Vitality Living

We did not have a great experience here. They had a [large] community fee, which is refundable until after thirty days. I feel like they stalled me to keep me from moving out until that time was up. Their elevators are small and antiquated, and they only have two. They can't lock the doors to the rooms while the resident is away, and they aren't well supervised, so they are free to roam around, and go in and out of peoples rooms [removed]. They have a second and third floor, one is for more severe residents, and one for more mild, they told me they were the opposite of the ones they were. The staff to resident ratio is not adequate. The food is terrible, and they don't seem to accommodate for special diets. There wasn't a lot of mental stimulation going on either. I will say they have a good security system with the bracelets, if they go out of the area, the alarm goes off immediately. They were also a pretty okay price. However I was very disappointed in our experience.

This facility's management act as if they are novices at care. [removed] There is constantly trash in the front entrance of the facility. The best part of the facility is the physical therapy- and that is contracted out. Nursing staff is poorly trained. ALL LPNs. Abundant mistakes. Care staff speak harshly to residents.

Privacy issues were a problem for my family members. Staff would enter apartment at any time without notice. Food quality was poor. At first the staff was friendly and helpful but as a few days passed, that ended. We felt the security was lax. Back door was unlocked and accessible late at night. They lived in a back building so security was important. Also the dining area for their building was so far from the main dining room and kitchen, food was often cold and some foods offered in the main room were not available to them. So many promises to fix things in the apartment went unanswered. It was a bad experience for my parents.

I have not been in this facility as I live out of state. However, I feel compelled to write a review. My brother was a resident at Vitality Cour. He was being taken to group therapy at a nearby mental health facility each day by the Vitality Court staff. Yesterday, by their own account, they forgot to pick him up at 6PM as they were supposed to do. They didn't even notice he was missing until 1:30AM. That is seven and a half hours. How could it have taken that long to notice that one of their residents was missing? I don't know what time they called the police, but the police didn't pick him up until 3AM. For nine hours, my brother was wandering around near the mental health facility confused and without a phone. Last night could have easily ended in tragedy. Fortunately, it did not. [Date removed]

I can tell you that I would not referee that place to any body. The admin is not helpful at all they left our husband and father there I had cameras in his room in a 12 hour span no one looked in his room. They forgot to take him down for dinner and the other time was breakfast. I had someone come 3hours a week or more to check on him He was three months and I took him out.

I would give it a 1 in all counts except for cleanliness a 3 and that only because it is very superficial clean. We moved out after only 2 weeks because they mismanaged the medications, the food was inedible, the staff was almost all temporary workers who were untrained and didn't know the residents and mostly sat around doing nothing. She was not being changed at night nor the linens kept clean. There were barely any activities at all. It was absolutely horrible and a disgrace that you are sending unsuspecting people there.

These are my thoughts on Legacy Village : If you have a loved one who needs just a little bit of attention - this is alright. I personally would give it a C+ If you have a loved one that needs help of almost any kind dealing with mobility, then I'd give them a D+. The food was NOT GOOD at all. Everything that they served tasted as if it was straight out of a can - no seasoning, neither hot nor cold - room temperature. The only staff that I noticed that seemed happy to be there were the front staff that you see when you came in. The other staff complained and didn't have a good word to say about the other staff. It just seemed, on a whole, not a good place to be for the workers or the patients. Some rooms smelled of urine when you went by the door. My husband regularly saw those huge water bugs in his room, a long with ants from time to time. He complained, but only saw an exterminator once. They were supposed to do laundry, but my husband's clothes piled up and I began taking them home and washing them. When my husband didn't go eat in the dining area, he had arranged for them to bring him a tray, but it usually arrived an hour or more after the dining time. My husband was only there approximately a month, but as it got closer to the time, most of the services that he was supposed to receive, were nonexistent. For the price that I paid, Legacy Village really made out substantially off our family. It is awful to write a review like the one above, and that is why I hesitated so long before sending it to you. Maybe things have changed since last year. I really hope that they have because it's such a beautiful facility and I would hate to see it go down because of the staffing and\or the main administrators or administration.

They do not manage medication very well. Planned activities don't always happen. TV service was promised but not delivered. I liked the management there and I want them to succeed. But they made an almost fatal mistake with my mother.

