We moved my mother to Heartis earlier this year and since then we have had one problem after another - most involving response times and level of care. The former Director of Nursing was awesome but overworked and she eventually left. The current patient care coordinator is also awesome and goes above and beyond the call of duty. However, we know she is also overworked and we wonder how long she will stick around. Some of the caregivers are great and truly love the residents, but they are all overworked and underpaid. Sometimes only one aide is on duty to respond to the needs of an entire wing of residents. Many aides have no experience working with disabled people and don't know how to help my mom transfer from wheelchair to bed to toilet to recliner, etc. Heartis remains consistently understaffed and my mom often has to wait an hour and sometimes more to get a response when she calls for assistance, such as for help going to the bathroom. For what we are paying for her so-called level of care, this is all extremely unacceptable. I always believe that problems with a business start at the top - with management - and that is definitely the case with Heartis. The executive director is never available, won't return our calls and doesn't follow through with any promises he makes. I could go on and on, but I think I've said enough. I'm praying there is another, better place out there that we can move our ladies to. My family has had enough of Heartis. My recommendation: Don't take your loved ones there.

The place is beautiful BUT the Assisted Living side lacks enough staff to check in on its residents. It's as if they are in Independent Living. 1. restroom not cleaned, had to tell them several times 2. carpet smelled, again, had to tell them. 3. you have to buy your own toilet paper? Odd considering how much we pay. 4. Poor nursing staff. 5. If you want more care, you need to hire By Design, which is very pricey for night check-ins/care. Staff ratio is very low at night, so beware. 6. Be very careful--and make sure your loved one doesn't need more care even though they rate them. Sometimes it can be a fine line. 7. Re activities, the calendar sounds great but the staff don't encourage the resident to come out and join the rest of the folks. Sometimes people need a little encouragement. Beware of large AL's, especially if run by corporations. They're interested in meeting quotas.

We moved our mom to Heritage Oaks after a stroke left her unable to live by herself. She lived there for 4 years. We saw many, many staff changes, administration changes and deterioration of service and care. It is a beautiful facility, however, that is about all it has going for it. Problems occurred more and more, residents started leaving, staff was let go, which lead to worse service and care. We were paying $5,000/mo. for a number of services and a level of care that was not being provided, despite repeated discussions and complaints with staff and management. Mom's oxygen was not being administered as prescribed, her meds were not being administered as prescribed, the dressing and bathing assistance was often times not happening due to "I forgot" or "I got busy" by the staff, her linens were not changed when needed often times, laundry was lost, and the list goes on and on. I read an earlier review where someone asked for the door code to get out-there were a number of situations where the staff didn't know the code to get in or out. The staff didn't have family contact info if management was gone for the day, to contact in the event of an emergency. When we moved mom into Heritage Oaks, it was the highest rated, newest facility in the area. It is sad to see how much it declined over the four years. You can tell by the dirty carpet throughout the facility how much the maintenance has deteriorated. It took me over 10 times reporting mom's toilet running to finally get it repaired. Bottom line, Heritage Oaks was once a great place. Maybe they can get their act together and be good again. When we left, on average, they were loosing 1-2 residents a week to other facilities, the Executive Director had been fired, and the Sales Manager was "Acting Director", It was sad to leave, but for mom's sake, we had no other choice.

Tough moving in...suggest a walk through and still you will have issues. Get as much as you can up front. Book out your 25 hours of move in help for the $2,600 one time lose it moving in fee. Otherwise the people may not be available. The departments seems to offer more to some people than others wonder if she is getting what she is supposed to. They told us to buy our own hand held shower head and they purchased one for someone else before the install. Read the move in packet and get it ahead of time. Do Not Use Price Rite for moving unless you want busted glass, scratches on furniture, sentimental pictures damaged, stuff left behind and a dirty truck from moving a house with lawn equipment the day before. Dead grass dumped out from their truck...when they arrived...found more than expected left as well as one of the movers drinking cup...

My mother is in the memory care. Since her arrival many residents have moved to Autumn Leaves. They do not have many activities for people there to do. They have a large turnover from the care giver to the director leaving (should have happened sooner). There is a lack of leadership in training of the care each resident special needs are. I have been promised many changes with little results. I would recommend a camera to view what all goes on. Meals are not what are listed on website as samples. If your loved one is needing assistance I would recommend checking other places. Look at the number of empty rooms as you visit.

There is no communication concerning medication changes, no status updates which is very important for the memory care residents. Trying to get in touch with someone in charge on the weekend is a exercise in frustration. The concierge will tell you outright that she has no idea who is in charge. She then will connect you to numerous extensions to leave messages just in case they are the director in charge. The facility is responsible for the health and well being of our loved one and if nobody knows who is in charge it is impossible for them to act with the speed necessary to address an emergency situation. As nice as the facility is, to receive the necessary care we made the decision to move our loved one.

