Overall experience of Memory Care: 3 Services to Memory Care (MC) residents are very basic — what the law requires and rarely more. If not for the tenderness of many of the on-floor caregivers, our overall experience would drop quickly to 2, maybe lower. As a result of minimal services, the MC unit is tragically understaffed. The staff there when Dad was a resident was for the most part, tender, dedicated, and completely overwhelmed. To give time, attention, or care to one resident is to take away from another. The claim that there are always two staff members on the MC floor is untrue based on our experience. Residents are especially underserved on weekends. Our loved ones are so very vulnerable when we rely on others to care for them, and overall care, meaning safety, nutrition, human dignity, mental stimulation, and having the best day possible given your circumstances was low in our opinion. Overall experience of Independent Living: 1 The Independent Living facility looks good, but no. We would not recommend it. Here, as in MC, the general attitude is "good enough". They appear to have trouble keeping staff; food, which is the reason we chose this place, is unimaginative; staff sometimes doesn't even show up — which results in buffet-style service. Resident feedback meetings are held, but response is very unsatisfying and residents are routinely reminded that "this is INDEPENDENT living" and they must look out for themselves (even in case of fire). When there's a problem, the response is frequently an excuse, no apology, no plan to fix things. Cleanliness: 3 It's okay. Like other categories, "good enough" rules. My mother's apartment, which should be cleaned weekly, has regularly not been cleaned as promised. In MC, it's difficult to keep up with the various causes of pervasive urine smell. The staff tries to do so but isn't always successful. Friendliness: 4+ Friendliness is one of the strengths at Legacy at Fairview/Legacy Reserve. Most staff members know the residents by name and also sometimes the family members. We were very impressed by this. Memory Care and Assisted Living front desk personnel (concierges) are flat-out amazing; we were impressed by this factor, and I think now, comforted and encouraged by it. Quality of Care in Independent Living: 2 The administration doesn't want to be responsible for any kind of "care", and the services promised in the agreements and handbooks (food, cleaning, etc), have been declining steadily and markedly in the last four months. Quality of Care in Memory Care: Depends on the Shift, overall 2 Individual staff members are amazing; however, their abilities to assist and really provide care is profoundly limited. They do a fairly good job of redirecting residents in their care, but this is not a place where residents thrive. Our loved one did not thrive there. By experience and observation, this MC unit is more of a place to "put" your loved one; there is no special intervention for MC residents that is uniquely designed for their needs. There are some activities, and MC residents can also "share" in the Assisted Living activities, but only if the resident is sufficiently independent so that there's no particular need for supervision, or if he/she is accompanied by a family member. Otherwise, these programs cannot be attended. Again, it's a matter of minimums — what is the least that can be done? Insufficient staffing is a factor in quality of care because of simple watchfulness: Dad fell often, sometimes within , ,sight of a caregiver who simply could not get to him in time to assist. By "insufficient" we mean for the need at hand, not necessarily in comparison with staffing minimums established under law. We were profoundly grateful to have a Ring camera set up in Dad's room. On at least two occasions, he had slipped to the floor and went for many hours unnoticed and unchecked-on. Administrative Staff: 1 And by this, I mean management staff. Corporate goals appear primary. I witnessed, more than once, the regional/area manager park a vehicle blocking the access of other vehicles to the front door of IL and hold up his finger for other cars to continue waiting while he entered and spent more than a few minutes in the building (he did not appear handicapped). When my father was dying, our family was able to reserve the private dining room so that many of us could attend to Dad without overcrowding his room. The regional/area manager came in and congratulated us on our new employment there (mistook us for new employees). On-site administration is superficial. Food in Memory Care: 1 Awful. Unimaginative, carb-driven, low in protein, vitamins, and minerals. Dad liked the meatloaf, though. This is not what it seems when you see the printed menu or the administration describes how seriously they take nutrition. Between design and delivery there's a great gap; what is delivered is troubling. We visited Dad almost daily and never witnessed hot food on the table — always lukewarm at best. My dad was a great food lover, but not there. There was no back-up: protein supplements, etc. Staff did not have time to notice eating patterns and make adjustments or make sure that daily requirements were met. Food in Independent Living: 1 As previously mentioned, food was a primary factor in choosing Legacy at Fairview/Reserve, but it has deteriorated quickly and greatly. Staff turnover and call-ins impacts the food quality and service. Normal service is now frequently by buffet. There are fewer offerings at breakfast and food (even whole fruit) isn't replenished. When a resident asked today for a second yogurt, she was told, "I'm only allowed to give one." My impression is that cost-cutting measures are seriously underway. Activities in IL: 3 My mother finds these uninteresting and especially the exercise opportunities sadly lacking. Again, there's an appearance that cost- and attention-cutting measures are underway. Also, shade outside the buildings is very limited. Think about that in a climate like this. Activities in MC: 1 These are uninspired and not particularly directed to the needs of persons with memory issues. Things are scheduled and the staff really tries, but sometimes there's no execution of activities. Activities are time-consumers rather than designed to meet mental or emotional needs. There is no music therapy, apart from performances in Assisted Living; but again, getting residents over into the AL side of the building poses supervision challenges. There really were no signs of activities offered to address needs particular to MC residents. There are calendars and day-sheets, but the offerings are designed more for passing time than engaging residents. Overall Value for the Money: 1 The company presents itself well, and many visitors will say, "this is one of the good ones!" We desperately wanted to believe that Legacy would do what it said and be what it seemed, and so we chose this company. Because of the tremendous investment and the tremendous hassle of making changes, as well as financial commitments, changing is difficult. Note: there are no discounts, no considerations for circumstances, no adjustments. The "corporate" colors show clearly in this arena. My father died here. The experience of his passing was peaceful and beautiful — because of hospice care and the individual personalities of caregivers who went out of their way for him and for us as a family. But his day-to-day enjoyment of this place was

