They promised lots and did very little. The rooms and bathrooms were dirty. The memory care side was locked behind doors that made a terrible alarm noise every time they opened which was a lot. Communication was terrible. Pretty much hated the place. Sorry we put our loved one there.

One reason our loved one is still at the facility is because of [name removed] who is the sales rep - we found her to be so lovely, and so caring towards our loved one. She has done an awesome job and we wish they had more staff like her. We would recommend this community to other family's if the following improvements are made. They need to improve on their friendliness towards family's as well as their follow up and communication. The cleanliness could be better and the food is terrible.

I think Eden Vista is appropriate for some people. But they assessed my person, assured us they could care for him, and then did not have the personnel to do so. I would not recommend it for somebody who is very sick. But for folks who are still fairly independent and don't need a lot of care, I think it would be fine.

Overall it was not a very good experience. The staff was friendly and you can tell they have good intentions but lack of training and lack of staff round the clock was frustrating. Also they need more of an activities agenda/calendar. Very disappointing. Also leadership stability was missing. The short time my mom was there management and nursing saw a turn over. The nurse that assessed my mom was gone a week later and unfortunately that caused a lot of disruption in care, services and Mission Creek did not fully communicate the changes.

My loved one likes the food. On the other hand, it has been, a very unpleasant experience. The room rarely gets cleaned. Ther has been lack of communication, with mangement on getting and keeping my loved ones room cleaned. Also, they do not have regular activities.

Overcharged two months in a row and did not correct it. As soon as they took over the Allouez village location they shut all the residence in their rooms and stopped activities stating covid but the prior owners did not do that. We are not impressed with this new ownership and do not recommend a big corporate facility.

I was not a fan of Maple Ridge. The facility itself needs some updates and TLC. The bathing is very odd, you have to schedule when to be bathed and there is 1 main bathing station so you get no privacy. The rooms here also looked very tight.

I hope the change in management helped. They had a management issue when we were there. It is a beautiful place, and it looked like the staff was very friendly. The unit director was very cold and not talkative. It didn't seem like a good fit for the director. I never saw her interact with any residents. The unit director never called me about my loved one's personal belongings being stolen. The community was understaffed. I would see residents with their call lights on and the staff was in a different unit. The executive staff never called us about anything, until after we left.

I want others to know that we did not have a good experience here at Bella Vista. My mom fell and hit her head after a week of being here and passed away. They were always short staffed and never had enough people to care for her. I would not recommend to others looking into this facility.

Older and dated but was told that they were planning a huge remodel and update. Convenient location to Willoughby VA. Positive comments from residents

As you enter Bella Vista you are greeted by the most wonderful friendly staff. So helpful and kind. Unfortunately, lots of problems. We lived in an apartment and received Assisted Living services. The service you receive is far too costly for what you receive. Waiting up to 1½ to 2 hours for care (especially to use the restroom or go to scheduled meals, etc ). Explained as short of staff. What the real problem is they can’t keep the excellent staff. Staff is poorly trained and treated lightly. Upper staff also need training. Meals are great!! Service is good. Staff is always changing because of harassment and bullying that is done openly in front of residents as they eat. Housekeeping is kept to a minimum. Barely time to mop the floors in your apartment. Here too staff is short. We have gone without cleaning for up to 3 and 4 weeks at different times. Poor attitude from upper level staff. Carpets, vinyl flooring, draperies with dust are common. Large black spots on carpets on all 5 floors, wet and spotted flooring also common. For the high expenses you should live in a place that is up to date and clean. Staff turnover here too. The residents are fun, friendly, and exciting. They share stories and are always helpful. Can’t say enough about the people who live here. Staff is somewhat friendly, but there is definitely a do it my way or no way by upper staff. Decisions change, unclear, and how it would effect residents is not considered. Upper level staff is hardly ever seen greeting and showing how Bella Vista cares. Activities are quite good. Outside trips are often, but more throughout the month would be nice to get residents out. They can change without notice. And most activities are left up and run by residents. Far too expensive for such poor services, cleanliness, with little care for residents. We have now left and found a wonderful place that gives so many services and care for its residents. Really tried to make it at Bella Vista for 8 months. Things just never changed and our treatment of care was most important.

