They did a good job getting her walking again, but I think they didn't supervise her enough. When she arrived she was too week to walk to the bathroom on her own. In two weeks she was walking. But I believe they should have had staff escort her to the dining hall daily instead of letting her walk on her own after only two weeks. She had a fall in the dining hall, broke a hip, was hospitalized and went into cardiac arrest and died. After she died I was surprised that no sympathy card was sent to our family. But they did send a bill for the meals she had served in her room. She wasn't there for a month before she died from this accident. I am very impressed with their physical therapy services, but not impressed by the supervision. Their dining room and meals seem better than most. Activities... Not terribly impressed. The only thing I ever seemed to catch when I was visiting was chair aerobics. But my mom was only there about 2-3 weeks. Location is great. Easy to take your loved one out for lunch or coffee. Mall within walking distance.

My Dad was a resident in the memory care center for 3.5 weeks. They lost his dentures on the third day so he couldn’t chew food. They never altered his menu and continued to give him meats. He should have been on mechanical soft diet. I don’t believe that they woke him for all meals as he sleeps a lot. They do not participate in the Medicaid waver program and we will need that so we had to move him. We knew that they don’t participate but the changed his level of care the day before he was to move in and we had no other options.

I really liked the facility however I did not care for the way we were made to feel it was all about money. the way that they told us to leave and let them do their job and then immediately called us back and told us that my father was an elopement risk. We had to hire a babysitter to sit with my dad for 12 hrs a day (at $[fee removed] per hr) for 4 days because they said they could not keep him there without assistance. Then my father was so adamant about going home when we tried to visit him that we had to drop things off for him (because we could not see him the first few days without him getting really upset) that were not given to him or put up in his room like his shower curtain. Then we found out that after leaving him be for a few days so that he could get adjusted to the place and get familiar with the people. They were not making sure that he was taking care of himself like showering. My aunt went by to see him and he had not showered in days and was wearing the same clothes that he was wearing when we dropped him off, which was nine days later. So no, I was not very happy with their treatment of my dad, the quality of care that he got or the compassion that they showed.

We found that while a handful of the staff were very caring and diligent about patient care there were times when my father-in-law was left soiled and his bedding soiled. This unfortunately happened more than once. The staff generally seemed overworked and stressed out. Unfortunately, my father-in-law actually fell while there and broke his hip. While this happened under their watch, the management kept calling us and pestering about moving his room and our plans going forward, hoping to get us out in order to fill the bed. Sadly, my father-in-law did pass away after the hip fracture and only one of the staff members Gave any condolence.

Need to know what their communication system looks like . How many caregivers are working daily and how do we know if the patient is getting his or her medicine? What kind of activities are there on the weekends when the director is not there. How many nurses do they employ and how many/how often does the Nurse Practioner come to visit?

The recent violations listed are indicative of the experience my loved one is having. I notice a new complaint. Seems like conditions are deteriorating.

There was a shortage of staff due to firings after the hurricane evacuation. 1 person on duty for 2 floors if residents was too thin. Employees were friendly and helpful when they got to mom but sometimes there was too long of a wait. The cleaning lady did not do a good job. The laminate floors were filthy when mom moved in and out. All she did was swiffer down the middle of the room. As mom neared her move-out date there were days when they did not come to take her to lunch or dinner. I was also uneasy about whether they were giving her her medication properly.

The memory care is not good. The food and dining room is not good. Staff training needs improvement. In memory care the staff are not engaged More activities for memory care Healthier food and drinks encouraged -- dementia patients dont remember to drink

Having had a very difficult year with my father, getting him into a facility where he could thrive was a dream ending to 2024. Touring Vitality Hendersonville and seeing their apartments was great. Residents were at music and the facility is nice enough. Now, after 7 weeks, his room AC/heat unit has been broken for five of those weeks, he doesn't have a key to his place, and they've seemingly shuttered an onsite PT gym that was one of the key benefits for him being there. I have been called names by the staff and he's gone as many as 3 hours, after pressing his pendant, before having someone show up to help him. They've adjusted the response times, however issues remain abound and now I'm hearing rumor they're selling to someone else. Shopper beware.

The price was better, now it is not! The experience is just okay.

You have to stay on top of them to get just the basic services that you pay for. We are always asking them to do laundry and assist with bathing which is supposed to be done twice a week. In a years time they have bathed her less than ten times. My wife has to go there weekly to make sure she gets a shower. Plus they have increased our monthly bill by $[removed]in a years time. That is 34.6% and have offered or performed no more services for the increase. It is has become more about servicing the well being and interest of the staff instead of the client. The is no stability within their memory care unit. Staff changes are frequent and no one knows what is happening since there are such frequent turnovers.

My family cannot in good conscience recommend Traditions of Smyrna to any potential residents. We have experienced poor management, medication mismanagement, multiple falls, and lack of communication of things we contracted for based on my MILs level of service we are paying for. Verify the credentials and experience of those caring for your family member as their decision have long reaching consequences.

The management is not very good and therefore even good employees fail at their job because of poor direction.

They left my mother on the floor for 24 minutes after she pressed her button she called me and I called the front desk they didn't send anyone in 15 minutes later they finally did I have now moved my mother from there.

First I do not want to be unfair since Vitality Living took over management of The Hearth shortly after my wife went there. I recognize that things fall through the cracks during a change of management. However, I was concerned that problems were brewing when many of the staff members left or were let go immediately after Vitality took over. The workers dealing with my wife were not always pleasant. My wife was not always cared for adequately. The activities were not consistent. I definitely feel that my wife would still be alive if I had hired workers to keep her at home. As best I can calculate the cost would not been that much more than Vitality since I was having to pay a higher rate because they required that she have two person support for her. I definitely would not recommend Vitality for anyone in the memory care section.

My wife was here for a short time. I was told that they only have one staff member in the evenings for the 15 residents. My wife was sent to the ER 5 times during her short stay. I met with them and was told it's protocol to send a patient to the ER when they fall. But my wife says she falls when she doesn't. I guess that's part of Dementia. She's been home with me for several months now and no falls.

Our family had an in person tour and we were not impressed at all, it was just not good and we left in tears

I was not impressed with Vitality Living Spring Hill. We waited for someone for 45 minutes to give us our tour. The staff were rude so we did not even continue the tour to go look at the rest of the facility.
