Disappointed

Medications were not given on time. They do not have enough staff there to care for the residents they have. The social worker was great. They said they would take residents to appointments and they would cancel the day of, so you had to wait months for a new appointment. The food was not good, they would serve corn dogs and soda for breakfast.

Do not be fooled

Do not move here. It is really hard to find good health care. They do not have a quality staff. They are not friendly or helpful. The staff is lazy. No one is accountable for anything, they blame each other when things happen. The food decent. The administration does not get back to you. The social worker is wonderful. The front desk staff is excellent. The maintenance staff is good. The rooms are not kept clean.

A Big Learning Experience

Lacking in leadership. The staff is always shorthanded and help is constantly calling off. They aids aren’t trained in memory care. Recently on a Saturday there was one aid and no nurse in the cottage, for sixteen residents of which three were high risk for falling. Consistently behind on laundry. They left my parent without a shower for two and a half weeks. Too much to continue with. We pulled out after a month.

Buyer beware. Not as advertised. Food is terrible, repetitive, and lacking in fresh fruits or vegetables. Activities are very limited, despite the fact that Beaver Dam has a lot of cultural events going on. Management is not responsive to concerns or suggestions. Seems to be run by people, who have little interest in or knowledge of the elderly. Nursing assistants are kind and helpful, but there is an extreme level of turnover, due to poor management.

Changes Again And Not For The Better

We put my mom at Rock Run back when it was under Brookdale. [Name removed] was wonderful and instrumental in helping us with this transition. It was hard for my sisters and myself as the guilt was overwhelming, but [Name removed] helped ever step of the way. I only get to visit once every few months due to living in another state , but I came Labor Day weekend and was floored at the staff and yet again more changes. There was a new nursing director I did not get the chance to meet. There were only two CNAs I recognized and the entire staff was different. My mom was tired after lunch so her CNA took her back to her room after we waited for assistance after 40 minutes. Finally I took mom myself. In that time she had an accident which ruined her demeanor and still no help, finally one of the regular CNAs came to assist. She apologized for taking so long but that there were only 2 CNAs working. I was shocked, it’s a weekend people are visiting families and only 2 CNA members on a afternoon shift? She said staff just come late or not at all. I was a bit upset that there were no activities going on. It was a holiday weekend and a dozen or so of the residents were just sitting on the front patio. Most of them smoking that mixed with the heat did not make for a great visit. The CNA came out to sit with us, she proceeded to talk about how many hours a week she puts in, she’s tired and she expressed her disenchantment with the job. She went on and on with information I personally don’t think should have been shared with family members but it was clear she needed to vent. With that I added that my mom’s room and bathroom were pure filthy. My sister usually has to come in and clean it . She commented that the CNA don’t clean rooms or bathrooms but that the cleaning staff do it and they are all gone by 2-230 during the week and maybe one there on the weekend . Needless to say I was very unhappy with my visit. My sisters had reported similar incidents so obviously lack of staff is hindering the care. I told the CNA I would be back to speak with the nursing director and the Executive Director regarding my observations and complaints. She proceeded to tell me the ED I asked for and am use to and had good conversations on issues in the past that she tended to immediately and rectified was gone no longer there. I was floored she was the one we knew all that time. It was a small detail my sisters forgot to share. Needless to say we are looking at new options. Instead of having your staff tell family members they are short staffed or there are only 2 on the shift, maybe you should hire more staff. The company is sure not shy about charging for these services obviously we are not receiving, With all the changes, not positive in our eyes and a price increase again on the horizon this place is no longer worth it. We toured a few places and sad to say they had nothing good to say about this community. Time to move on. It’s unfortunate the one it will effect will be our mom,

