Brookdale is extremely expensive, understaffed, and has had lots of turnover in caregivers and additional services like the salon. Due to understaffing they don’t seem to be able to keep up on the hygiene of their residents or their clothing. They have made it abundantly clear that in memory care they just want residents that will sit in a corner and not create any extra work for their caregivers like walks for residents that suffer from sundowners. The director may try to mandate a 24/7 babysitter for your loved if they are seen as being an inconvenience and charge the family a huge hourly rate for it. Your loved one deserves better and deserves a facility with a higher caregiver to resident ratio.

My mom went into this facility on [date removed]. My sister took her to the ER on [date removed] where we discovered she was severely dehydrated and septic due to poor care given to her peri area at Sebring. We are now in the process of watching our mother die! This place oversells on what it can deliver. Completely understaffed!! Several times my mom waited anywhere from 30-60 minutes for a call light to be answered. If you are thinking of this facility as an option for a family member I would look elsewhere.

My loved one used to live here and passed here was often upset about the treatment this patient received from the Medication Technicians on the top floor...and im referring to the women that worked in the evening. Extremely rude, never helpful..disrespectful. NO bedside manner.... Always issues with my loved ones medication I cannot believe these women are legally allowed to work in personal care!! The way they talked to me I can only imagine how they talk to the patients when no one is around! The Nursing Assistants in the evening sit around with a disgusting attitude. Came to visit my loved one soaken wet in urine.. hadnt had a shower in weeks. From chatting with other patients families I know im not alone in these issues.. the Director of Nursing and Assistant Director of nursing clearly dont bat an eye to these issues with the night shift team... Do NOT put your loved one in this facility!!

Putting my loved one in Brookdale was the biggest mistake I made. The lack of communication is astonishing. My loved one was injured and I was not told until I went to visit and saw what happened. The caretaker ratio *meets* minimum requirements for the state but understand that it is not enough to properly care for and monitor residents. This facility has been nothing but a headache and they have shown their true colors.

I must say after visiting this location in the morning the staff did not have any idea what they were doing. The nurse was two hours late. Therefore the residents did not get their meds on time when I ask was the head nurse contacted about this situation the staff show me that they have reached out to, the DON more than once, breakfast and lunch. I seen better at the 2100 shelter building. This building is very shameful especially to be located in Medina. I seen better living situations in Cleveland Ohio in the kinsman area and if anyone know about the kinsman area, it is not a good area , this place look like the projects

My loved one was previously a resident at Brookdale Forest Grove and the overall experience was terrible. Everything from management to day to day care was bad. She ended up in the hospital for two weeks and we instantly removed her. I would not recommend this facility to others.

First, this is not a review of the kitchen staff, aides, cleaning crew, and maintenance personnel who are all top notch. The administration and nurse here simply do not treat residents with respect. They refuse to listen to residents, family, or other professionals who directly work with the residents. They speak down to residents, making them feel like children. They make assumptions and generalizations about medical conditions or disabilities without consulting doctors who work with the patients. They are rude, combative, and unwilling to listen in conversation. I strongly recommend to look elsewhere because places with respectful admin and nurses exist in the area.

Bad things happen when room checks aren’t done every two hours. Pts need to be evaluated for post surgical problems that may arise. Nursing homes should be more vigilant when taking care of the elderly even if it requires extra time.

This is a beautiful facility but they did not take care of my mom. She fell and ended up in the hospital because they were not putting her compression socks on her. I was paying them to dress her in the morning. When I asked why she never had her compression socks on, their answer was "She came down to breakfast already dressed." They missed her medications several times. She had a severe UTI. It was a 30 day supply of medicine. 3 weeks later, only 1 pill had been given to her. Her shower leaked all over the bathroom the first time she took a shower and it was never fixed. She had to use a vacant room down the hall to shower. I wasn't allowed to have a meal with her in the dining room. We had to reserve a room for me to eat with her. The food was not good. It was $18 for a guest to eat with her. We spent a lot of money buying her take out because she didn't like the food. I paid [fee removed] and she was there for a total of 39 days. After she left the hospital she had to go to rehab. I told them that she would not be going back there and they still charged her for rent and care and utilities. I would not recommend this facility unless you are just looking for somebody to do your laundry, Cook your meals and maybe clean your apartment once in a while.

