Since my mothers move into this community everything is going decently well. There is room for improvement in some areas around the community. I do not feel they have been responsive to my concerns. My mother, and I come across the same problems a couple times a week. They do a good job with the cleanliness around the community, I have no concerns in that area. When we first moved in there was no activities director, they have one at the moment. However I still have not noticed many activities available. I have offered to do a bingo night myself. The meals they provide are well balanced, and they have a nice variety too choose from. Until the issues that I have stated are corrected, at this moment I can not recommend this community.

My loved one and I where not very impressed with this community, it was a little to depressing for my loved one. I had noticed during our tour that there was not much light throughout the building and the atmosphere did not have a comfortable living environment feel to it. My grandmother said she would not consider living here and would not recommend it.

We looked at it and liked it as far as aesthetics, but the atmosphere was cold, not from the staff, but from the residents. After the tour and seeing how the people were, we had lunch there and requested to have lunch with the president of the residents council. She pretty much confirmed what we saw that people are pretty crabby and not friendly. We had made an appointment for the tour, but when we got there, the woman was not even in town. The activities director gave the tour, part time guy. Finally, the manager came to apologize. After we left, they never called if me to find out if we made a decision. I never got a call from anyone, I thought that was very odd.

Salesman was not truthful. The care was mostly good. I was there daily & I feel they could tried to engage him some activities given that I informed them of some. People were given ice-cream an hour before dinner. Not smart.

To me they were a glorified babysitter and did not know how to handle sun downing patients. My mother had bruises and skin tears on both forearms. ( restraining?) EMS was called at least 4 times when she had sun downing episodes of agitation. I would not recommend this facility

I've moved my mother to a different facility. In my observation the weakest point of all assisted living are the care providers. Atrium weren't well supervised, under staffed over worked and not very motivated in their responsibilities. Once you need more care they encourage you to get private providers for that service. As for the facility it's old and in need of repair and refurbishing. They don't keep the rooms clean. Or not cleaned often enough even with notification to the issues. There were insect infestations in the room and leaks in the facility that they're finally investing in fixing because it was damaging resident rooms. You get what you pay for. The facility my mother is in now is much cleaner and modernized but we pay for it.

I didn't appreciate that after only a month that the Sales guy told me that there was gonna be a price increase in one month. It seems like something he should've mentioned while I was still signing paperwork a month earlier. And he said if I wanted to lock in the price I needed to pay another $[removed]to do so. Just seemed like I was bleeding money to him every time we talked. My mom only lasted 5 months in that place before she passed away.

My mom was medically mismanaged twice while there. I was asked to give them a second chance after the first incident and did so because they admitted they dropped the ball. The second time, they blamed it on their pharmacy. They required community feedback that they said was like a deposit. They refused to refund it after I moved my mom. I wouldn’t have moved her had they not mismanaged her medications so my dad was out the additional community fee of $[removed] with her being there only 2 1/2 Other issues: They are frequently short staffed and their staff members are constantly playing on their phones. They also were losing my mother’s clothing constantly. They withheld hand soap for a couple of weeks when they removed all of the resident's hygiene items from their rooms in the name of “safety”. It was only after I brought it to their attention that handwashing with soap is essential to fight germs that she was allowed to have soap to wash her hands. That place was a huge disappointment

