh telling us they receive no training at all and they simply follow another untrained person around for a few days then they are on their own. Stayed for a meal and was less than impressed. We sat with the daughter of another tenant and her dad who told us the staff is constantly changing and that her dads care doesn’t seem like a priority. We both felt the money being spent didn’t match the care being given by a long shot. Ironic that we said that before the food was served, because that’s yet another thing that was atrocious. The person who previously came to the room was now the one serving our food and she seemed to have received just as little training on waiting a table. Fresh baked bread was the only good part. We met the housekeeper and she was really the only person with a smile on her face who greeted us happily. The room was clean, so that was good. Wish we had asked her name. There’s an obvious problem in this building and I sure hope it gets fixed, in the meantime we’ll be making our recommendations to have our loved one moved elsewhere.

My wife was here and they never checked on her. They are very understaffed here. Everything started out good here then just went downhill. They would charge me things I never used. I had a lot of issues with the rehabilitation that they contracted. The only good thing here is it was a little bit less expensive, and they had a Dr or a nurse come 1x a week. I would not recommend this community. When I was moving her out the door was open a good 45 min with a buzzer going and nobody even came to see if it was a resident

Very understaffed. High staff turnover. Staff not adequately trained. When family member was having a good day care was adequate. When family member was having a bad day family member did not receive meals or prescribed medicine. Did not feel family member was safe in this facility.

My wife was there for about 9 days. 3 days in, she fell. On the 9th day, she fell in the same spot and broke her leg and hasn’t been able to walk since. I guess the staff was okay, I think if the fall hadn’t taken place then it might’ve been alright. But ever since then, it’s all been downhill. The cleanliness of the facility was fine. However, after 9 days of her being there she still had on the same under clothes that she went in with. I don’t think anything had been cleaned the entire time she was there.

Farmington square is the most unfriendly ,uncaring, place ! There was no interaction with my husband , He would be in his room or sitting at a table all by himself . most of the staff are unfriendly will not talk to you or look at you . That includes management . when my husband passed away I received NO compassion from anyone except the housekeeper,not a word our phone call from management The last time I saw my husband he had feces on his hands and face his hair had not been combed and was setting at a table all by himself. this is how he was every time i came to visit. He was only there 2 weeks ( BAD PLACE)

I don't want to place the unprofessionalism blame on the entire facility. However, I think that a particular employee by the name of [Removed], needs to practice more on professionalism. She really disappointed me because she has a kind demeanor, but un professional. People who come in seeking a job deserves to be treated with dignity and professionalism. Get it together, [Removed]!

The one thing they never screwed up was cashing the $2500.00 plus a month check for their “services”. My elderly parent was given another residents powerful narcotic pain medication and rushed, traumatized, to the Emergency room. My elderly parent has terrible arthritis and can’t shower without “assistance” which they were supposed to provide. I found my elderly parent, more often than not, filthy, in a soiled depends undergarment. Often when I sought help to clean my arthritic elderly parent it was not there. I was told you can shower your parent, they bathed you when you were little, by one of the nursing staff!? And, of course, I ended up showering my elderly parent frequently. There’s nothing more traumatic than finding your elderly parent on the floor when checking to see if they’re safe. My family and I paid for the depends undergarments required, and if you don’t write the residents name on them they “disappear” somehow. These undergarments are not cheap! The cleaning staff does a poor job, at best, keeping the residents rooms clean. My advice, look closely before you subject your elderly (The Greatest Generation) or disabled loved ones to Rittenhouse! I am a proud military veteran, like my father and his father before him. I have not exaggerated or embellished this review or used a pseudonym. My elderly parent has since moved from Rittenhouse and my family and I are relieved.

WARNING ** PLEASE READ ** Do not place your vulnerable loved ones in the care of this facility. They make promises that they refuse to keep. We were promised a private room in a neighborhood with higher functioning individuals. Further, we were promised a secure location for our father who has alzheimer's. Finally, we were promised constant communication and a role in the care they provided to our father. They placed our father in a neighborhood where the windows are not locked, secured, or alarmed. They did not monitor him following his placement and he was able to crawl out of the window, into the street. Following his escape, they placed him with a roommate in a neighborhood with the lowest level functioning individuals (the ones they do not show on the tour). When we repeatedly stated this was not where we wanted our father they refused to give us any options. When our father's mental and physical health began to decline, they continued to refuse him adequate care. Finally, our father was pushed over the edge and lashed out at a staff member who appeared to be inadequately trained to deal with his behaviors. They immediately sent him to the ER and recommended a psych. unit. They continue to refuse to take responsibility for our father's significant decline under their care. In less than a week, he went from one of their staff stating he did appeared more like a visitor, then a resident, to someone who can no longer form two word phrases. Do not trust this place to make decisions on the best care of your loved one. Once you sign on the dotted line, they will disregard all of your requests and wishes. Further, they are one of the most expensive placements in MN. They are many better placements for less $$.

Be Aware: Great sales pitchers! So far, poor follow through. . Weekends: "skeleton crew". Clogged plumbing, no water. Several days later, commode is stopped up. Weekday, day shift kind and helpful. Weekends are not good for patients as staff is too limited. Bath assistance Mon., Wed. And Fri. Only. Laundry? Find a dirty bed, change it yourself. I will update this review. Just question and be aware of these observations. Any health issues, doctor sends them to the hospital. Be ready to be on call. It seems they don't want to generate any bad statistics for their facility. I am undecided as to where the family will go from here.

I had a family member here on the memory unit side. We live long distance and aren't able to be there often, but we had trust in this place to make sure the proper care was given, what a mistake. I would like to say how unprofessional this place is. Not only did they fail to communicate to the family when a change in condition occurred to our loved one, but the only time we DID hear from anyone was when they didn't receive payment for the month due when it was their fault for having a change in staffing causing our family to not receive monthly statements. Apparently that is the only thing this place is concerned about, getting paid. They waited several weeks to let us know that our loved one had lost 30+ pounds and was not eating, the immediate phone call to us was "we are seeking hospice treatment" As many may know that is a end of life treatment to make someone comfortable and pass peacefully... well no other update was given prior to that so of course we were taken VERY off guard!!! Our family should have been notified immediately when her weight started to decrease NOT when things were getting serious for them to then notify us. Our loved ones well being was obviously last on the list. I have never experienced anything like it.
