Loved ones room was dirty, he was never bathed, often rooms were often locked, clothes disappeared, they put other ill fitting clothes on him, staff was uncaring, couldn’t answer questions and overall care was horrible..he lost a lot of weight, no one could tell me if he had eaten..staff is not memory care trained…I would get calls in the middle of the night for things staff should be handling..I will never recommend this facility to anyone..

I would not put my worst enemy at Belvedere. If they want their family member to be cold, hungry, dirty, treated with no respect by staff, and get no assistance whatsoever then Belvedere would be the place for them. My family member had feces on his floor in his room, and they never cleaned it up. He asked them where he might find a lightbulb and they told him to go to Home Depot. He has dementia the director told him by the automatic opening doors that he'd need to go to Home Depot. Even the medical staff where their coat while the two office workers sit in the front of the facility with their doors shut so they can stay warm. The staff is rude to family and residence. The laundry does not get done. The bedsheets are not changed. They had bedbugs. I saw people residents, days in a row and the exact same clothes not having been bathed or cleaned, and God only knows how long. If I was a company recommending assistance for families looking for a place to put their loved ones I would remove Belvedere from my list! I doubt they could pass a health inspection by the health department! Needless to say my loved one was there less than a month!

WE ARE PAYING FOR A LEVEL OF HIGHER CARE AND SHE HAS TO ROLL HERSELF. THERE IS NOONE IN THE DINING ROOM THAT UNDERSTANDS HER ORDER. I WILL BE LETTING EVERONE KNOW HOW THIS IS THE WORST PLACE EVER. IVE WORKED AT THE CARLISE AND AM NOW AT COURTYARD GARDENS. I SHOULD HAVE WORKED HARDER TO GET HER SOMEWHERE ELSE. MY FAMILY DOESNT APPRECIATE BEING LIED TO. YOU SHOULD REMOVE THIS PLACE FRIM YOUR LIST. IF YOU WANT YOUR PARENT TO DIE QUICK...THIS IS THE PLACE TO GO. DID YOU KNOW THERE IS NO HEATER FOR THE POOL..NO REAL CLEANING CREW..BEDBUGS AND ROACHES. WE ARE BRINGING HER HOME. I LIKE A PLACE FOR MOM BUT YOU SHOULD EAT IN THE DINING ROOMS YOU ENDORSE AND REALLY PAY MORE ATTENTION TO DETAIL.

After raising his rent to over double what we originally agreed upon, I moved him to another facility. Then, they charged him over $[removed] after he moved out. I've been trying to sort this out with [name removed], the General Manager. She won't return my phone calls or emails. I could go on and on about the myriad other problems with this place. If you care about your loved ones, DO NOT move them into any Truewood facility.

My aunt, whom I am POA for has lived at the Pavillion for almost 8 years now. I DO NOT recommend this place. In the last few years it seems they cannot keep a director more than six months. Phone calls are never returned. Note that I live in Venice and can visit when I wish and I have been hard pressed to find anyone in charge any weekday that I went there. I must say however that the CNA's and aides are caring but they can only do what they can with an individual owner that seems to care little. They had a major mold problem about a year ago, that has so called been fixed. The last year or so the elevator does not work. They are continually having to reset it. This is an extremely old building with 3 floors. Most of the residents are on walkers or even some in wheelchairs. Imagine these people trying to make it down a stairwell in a fire. I even spoke with the Fire Chief of Venice, he said there was nothing he could do because the building has sprinklers.

Choosing a memory care facility for a loved one is one of the most important decisions you can make. Allow me to share some things that could possibly happen at Brookdale: -Your loved one may be left in the care of rude, belligerent, teenage-like staff who refuse to put down their phones. -You may have to ask multiple times, every time, for rooms/bathrooms to be cleaned and laundry to be done. -You might get so tired of the pile of laundry that you finally just bring it home and do it yourself. -Prescription meds might be administered regularly, or they might not be administered at all. -At some point, your loved one might fall in one of the common areas, breaking an arm and suffering a nasty bump on the head. When you ask what happened, nobody knows how it happened, or even when it happened because nobody had been paying attention. -Bottom line, if you care at all about your loved one, Brookdale-Sun Prairie is the last place you should entrust with their care.

