Bonaventure

Poor, poor quality of care at this facility. Critical medication was missed, showers not given, laundry not done, personal attention not given, clothing was lost. After I asked twice they finally showered but did not give the resident the help they needed. I observed the staff loitering around and socializing more than they were helping residents. Management did not appear to be overseeing this behavior. I went up a level and spoke with human relations at the corporate office. All they did was give me a credit on the monthly bill. Management problems were not rectified. I'm concerned for the residents at this facility since no one seems to be doing their job. They don't seem to care, just waiting for another paycheck. Shame on the corporation for making profits at the expense of the residents and their families. I moved my parents out after just two months. I tried to help them do a better job but they proved to me they are not interested. BEWARE

What they could improve on would be food quality, inventory, caregivers response times, medication times, dispensing, handling. Cleanliness of rooms and facility

We moved my Mother in the middle of July and moved her out the middle of September. She had fallen and broken her leg and her memory/dementia was such she wasn't able to go home with our full time care. We paid over $[fee removed]/ mo and they didn't fulfill their portion of the contract. The care here was horrible and we have had issues with them from day one: -Not responding to the pull cord for help. I myself have pulled the cord twice and got no response WHATSOEVER. - Not locking the door to my mom's apartment... The other residents, 2 in particular, regularly just walk in and take things or in one case a gentleman left his clothes in her bathroom. -Not bathing my mother -Not cleaning her apartment -Not doing laundry regularly as stated, only when asked -Being so short staffed that they did not get my mother for lunch or bring her a tray of food. -Informing us of her prescription needing to be refilled WHEN SHE WAS OUT on a Friday afternoon! Do not leave your loved ones here!

It started out okay in independent living. But we had to move them to assisted and that's when the nightmare began. My mom had a bad fall and the Care plans that we had made with Bonaventure were not followed through on. They did not deliver meds on time. They did not check on them and we ended up hiring an independent caregiver to come in which we should not have to have done. My dad spent the day in memory care had falls that were not recorded and reported to me and lots of other items. They are very short staffed and cannot take care of their residents.

I would give it a 0!!! If you want your loved one to get bed sores, poor care, and pay lots of money for this, then you should come here!!

They do not have adequate staff to care for their residents.

Not a place for your loved ones. Poor quality of care. Staff not trained properly. High turnover, management only care about your money. Meals are made on a low budget level, mostly rice and pasta based.

The in floor care staff is very nice but I would not recommend this facility to anyone. I have my mom on several waiting lists trying to get her out of there.

The administration was really horrific to work with. They were very passive aggressive with my Grandmother. They could work on behaving more professionally. My Grandmother never actually received any paper invoices via the mail... though, she tried to resolve this many times. I would assume this is some sort of book keeping error/process error? In person they would tell her how much she owed each month and she would then write a check to them onsite. Food was a big problem... my Grandfather was on a dysphagia pureed diet and they were just feeding him whatever they fed everyone else just cut up. Often the food was quite hard/difficult to chew, also was cold and was a definite aspiration risk. They were making no effort to accommodate his diet and yet charging my Grandmother extra for his diet... yet, not actually feeding him according to his orders. At one point he needed assistance with meals and they did not have the staff to accommodate this. They told us we would need to hire an outside agency for this. The outside agencies we could find wanted 4 hour minimum shifts and were booked a month in advance.. so it just wasn't practical for many reasons. Bonaventure advertised as being a memory care unit... but, then didn't have the staff to assist him with his needs. Needing assistance with meals is a common need for patients with Dementia. My Grandfather lost about 20lbs in the care of Bonaventure... he would have wasted away if we hadn't pulled him out and engaged in home care. Cleanliness: my Grandmother witnessed multiple weeks when his trash was not taken out for two weeks at a time. As well as the floor being sticky in his room... when your shoes stick to the ground it's generally a sign that the floor isn't clean. I personally witnessed my grandfather soiling himself in the restroom and the staff couldn't find gloves to protect themselves. The staff member that cleaned up my Grandfathers diarrhea that was on his clothes and the floor... did so with one glove. As a nurse, this was traumatic to witness to say the least. In their contract it stated that the residents were to receive 2 baths/showers per week. We got a call from a home health nurse that visited him who had high concern that he wasn't being bathed properly as he smelled and looked quite unkept. Lastly, I just need to mention. After my Grandfather moved out... the administration of the Salmon creek Bonaventure passive aggressively threatened my Grandma that if she left even one belonging in his room they would continue to charge her for the entire month under the assumption that he was coming back... despite receiving written notification of his move out 30days in advance. I have never seen my Grandmother so stressed in my life... she was often crying and deeply concerned about leaving the shower curtain in the room... under the assumption it was provided by the facility. FYI, they also do not provide a single personal care item including toilet paper... even though in their contract it stated that they provided personal care toiletries. The staff that actually cared for my a grandfather where really lovely, friendly and worked hard. I personally heard one denying her lunch break because she had too much work to do. They clearly were doing the best they could... but, were stretched thin. The problem with Bonaventure is clearly administration.

