There was a complete turnover of Administrative Staff... this negatively impacted their ability to communicate with us as a family AND when they reassessed her, in [date removed] , the price for her care was OVER [fee removed] per year. They did not assist us (as promised when we admitted her) in the process of getting government medical assistance "waiver"... we are STILL in the process of this and getting retroactive monetary support. There is not a nursing/caregiver staff was present/visible at all times to supervise and support the patients when necessary. Many of the hired Staff were NOT trained properly in the needs of a MEMORY Care/Dementia patient. Despite a change to MInnesota Healthcare insurance change from Humana to Blue Cross Blue Shield, bills were still being sent to Humana which has resulted in over [fee removed] of unpaid bills and mailings to get bills paid. Hospice was introduced in [date removed]. We felt that This was the only consistent nursing care that my mom was actually given personal care for. My mom fell at the facility twice and was sent by ambulance to Hennepin County ... Hospice WAS NOT CALLED FIRST. This also has resulted in numerous medical bills because insurance now doesn't cover the visits to Trauma Hospitals, because she is on a Hospice Plan. Having the "Rules" of care ignored had us very concerned for her safety and quality of care by the facility. Then to add insult to it all, they wanted thousands of dollars MORE for her "proper care." We felt it insulting because the care that she was previously getting wasn't even "up to par"... We told them at a phone conference meeting with several people present, that she would be moving out by the end of [date removed]. When I again mentioned that in an email to them to send documents to her new home, they told me that I had to give a 30 day notice. Even though we had given it at our December meeting, that was unrecognized and we had to pay through [date removed]. This is in our past now. I wish them no ill-will. I just HOPE AND PRAY, for the sake of the comfort, care and peace of mind, of present and future family's loved ones, that the new administration will step up and make that place WORTHY of the care my mom and others like her actually deserve.

Good place with lots of activities for residents and transportation to and from shopping and appointments. A little patching and paint would make this place be so much more inviting. Facility has pool and fitness center.

My loved one is currently a resident at Layton Terrace and overall the experience has been okay. As for care services, my loved one does not need major help, but what she does need is not provided reliably (such as help with teeth brushing- point out the toothpaste and toothbrush and ensure that the brushing is done, face washing- wet a warm washcloth and point out the cleanser, and choosing fresh clothing.) Her weekly shower is not always done. Staff and friendliness varies from person to person. They do not deliver on their promises and do not respond to emails. I don't know if this is typical of memory care facilities or if they are subpar. That said, it has been a very rough transition for my loved one, but it is getting somewhat better for her.

The staff work well together and care about the residents. The CEO, [name removed], however, does not get along well with either staff or residents. She tries to run the facility like an autocrat, so the residents are forced to take care of each other when the facility could have been involved. There is a very positive atmosphere which prevails. People make positive remarks, smile and greet one another in the hall, and help each other out. The hot tub was useful as a way to reduce pain. The food preparation/organization is excellent.

Assisted living and memory care never have enough staff. I love my apartment and the people but I would never stay if I had to transition to [removed] memory or assisted living. They also have two vans but go through drivers .... and this is the only transportation we have. Uber is leaving Minneapolis and St. Paul unless Uber pays their drivers more.

The residents are very friendly. We help one another. We honor privacy if that is your choice. We support each other & look out for each other.

Cleanliness could be improved on. Also the professionalism of the med tech staff. There were a number of med techs who badmouthed the facility frequently. [name removed] , the Executive Director, was fabulous. She went out of her way to help our family. She is outstanding. Would/would not recommend is really based on the individual's needs. It didn't hit the needs of our grandmother. She needed much more care. But if the resident is relatively healthy, it's a good facility.

I would say "Be careful and do your research".

I would recommend it to others. They need to increase their kitchen staff. The food is pretty good but the time between the first course-fruit or salad-and the main meal can be so long that my mom falls asleep and then gets embarrassed that she did. Most of the staff that tends to my mom is good.

My brother has been at Orchard Park for : months. We are very concerned that he is not getting his meds according to the doctors orders. The on site nurse has resigned so this is very concerning. They are looking to replace the nurse, hopefully this will take place soon.

At first it was the best value as far as cost. The nursing staff seemed stable and concerned about the residents. But during the year ,there has been so much turnover there that it has changed for the worst. I am considering moving my mother.

My loved one was a resident at Brookdale Eden Prairie. The experience there was okay. We grew concerned with all of the turnover - leadership and staff. Activities were limited. Communication was not great - we had to initiate for information. I can't say that I would recommend it to others.

I put all 3's because it's an average place in all ways. My dad is comfortable there, so that makes me happy. When my dad first got there, he said the food was good. But he noticed that it has gotten worse in the last 3 months. There is a lot of staff turnover. The transportation has been very reliable. He also appreciates housekeeping. He likes the grounds, too.

Some of the staff has been mean and demanding make my mom go to bed or stay in bed. Some of them rush her to go to bed, get dressed or finish going to the bathroom and there isn't enough waiters so they wait for an hour to get any food not to mention they don't bring the right foods and won't correct it forcing patients to eat what they don't like or want. My mom says the menus is very repedative and not very appetizing. Ibe morning there was no coffee needless to say I called the director to complain and the next morning they had Duncan Donuts coffee. I actually bought my mom an ice machine for her room because you would have to move heaven and hell to get some! There are staff members who are not like that and are caring too but there shouldn't be any of the negative because she is paying for it!!!

Be sure to follow up on care plan and charges for services. The food was very bland and we were charged 4 special meals, not needed. I would still recommend them but with caution...

I have found administration not very responsive to my concerns and needs. Unfortunately I think for that price range it's probably as good as it gets

They are quick to respond. They are always friendly. Food was very good but not enough variety that he found he couldn’t eat a lot of things. Food needed to be cut up for him or options needed to be provided that would allow someone with minimal teeth to eat. In my opinion, the cost of all of these places is too high for the amount of care and personal attention they actually receive. Especially for people that have to spend every last penny to stay there.

I would say based on some of the others we looked at Leisure Park was better. We also wanted someplace with Assisted Living and Independent Living in the same place. It was also conveniently located for us to visit on a regular basis. I do not regret our decision to move our parents there.

The management staff is very friendly and communicates with the family. Activities are a priority and are thoughtfully planned and well done. The dining arrangement is very different. No central café. Food is served in one of several dining areas on different floors. This can be confusing to new residents. Staff needs to help with integration of this approach. Food is decent. They serve homemade soup every dinner as an appetizer. Care (so far) is inconsistent and below expectation when compared with other Assisted Living facilities. We had to hire a Personal Care person to augment the provided care. The community doesn't provide Memory Care and therefore residents with dementia do not receive the level of care (prompting, redirection, wandering assistance) that they would get at a memory care facility. Several things (pendant capability, specific care fees, pendant response time, care provided, etc) were over stated by the Sales team in comparison to what is actually delivered. Housekeeping and laundry are average in terms of dependability, consistency, quality. The move in transition was smooth. They were helpful in allowing items to be moved ahead of the official occupancy date. Staff was aware of the new residents and made efforts to integrate them. Two weeks after move in, management to meet with the family to cover issues, gaps, expectations.
