I would rate them worse than zero. The patients who were in memory care seemed to be wondering around lost. It was lunch time and total chaos. And the memory care suites were in the very back. There is no caretaker anywhere close to them. And the "sales person" conveniently failed to mention the money was non-refundable. It was so chaotic she suggested we write the check and take the contract home and read it. Who does that? I would tell any family to stay away from those thieves! They stole our money. My Mom never spent one minute in that whacko place. We paid them [fee removed]. Then immediately decided not to put Mama there. We let Loren know within hours that we were not going to put MAMA there. It was too chaotic and we were told we would not get our money back. I wouldn't recommend that bunch of thieves to anyone. We were promised a [fee removed] refund of the [fee removed] Of course we have never gotten a dime back. Disgusting unorganized bunch of liars.

What the community could really improve on would be all of the communication. [name removed] was the absolute worst nurse I've ever seen in my life. Unavailable doesn't return her calls in a variety of other problems. There was multiple issues at that facility enough so that I would never recommend them. Everyone was sweet and nice but execution was absolutely a zero.

Heartis is a beautiful facility but the management is terrible. We paid a large deposit for my mother-in-law to move into their assisted living, only to find out almost two weeks later that they would not take her due to a recent shortage in caregiver staff in assisted living and concerns that they could not take on another high level of care patient. We were annoyed that it took so long for a decision and that the communication was terrible. The lady I spoke with had only worked there a week. In the end I am glad we did not move my mother-in-law there due to the incompetence that we have experienced. Now we are waiting for them to return her medical paperwork and check and I am being ignored.

My mother was not taken care of I. This memory care facility. I would not put my dog in their memory care unit. They need to have more staff to take care of their patients and feed them the proper food instead of the crap that the patients would eat or couldn't eat. Also do things with them besides sitting at the dining table all day long or in the bed.

This facility is simply a sales pitch. The tour was peaches and roses; everything fell apart after mom moved in. First, we waited 10 days for keys to the apartment. When Mom needed more care than their initial assessment(10 days after their assessment) and they couldn't handle it. The executive director told me that if mom didn't improve within 2 weeks that she'd have to move out. Even when I decided to put mom in hospice they doubted my decision....I never felt supported by the leadership....only challenged. I highly suggest you continue your search elsewhere.

My loved one had sitters 24/7 because of her fall risk and she had the money. Administration was against having sitters but I refused to place my loved one there without them. If not for the sitters, and what I saw myself, I would not be have seen med-techs placing pill cups on the dinner table between residents and leaving them alone to take. Some staff were wonderful and caring. I was told to mention scabies was found on the resident by family and staff was not notified nor preventative measures taken. It was a common problem. Residents sat in front of television without sound until bedtime. Activities were posted and not done. When an activity director was hired for memory care. She was pulled to Assisted Living with little time left for MC. Food was unappealing and too little. Staff did not watch residents eat nor notice if needed help. Cell Phones dominated their attention. Staff who were good, were very good. Too many were not trained properly nor supervised. The assisted living area seems to get the attention from appearances. I encourage family to insist on observing during supper time, as medications are given, and how residents are treated with respect and appropriate knowledge on how to react to dementia. How residents are left before bed and what activities actually take place should be observed before deciding on a facility or where your loved one will live.

My family member is a resident there and while they receive great care from some of the nursing, dining, housekeeping staff. The nurses [name removed][name removed]and [name removed]. [name removed]and [name removed]in dining, and [name removed]in housekeeping are always so helpful kind and knowledgeable. To bad my mother and I can’t say the same about the Director [name removed], she has always been so rude, and incompetent when we ask her questions, she never seems to know what she is talking about , she doesn’t answer any questions, respond to my emails or phone calls. Guess that’s the price you pay for care when the Director isn’t even a Licensed Nurse.

The Golden is a beautifully designed facility. With some people who are obviously professional and wanting to do the very best that they can given the situation as it is. However I would never recommend this facility to anyone and I have had both my mother and my father in that facility and I do believe it to be terrible at best. My mother is there and actually died in the facility which was to be expected. Yet while she was sick she could get no one to change her bandages and there were times when she had human feces left in her underwear for hours on end. It got to the point that she had burns on her skin because of this and I had to hire someone from outside the facility to make sure that she was checked on constantly.