Very high Staff Leadership turnover exists and daily care staff short handedness is common. Leadership promises everything, but delivery and follow through is consistently disappointing. Individual Care Plan language is very vague concerning what comprehensive and diverse care actions and services will actually be taken or provided and at what frequency. There is very poor daily care staff commitment and ownership of responsibilities. There is too much looking the other way instead of looking into resident events that happen; where's the love of being responsible? Activities and longevity of activity coordinators are inconsistent, especially in Memory Care. Daily stimulation is not the norm. Outdoor courtyard activities are rarely observed. The Assisted Living courtyard is resident's natural world and is severely underutilized. Thanksgiving and Christmas events including resident families are appreciated. In Memory Care the norm appears to be daily corralling of most residents in the common living room with 1 staff member hunkered down in a corner (focusing on their phone), thereby 'monitoring' the residents in the room. The cable TV service is not the same as in the rooms and only appears to broadcast old reruns. How monotonous and mind numbing for memory care residents. High fall potentials are common; our brother has fallen 7 times over the past year resulting in 7 stitches to a bloody split left ear, a broken right finger head lacerations, bodily bruising and 2 falls in the same day. Communication of these events with family and fall event response and follow up care has been poor. Costs are high for the overall health care, environment safety and lack of consistency of housekeeping. The Memory Care dining room is warn out with rickety chairs upholstered in seconds fabrics splashed white with bleach splotches. The food nourishment values, portions for men and overall variety warrants updating. Fresh vegetables, fresh baked breads and fresh beef (steak) proteins are extremely rare in Memory Care. Frozen and canned foods dominate the offerings. Where are fresh fruits for Memory Care? We had to pay a non refundable $[fee removed] to lock in a monthly rate of $[fee removed] This is too expensive for this old warn facility. How does Atlas expect fixed income residents living predominantly on Social Security to pay $[fee removed].+ annually without draining any and all resident low savings amounts? Daily snacks and especially hydration provisions are nonexistent to inconsistent. Dementia sufferers cannot communicate for themselves well enough to obtain a healthy snack and a bottle of water on a daily basis; why don't we see hydration provision every day as part of basic care. It appears that when a resident is prescribed Hospice, Legacy Ridge daily care staff defers lead care actions to Hospice staff and really scales back all separately paid-for routine basic care needs. Legacy Ridge 'policy' provides 2 baths per week, but if Hospice provides baths, Legacy Ridge does not perform bathing. In this situation the resident is in effect or actually double-billed by paying Legacy Ridge for bathing and paying Hospice (ie Social Security) for the same 2 baths. This is sketchy and unprofessional business on Legacy Ridge's part. Resident belongings management is very inconsistent. I don't know how many times I've found my brother in someone else's clothes, and towels, bedclothes, and other resident provided provisions/items have gone missing and or ended up broken. Personal hygiene needs including routine shaving, teeth brushing & flossing, handwashing, etc. are not provided on a daily basis. The current kitchen manager [name removed] (?) and server [name removed] in Assisted Living are loving caring professionals.

I had parents at Tribute when they first opened....it was fabulous and reasonable. We moved my ex husband in January and it has been very difficult to communicate with operational staff. The room rates are reasonable but their level of care policies are ridiculous and the staff does not follow the care plan. I am very disappointed and will be looking for another Assisted living facility.

High staff +administrative staff turnover, not setup to care for residents who have higher needs. Definitely not able to assist residents who are incontinent. Not competent to identify/help residents who are injured or ill or who need to be on hospice. Family must be available at all times to advocate for the residents. Staff does not read medical notes and will perform procedures on residents that they've been directed not to perform (such as forcing a resident on hospice to get up and use the restroom when the care plan specifically says not to do that).

Place is brand new so very pretty. Rooms are nice size and well laid out. Food is surprisingly good. Communication here is terrible. Getting anything done is near impossible. Any concerns are disregarded. Some staff is good. Others are terrible and only grudgingly provided the bare minimum of care. Complaints fell on deaf ears. Leadership is not good especially at the top. Director seems unconcerned and unqualified. Any concerns she turns on the resident. Nursing administrator is better but follows her like high school BFF (very odd behavior from both). Sales is friendly and kind. Head of dining is nice and food is good. Big problem I noticed was lack of hand sanitizer stations. A virus went thru the facility like a wild fire several months back. Sad because at least one resident was hospitalized, which could have been prevented with hand sanitizer or hand washing. I think place could be greatly improved with new top leader.

They were super responsive when my grandfather pressed his button for help. Staff seemed friendly. He got multiple UTIs there. I think it's because they had him use multiple pad liners to "save money on depends". The last time I asked them to test for a UTI (he was lethargic, sleeping a lot, moody and grumpy) and then 3 days later his PT asked for the same thing. The next day they called me thinking he had a stroke... it was just a UTI that went untreated. The nurse who called me said they never tested, the head nurse said they did... but yet never treated for one. His UTI was so bad at that point he was hospitalized for 2 weeks. They then wouldn't take him back saying he needed too much help. However, now he is back to needing 1-1 assistance for transfer and is using his walker to walk short distances (across a room) at PT. I feel like on paper they show one service, but it's not what you always get. Mentally he wasn't himself there. Unhappy most of the time. He's doing much better since leaving.

None of the directors in their positions take responsibility for their negligence. Evaluations are not completed. Treatment plans are not in place. They need to hire people that are qualified to clean and not just throw somebody in there and say clean a room because it doesn't work. They could be a little bit more creative with their food. The same meals were given over and over again. They also need more staff they only had one or two people in the facility that did the cleaning and the maintenance person was in charge of cleaning, and would often have to clean the rooms rather than do his maintenance responsibilities. Everything changed when they got rid of [name removed] and [name removed], all of the really dedicated staff left.

I've lived at Lake Howard Heights and it's not all the bells and whistles you'd think. They increase the rates several times a year, the food isn't the best, and it feels like a glorified detention center. The parking is horrible and they only have laundry on the first floor. It's a pretty place and they keep it clean, but not the value for what you pay.