Moved my dad in on [date removed] of month and had to pay the entire months rent. I didn’t realize there’s a $1500 non refundable community fee. The staff( nursing assistants) seem to have no training for Alzheimer’s/dementia residents. Took over 2 weeks before he got a shower. Staff never offers fluids, no plastic cup of water left in room or offered to residents on dementia unit not even with meals. Took 9 weeks before a plan of care was posted in his room so staff could read and figure out what to do with resident. Has never been shaved daily- hand him electric razor turn it on and show him what to do. Fortunately my dad is high functioning just no memory, walks, continent, feeds self so the care is extremely disheartening. The staff very friendly and polite just never any FOLLOW UP with any staff clear to billing office

My Mother has lived at Brookdale Burr Ridge since 2017, when the business was well-run & a good place to be. Though they can address all of her myriad of health issues well, of recent (since Jan 2019), things have changed. The Maintenance Manager, Dining Services Manager, Housekeeping Manager & Executive Director have all quit. It has been a revolving door of new faces, including cooks, servers, receptionists. Residents are very upset because they have not had room cleaning/linen change in 2-3 weeks!! And there is always some excuse from management. Food service is a joke. 3-4x/wk they RUN OUT of entrees on their printed menu. Or food is not prepared as listed, or components of the meal are "out", or the portions are tiny. Again there is always some excuse. It is quite sad. I have contacted their corporate office in the hope that change will occur SOON. This is no way for my Mom to live.

My mom was here for several years. At the beginning all was good, however over time the prices kept increasing and the level of care decreasing. They were always calling for supplies that were depleted way too soon for refills. Items always missing; such as shoes, slippers, clothing and personal items. It was expensive to keep replacing items that were properly labeled and "gone". When I would inquire about the missing items the staff would act "befuddled" as if I didn't know what I was talking about. Also, the administrator does not return phone calls. The activity department and the food however were excellent.

I'm sorry but I really have a hard time recommending this place to anyone with a parent that is going through dementia. Just got off the phone with my mother who said There is a man who took her hard by the wrist and pushed her in her room I'm not sure what's going on but this had better stop

It is still early with only 2 months into this living arrangement. But after the first 10 days we were handed a bill for $11,800 . That's $35,400 a month ! after they agreed to $4870 a month . It brought my wife to tears after all the other stressful things she has been going through trying to help her mother with doctor appointments , living with us in our home . medications , therapists every other day , preparing meals , laundry . bathing her mother and other hygiene and more . In the first 10 days Mission Creek did not bathe [name removed] nor do any of her laundry after it was discussed that both things would be done once a week .

Although very friendly staff, I felt this facility was more geared to nursing home and full time care. This is not what we are looking for at this time.

I was oversold on the Woods. Live and learn, 48mom seems content. I guess that is all that matters.

Lakeshore Manor was DETRIMENTAL to our Father's health and well-being. This was his first move to assisted living and we were encouraged with the staff up-front. However, once Dad was living there his CARE NEEDS WERE NOT MET. He was not kept clean, his place was soiled and unsanitary, he was missed for meals, his laundry smelled of urine, there was little communication from nursing director to family. Dad's VA CANCER DRUG AND OTHER MEDS WERE MISSED ON SEVERAL OCCASIONS prompting him to become ill and his PSA to rise sharply. Staff that is on hand tries but there are TO FEW STAFF TO CARE for all of the residents. Activities were a positive but who can enjoy themselves when they are unclean and not cared for. Even with a new director making promises in [date removed] OUR ISSUES WERE NOT ADDRESSED. As recently as [date removed] the State Board of Aging followed up on our concerns with a surprise inspection and SITED LAKESHORE WITH CODE VIOLATIONS. Lakeshore seems more concerned with outward appearances versus patient care. We were fortunate to QUICKLY MOVE DAD OUT to an assisted living home that's mission is care.

I toured here while looking for a new place for my loved one, and overall, it was just ok, but that's it. The staff wasn't particularly warm, or cold to us, the place wasn't more bright, spacious, or grand than any other place we saw, and overall, I just was not impressed with it.
