I had written a good review in summer 2020 and it no longer applies. There is a new director and many new care staff and the care just isn't as good as it was then. I hope things get better again, but unless it does I can't recommend Brookdale Sun Prairie.

I would not recommend Brookdale. Some of the staff members are good here like the executive director and the main nurse, they were great to work with, but others are not so good. The food was not good at all.

Make sure it is the correct place for your loved one. They will tell you anything to get your money to get you in the door. My loved one ended up with some additional health issues and they used a 1918 form to see if my loved one was a fit for the community. They knew that my loved one was a fall risk and still admitted my loved one. The community has a fall policy of three falls and they are out, supposedly. The community dodged the question when I asked them about their fall policy. Due to these additional health concerns we had to bring in hospice. All communication with Grand Villa was cut off when hospice was brought in. I feel the community lied about the form and being qualified to take care of my loved one. The communication with the staff was not good, they wouldn't answer me about my questions or concerns. My loved one was not supported by the community with using the phone. I had to make several calls before the staff would answer my questions. The community had a list that my loved one needed during their stay that we did not give until the day of intake.

I would not recommend anyone have a loved one here that needs any kind of healthcare. I suppose if a place for a room and meals is what you seek GVBB is ok. I had a brother there that needed health care and after we had paid and moved in they then told me if it is not with in 7 miles we won't help out . They could not would not help find health care for his condition. A number of times they said they would find him the proper heath care and the never did I really don't think they even tried . The director ( wonder if the same one is still there ) was uncooperative and at times down right nasty. The good that came of it after much time and money wasted I moved him to a different facility , with in the same area the found the care he need right away and he is doing much better . it just took a little effort and caring which Grand Villa was not about to provided

LOOKING FOR MEMORY CARE ONLY! This is a very quaint property and a bit aged. There are two buildings one for assisted care in the other is called the village for memory care. The buildings age is reflected in narrow hallways but also has a very homey feel. There are three sections that house about 10 rooms each and open up into the hallway that leads to a common area with a lunch counter style sitting area for meals and activities with tables placed at various areas. Each section is dedicated one for males, one for females, and one for combination. The three sections open into a common hallway that they are able to go into and enter any of the sections they wish to, but they need to be able to be able to grasp the doorhandle and open it. Otherwise the doors stay closed. There is access to outdoor areas with lush vegetation. The rooms are tiny, and able to convert into double occupancy rooms. However, even with a semi wall separating the two beds the room becomes very cramped and the roommates very close to each other when they sleep. We feel on mom would not have the presence of mind to open any doors to explore and would feel closed-in by the tight surroundings around her. Staff appeared to be very nice and competent. I wasn't very impressed with my experience with this community. When I first called to set up my tour, the lady that I spoke with informed me she wasn't going to be there, but someone would help me. When I arrived on time for my tour, nobody was ready to give me the tour. Once someone finally became available, the tour was decent. I didn't feel this would be the community for me.

We toured this community when searching for a place for my mom. We were not impressed by them. They didn't offer many activities at all. There seemed to be a very sparse staff, although they were friendly. The food we tried during the tour didn't WOW us either. We decided on another community in the area.

65 residents - very sterile environment - not very homelike. Noticeable odors - one area smelled like feces had not been properly cleaned up - and another area smelled like there was mildew damage. The memory care area seemed dark and like the residents were in another world from the rest of the building. I did not like this at all for my MIL. This was not the place for my loved one but the administrator was very cordial and answered all questions and concerns that I had. Very affordable - but needs work so that the atmosphere is more homelike

I was not thrilled with Village place. They really courted us in the beginning, calling us, twice a day, and trying to be friendly, and very helpful. We decided on Village Place. Sarah couldn't have been nicer, she said that she would be there and stay and help get my loved one oriented. But that's not what happened, she was transported, and sat in reception and no one wanted to help us, and Sarah wasn't there, and didn't follow through with what she told us she would do. This seems to be the theme of our experience. I strongly feel that the senior staff could have handled us and our situation with a little more compassion, and class. The aides and the maintenance people were just as nice and helpful as they could be. I would not recommend them for anyone. I think the staff needs some sensitivity training. I also think they have some in there that shouldn't be, and cause chaos and commotion on a regular basis.

This is definitely not for people with Alzheimer’s Disease. The parking was fine. The problem is that they advertise that they provide care for people with Alzheimer’s and Dementia, but they are not equipped to provide care for these patients.

in 2019 this facility has reduced the quality of care significantly in treating my brother in law. Lack of communications, understaffed, and lack of training. Couldnt regularily cut hair, shave, cut finger nails and more important report medical issues. Twice he developed urinary infections because of hygiene issues. Then they increased is room rate by 80%

The director of excitement is what keeps us there. Without [Removed] there would be no Grace!!! The great staff i.e. CNAs and HHa are now scheduled off more than they work. How long will they be able to keep them? A number of residents are way down...like it was when the state came in last summer and shut them down. The residents are the ones that suffer when numbers are down. They have a marketing guy but really he's in the building too much to really be selling Grace. So once again the so-called chef-inspired meals are white bread hoagies with chips for dinner. No nutritional balance for seniors! Where the fruit Salads etc. Every meal needs vegetables for fiber etc. Not one person in the meal prep is a chef. Cooks yes!\There an executive director but she is never there and very out of touch with residents and families and I have observed her with the staff she seems to be mad all the time. A micromanager who doesn't know her job. Bring back [Removed]!! I see that the staff is struggling to met the residents needs due to the lack of hours and support . The Med Director is great. Smiles and accommodates everyone and is another reason we stay. She knows her stuff. We pay a pretty penny and to be honest the level of care, food are way down. Have been for 4 months. All summer. Thank goodness for [Removed] you keeps everyone busy and engaged!! It used to be a 4 star...not anymore!! Lets hope they get it together or 5 of us are going to move and then there will be only 12 on the assisted living side

We toured this community. We did not like the atmosphere. It was not clean and needed a lot of improvements in the cleaning. The community was dark and we wanted to the community that was more cheerful for environments. We would not recommend this community. We did not see a menu of if activities are offered.

I took her out of there after crazy price increases without notice and poor care, rude staff, and paying for services that weren't performed! Food was horrible like heated Gerber baby food, staff left patients in soiled pants for hours, there was other people's incontinence left in bags in the hall for hours and it made the whole place smell, I was charged for housekeeping and laundry that was never done. There was dried poo on my moms bathroom door and the poo in the toilet had dried as water evaporated and molded. Dried skin piled up in bed sheets and on floor under bed till it looked like a snow globe. They lost some of her meds and had no procedures in place for me to take my mom for a visit. I was given random meds in a taped sandwich bag unlabled without instructions. Again I wouldn't recommend this place to anyone!