It was my sister who was there. It was 90 to 100 degrees weather. She has no air conditioning. I couldn't hardly visit her because I couldn't stand the heat. The administration kept giving a different answer every day. She was only there 3 weeks before she died and part of time in the hospital. Then they kept billing me and I had to go to corporate after months, and come to find out they had her down as the fixed monthly payment for 3 years and we never signed up for it. They did finally remove it.

Monark Grove gets less than one star. It is a place to avoid for loved ones. My husband was treated terrible. I had to remove him very quickly. So many bad things went on there. The new mgt gave no support. They forced my husband on hbp meds - even though it was specifically against his health directives. If he didn't take they would 30 day him. One employee took a special odd and weird special interest in him. He got covid and I wasn't notified until he ended up in the ER and I had to go get him and take him back. My husband at that time couldn't speak nor hardly understand things. He was mild mannered and cooperative. More things happened. Beware.

The director of nursing refused to follow the doctors orders for administering medication, even after 3 requests were sent. She had previously lost her license for a period for the same reason at another facility. She also did not know how to properly care for the patients in memory care. My FIL was there for two weeks before being helped to bathe, even after we asked about it multiple times. Because of her poor skills and the facility's lack of care my FIL died after incorrect medication was administered. Terrible place!

Imagine you are a senior who is fully cognizant and independent, and you wake up every day inexplicably in an unfamiliar place where people you don't know do the following: say you need to take a shower now and inform you that they will be staying in the room while you do so; tell you that you're wrong when you bring up a concern you have; and tell you that you can't leave this place without a doctor's order. Would you respond calmly in these situations, or would they cause you confusion, worry, indignation, and agitation? That's just a taste of what my dad's experience was like at Madison Crossings. My dad has Alzheimers and now needs a memory care facility. He also has anosognosia, which means he is unaware of having any disability whatsoever, physical or cognitive. Anosognosia poses additional challenges to caring for a person with Alzheimers because the person doesn't realize that they no longer perform ADLs independently and therefore need prompting and supervision, yet such prompts and supervision must be delivered in a covert manner in order to preserve the person's dignity and avoid offending, upsetting, or angering them. One would assume that memory care facility directors and staff would utilize strategies such as therapeutic fibs and appropriate words and demeanor with dementia patients in their care, but that is not the case with Madison Crossings. In addition to not demonstrating such strategies, their (now former) memory care director exhibited extremely unprofessional and legally questionable behavior in the following instances: she was complicit in changing my father's medications without the knowledge or consent of our family; she argued with my dad about the accuracy of a complaint he came to her with (arguing with someone with dementia is pointless and only serves to agitate the person with dementia); during a conference call with our family about Dad, she was hostile and often interrupted and spoke over us when we were talking; and she outright lied about incidents involving our dad on many occasions, especially to create an inaccurate portrayal of him. I also had concerns about many (not all) other staff members, some of whom are on staff and some who are from an outside agency. During most of our visits, we rarely saw staff engaging with residents. On one of our visits with our dad, a female resident intruded and began telling Dad to get back to work, to which Dad told her to mind her own business and leave; the conversation got a bit heated, yet three staff members who were chatting together nearby didn't intervene to distract and remove the intrusive resident. It also didn't seem like many of the staff utilized strategies such as I mentioned above to de-escalate things when a resident becomes agitated or to notice patterns of behavior and intervene proactively to circumvent agitation. Another concern we had is that even though they publish an activity calendar, we saw very few activities during our visits and those were either BINGO or coloring pages. People with Alzheimer's need cognitive, social, and physical stimulation, yet during most of our visits we saw residents sitting in front of the TV or wandering the halls if they weren't in their rooms. Don't be swayed by the newness and hype of Madison Crossings. Our dad's two months there were a nightmare for him and all of us. I could write pages about the horrific ordeal we went through, but there is a character limit.

Cleanliness of rooms and hygiene of residence is definitely not a priority. We also had issues with staff not following the documented care instructions - simple things like use of a bedtime adult diaper. Monark seemed to be betting that our family wouldn't be visiting Mom much and wouldn't notice their lack of care. Staff literally told us they weren't given adequate training. Someone in housekeeping also told us their instructions about laundry service was not consistent with what prospective residents were promised. It was a hard decision to leave Monark, however, change is difficult for memory care patience. Six months later, we're very happy with Mom's current facility.