My loved one was previously a resident at Truewood by Merrill Charlotte Center. The staff communicated with the family when a problem was apparent, but either did not pick up on sublet signs of decline or did not feel that it warranted a call to the family. It seemed that there was not consistent communication from direct care staff to those with higher levels of licensing until an issue arose. Honestly, it is not entirely the facilities fault that things did not go well for my loved one there. I do think that families need to have a better understanding of what a facility is capable of doing, in terms of level of care. My loved one's physical mobility greatly declined over the five weeks that he was at this facility, in part because he was not interested in participating and in part because the facility did not have the capacity to be consistent with ensuring that he was getting up and moving around. Also, I'm sure that the meals were fine from a nutritional standpoint but my loved one's tastes were not satisfied by them. And the 'extra fees' that were incurred were not clearly communicated. I would not recommend Truewood by Merrill Charlotte Center unless family members have the ability to visit their loved ones frequently to ensure that there is not decline over time.

I'm a former family member of a resident, I don't not approve any body to stay at brookdale , bland food , unsupportive staff , inappropriate outburst from other residents staying their , it was just a overall horrible experience and my mom told me alot of staff would vent to her about their own personal issues and other staff members, it was just an overall negative experience for the both of us .

The LPN that provides health assessments is terrible she spends a brief 10 minutes asking questions like: Do they know the date, can they memorize 3 words and repeat them after 10 minutes? ,etc. She makes a decision on their need for memory care after this brief encounter. If your loved one doesn't pass, nobody tells you, they just pretend like your going to come back and sign a lease and then they have their memory care unit call you the next day. Brookdale puts people in memory care if they don't know the date. That is a great financial decision for them, but not the best decision for the person. I had 2 other facilities recommend my loved one for assisted living, not memory care. The rooms are small, but cute. One complete hallway smelled like a dirty catbox due to one of the residents. That was really the only thing I found bothersome, other than being completely wronged and not communicated with.

Our family had a terrible experience related to safety, cleanliness, and overall care. My husband was able to get out of facility twice. Housekeeping was negligible was odor of urine throughout! The care an services provided were grossly misrepresented from the start. For starters, he was able to get out twice. Once due to gate left open and unattended. The director actually saw him walking outside in front of facility headed to Royal Parkway. On another occasion, he was brought back to Belvedere by a nice couple who saw him walking on Royal PKW and knew something was wrong. It could have ended badly. Food portions were small and cold. I was told he would be assisted eating. Only one of the aides did that. She worked nights and I can't remember name. I took food myself almost every day and left snacks. Prominent smell of urine throughout facility causing feet to stick to floor. The toilet in the commons areas was beyond nasty with excrement and urine. I was told he would have some type of physical activity. That did not happen. One of us (children or myself) were there every day at different times. I realize he had Alzheimer's and might not be able to do group activities, but was told he would get 1 on 1 stimulation during the week. Difficult to get information. I was told I could call and check on him. Seldom did anyone answer...especially at night. It was equally difficult to reach director or nurse for information. One nurse spoke very little English. We were in process of locating another facility when a UTI sent him to [name removed] and thankfully a spot opened at [community name removed] where he received quality care. Interestingly, [community name removed] cost comparison with Belvedere was negligible. Very short staff. 1 person on duty at night. It was a nightmare experience. I don't understand how it has remained open.

The food is terrible. They can't keep staff. They raise the rent shortly after you move in. Someone stole money out of my loved one's purse. We are now actively looking for a new place for our loved one. What you're getting isn't worth what you're paying here at this facility. They have made some improvements in the staff and services, but it's not enough. I would not give this facility any recommendations.

My father lived at Belvedere for 9 months in their Memory Care facility. I would not recommend placing a loved on at Belvedere as we had too many issues with the level of care provided. Hopefully my review will help other families with their search. In spring of 2022, Belvedere Commons was purchased by Opal, prior to that it was owned by Grace Mgmt. After Opal's purchase, they cut nursing hours without formally notifying the residents and their families in writing and many of the staff and nurses have left. The management makes promises but does not follow through. My father fell during the middle of the night and no aide was given to him. He had a cut on his head and they didn't call 911 or have him sent out for evaluation. They didn't notify me. I had to take him to the ER the next morning when I arrived and found blood on his head and on the floor. No one contacted me.. There are many issues at Belvedere and the management team isn't transparent with the families.

We have had terrible communication from them. All the good staff have been fired and there has been a ton of turnover. They have 2 buildings and they only have 2 people on staff for both buildings over night, so they are severely understaffed. They don't try to incorporate activities for those who are less able. Overall we have been very disappointed in our experience and I would not recommend them to others.
