My grandma is currently at this facility, she fell the other day and the nurse told her “I don’t want to do paperwork if you fell, because it’s a lot.” As my grandmother is laying on the ground. My grandmother will press the button for the nurse and it usually takes a nurse an hour if one even comes to see if she needs help. This facility needs a better director of nursing, the staff needs better training and to be kinder to their patients and the families coming in.

Our Mother with dementia was there twice. Both times she fell and required an ambulance to take her to the hospital with injuries. The first time, her granddaughter found her in the bathroom floor, upon finding her she called the nurses station and it took 20 additional minutes for someone to show up and finally decide to call an ambulance. Today, she was found in another patient’s room laying on the bathroom floor with a head injury, again, no idea how long she had been there. Ambulance called again, we were notified that she had been taken but no one escorted her as a dementia patient. While inquiring with the nurses what had happened, we were informed she didn’t really belong in that facility. They do not handle the dementia patients and she should be on a different floor. Please use caution! If I could give it a zero I would have. Please note: no shower for 5 days.

I toured this facility and was so impressed I requested a transfer here following my Total Knee Replacement Revision surgery. The care and attention at this facility is almost non existent. Non of my regular medication was given. I was told it is normal to wait 24 or the next day for meds to arrive. I did not have pain medication for hours. No one checked on me. No one knew how to fill the ice machine to keep my swelling down. No one answered the call button. I had to shower holding on to my walker because their was no shower chair. There were no towels so I dried off with my tshirt. I could not get them to confirm my need for warfarin I got no medication and my INR was 1.0. Dangerous for throwing a clot. My only meal was beef and I tried non stop to get someone to answer the call button. I cannot eat beef. I have had my esophagus stretched twice. I can choke on this. There is no compassionate care at this facility. My daughter took me out in less than 24 hours.

They lie, they raise the rent too much. The don't fix things that are broken. They said she could move in [date removed] it wasn't ready, she couldn't get in until August and they charged her for July. The place wasn't clean at move in. They said she didn't pay the community fee but she did. I was there!! I will NEVER recommend this place to anyone! She is moving out. They are not helpful at all when there are issues with the bill or maintenance.

This place is not a good place to send your loved ones. Seems like the aides are too busy talking to each other rather than helping the patients . I visit a few friends there regularly n I worked many nursing homes n I never would just put someone in a room n leave them without making sure they are ok. The light was on over an hour- who’s watching these lights n an hour is totally unacceptable . The food is mediocre nothing to brag about! Someone needs to make these nurses n aides take classes for respect n caring! I wouldn’t put my loved ones here. Just my personal opinion- and the patients look like zombies all coped up! Not ok by me!!

Would love to give a minus review! Horrible administration that lie to get your loved one into the facility. Save yourself the heartache and look elsewhere! When you start complaining about the care they turn on you and talk about your loved one to the public. HIPPA violation

Would not recommend Mission Chateau or any Tutera elder care property. Based on my parents experience, Mission Chateau has minimal empathy and compassion for the elder and are totally focused on the bottom line. Beware of all the add on costs that can easily add up each month. You only get two meals a day, with minimal portions and pay for sodas, deserts and other amenities. If you have a loved one and expect, at some point to move them the memory care at Mission Chateau, they are very selective and only take “high functioning” memory care residents. But, are not up front in telling you this. You find this out once you move in. If your loved one is in latter stages of dementia and is at end of life, you will have to move them to another facility. Beware of Tutera properties. Beautiful facilities, but look beyond the millions that they spend. Money does not buy compassion and a caring and loving environment.

Do not send your loved one to than side. My mom was in here for 8 days, and it was 8 days too long. First, one night she was left sitting in her feces for hours. When my dad brought it up he was told that they sometimes get outside staff in on the weekend. Second, when I arrived for the first time, my mom said the staff don't answer the call buttons at night, and when she yells for help she gets yelled at. I actually tested this call button issue out for myself. I did this different days and times of day over the course of my stay when I needed assistance, and I noted what time I called. They ignore calls for help.. This place is hideous. If you love your family member do not send them here. I follow the reviews on this place. Believe what the people are saying about the patients who can’t advocate for themselves. Like any horror movie, beautiful outside, horrific inside.

Looks are definitely deceiving. This is a beautiful and well kept home. The nursing team needs a good overhaul. I was sent here by an agency company to help care for the residents. I was left by myself the entire night. I had an emergency with one of the residents. No clue who to call so I finally found a list, she gave me the nurse on call #. Straight to voicemail. I had to make several unnecessary calls before I was able to get the nurse. So eventually the nurse called me and she thought it was funny her work phone didn’t work. Thank god for the rescue squad and the hospice lady that helped me that night. I will never go back to this place. If you have loved ones I would recommend taking them to a different facility that will make sure they had adequate staffing.

For the expense it takes to have a loved one in Tiffany Springs Rehab, you would think they could clean the place and pay better attention to the clients. My father couldn’t get anyone to take him to the bathroom for over an hour, yea he pooped his pants. All the nurses do is sit behind the corner desks and look at their phones, ignoring the patients. The Medical care is substandard, the head of nursing ignores the complaints of the family. Patient bathrooms are not cleaned according to industry standards. Take a look at the Medicare reviews, its an eye opener.

