I would want others to know that we did not have a good experience with Sugar Grove in the memory care unit. I would tell others to look for another place and not place your loved ones in here.

Executive Director was not honest about the pricing and the transportation. I am very unhappy with the cleanliness or lack there of with the facility. Smelt of dog urine and fecal. We moved our mother out to Brookestone. SO MUCH NICER and great staff

If there was a -10 rating. That's what I would give Carnegie Village Rehab. My 93 year old mother has been there for 3 weeks with a broken hip. We have turned in a grievance almost every day. The days we didn't was just because we let whatever happen that day slide. My mother is quite alert. must be something they aren't used to.. I feel sorry for the people that don't have family to check on them or they cant express that they need something. My mother wasn't to be left alone when she was in transit. About the first day a RN took her to the bathroom and while we were there in her room, left. Didn't come back Left her standing in the bathroom!!!!. Another time when we wasn't there, they put her in the corner in a chair way across the room where she couldn't get to her call light or water for 2 hours... The call light, well they might as well not even have them because they don't pay ANY attention to them. So many times her call light would be on for at least 1 hour or more before anyone checked on her.. If she had fallen, she would have laid there for more than an hour.. She had her call light on for 1 hour and no one came. I had called her and she needed to go to the bathroom. So I called the front desk and not very nicely told them she needed help. I'll tell you, it was still 30 minutes later before they came in. Unbelievable. Oh but they are very sorry, Don't know how many times I heard this from the nurses and heads of staff. When we looked at this facility before we took my mom there, it looked so nice on the outside, the rooms are very nice, but no workers . There are a few that are very nice but most of them don't give a hoot. She was in this facility for a week and a half before they helped her with a shower..Then we had to ask for it.. A lot of the time the food is cold by the time she would get it and then not what she ordered..We had a RN tell us that mom had a fentanyl patch on (which she isn't to have any opioid) when I questioned her about the opioids she said they wont bother her because they wont go to her stomach. They had just put ted hose on her instead of giving her Lasix. When I questioned a different RN about this problem she says, "She doesn't have a patch on and she had her Lasix this morning. Go figure. However the rehab was very good..

Three chefs in the past few months. No budget for activities. Something is not working at this community. Would not recommend. It is an unfortunate situation.

I placed my wife [name removed] in this place and informed them not to give her certain meds as she has bad reaction to them, [name removed] loved to walk and I got word they found her walking around in the parking lot in pj's, this is suppose to be a secure place, next I found out she had made it over to the admin, bld, again unassisted, right after those 2 times she started falling and having to be taken by ambulance to hospital emergency room, I asked for list of her current meds, the medical staff refused this and during the next 9 months she fell at least 10 times, each time required the emergency room, they finally told me they couldnt care for her anymore and to move her to another facility, gave me 30 days to find new one, and that during that 30 days they would put her on 24 hour care at my expense, found her walking around naked, her sofa soiled, she thought it was a toilet, this is just a short list of some of the things that happened in the short time she was there, this is a hell hole

Terrible rating. For Rehab Services - check Medicare and comparison stats first. My parent has been in Carnegie Village Rehab - terrible care by hands on nursing and medication administration. It took more than 48 hours for all of my parent's meds to be provided from "their pharmacy." Less than - welcoming - to an elderly person who just arrived from acute care to rehab. My parent stated they were "afraid" in the first 24 hours. Gee - I did not desire to leave my parent. More moves would have been - unsettling to the parent therefore - we made our presence known most every day - even with our working full time. So much for our Medicare benefits that my parents worked so hard for during their life! Bedside nursing - Stinks! PT, OT, Speech - well done! Bedside - my parent's call light is NOT answered for 28 minutes most times - or longer! I timed it - after I arrived and the parent's light was already on! How long was that?? Therefore the parent was soiled - who is cognitive (unpleasant experience by a cognitive elderly adult with dignity) - and clothes to be washed by family unless you want clothes to be combined with everyone's clothes in the laundry from the same - skilled nursing hall! NOT! What an ongoing very unwelcome to a debilitated, weak, afraid, former government employee! I would not recommend this "my poor rated" facility for rehab. I heard it was - good, NOT! The PT, OT, Speech - is not the only care a person in rehab needs - they need their medical ongoing care in a timely manner - with professional caring, courteous, compassionate staff. NOT! It would be great if all staff wore name badges - a part of their uniform so as you could take names for Admin. Excuses - "I'm new here, I lost name badge, I don't know, I faxed the med orders to our pharmacy." Where is the accountability? Spoke to Admin on numerous occasions -- same ole lack of med care - which is important to rehab in conjunction with PT, OT, Speech. I would NOT recommend this facility as a rehab program. Location for convenience is not always best. Next event we will go to Lenexa, KS where the reviews are great - and the care is compassionate for the ailing elderly. Our family has talked and taken an unguided unannounced tour with clients at the Lenexa, KS facility. We made a mistake for our parent - that we will have to live with! DO NOT UTILIZE CARNEGIE VILLAGE, BELTON, MO - FOR REHAB SERVICES! Check your options.