Ask questions [removed] turnover, ask to see the state inspection ratings. Do the residents seem engaged & happy. Or are they sitting around unattended. There are several rude employees from the top on downward. Very little communication to family members. Its very difficult getting a hold of staff. The rate on this SeniorAdvisor website states average mthly rent is approx $1,700. That is incorrect, it starts around $4,000 on upward. Please correct this information. Senior staff can be quite aloof towards visitors/family etc. They need to be more involved with the hourly workers & residents for better quality of care. There is significant down time & difficulty in finding there Maintenance Man, Al. The head nurse Diana seems disconnected from the residents. There are definitely clicks within the entire staff. However the marketing person Cecilia seems genuine about her concern for residents, I wish all staff with offices and those without offices shared her compassion. I am looking at moving my parent out. Its too bad, we pay alot of money every month for the level of attention we are getting. Perhaps a smaller resident to staff ratio is in order.

My parents have been living here since the place first opened. When my wife and I first visited, we were extremely excited about the possibilities. It was unlike any facility we had visited or ones that my parents had stayed in before. We signed up and moved my parents here several months ago. Like with any new facility, there are growing pains. One of the main reasons we selected this facility was because of the food. Their kitchen is littered with state of the art appliances and the idea of a home cooked meal was music to our ears. We visited for lunch before moving in and they served a tenderloin that was out of this world. This was a great relief as my dads biggest complaint in the two facilities he was in prior was the food. Well, that was short lived. The chef either quit or got fired and the food service from that point on suffered greatly. My dad started to complain about the food and the lack of it. For dinner, for example, they were serving hot dogs, grilled cheese sandwiches, or a bowl of soup. My dad complained regularly. We talked with the management staff on many occasions, even spoke personally with the chef explaining that my dad does not like the meals he is being served. Although the staff acknowledged our complaint and assured us that my dad would no longer be served these types of meals, all fell on deaf ears. During one discussion, the management staff promised to keep a log of the food my dad was being served and track how many meals he was refusing to eat. Promises, promises. When asked to see the log, one was not being kept. Since he moved in, my dad has lost 14lbs. In summary, this place has a long way to go to measure up to the standards by which they market. At over 6,000 per month, one should expect a level of care commensurate with the cost. They are terribly understaffed and often do not meet the needs of the residents in a timely manner. We are now looking into moving my parents to another facility.

We were really disappointed because this one was one of the newest and they had lots of plumbing problems. We visited 2 or 3 times and they never got resolved. The food was really bad, like cafeteria food. I had a fish with a lemon sauce, the fish was definitely overcooked and it was very rubbery; they tried to hide it with a bunch of sauce and that didn't work because it was too lemony. My son had a hot dog and that was also overcooked- I don't understand how you do that- but when you grill hotdogs they have grill marks, these weren't grilled but they were burnt and rubbery... overall it was all burnt and it had no taste, except for my fish that was covered in sauce

The dishwasher was not installed into wall correctly. It was falling out of the wall…it was not screwed into the top of the cabinet to hold it in the wall. The electric cord was hanging outside the dishwasher. Ready to fall on someone. I opened the clothes washer and smell of mildew like to have knocked me over. That would make anyone sick. The shower floor looked like it had been painted several times and the pint was peeling off the floor. It was very disgusting. The medicine cabinet had brown stuff all over the door. Look like something that was body waste. The carpet was very worn down. We loved [Name Removed]. He is a great guy. Very knowledgeable about the business. There was no way I would even [amount removed] a month to live in that unit. It was dangerous for someone of our age.

Long story but he fell repeatedly, knocked out a tooth which nobody seemed to know how that happened until the week he left… he had two sunburns, was chronically dehydrated, dirty and I was told they just let him go and staff were told not to bother him! ONE STAFF member took great care of him while he was there but the others were petty, offended by dementia behavior and frankly just ignorant and rude. I don’t know about the AL side but please, DO NOT place your loved one here! They are NOT safe. My husband got out of the doors so many times I lost count. This SECURE memory care literally had a sign on the door telling the residents how to get the door open. Just a big no

My mom enjoyed the assisted living area and the staff. The food seemed to have very little variation however. They said we needed to move her to the memory care unit so she could get more hands on care. So, we moved her. She had less care. They left her and her room in filth, the staff did not engage with the residents except when they had to. They instead sat on their phones. It was awful. She had bruises from what I can only assume were falls. The management failed to properly submit her claims to the extent of almost [Amount Removed] in claims not paid in a reasonable time. There was a lot of turnover in management staff. I think at least 3 or 4 directors in 1.5 years. We moved her out at no notice due to the condition of her care in the memory unit.
