I would rate everything a 4. They are very prompt answering questions, keeping family informed and if we have any concerns they address the issue right away. I'm very happy with how they take care of my mother. She has dementia and can be very difficult to deal with at times. The staff is very kind and treats her with respect.

The staff I liked. The administrator didn't want Bernie so he was there on a weekly basis as respite care. At the start of the 3rd week he fell, then he couldn't walk. Hospice sent him to hospital and we also looked for placement elsewhere. He ended up at [facility name removed].

Staff here knows everyone by name ... all 200 of us. Cheerful atmosphere. Our location in the woods is a plus. Several different apartment layouts. 4 "wings" and 3 floors to choose from. Price is all inclusive. The bus here has weekly out to lunch trips on Tuesday's and shopping every Thursday. Also the senior shuttle from Menomonee falls is available daily for $2 for shipping or appointments. Happy hour every other Friday with appetizers. Exercise daily. Games daily.

I noticed the residents were happy. They really support each other with no judgment. Price stuck out and it is where she wanted to go

We brought her there on a Saturday on [date removed] and she passed away [date removed] so we really don't have any experience but they were very nice for the one day she was there. The Myrtle Beach was very helpful. They listened to what we had to say. They were gonna help out with her favorite foods. I just wish she had gotten to spend more time in there.

They need to change the menu. It's the same old same old and I've eaten there a handful of times. Needs variety in food menu. My mom n her entire table complained to me several times. Get residents more involved. Not enough tables for all to play in games. Other than that it's been great. My mom doesn't like it but she wouldn't like it anywhere.

It's great and she has adjusted. The food is very good. She has favorite nurses....one she loves and she calls her Sweetie pie. Yes, we would recommend Fountainview to other families based on the experience we have had so far.

I'll give them 4 1/2 for all of them..My mom was only in there for about a week because she died then so I don't a lot about them. But I liked everything I heard and saw.

The facility is well maintained and the staff is well equipped to handle any personality type they meet. The overall experience was a 4 because there was one of my father's childhood toys that was taken and never returned. It sat far back on a bookshelf and would not have just fallen off. The issue was not resolved.

Walking areas are good, lots of old oak trees. Assigned parking is good, covered parking available. A Luke warm recommendation, no. Walls are not well insulated, noise between units is annoying.

Staff are are good and a new chef, [name removed], has given us new offerings. Physical therapy and occupational therapy staff are excellent. I am glad I chose this facility.

Great staff but sometimes short-handed. Staff was friendly and caring, but there were days when short staffed affected their efficiency. I liked the fact that rooms were furnished. They also give a 5% discount for veterans and their spouses. Visitations were not restricted.

The facility is clean and the staff is mostly friendly. I believe some of the caregivers need to be a little more conscious of the residence. There were a few times my parent buzzed for assistance and no one came. I understand that there are only so many caregivers per resident but, a 20 minute wait was a bit long. Again, it is only a select few. The food offered is not to bad. My parent seems to like what is offered. There are activities offered but my parent does not want to participate. She occasionally participates is SPA days but not all. The activities offered are Bingo, word games, guest musicians and outings. The administrative staff is fine they usually are noticeable in the facility. They have meetings with residence and their families. Other than the meetings we haven't had much contact with them. Overall the "value for the money", I am satisfied. Their care costs are based on individual needs as opposed to other facilities that have tier charges. I recommend this facility.

The people are very welcoming and friendly. The food is ok, but you have to wait when they are busy. They always offer dessert!?? They have exercise classes every morning during the week. So far it’s been good

My MIL has only been there a few months. She seems to be happy so far. There has been a few things that need to be dealt with. Their van has broken down, so no field trips or rides to dr’s appts. Sometimes the food is not always up to par or they run out of items. Her toilet needs to be fixed/replaced. But mostly minor things that will hopefully soon be corrected. She seems happy there so far, so that is a huge plus

Cleaning details like on the glass walls of the dining room and also on the windows… I recognize people probably touch those areas however it seems like they're always smeared. The communication between the nurses/doctor/care providers concerning your love one falls through the cracks! It seems to me that they need to have consistent scheduled meetings to update all members on a patient. We have had a few incidents where the medication was increased by our doctor and was communicated, but was not followed through by the Care Provider at Stone Ridge. These are the primary reasons for my rating

The nurses are good about keeping us updated on issues or events that occur. Staff always friendly. Had issues with getting cloths washed on a regular basis. Have to request the room to be cleaned from time to time. The meals are average.

Brookdale Town Center was efficient and available to assist us getting our loved one the needed assisted living care for what ended up being her final week of life. Overall the staff was kind, cheerful and helpful.

The friendly staff and the how clean and nice facility is. The restaurant seems to be a very nice addition and my sister really likes the food. The amenities are nice the hair salon and nails and the therapy available

I would say the biggest thing was that they listened to my mom's needs. And they were really excited about her living there. Yes I would recommend The Forum.