We moved my loved one into the community and tried to make my loved one feel comfortable by setting up their room. Family members saw a resident that had fallen and hit their head. The resident was yelling for help, and there was no staff around to assist this resident, and when the caregivers saw us they told us to leave the resident's room. The caregivers let us know that hospice was going to assist the resident, and the caregivers didn't seem to be very concerned. Another resident had fallen and had bloody blankets and towels, and the caregivers hadn't removed them from this resident. I removed the towels and blankets for the resident and took them to the front desk, and they were still there as the family was leaving. I don't feel there is much concern for the resident's care here. The staff told the family that they would take care of my loved one's medication and they never did. The family had to step in, and do this. The staff would wake my loved one up at 4 am to give something and this was disturbing to us as we didn't know what it was. I spoke with [name removed] to discuss my grievances, and he blew past these concerns, and didn't take any care to try to make resolutions. I think this place should be shut down.

My father in law was transferred to a nursing home 5 months ago and was charged for a full month even though he was only there for a half a month. They said he would be refunded for the unused half month. That was 5 months ago and they still haven't refunded the money. When my wife calls them, the manager is always busy and says she will call back but never does. He has had clothing and towels, all new, stolen from him numerous times by the staff, even though his name is on them. NOT A RELIABLE OR RESPONSIBLE PLACE!

This place is understaff it isn’t even funny.Employees don’t care don’t even show up to work. But yet they get to keep their job!! How is that. Rough with the patient. The state needs to get involved!! Don’t send your loved one here!!

We used Brookdale Edina for 30 day respite. Worst facility and experience ever. They did not help her bathe brush her teeth stayed in same clothes for days. Not a place I would recommend. Also, they knew she was only going to be there for 30 days and charged an extra 3 days which amounted to $[removed] because they did not receive written notice the same day she was admitted.

What a disappointment. I moved here [date removed] and now it is a month later with an inoperable refrigerator. I have had altercations with director of maintenance. All he does is yells and blames me for the refrigerator. He is obnoxious band tried to intimidate me which I will not allow. I have put in three requests to fix or replace and i am sitting here on a Saturday afternoon refrigerator not working. I drive and when maintenance comes by once yesterday I was not home and do not allow anyone in my room without me. Is this good service? Refrigerators should be treated with priority. I have spoken to appropriate individuals.

Was able to move my spouse out of Sebring-Coventry this month. In a recent 7 week period, my spouse was in the ER 5 times -- a head injury requiring many stitches, bruises/cuts on various parts of his body and a displaced fracture of his collar bone where surgery wasn't an option due to his dementia. On the last ER visit, the doctor told me my spouse was not getting the level of care he needed from the staff. Also, the ER notes stated that he was "unclean". On a recent visit, I took him to his bed, which the staff had already made, only to find a huge amount of wet urine on the sheet which had been covered up. Three days later, the sheets still showed the same now dried urine and the sheets had not yet been changed. These were sheets I personally bought and were a different color so I know what occurred. The staff are uninvolved and unsupervised. My spouse has filthy, stained clothes on for 4-5 days at a time. Rarely does he have hair or body washed. Stay away!

The employees are getting high on break/lunch and then “caring” for their residents. One in particular [name removed] posted a video of herself smoking marijuana on break. They are caring for our loved ones, HIGH. That puts our loved ones at risk. Horrible, do not send your family here.

I believe that if somebody just needs a room and they are more independent, then it is probably a very nice place to live. However, if they need at care at all, (even meals delivered etc.) I would avoid it. For our family, it was a very bad experience.

We had a very discouraging experience - they could of had more staff members that are knowledgeable and have the training for dementia and Alzheimer's residents. They could of had better communication and been more organized. We had to do a lot of things ourselves and stay on top of things and we felt for the money being spent it was not worth it.

The staff at the Denton, TX facility was very rude uneducated and unwilling to assist in any way to provide proper contact information for medical records and lacked basic human decency. I advise you if you are not from Denton, TX do not take your loved one to this facility.

Where do I even begin? I’ve been trying to get in touch with the Regional Manager, but I can’t get a call back. So, I will post here in hopes that someone can call me. My dad has only been there a little over a month, and I have seen more bruises (attached photos) on him during this short time than I have seen on him his entire life. [Name Removed], the LVN in charge and only point of contact, will lie to cover up. [Name Removed], the director, will never call back. We’ve asked for them to investigate how these bruises happen, then we are told, “sorry, it’s internal and we can’t let you know.” My dad had a bad fall in his room and was found in urine and throw-up. How does this happen?! Had to be transported to the hospital with a 2 night stay. There is so much wrong with this facility, it would take me days to write about it. Please, please, please do not take your loved one here!