Stratford Commons Memory Care is severely understaffed. I wanted so much for it to be a good fit for my mother because it was close and I could visit easily and often. It was obvious by visiting often that there were major issues with the administration and day to day care of the residents. My advice is to visit often, at different times of day, to see the true picture of your care facility. Our loved ones deserve the best we can offer.

[name removed] the admittance coordinator was very rude to me when I wanted to place my father there at rehab. I thought she was bipolar, one minute she was happy, the next she was horrible. I then met with [name removed] and she wasn’t very professional at all. Those two people gave me a horrible experience and I would not recommend the facility to anyone. After talking to a friend I learned that I wasn’t the only one who has had a bad experience there. I would pass if you are considering this place.

I had a short experience with this community. The way they chose to evaluate my father was by calling a community he used to live at. We did not have a good experience in this community. So I was confused when they didn't take the time to come visit my father, and evaluate him themselves. I didn't think that was very professional.

My step mother and I are very disappointed in the level of care and the condition of the facilities for when my dad was staying there. His bed was broken, his call light wouldn't work, the light over his bed wouldn't work - finally had to go to reception desk at front office to specifically request maintenance assistance. One day my step mom walked in and my dad had ants crawling on him, another time it was noticed there was black mold growing under the window. When they moved him to a new room several of his personal items went missing, i.e., electric toothbrush, electric razor. Often he was not clean when my step mom came to visit. They never helped him find his teeth when they went missing - I usually found them in a pants pocket in the wash when I picked it up - which meant for days he went without them. He lost over 20 lbs during the several months he was here. We also had questions about the billing but no one would ever contact us back - even after sending a long letter and requesting the courtesy of a reply. We believe the billing is incorrect but paid it because there was a $250 a month late fee for unpaid balance. Horrible experience. BUT the care staff were wonderful, caring people - just understaffed and overworked.

This is not a place for anyone to live, staff always walks out within 6 or less months. Price changes monthly and large one if you move out. Bad food - bad staff and the one charge has a temper, she her make people move in just 8 hours. If you do not believe me just live there and see. There Boss Jackie is unruly ----do not make her mad, it will cost you dearly. VERY TRUE

We did not have a good experience with this community. They did not care for my father and his needs well at all. The management is not very good here. They are not good at responding to my concerns. I feel like they are not very honest and are hiding things. Their skilled nurses do not have licensees. Also we had some insurance problems. They lied to my father and I would never recommend this community to anybody.

Another new Director-huge turn over in staff and still not managed efficiently. After 6 months someone tried to make the ground level outside of the patio area- it is a joke...it is worse than it was. Still no security cameras on premises. Very poor customer service AFTER you move in! Promises never fulfilled-rude comments from the staff, and NO RESOLUTION TO THE BREAK IN!

My mother was there for six years. Unpredictable staff and services. Insensitivity to the residents. Treating them as children. Focus on medical not emotional well being.

Out, horrible reviews and no on site care physicians

Sometimes no one would eat the food, sometimes I think they try to make too fancy of food. I did not like the management, they try to take control, completely. They would change the medication, without my permission, or even informing me. They would make her wait over an hour for pain medications. If you are dependent to have them take care of you, I wouldn't go there. They were very rough with putting her in bed. She wandered and was gone for half a day before they even noticed. I told them that we needed finger foods and they said that's they were giving her, but it was spaghetti when I saw it, that's not finger foods.

Probably too far for me and relatives who may visit. Very good follow up calls. no

My mom was in the independent living and then skilled nursing, the independent living wasn't too bad, but the nursing place I wouldn't take a dog to. You could walk in the door and my mom was just laying there all day, you could smell uring and they weren't checking on her. The food was terrible and she had to send the food back several times. There were so many times when the food was literally unedible and most of the times when she was sitting in the dining room and people would just complain about how bad the food was. I would be there 5-6 hours just to make sure that she got the basic things she needed and was taken to dinner. That place was terrible, I was having so many problems with them, I was just disgusted. My brother and sister had a meeting with them and absolutely nothing got accomplished from that. The independent living wasn't too bad but the skilled nursing was terrible. Somebody from medicare needs to investigate it because I think it's just one big scam.

They were medicating my aunt so she couldn't get out of her wheelchair without falling, they lost her wheelchair...we had a very difficult experience with them. They didn't keep up with things nor were they very good about seeing her and they had her on such an extreme amount of medication. The doctor she had there probably perscribed the medication based on what they were telling him and they probably just wanted her sedated so they wouldn't have to care for her. I would not recommend that place to anybody, we didn't even know what all she was on until we left there. They also wanted us to provide 24 hour sitters if we kept her there, and I think that would have been an additional 20 dollars an hour which we wouldn't have been able to afford. The medication thing became quote an issue and the fact that they wanted us to provide sitters.
