My mom has been a resident for over two years now. At first her experience was great, but under new management the care has deteriorated. Staff turnover is very high which is concerning and upsetting to my mom and other residents. Staff members just disappear with no explanation of what happened to them. There are a few excellent staff members and all the Med techs are great but how long will they stay? This place has turned into a sad and lonely place for my mom. After she was released from a week long hospital stay my mom went back to the Village. We were told her care would be more attentive, as she needed more after the hospital stay. After two weeks that stopped and I was told by the director that the extra care was just temporary and a “courtesy”. Had we known that we would have placed her in another place after her hospital stay. We are very disappointed and saddened by the change in care.

Although there are staff members that truly care for my mom in the facility, overall there was an incompetency for nursing management and misdiagnosis of my mom's condition. There were a number of staff members that caused more conflict than resolution and were unwilling to encourage activities and getting residents out and about, even within the facility.

I had a family member staying at a ‘5-star’ skilled nursing facility in S. Florida for over 2 wks. We were there to visit nearly every day, for at least 3-4hrs per day, sometimes more. Therapists were very good. Nurses, generally, did there job. The rooms were kept relatively clean. There was a handful of staff that would give a little extra from time to time. When it came down to my family member complaining of pain and visiting family speaking to nursing staff & supervisors about worsening issues, little was done. Within a few days this family member was rushed to the ER in critical condition. Had the doctor and nurses acted responsibly & ethically it wouldn’t have come to this!

Worst place I ever placed my mom. Do not think I will ever go by your recommendation again. Staff was rude and disrespectful. Care was totally disorganized. Many times ran out of gloves, supplies, etc. Right hand never know what left hand was doing. Poor communication. Staff disregarded rights of client and they dropped her and cut her leg and she wound up in the emergency room...twice at which time I moved out!! Total waste of time and money. Wasn’t a pleasant stay but nightmare.

Non-transparency when loved one injured on site.

The food served is poor to say the least. Even if residents order off the menu the quality is poor. Nothing is really fresh..mostly processed. This has been going on for years. Sad with all the land they could have their own gardens. ( ask a school or group to tend it as a project)

My mother was in Classic Resident that turned to Five Star. These are the facts. First and foremost terrible discrimination against the people in Assisted Living from residences in Independent Living. People in Independent had aides, made messes but they were allowed to turn theirs noses. I liked when these same people ended up in Assisted Living.my brother brought this up to the Director and she never wanted to discuss this. 1-Aide stole money from residents, my mother was one. Police involved never heard back. 2-Director of Nursing wheeled her and others in wheel chair at 7:00 AM left in a room, found y mother warehoused, by accident showed up to bring something, was very upset seeing this. Why? 3-Several times Five Star car service did not pick my mother up from doctor's appointment, it was winter. I had to get a taxi to pick her up, very poor response. 4-Many more incidents, the last one put her on the path to death. 5-I was the Medical Proxy, they called in a Psychiatrist without my permission or knowledge, she was acting oddly from a bladder infection. They never called me. This doctor decided she needed her meds changed, at this point I agreed. In a day I saw a horrid response; I ordered the meds to be changed back. In the next three weeks she stopped eating and starved herself to death. But before that my mother had a on site doctor's appointment Christmas week 2016, I went to the office to be with her, not there, I rant to her apartment and surrounding her was the doctor, nurse, head nurse , they never called me!!! She died [date removed]. I blame Five Star and the Director of Nursing at the time, [name removed]. The food was okay, chicken always looked under done, the Director would come in and say it was cooked according to the thermometer, it was red. If you have no where else to send your parent, then this is your option. I regret not sending her to The Jewish Home , not the Nursng home the Assisted Living.

We are not having the best experience here in this community. My mother has been to the hospital four times due to her blood pressure being to high. I do have concerns regarding the high turn over rate. They seem to be under staffed often. There needs to be more onsite activities available. At this moment I would not recommend this community.

