Too make sure your family members as much as you can to make sure they are taking care properly

Good place to be. The residents are cheerful and eager to attend activities. The new Learn information presentations are interesting and informative. Our director takes a personal interest in each resident and follows up on our welfare.

We picked this community, because of the availability and price. It was adequate. Hell of a lot nicer than, the other place that my loved one got placed in.

I would tell others looking that the staff at Prairie House Assisted Living and Memory Care is easy to work with and friendly. But, when my loved one was there over a year ago, they were shorthanded and seemed like high turnover in regards to nurse an aide staffing. We moved my loved one to another memory care facility.

Our experience was the place was clean and the staff helpful. However, there appeared to be a severe staffing shortage which affected other areas of the facility. As far as the meals go, they should hire a good cook to buy local fresh food instead of what they are currently doing. The food served was not very good and tasted frozen or or precooked or out of package. Folks look forward to a good meal.

Would not recommend. Handled health emergencies very poorly. Exceedingly bad at communications to family. Took no ownership of responsibility. Would not pick again. We picked this community, because it was recommended by a friend whose dad was there previously. It was spotty! Care was lower than needed and expected which was the main reason we were there. It looks pretty though was overall disappointed.

This facility is beautiful and clean. My loved one, says the food is awful and often served lukewarm. Sometimes they run out of ingredients, like beef or chicken There is no chef.The resident assistants are wonderful.[names removed] did a great job when hiring them.[name removed] from the finance department is awesome. [name removed]from maintenance is a resident favorite. So is [name removed] the health director.

Legend was not available to us at the beginning for my loved one's care. The staff was good, along with the management team. The food was also good. The staff to resident ration was good as well, and this impressed me.

Over the last year or so the care and attention to detail have really gone downhill here. When I see mom I notice things aren't like they used to be. The new staff doesn't seem to care as much, or maybe they are just stretched too thin. The meals are alright. The staff are nice enough but I feel like they could do a little more in my opinion because at one time this place was better off than it is now. I don't think we could find a better place in the area but this one is lacking. Oklahoma isn't a shining star when it comes to mental health or healthcare in general.

We keeping having to ask to get the same things fixed over and over and they still aren't done. They do a pretty good job of keeping the indoors clean. Most of the staff is nice. The care services all depends on who is working. Our loved one is not a huge fan of the meals. When it comes to the activities they could have more. Value for cost - they are not cheap by any means, but they are not the most expensive in the area. Pretty facility but seems understaffed. Took my mom awhile to get comfortable there, but she is now doing well

Our family had an in person tour with Sunrise of McCandless and from what we could see it seemed like that had good care but the community itself just seemed to be a little dated

We toured with HarborChase of Tallahassee and it was built like a maze with interconnecting circular hallways on multiple levels, which is a nightmare for dementia patients like my dad. That was the main reason we did not choose this facility.

They often run out of food, because people are not choosing the fish option, which is a major issue. The chef is wonderful. My loved one loves activities.The staff during the week is excellent. Although, they have experienced staffing issues, and they don't have enough help on the weekend. The cleaning staff is not changing the sheets as often. They take my loved one to physical therapy, twice per week, which is wonderful.

Most of the staff is friendly and helpful. The only person not friendly is [name removed], the manager. [Name removed] (head nurse) and [name removed] (front desk) are especially great. The facility is kept clean. It is a comfortable place. Probably a little overpriced, and they will charge you for everything they do other than the room and meals. Their long time chef left a few months ago and quality of meals has really declined. Could have more activities for the more active residents. There have been some people moving out, and they seem to be having a problem getting new residents.

We didn't have a good experience here with our loved one. I will say, though, that they now have a new Executive Director. Hopefully that has helped change some things because people were not doing their jobs here. My loved one started out in Assisted Living and then moved over to the Memory Care Unit. One of the main reasons we initially chose this facility is because they allow pets (On the Assisted Living side only and with a pet deposit, of course). Then we ended up having to move her from the Assisted Living side to the Memory Care side. The cleanliness upon moving her into her new unit was not good, we had to clean it ourselves. A lot of what we were promised for our loved one's care in Memory Care didn't happen (for example: staff working with her and doing things with her). The only people who helped our loved one were the outside care staff that came in to assist (nurse, physical therapist, speech therapist, etc). The regular staff promised that once they got her settled they would start working with her, but they never did. Communication with the family was horrible. For the amount of money that it costs to live at this facility, you would think that they would have been on top of their game. As far as bang for the buck, they either need to up their game or lower their prices. The cost is what you would expect to pay for a top of the line facility and this facility is not top of the line. Their cost isn't all inclusive, either. Pricing varies and is based on care level and individual needs, so it not only varies but also changes. I would not recommend this facility to family and friends.

It was nice when you first walk in. The memory care unit, was separate and there was not a good smell. It felt like an enclosed area.

They need to work on, transparency as far as what they will and wont't care for. Take responsibility with behavioral issues. They have failed miserably. It's a memory care facility and they should act like it. This has to do with upper management. They do need to work on communication. They don't return phone calls. They can also, improve on the medical side of doing evaluations, it is very weak. They did not do an acessment after 30 days, to see how they could improve on the care for my loved one. If some need additional care, then they need to step. The staff is amazing, and I am very pleased with some of the staff members, that is caring for the residents.

My loved one simply does not get much healthcare at all here at this facility. Supposedly the Dr comes through the whole facility every other week. However, months of assuming she was getting the care she needed was a time mismanaged. There's been delayed and mismanaged follow through with doctor recommendations and necessary appointments. I was promised that they would follow up, but that hasn't happened. I've since emailed them about it several times, but have not received a response. There's no communication about my loved one's health unless I advocate. Doesn't seem like the medical staff is adequate. Essentially, we're paying thousands of dollars a month for an apartment with basic services and some activities (the Activity Director is great). But as far as addressing my loved one's health concerns.....minimal services and communication.

Too many "agency" people working or filling in. That's fine, but these temporary workers were clueless of the needs of each resident. On the wing my loved one was in, all the residents needed help with most everything - including feeding themselves. Sometimes there was only three workers in the entire wing....not nearly enough for the amount of work they had to do.

We love the facility and the charge nurse [name removed] is wonderful. She is at the community every single day checking up on our loved one and the residents so we cannot deny that the care being received is wonderful. However for the amount of money we are spending there is a lack of activities and meals. They are having an issue with their kitchen and their food - including quality and service. We do know supply and demand is hard right now but someone is not ordering enough to last .