It was not a good experience it was disappointing and misrepresented

They can't keep any good staff and most of them have no medical background. I felt misled about the cost and the monthly tenant meetings always had a list of complaints that no one working there seemed to care about. I feel like they are a sinking ship and they are desperately trying to keep people from leaving.

The reviews are good online but since those reviews they have a whole new incompetent management team. From day one we were misinformed, numerous problems every day that never got solved, it is a horrible place to go. Severely understaffed and untrained care givers. It took a letter to the CEO of Bonaventure to get things rolling and then we left that place as soon as we could! We could go on and on and on of the issues we've had there, I'm telling you do not let anyone stay there. It is a bait and switch con job to get you there, sure it looks beautiful but looks were very deceiving. Can you imagine having your loved one stay there for 5 days with no phone, a broken heater that didn't get fixed for 8 days, the most unskilled caregivers and on and on and on.

It's hard to put in a single comment. Just to remember that the place is in business to make money and that is their most important goal is to make as much money as possible off of you. They had absolutely no problem TAKING [price removed] out of my dads bank account for his 20 day stay. They took [removed]k out 9 days after he died. Absolutely no consideration that my dad was taken to hospice and died within the week. While I was trying to grieve and plan a funeral I was getting checks returned to me because they were busy taking every penny he had left from his checking account. No human compassion, nothing! It's money money money It's disgusting! I've told one of your employees already that I planned on making sure that I take every step possible to insure the most amount of people will hear about my experience at Bonaventure.

I brought my mother here 7 months ago and from the very beginning it was a nightmare. no onboarding, no welcome, the room was not ready they didn't have her name in the computer so I had to hold on to her medicine for several days and administer it myself. Things progressively got worse: meds given by someone who didn't even know what they were, delayed medical attention by the ball being dropped for 2 days, never ever had her room cleaned weekly the food was horrible until about a month ago when I went to the regional manager. now they're being fed with the rest of the facility is being fed which should have been all along. Them doing her laundry is a joke. they ruined more than half of her clothes so I do it now and I can't get them to wash her sheets and towels even though it's included in the contract. I believe the rest of the facility is okay but Memory Care is broken and so far has not been fixed. I'm waiting for another facility to open up and will be moving my mother soon.

I moved out the place was not clean. The staff was always changing. Meals were not on time . One. Night I had no dinner. They said they lost the ticket. One hour after that they said they were out of food.

All so friendly and nice then I left [name removed]in Memory Care. Didn’t visit for a week as their suggestion was a couple of week for adjustment. Went after a week He Wa still on the same clothes, sick and shoes never removed, sat in chair to sleep not bed, no teeth brushed. He’d fallen and not called me. He was so traumatized as was I. Ankles swollen over top of shoes!!! I had paid extra for special care for him. It was awful. Took him out of there the 9th day. I reported them to all State agency’s and surprised your rating was 41/2 stars. State found no infractions after speaking to staff yea, residents Yea, and family members who never visit. AWFUL PLACE JUST LOOK NICE AND PUT ON A FRIENDLYFACE!

At first glance, the facility is beautiful and new. The sales team was friendly and accommodating. But that all changed once we moved into the facility. They immediately raised the price [fee removed] in the first month. They were grossly understaffed. We wound up doing basic things like laundry and linens as they were not being tended to. Packages were not being received or being stolen. It was all around a bad situation and I regret making the choice to move my mother to the facility

All ones! Presentation was great but help was not there. He fell in the night hit his head on the dresser and laid there calling for help for an hour. Sent to the hospital alone and no follow up. Called me in Mexico instead of daughter [name removed] who was the emergency contact. He came home with cellulitis on both legs which is life

There needs to be more bus van trips. The food was salty and unhealthy. I had to order a salad with my dinner and I think that a small green salad should be included with dinner. Less ice cream and more fresh fruit. Being a type 2 diabetic l didn't have too many choices.

This place was highly recommended to us. Going on tour of the place was fabulous and so many things promised. We put our dad there and he was able to walk, talk and pretty much care for himself. After 2 months, he was not able to walk, could barely speak, and had lost a lot of weight. He had bed sores, a very bad u.t.i., couldnt walk anymore and was only getting help to eat one meal a day. His roommate was stealing his personal belongings, and even got into my dads bed several times. I would not recommend this place to anyone. They tell you whatever you want to hear, but they dont follow through.