One Star only because of great Reception staff

I would and will never refer anyone to this place. My mother was there for one month - she had severe depression and anxiety issues and I asked while signing the paperwork/lease agreement if they had a doctor that could help with those issues and was told yes, and that they could have someone work with her (psychologist/counselor) to help her with those issues. They never did. The room was beautiful, the facility is very nice, but there is no 'hominess' to it at all. The care staff was exasperated with my mom because she wouldn't cooperate at times and instead of trying to address the issue (anxiety and anxiousness mixed with short-term memory issues), they would call me at all hours to tell me she was refusing meds, was yelling at them, etc. There was a time I was visiting and as I was leaving, I ran into one of the aides who gives out the meds. I told her I was sorry she (my mom) was so difficult and apologized. She said nothing, turned and walked away. All the staff did was complain about her, got the director involved. I would try calling the doctor and wouldn't get calls returned. And any time I did get a hold of the doctor, she was in the director's office with the legal person in the room as well. The person who initially gave me the tour never went through the proper channels to determine the level of care she needed, if that had been done correctly, perhaps they would have told me that they wouldn't accommodate her. Instead she endured extreme anxiety, and all she did when I visited was cry. Instead of figuring out how to help her anxiety, they sent me a letter telling me that I had 30 days to move her out, that they didn't have enough staff to provide for her needs. The only people that I encountered that were helpful and nice were the gals at the front desk! I was thoroughly disgusted with the way my mother was handled there - my feeling was that she wasn't a "perfect" resident, so they didn't want to deal with her because she was difficult, but that was all due to her anxiety and depression. I took my mother to her doctor when we left there and her doctor had a long discussion with her and determined a regimen of anti-anxiety/depression meds, a pain medication for her extremely painful shoulder, and she is a new person. We moved her to a wonderful facility with caring staff that treat her and the other residents like family (I observe this every time I am there and it isn't a show - they are like that all the time) - they hug them, joke with them, the residents are all treated like family. They are just wonderful and my mother is happy, she smiles, laughs, participates in all of the activities and has even made friends with a bunch of the ladies there. Although Mission Creek is a beautiful facility, and I'm sure some of the staff is great, our experience was far from great. If your loved one doesn't have any issues, they'll be fine there. I also know that, when my mother was there, all of the residents got letters from the parent company stating that they would not be able to stay there once their money ran out and they go on Medicaid. I was told they only have "one or two beds" for Medicaid residents, so 98% of those who are self-funding, will have to leave and go to a Medicaid facility that doesn't require 2 years of self-funding. Know what those facilities are like? Know what kind of care they get in some of those places? How rotten. Would NEVER recommend this place to anyone!!

Horrible experience

We had first-hand experience with our loved one at Lakeshore manor who was a resident for approximately 3 months. We had the most horrible experience and ended up moving to a different facility ([Removed]) which we absolutely LOVE!! Lakeshore manor consistently missed doctor ordered medication.... never notifying family or the doctor according to state regulations, we found our loved one soaked in urine from waist to knees on numerous occasions, there were 3 occasions (that we know of... could be more) where he was totally forgotten for meals, the facility was greatly understaffed with some staff working consistent overtime creating unsafe/ineffective patient care, clothing was consistently misplaced and found in other residents rooms, the meals are NOT chef prepared as stated in the brochure, the whirlpool bath was never used EVER even tho we requested it... one employee stated the “girls don’t like to use it because the have to sanitize it after each use”, the list goes on and on and on and on! The nurse in charge is new (according to another family there the turnover has been quite high!) and she lacks the motivation, knowledge, and skills to properly train her staff and follow thru with doctor orders and resident needs. This is a memory care facility and we had to keep reminding the staff that our loved one could not remember how to brush his teeth, do personal cares, come down for meals, etc. If we did not do those ourselves on a daily basis they were often missed! However...they were charted as being done but we emphatically knew for certain they were not being done. Even some of the staff said “some workers will chart it as being completed even tho they didn’t do it out of laziness “. The ONLY positive was Kristi and Vickie(activities staff).... they kept the residents active. After talking to many people in the community I found that MANY other people had the same experience we did and also pulled their loved one out and took them elsewhere. PLEASE PLEASE reconsider before going to Lakeshore! The only hope is for CHANGE! Please not that there are positive reviews on Facebook however some of those are written by staff..., not families.

