Understaffed, bad housekeeping, missing possessions, undertrained, mistakes in medication management, meals/food presentation terrible and usually not on schedule, facility looks nice but care for the residents lacked. Administrators at the facility avoided conversation regarding complaints . We had to remove my mom from this facility because we were concerned about her safety. We made a report to the State of Oregon licensing department in [date removed] . Mom was in the memory care wing at this facility. I would recommend that people should not consider this facility due to lack of care and respect for the residents from Administrators to Care Givers.

I would never send anyone there again. It is beautiful, but they do not help and do not do what they say they will do. Staff changed like weekly. No activities in the locked wandering side. They cleaned and did laundry when we complained. Meals were not bad, but service was not nice. It was expensive.

DO NOT USE THIS AMATURE/UNTRAINED FACILITY! My mother-in-law was in memory care. She was not cared for. During an altercation with my mother-in-law, a staff member actually cut her skin leaving a 1" gash (we are talking torn skin). This alone demonstrates staff are untrained. My mother-in-law's hair look wet with grease, because she had not been bathed for many weeks. Her room was filthy, trash wasn't emptied, it took over a week for the facility to unclog a sink drain. Feces was on carpets, floor and shower. Urine stains were left on the floor and cleaned up easily. Staff spends most of their time in a central area and frequently yell at residents to go back to their rooms. This is one of many review posting I will be submitting. TAKE YOUR MONEY ELSE WHERE!

Bonaventure in Keizer was quite a disappointment! Memory care was not what they told me it was. Nurse was supposed to be on duty 5 days a week, didn't happen, hot dogs were on the menu constantly. Contract was quoted at one price, then they presented contract with different numbers. My husband did not get the care that Bonaventure told me he would get! I would not recommend them to anyone!

Upper management made promises that were never followed through with. High employee turnover, terribly understaffed. Meals and meal service is horrific. Sub-standard at best. Housekeeping staff is overworked and not given sufficient time to actually clean. Communication with management was next to impossible. Had to call several times to ever get a call back. Loved one ended up with serious injury, which was not reported, or family notified. Loose meds were found in loved one's room, that were supposed to have been dispensed and witnessed by med tech. It is a beautiful facility, but the looks of the place don't outweigh the negative experience of having a loved one treated as just a rent check. Utterly disappointing.

My mom first lived in the independent side at Bonaventure Castle Rock. For that purpose it was OK. The apartment was nice, the location good, etc. The disfunction of the facility really hit us when she moved to assisted living. This was against the advice of her care team. Bonaventure has a pretty bad local reputation. But we felt it would be less disruptive for her, so we believed the promises of management and took a chance. They acknowledged the problems and their poor rep. The managers will say all the right things. You will want to believe them. Unfortunately, all of the people we worked with were gone within a few months. In fact, there were *three* complete changes in facility management in the first eight months she was in assisted living. There is no continuity, and no integrity. If you do decide to risk putting a loved one here, get everything in writing. Until they make fundamental changes to improve their culture, do everyone a favor and don't support this facility.

Our experience was horrible here. I wouldn't recommend you place your loved ones in the community. My loved one was here for a short amount of time. During the time my loved one was in the community, a family member was with our loved one. The family was having to request that the staff come and care for our loved one. We were told that the community would be able to provide full care, and this wasn't happening. There were times that we had to request the staff come in and sit with our loved one to care for our loved one. There would be meals left at our loved one's door; however, our loved one was unable to leave their bed and the staff knew this. The management team would always make promises but they never followed through on the promises. They over promised and under delivered. Hospice was doing more care and work with our loved than the staff in the community due to the ratios. Hospice was better at caring for our loved one as well. The biggest complaint I have is the fees from the move-in fee to start with, and weekly payments. It was higher than I would have imagined for the short amount of time.

It was NOT a good experience. I could not get my mother out of there fast enough! Misinformation; NO follow-through on anything; inexperienced or inept maintenance workers tried 3 times to repair leak under bathroom sink, when hooked up washer and dryer tightened hoses with his hands so first time washer used sprayed water all over; mother was there 3 months, never got name tag for her door; was told Wi-Fi provided but never worked and no adjustment given just said room too far from equipment - not our problem; new chef and food terrible, many complaints from residents; halls not vacuumed regularly. Provided feedback to management who apologized but did nothing nor did they return calls! And worst of all on 2 occasions my mother was taken to hospital and there was no notification to me.

It was not a good experience for us. In the beginning it was fine, but no one ever did anything that they said they were going to do. We got no orientation or greeting. Administration was never there or in contact. The aides were great who worked with my loved one daily! I think it might be a good place for those who are more independent and can take care of themselves. But as far as assisted living goes, I would not recommend them. My loved one just needed more care than they could provide.

From the moment our loved one moved in, it was a terrible experience and we felt that it was just a money grab. We cannot speak for anyone else but this was our experience. The staff were not able to handle our loved ones care, there was not enough staff around and the staff they did have were inexperienced and not adequately trained. They had slow response times for when our loved one would call out for assistant. It took them 2 weeks to give our loved one a sponge bath which we had to raise hell about. It is a beautiful community and maybe it would work out for someone who does not need a lot of care, but it was terrible for us.

I wouldn't recommend the community to others for their loved ones. The community is very friendly; however, they are short staffed and has a huge staff turnover issue. The food is terrible, and is coming out of a can! It is inedible. The family is providing food to our loved one, because the food offered at the community is our my loved one ill. The housekeeping was supposed to be done weekly, and my loved one was lucky to get 4 times out of the last 7 months. We aren't getting what we were promised and they are not fulfilling the services as promised. In addition, we don't feel we are getting the services that we are paying for monthly. We have been informed by staff that they are sent to different communities and some staff have to travel over 50 miles to get to a location to assist the residents. The apartment my loved one has is beautiful, but it is rarely clean.