Marketing people were very nice and helpful as was the director. Once you actually get into the place, there could be more cerebral activities other than bingo and Christian related activities. Many people aren't Christians. I think the colors inside on walls are dull there and don't invoke cheerfulness when you walk into the place. I thought maintenance was very good though- and their attentiveness to issues and maintenance needs with the individual resident was very good. There needs to be more orientation taken with the new individual and many times they're just given brochures and packets of info. Well, when you're disabled & have some memory problems that's not the best way to orient a person - other than that, I think it was a better than average place, but could be even better. Also, the landscape outside desperately needs attention and does not match the nice interior at all. You see that when you drive up in the place and that's very important I think - what you see First definitely makes an impression -it just has never had the attention that it needs.

Cleanliness of the facility 5 for common areas. 4 for rooms. Friendliness depends! Front desk 5. I think the facility is very nice and the activities they do and the energy on the first floor is fantastic. I think for patients that need a little more care it's very hit and miss depending on who the aide is that day.

Food, and facility itself is nice, but they're not very good with communication. Emails and calls go unanswered. It's just ok, not great...not terrible

They seemed more concerned about the camera in his room than actually helping him. They frequently told me they had to be rough with him because he wouldn't listen. They were worried I saw it on the camera.

3 separate areas within one facility...unassisted/retirement, assisted, and memory care. There is a calendar of events for each area for those residents that want to participate. Rent is raised around $[Amount Removed] each year. Turn over rate for care staff has been high in the last year. Care staff in assisted areas need better training and need to be monitored more by administration. Urine smell and bathing is concerning in this area. Breakfast, lunch, and dinner are provided within the cost of rent. Food is mediocre here. Walmart and Publix outings once a week for residents who can go. Transportation is also available to those needing to get to medical appointments.

From my observations, the assisted living residents have a completely different experience from the memory care residents. It's obvious why that may be the case. However, I would ask very specific questions about the quality of food for memory care patients. I know that families will not get a truthful response but show up for lunch and dinner when it's not part of the tour. I will add that I would suggest looking at facilities that allow cameras in the room.

The people that were and they were great. The situation we had was financial mistakes that costed us quite a bit of money and I had to get a lawyer

At the price point and accepting Medicaid, it was by far the most clean and least depressing place. Everywhere else was extremely expensive for someone that qualifies for Medicaid or put two to four people into a very tiny room. Lots of places were dirty and dark. This place seems to have more personality and happier residents and stuff overall at least compared to the others in the same range. I would recommend it only if someone had a similar budget to my dad, otherwise I'm not sure it's ideal but none of these really are

Tribute at One Loudoun- it is a nice place, clean, friendly sales people. The food is decent. There were a few challenges with nursing staff and making sure they could accommodate my dad with his medical needs, but once we got that sorted out, things went well.

They need to put a better transition team in place. My mom's adjustment was very difficult. I had no idea what to expect and it would have been helpful for a bit more assistance getting her settled. Also don't move in around the holidays or staffing transitions. There weren't enough people on staff for a time.

The facility is pretty. The dining area was nice. My Dads room was smaller then I expected. The people working there were not very responsive to issues. Lost glasses, transportation issues, wound care, hard to get in touch with people. I probably could have alleviated some of these issues if I lived in the area. It was hard being 1000 miles away.

It was a nice, clean place, could have used some general sprucing up and brightening of corridors and various rooms. PT/OT was almost nonexistent. They did have a lot of nice activities. Difficult to judge room since it was completely empty though the rep said it could be brought in if needed. The rep seemed more interested in what I thought about other sites vs trying to tout what Goldton has to offer. Many of the same activities were available although other sites had much more to offer as far as PT/OT and medical services. The facility was located in a nice area, had nice grounds, but is purely Assisted Living and didnt offer higher level care.

The facility itself is beautiful and the administrative staff in Memory Care are very supportive and caring towards the residents they care for. However, the Director [name removed] is not suitable for the job in my opinion!

When we got here [time frame removed] the menu was better than it is now and the dining room service was much more prompt. We have not even been to the dining room in two months, because we are much happier preparing our own meals in our fully equipped kitchen.

We are new to memory care and I rate it as fair because there is not enough activity and the staff does not engage with the residents as they sit idle all day.

Members of the staff did not speak English to the patients. It was very confusing to them. I would not recommend this facility for those looking for memory care.

[name removed] is a super nice man and he will bend over backwards to help, but some of the people that work for him are something else. The majority of the people there are super nice that take care of my wife. The building is always clean and everyone is nice to me. If not for the problem with the meds, I would give it a 5.

They need more staff and the building/rooms are outdated.

They present at the tour much better than they perform in reality. I would never again put a loved one into a facility that doesn't have fall cameras and also doesn't provide residents with pendants to call for help.