My Grandpa spent his last month of his life here and it hurts me knowing that it was such a hellish experience. I would often come late at night to visit, anywhere from 11pm-2am. I would find empty hallways with residents literally screaming for help. “Please help me!” “I NEED HELP” “PLEASE HELP ME!”. It was honestly horrifying and felt like right out of a horror movie. There is only one nurse per entire hallway, each hallways has at least 25 residents who all need full time care. Once I got to my Grandpas room, he was lying in his own feces and piss for over two hours waiting for help. He even developed a rash that had to be treated because this happened so often. The nurses would just be on there phone at night ignoring the screams and calls for help. Some of the day time nurses were kind and attentive within the hour. Most were not. Grandpa needed oxygen and they once just forgot to refill, which is blood pressure dropped and it could of been deadly. Don’t take your loved one here

This is a beautiful community and building, but the services are horrible. The food is lousy. The doorways are hard to get through with a walker or scooter. The people behind the desk don't know what is going on. The resident have to take their own trash out. There are washer and dryer in the apartment, but there is very little water pressure in the apartment for the residents. I can get cold water, but not hot water.

Visited in 2019 and again recently. Things are worse. Staff was rude. Community fee was too high and rent was outrageous for what they offer.

Our 87 yr old mother with Parkinson's died at TS last fall. Her death was not a surprise, we had been preparing for it as a family. My sister and I had DPOA for Healthcare decisions and chose this facility after a hospitalization. Mom was a DNR in the hospital and an outside the hospital DNR form was signed by us on the day of arrival at TS rehab and then signed by one of their physicians. The SW invalidated the DNR 14 days later, which was 2 days before mom's death, and failed to get a new one generated as she promised. EMS was called when our mother's heart stopped and CPR was performed against our directive for her care. Our mother did not revive, thankfully, as she would have been in pain and only have to die all over again. It has been 4 months today since her death and we are still angry and traumatized by these last events which robbed our mother of a peaceful end to her beautiful life. Please do not take anyone you care about to this facility.

But my significant other in there and August for a week took him out in five days. Did not help him adjust to his new surroundings he fell four times. Never made sure he took his meds just left them in a cup by the bed.

As a family member, who's loved one still resides in this community, it has become an unreliable and unfriendly place. Since they fired an amazing director and the best staff left because of it, it is a barren place without personality or character. They had to deal with a Covid infection without the support of the corporate people that were supposed to help. Several residents passed away and corporate attempted to misplace the blame. I would have once recommended this community to anyone who asked about it but is just a shell of what it once was. I will be moving my grandmother very soon to another community within the Sioux City area. Horrible place. Do not send your family here.

I would want others to know that we did not have a good experience with Sugar Grove in the memory care unit. I would tell others to look for another place and not place your loved ones in here.

Executive Director was not honest about the pricing and the transportation. I am very unhappy with the cleanliness or lack there of with the facility. Smelt of dog urine and fecal. We moved our mother out to Brookestone. SO MUCH NICER and great staff

If there was a -10 rating. That's what I would give Carnegie Village Rehab. My 93 year old mother has been there for 3 weeks with a broken hip. We have turned in a grievance almost every day. The days we didn't was just because we let whatever happen that day slide. My mother is quite alert. must be something they aren't used to.. I feel sorry for the people that don't have family to check on them or they cant express that they need something. My mother wasn't to be left alone when she was in transit. About the first day a RN took her to the bathroom and while we were there in her room, left. Didn't come back Left her standing in the bathroom!!!!. Another time when we wasn't there, they put her in the corner in a chair way across the room where she couldn't get to her call light or water for 2 hours... The call light, well they might as well not even have them because they don't pay ANY attention to them. So many times her call light would be on for at least 1 hour or more before anyone checked on her.. If she had fallen, she would have laid there for more than an hour.. She had her call light on for 1 hour and no one came. I had called her and she needed to go to the bathroom. So I called the front desk and not very nicely told them she needed help. I'll tell you, it was still 30 minutes later before they came in. Unbelievable. Oh but they are very sorry, Don't know how many times I heard this from the nurses and heads of staff. When we looked at this facility before we took my mom there, it looked so nice on the outside, the rooms are very nice, but no workers . There are a few that are very nice but most of them don't give a hoot. She was in this facility for a week and a half before they helped her with a shower..Then we had to ask for it.. A lot of the time the food is cold by the time she would get it and then not what she ordered..We had a RN tell us that mom had a fentanyl patch on (which she isn't to have any opioid) when I questioned her about the opioids she said they wont bother her because they wont go to her stomach. They had just put ted hose on her instead of giving her Lasix. When I questioned a different RN about this problem she says, "She doesn't have a patch on and she had her Lasix this morning. Go figure. However the rehab was very good..

Three chefs in the past few months. No budget for activities. Something is not working at this community. Would not recommend. It is an unfortunate situation.