I wouldn't recommend you place a loved one here or move here personally. The staff and community is inept in everyway, there isn't one thing I am satisfied with, not even a little. The staff communication with the family is horrible. There is no follow up phone calls, or they will miss appointments to the point the family will keep calling and until the family eventually gives up. My loved one overall doesn't care for the food. Only four meals have been enjoyable, others have been non-edible. There are no napkins or drinks and some meals only consist of a hot dog and canned corn. There has only been one key given to our loved one for their apartment. The staff have a master key; however, they don't know which master key operates which door for the individual apartments. If a resident were to use the pull cord for assistance the staff wouldn't be able to access the resident's room in a timely manner. The community has had months to try to fix this, and it still isn't resolved.

Absolutely horrendous. Don't be fooled by the beautiful building and nice location. Looks good from the outside, awful on the inside. I had to pull my L.O. out of this facility after living here less than a year. Where do I even begin? The first red flag should have been the filthy condition the room was in upon move in. Manager "[name removed]" said she would have housekeeping come clean the room for us prior to move in. It never happened and we had to clean the room and bathroom ourselves. It was absolutely filthy and obviously hadn't been cleaned in months. Restroom had feces all over, hair, and urine drippings. We were willing to overlook this in hopes things would get better. Unfortunately, they only got worse. This place is very understaffed which leads to an extremely poor level of care even though you're paying top dollar. There is a real lack of leadership and structure from the top which results in unhappy employees and a high turn over rate. I never saw the same employees for more t

I finally have a regular housekeeper and he does a great job, so I gave cleanliness a 5. As far as the facility cleanliness, I'd rate that at a 2. The staff really does try here, but the management is terrible. There is no communication within the staff. Communication with residents isn't good, either. For example, the other day I went to use the elevator and it was shut down. I use a walker and rely on the elevator, but it was off. I finally made it to the front desk to tell them. They said that they were aware, that it was shut down for yearly inspection that was happening sometime during that day. That was never communicated to the residents, we had no idea. They could have at least posted signs by the elevator doors to let us know. The food is horrible. It's not good quality, variety, or quantity. The menu seems to be made for a 14 year old boy. Full of junk and processed food. They serve pre-cooked, processed, reheated food. I don't eat it anymore, I make my own meals, But I still pay for it as 2 meals a day are part of the monthly cost, whether you eat them or not, it's not an optional cost. They try with the activities. We usually have one per month, maybe, and most of them are led by residents. We do have a new executive director, so we'll see. I feel so bad for the staff, they try so hard but there just isn't enough of them. You can only do so much with nothing. They're worked to death, get yelled at, and most end up just quitting and leaving. The monthly cost for what I'm actually getting here is outrageous. But I don't want to move. I make it work in Independent Living. My apartment itself is very nice and the pharmacy and gym are both within walking distance and I really like that. However, I wouldn't recommend this place to anyone. They talk a good talk, but don't deliver anything they promise. I'd say think very carefully and beyond the promises they make during the tour. Also, if you find yourself needing long term care you have to move to the Assisted Living side. That's one of the things I asked about prior to moving in and they weren't truthful with me about it at all.

VERY DISHONEST PEOPLE THERE. DON'T TRUST THEM OR LIVE HERE. ONE STAR IS TOO MUCH FOR THIS SKETCHY PLACE. HORRIBLE EXPERIENCE. CRAPPY SERVICE. AWFUL STAFF. ONE IN PARTICULAR. SAVE YOUR MONEY. VERY SKETCHY

Do your research! these places LOOK nice and are maybe fine for people who don't need much care. For those who do, they are grossly understaffed. Caregivers are good, but I don't expect them to be there much longer. The director is a [profanity removed] who's only interested in SELLING (which is probably the corporate directive). They need someone capable to run the place

The actual staff that works with residents is great. but as for sales and marketing very overpromised but very much undelivered once actually got into the Bonaventure at Keizer Station. The facility is very pretty on the outside but the inside is not near as great overall.

When I first moved to Oswego Place, I was very satisfied. The executive director was kind and caring. She could be tough at times, but I never thought she was unfair. Then when she left we got a executive director that is the housekeepers daughter. In my opinion she doesn't know how to be a leader. If she gets upset she attempts to lead by intimidation. She lied to me several times and when she got caught, she just got angry. She is also holding approximately [removed]. You can tell how well the place is doing by the number of empty apartments they have. When I left there was 16, that is quite a few for a building with 62 apartments. The place I am at now has "one." If you are considering moving into Oswego Place pay a unannounced visit and talk to the residents, they will tell you the truth. There is one housekeeper for all the apartments, my current place has three.

If you love your loved one, do not put them in this facility. It's a beautiful facility from the outside. However, once your loved one is at the facility, promises are not kept. They provide poor care. Bonaventure is geared towards making money, not caring for your loved one.

We reached out to Bonaventure for some information for our aging parents. We let them know that we were not ready yet and had only just began looking. We gave them one phone number. After visiting, we were continuously harassed including them calling us on phone numbers we did not give them and SHOWING UP AT OUR HOUSE unannounced when we did NOT give them our home address either. This company will invade your privacy if you contact them for information. Please be aware before you share ANY personal information with this facility, especially if your loved ones are unaware that you're looking for a long-term place for them. My husband had to call them and demand they remove us from their list in hopes it will stop the EXTREMELY invasive harassment. Fingers crossed!