The aids and the kitchen staff are top notch, no complaints in this area. However, the management team is less than cooperative and helpful when dealing with medical treatments and concerns. On more than once occasion I was given conflicting information by the staff RN and the facilities in-house doctor when asked directly about my mothers medical condition and needs. Due to under staffing with only 2 aids on duty during the evening and night shifts, patients are not able to get the individual care and treatment as they may need. A few of the issues I have experienced from minor to worst is loss in clothing, no routine bathing, and left in her own body feces for who know how long before found by a family member. If you are wanting your family member to receive top notch personal care, then this is not the facility you would be interested in. Even though this facility does have some good points, there is need for improvement in many areas.

I have mixed feelings about this facility. They seem to have a lot of turnover and staffing issues. The main problem I have noticed is with the cleaning staff. When my loved one first moved in, the room was dirty. There were feces still on the toilet and the dining room reeks like urine. Everyone there is very nice, but they just need some major improvements.

Be very careful when dealing with these people regarding fees. We were quoted a specific price for my father in memory care. They honored that rate for one month and then raised it $320 the following month. They were told what his income would allow. We are looking for another facility. We are not happy. Beware!

I have a family member at Mulberry and the review in March 2016 is mostly accurate. Unfortunately you have to monitor this facility as it is not the safest for your love ones. No one is checking on the residents routinely.Seems to be more and more turnover and not enough help.

I have some concerns regarding communication, care, and security. If they have an issue they want to talk with you about, you'll receive multiple calls. If you need to speak with someone, they'll never get back to you. We often needed to leave messages to keep Mom off of the bus for appointments or family visits. Not once did the message get through from the desk to the bus driver. When we got there they couldn't tell us if she was on the bus. How can the bus leaves without first clearing the names of those on board with the front desk? How do they know the resident has not left the premises? We requested that they change the band-aid on Mom's leg after she showered in the morning. The nurse said the aides we not capable, they didn't always have band-aids (we provided them), it was impossible to communicate the message to everyone. I asked for her email to make sure messages got to the "medical" staff, she didn't know it. Then she called me back with a fake one. She told me many times, "I'm just temporary." So we can't expect any services from the health care department because she is temporary? Not impressed.

Hello my mother is there and it use to be very clean every since December the cleanup of the place is not so good I use to come in if I had a problem I would ask Frankie to come in and she was great at getting any mess cleaned up and helping mom get comfortable when it was the PCA job she didn't mind they use to have 2 housekeepers Garrett does good but he needs help he can't do it alone maintain both buildings I don't know what happened but they need her back you notice a big difference in the building when she was there whatever she did it was with a smile they need to have more outdoor activities for ALZ residents food could be better

Most of the staff were very nice though a couple of them made me feel uncomfortable. I think my loved one wasn't severe enough for this locked unit and seeing the others in various stages of mental illness disturbed him. The residents seem to all have severe dementia; just walking around aimlessly, clapping hands, plucking at the air, or sitting around the nurse's station on the couches. I expected more from the dining area based on the monthly fees, but the food was poor and they used plastic cups and utensils. I wouldn't suggest this environment unless your loved one doesn't recognize reality any longer.

They are not caring about their residents

Beware if needing to move out: First you do not get your 2500 deposit back, Second there is a 2 month notice if you are moving out. This is important, we needed to move my mother closer to home and into assistive living due to declining memory issues. There was no flexibility in notice, and typically assistive living facilities will not hold a room for 2 months. This is a huge financial loss for my aging 87 year old mother. Communication became an issue once mom was transitioning between being independent and having memory difficulties. The nurse is sweet but appears to have more then she can manage. OT and PT love to take on clients and provide services however again documentation and communication with family members very poor. Most likely not an issue if you are completely independent- but if functioning more on the border line- there are concerns. Overall we are very disappointed. Again fine place for high functioning, however beware if your parents are starting to decline in cognition- but not quite ready for assistive living- you can't count on the facility to notify you at all of concerns.

Nurses are great but assistants give most of the care which I feel is poor. On the alzheimers unit they do not even have a ice machine or area where family can get drinks for patients. I do not think pts get drinks except maybe at meals. Nursing asst at assisted livings do not have to be state certified so many have no medical experience

very poor staffed,have to beg for help on anything, food fair,too much saying no we cant do that or help you. would not recommend unless you have allot of patience and time getting help for your family member.

They do not specialize in anything other than assisted living! False advertisement! Unless your loved one is cognizant and can take care of themselves, this is not the place for someone with dementia or Alzheimer's.