The first thing I did when we placed my wife in Mission Creek was to give them a 3-page document I had typed up explaining how to handle her in various situations. One of the things I highlited to the management staff was that she sometimes will use the toilet and then a couple of minutes later feel she needs to use it again. I also alerted them to the fact that she was an extreme fall risk. Despite this, on her very first night there a caregiver took her to the bathroom and then returned her to her bed and left. A couple of minutes later she had returned to the bathroom by herself, fell backward into the porcelain tank on the toilet, shattering it into four large pieces, and ended up on the floor with a large gash down her back requiring an emergency room visit. In an attempt to prevent further falls, the next day my daughter and I met with managment and we all agreed that they would move her bed against the wall and remove a night-stand that was next to it. They were reluctant to put mats next to her bed or use a bed alarm. A few days later, she rolled out of bed in her sleep and landed hard on her right side, hitting her head on the night stand in the process. Neither the bed nor the night stand had been moved as they had agreed to do. Ultimately, this resulted in a five day stay in the hospital for her to recover from the bruising. A few days later, knowing her fall risk, a care staff placed her on a chair and she fell and hit her head on the floor. I immediately removed her from the facility, as I did not trust them to provide appropriate care to assure her safety.

Beware

Beware of all the costs.... What you sign up for becomes hundreds of dollars more... $798 to check blood sugar once a day.... you have to use omnicare for prescriptions that cost 3x's or more the cost even with drug ins. Don't pay for full care because they don't provide it.... my Dad's pendant didn't work.... even after we expressed concerns that no one was helping him to the bathroom... he sat for hours in wet clothes and all they did was put a pad on his chair... At first we were happy for a place we thought was going to relieve some of the stress of caring for our elderly parents... highly disappointed

Not enough help

Upper management is offering a 100$ rent discount for every 5 star review! There are only 4 to 5 staff on duty for 40 memory impaired patients. The food is not the best, whatever is easy ie: hot dogs, ham and cheese sandwiches. Staff turnover is weekly! I would remove my father if I had somewhere else to take him in town!

Horrible place to live

First I want to start off by saying this community was fine a few months ago. Now the moral is disgusting. Im thinking it has to do with the new executive director. I understand that all communities have their challenges but this is getting out of control. To this new guy is horrible he accuses people of owing money when they actually have a credit. He just don't care it's all about the money for him. How do you care about the money when you have no staff in the building. Multiple residents in the front yelling for help. Soaking wet mattresses and clothes. But yet you want my five grand a month. I suggest you put your loved one else where. It won't change until his arrogant money hungry ass is gone.

No Brookdale

Residents weren’t changed on a daily basis, many weren’t treated well, or thrown in bed when cnas wouldn’t want to help. I was Very disappointed.

Horrible Place

This place was in the basement of the assisted living home. It felt like a dungeon and it was dark. The apartments were not clean I'm not sure if someone had just moved out or what. There was nobody in the hallways or around at all. They had no decor it looked like an old hospital hallway.

Disappointing so far!

My loved one moved here recently from another Brookdale location and we regret that move so far and are very disappointed. I don't like the staff and he doesn't either. There is one evening nurse who does a good job. I feel like they are not following through with what they promised me. I was told he would be participating in activities but every time I visit he is sitting in a chair sleeping! We are very disappointed.

If you are looking for a rehab facility for you or a family member, it is strongly recommended that you hold out for openings at other facilities in Rochester, MN or surrounding areas. While the majority of the staff working direct care with the residents was kind and courteous, it only takes a handful of unskilled, unprofessional and disrespectful staff to create a worrisome environment. Management is neither organized nor competent to lead a team to deliver care to the elderly, most of whom are unable to self-advocate. I strongly recommend investigating substantiated reports against this facility.

This place is not a place to put anybody in there nice to you in front of your face behind your back they treat the residence like garbage not to mention there's different staff every few months coming down people just walking out on resident not caring only time they do care is when they know if family members coming by then the really sweet