Mom in Indep for 1.5 yrs. Apt very nice. Housekeeping poor, food poor, activities good, receptionist exceptional, bus driver great, bookkeeper on top of questions, director-little communication with tenant or family. Six months in Dependent. Definitely bad experience. Apt nice, nurses revolving door, care givers also and they had no certificate training. Some great but some very concerning and some very unprofessional. Nurses had great difficulty getting my Mom’s meds correct. I had to follow up daily. Significant communication issues. Housekeeping was very little in dependent. Seems could not keep it covered. Food poor in dependent also. Most concern fall back on admin. Traditions is an expensive senior living and should provide much better quality service. If moving into Indep , be sure to research dependent as most tenants plan to move that direction. My opinion is Vitality has some serious needs to evaluate in their purpose.

They promised a lot but they offered very little but I was not in the assistant living. I was in the independent living section. They seemed to like it there

I will say this I had cameras in room plus I had some one that I paid to help bath him. With that being said. No one checked on him several times at night as I said I had cameras in room and knew I did.

My mother had not one, but two, ambulance rides in the first week. The second one resulting in a partial hip replacement. She was a known fall risk. They took no additional steps after the first fall to prevent the 2nd one.. My father removed her from that facility. $[fee removed] to only land my mother back in the hospital. They ARE NOT a higher level of care facility. Would not recommend.

My sister was moved to memory care here, recently, since it received high ratings on A Place for Mom. Her son had 24/7 caregivers for her. He was assured that she would be well looked after if he cut down on the hours of the caretakers, so he did. They were supposed to care for her for only three hours (including dinner). I stayed with her until dinner, and saw only one person looking after the residents. As I left, I told the staff they said they would look after her. They didn't. When the night caregiver came in she was lying in the common room, on her stomach, on the floor, in a pool of blood with several people standing around her. She had fallen. It was discovered she had a brain bleed. Long story short, she died in a hospice hospital a few days later. Vitality staff were supposed to watch her for just 3 hours, and she died. Please be careful about using this facility.

Facility was not able to provide for my family member. Was lied to upon initial assessment and tour. They make you believe they will be able to provide what your family member needs and that is not the case. They want your money so they will make it sound good. Family member was hardly checked on, if family was not there with him nothing got done. Would forget to assist him with his medicine. Left doors unlocked that they could roam into, fell multiple times and left on floor for who knows how long. Family would have to come in and clean room. Staff would forget to bring him/ or assist with his food. Overall would never recommend this facility.

My mom was previously a resident at Traditions of Smyrna and I blame them for letting my mom get in bad shape. They didn't let anyone know the nurse left a month before we had to move her. I told her to get the Dr to check for a UI. I didn't know she was leaving. They never checked, and that's what mom died from. They said they could no longer care for my mom, and gave me only 3 weeks to find a new home.

My loved one is a resident at Traditions of Mill Creek. Initially I would have rated it at a 5, but now I would give it a 1. My loved one's room is never clean. I just cleaned it, the floor was black. The shower was black and the sheets haven't been changed since the last time that I changed them (a month ago). The food is gross and they served sandwiches and soup last night. My loved one's drink was 2 ounces of tea for her to drink with dinner. They forgot to feed her lunch one day.

Vitality Living Lake Forest was a terrible decision we made. They staff members at the community did the bare minimum. Would not recommend to other families based on the experience we had with them.

Somethings The Lodge at Aspen Village could work to improve on would be to pick up, brush teeth, quality of care. My husband was soak and wet on [date removed] for pick sitting in a stack of chair. No shower or teach brush since I left him. Very lethargic like sedated with something. He had two patches on him one dated [date removed] and could not make out the other date . According to pharmacy pamphlet if more than one patch could be deadly. I have tried to contact them and they will not return my call except [name removed]. Would not recommend. They need to hire some health care worker. Administration should return calls

The time she spent there she never had a shower. I had to pay out of my pocket, even though she had long-term insurance, reason being they wouldn't send her diagnosis to the ins company, they diagnosed her having dementia. Her staff was nice, she didn't complain, and the facility was clean.

My sister and I spent hours trying to find the best place for our mother and we were convinced she would be safe and happy at Tradition’s. They assured us they could handle her Ostomy. But…..After a Director change, 2 new Wellness directors and countless tech replacements everything changed. She would sit sometimes an hour before they would come and help her and no one seemed to be able to change her bag correctly nor did they care. We also started having things missing from her room. Toilet paper, coffee pods, bottled water and hair spray would all disappeared! Oh and beware you will get a 30 day notice as soon as you complain!
