Wonderful, kind staff but very under staffed at the expense of care. As a result, response time was slow. Especially on the weekends. It was obvious management was skimping on help to save money. I would not recommend for someone who needs a great deal of assistance. It would be ok for someone who needs minimal help.

I would not recommend this facility for Alzheimer patients. The only person that seemed qualified was the activity director. Housekeeping was okay. Communication was okay. Move in was great, after that it was down hill. Meals were excellent. If they are going to take Alzheimer patients, they need big time improvements. If they just have patients that need regular help, they would be great.

My mother was a resident at Amber Glen previously (April 2023). I could not recommend under former management. I found her room filthy more than once. They let one resident roam freely, stealing from other people's rooms-and they were aware. Activities were non existent after about 3 or so and not consistently any more than coloring or sitting in front of a tv. Only few of staff actually engaging with residents. Often short handed, so not adequate supervision and/or they combined those with severe problems with others causing verbal arguments and even a food fight that I witnessed. As for the food- usually good breakfast. Otherwise over reliance on soup and cold sandwiches - usually without lettuce or tomato or anything to make it look palatable. No call buttons. They use very ineffective motion detectors. Had many things stolen and/or misplaced. My sister or I went every day to make sure she was okay. Not much accountability as the Director, Head Nurse, Bookkeeper and Kitchens Manager were left or replaced within 2wks of our arrival. On more than one weekend evening there was a sub/contract nurse in charge of the whole place who knew nothing or no one- including the codes to open the doors. And get everything in writing- I believe we were unfairly billed based on the difference between what the salesperson told us and what we had to pay.

My mom got fairly good care. She was not there very long. Satisfied with care until she had an untreatable fracture and went on hospice. It was a battle to get medication to her for comfort. Mostly good people but needed a little more training on end if life care. This is something they should be better at. They really did quite well with her before she was dying. She had behavior issues and she wasn't easy. Mostly good people there. Like I said, needed to be educated more about focusing on comfort care when appropriate

The staff is incredibly nice, the food served is terrific and the building is more than adequate. However, there is an unusual policy in place. If the resident has an active contagious disease such as even the flu, they are not supposed to be in the building. In a community such as this, people will pass bugs around. One would hope for something minor, the resident could just be isolated and monitored for a couple of days. Instead, they are sent back home. If they have no home to go back to then what? Anyway, check 7.1.6 of the contract.

I think the cottages are great. Cleaning was very consistent. High turn over. Increase rates. Not enough staff. Never felt like I was getting people that really cared about my grandparents. I think the turn over is too high and quality has drastically shifted in the last 5 years.

This facility does have its bright spots. They included food, some staff, and engaging activities. But they also have significant turnover, very poor communication, and some snotty and disrespectful staff.

The Sales guy that we worked with just wanted a sale and completely RUSHED us through the paperwork. He verbally told us that 18 month private pay would not be a problem and told us this just to get us to commit. I had to fight corporate. It also wasn't mentioned that level of care is a completely separate cost. Not to mention they just had a 10% price increase, which coincidentally happened at the same time as a full remodel of the facility. The price increased by over $[removed]per month. I am very very unhappy.

They look overworked. But as far as the aides, they rarely interact with the residents on down time. They're mostly on their phone. Again, short staffed and they cut the staffs hours. When I visit, I rarely see someone. They are fed well. Almost too well. They bring out a 3 course meal with is not necessary. The amount of the food wasted is embarrassing. And my Mom is paying for the waste. Very little activity. Exercise in the morning and bingo in the afternoon. All of which are done through YouTube. Very little one in one. I moved my Mother from [community name removed]. [community name removed] was phenomenal with activities. My mother is higher functioning and very bored. It is an old facility. In all honesty, it is not a good facility. In fact, I'm looking to move my Mother. I would never recommend Brookdale if says anything. Horrible management

There seem to be no processes or procedures in place as it relates to the residents. I was always getting calls that my father was out of meds or hearing aid batteries etc, this was after providing 6 month supplies of each. No one seemed to know where to look for resident items etc. Personal hygiene and bed Linen changes and laundry for residents was terribly lacking. Communication among staff almost non existent. Facility needs structure and processes and procedures known to all and followed.

Keep your loved one at home. Brookdale Eden Prairie must have been a top rated facility at some time in the past. Owners of facility need to be questioned about their profits, their business model for future, their reasons for lack of upkeep in facility and staffing and maintenance

My grandma lived there less than a week before she entered hospice. They are understaffed and have to go into the hall and search for someone to help with caring for her. The lady seemed nice and concerned about my grandma. Once my grandma moved in. She did not answer my calls or help me solve issues that were evident from day one.

My loved one lives at Brookdale Twin Falls. This facility could improve on accountability in all departments including administrative, operations, transportation scheduling, to consistency in personal care. We did not feel that the admissions process was thorough, they seemed rushed to get a deposit and following was lack of communication. There was not a thorough orientation for my loved one, or the family. Overall, I would recommend Brookdale Twin Falls based upon its cleanliness, amenities, friendly staff, a well ordered dining experience and maintenance. However, I would suggest reviewing financials with the Executive Director before AND after your loved one's health review and BIM score.

Inexperienced and unresponsive management. We picked this community because they stated they we're building 4 PICKLEBALL courts, which my mom plays But, they have no programs in place/ no teaching pro, which they promised.[name removed] , the executive director of FountainView, is blaming the lack of any PICKLEBALL program on a missing fence.

Based on the experience we have had so far with Five Star Premier we feel they could improve on all areas. To be honest we would not recommend this community to other families looking for a place for their loved ones. The place needs to improve especially in the overall quality of care for the residents

When it comes to the friendliness of the staff we gave them an overall 3, however some of the staff members there should receive a 5. We thought we had found a great place. The staff at Ponderosa Lodge Senior Living were very accommodating to my sister's diet needs since she has celiac disease. We would recommend this community to other families if their loved ones do not have any physical issues, especially any decline with memory care

I came to visit my family member and she need assistance and I couldn’t find anyone. The call button went off for hours. The nurses seem to not know anything and are kind of dingy. Building runs with all agency who can’t really help much either. They need to do some reevaluating

Make sure people understand that they are not getting skilled nursing and that you will have to keep a close eye on the physical wellbeing of your parent.

Our family had an in person tour with The Gardens of Port St. Lucie and It was not very clean, and smell was very bad. Turned us off in arrival. Room they showed us as example very nice. Room my step dad was to go to very old and run down and next to an exit door which he would have done.

Pros: staff seemed engaged with the residents and they were nice. It was more cost effective price wise. Cons: The memory care floor was on the second floor - did not make us feel comfortable if there was an emergency. The memory care floor was also not very clean (floors were dirty and it didn't smell clean and didn't seem well kept). The rooms were my least favorite out of the 3 places - not as open and bright as the other two locations. There bathrooms seemed older and just not as well kept / clean. When we left Morningside, we all knew this was not where my mom would be placed.
