No time to eat when the food is eatable. Food is not cut so clients can eat it. Doesn't get changed when needed. Sometimes bowel movement ignored until it hurts. Rough thinks clients are rag dolls.

This place is a joke 1st shift 2 or 3 care partners 2 med techs. Med techs don't help care partners not there job 2nd shift lucky if we have 2 care partners 3rd shift once again only 1 person on duty they say that all that's needed NO NURSE IF SHE'S HERE IT FOR 3 or 4 hours maybe 3 or 4 days a week no one on duty at night and most weekends Front office are worthless [name removed], [name removed] and [name removed], [name removed] the maintenance is lazy The cooks in the kitchen are wonderful food is great Actives are good try to get everyone to join in Price is high for what you get Don't move here you will be sorry

This is a great community for people who are relatively independent. The activity program is one of the very best around. Unfortunately, most elderly people who are looking at moving to assisted living need special care. Ask any home health professionals who come in and out of this building and they will tell you that the care is seriously lacking. They focus more on renting out apartments rather than taking care of the people who already live there. Look for an assisted living who has a nurse there in person supervising caregivers during every shift (24 hours/day). This place is known among industry professionals as having ongoing problems with State inspections because of clinical concerns.

I would never put another loved one here again. They are understaffed. Room and board is about the extent of it. Food is mainly carbs. Pasta, rice and bread with very little protein. Medical attention is almost none existent. Turnover is high and employees are unhappy as a whole. My heart goes out to those residence who are at their mercy.

I placed my precious husband in this facility in January 2018 as I felt good about the activities, the staff, and the overall "good vibes" of the place. However, things went down hill quickly. [Name removed] hair was not washed, his teeth were lost and no attempt to find them. My girlfriend [Name removed] also went in about the same time and situation was even more severe than my own. She ended up dehydrated and in ICU for over a week. Her husband moved her immediately after that. I didn't; I liked the staff....those I could find. But when I missed two days and came in with [Name removed] diaper not having been changed for two days and his genitals raw, I decided to move him. I didn't; the staff talked me into keeping him there. He was seldom bathed - once it was five days before I got upset with them; the next time was six days and I just took him home and bathed him myself. They "claimed" he was unmanageable and sent him for 3 weeks to a psych ward to get stronger drugs . He returned a zombie, stayed in this state everyday but then they started double diapering him so they wouldn't have to change him as often. By the middle of June I had had it with the lack of proper hygiene and wanted him in another facility before I went to a Family Reunion in Wyoming. Because of the difficulty of getting all the paper work in before I left, the new place agreed to send two caregivers over to Autumn Leaves each day to make sure he was in good shape and bathed. They found him delightful and although he didn't want to be cleaned, the two could do it - not six as Autumn Leaves said it would take. But during my trip, I got a call and the caregivers told me to get him out of there; they could NOT believe how poorly treated he was. I rushed home and moved him on June 28th. Since the move, it's a different [Name removed] ; he's clean, he's not over drugged, he's loved and there are caregivers everywhere when I need to talk with someone. It has been the difference between night and day. If your loved one is early to mid stage in dementia, Autumn Leaves may be fine. If your loved one can bathe themselves, dress, etc. then the list of activities will work for them. However, if your loved one is more serious in the dementia. DO NOT SEND THEM HERE!!!! I have seen two people go down hill quickly and my girlfriend will never recover from her month's stay in Autumn Leaves. My husband [Name removed] is so much better at the new place.

We went to go on the tour that we scheduled and the person that we got was someone that did not know anything about the community. They were very unprofessional and that did not impress me. Also they had two floors and the community was very big. it just did not work out for my friend.

It was so close to the highway and the shopping center. I wonder about the noise level. I was not happy about the monthly cat rent.

have been there for several years hoping it would improve. they are often understaffed and have a tremendous turnover rate. the caregivers are poorly trained and some of them really are in the wrong line of care. the place has a housekeeping issue for the last year which is really reflected int the cleanliness of the place. there is a new director who seem to be trying hard to make the place better. if I were you I would consider other places

We had quite a few issues with this community, and were not happy with it at all. The staff were friendly, and tried to be helpful, but just did not seem very competent. We really had to stay on top of them to make sure things like regular cleaning took place. We also did not like that during the weekends they had temporary staff come in who had no personal connection with the residents. When we were selecting a community we had 3 main requirements. We wanted her clean, safe, and active. We were very disappointed in the level of activity here. When we toured it looked like they had all sorts of things planned, and marked on their whiteboard, but after she had been here a while we noticed that there was barely any participation, and the staff did not seem to actively encourage more people to join. And in terms of safety we never felt that the building was particularly secure. They locked it at night and you had to use a code to get in, but EVERYONE knew the code, and they'd tell anyone who asked. The building also very empty and unwatched at night. They also had a water line break, and it took them a month to get it taken care of, and during that time they would just crowd all the memory care patients into a common area for most of the day. And last of all were the problems with cleanliness. We would often come in and find Mom in bed in her clothes from the previous day, or find that her bathroom was dirty. We were paying to make sure that she had help getting changed in the mornings and evenings, and this was obviously not something that was happening regularly. I would NOT recommend this place to anyone else.

Our experience at The Heritage exposed repeated failures in four basic areas: communication, training, accountability and consistency. Communication broke down between shifts, staff, management and our family. Training became a serious concern when caregivers lacked information about care plans and physician instructions. Accountability was unacceptable when we repeatedly discovered missing blood-pressure documentation and medication discrepancies, including a software issue the Heritage acknowledged had omitted physician instructions. Even after corrections and meetings with management and regional leadership, problems continued. Consistency never followed. What happened often depended on the shift, day or employee. Some employees genuinely cared for my mother, but caring individuals cannot fix broken systems. Families should not have to audit records, verify orders and catch problems themselves. Our question remains: who catches these problems when the family isn't actively watching?

I'll give it 1 star because not everything was bad. I'll start with the positive...the food appeared to be freshly prepared, the activities are nice to get residents and family involved, the laundry of the clothing left them fresh smelling. Overall, the facility can be cleaner...in the dining area there were food stains on the wall and a smell of urine/stool in the residents corridors. The transport wheelchair I brought for my mom came home with a broken brake handle and a stain of unknown origin on the cushion. My mom ambulates in the house unassisted, and the transport wheelchair was used only for outside the home. This information was given prior to her arrival and during her 6 week stay...every visit found her in the wheelchair. She was never allowed to walk, and my mom with Alzheimer's learned how to "walk" in the wheelchair. So, upon bringing her home, my mom lost the ability to stand from a sitting position, had a difficult time bearing weight when i pulled her to a standing position, and her gait became unsteadier. She has regained very little of what she lost there. Another issue, why do male caregivers provide personal care to the female residents (changing diapers)? Yet another issue, upon one visit while she was eating breakfast, I took over because she has difficulty feeding herself. I did not realize until we left the dining area that her nails and hand were dirty...upon inspection it was stool/poop/whatever you want to call it...she was brought to the table for breakfast with poop on her hand...and this wasn't the last time. When I picked her up on her last day she was the most disheveled looking. It was 1130am. She smelled dirty, was dirty (poop in her diaper), and yet again had poop under her nails and on her hand. I will not complain too much about the bruising on her arms and hands because I know that is from (mis)handling...if they were having a difficult time with transfers/position changes then they need proper education and equipment. I've taken care of my mom for 5 1/2 years in my home with no outside help, tackling all of her changes and adapting to them. Now in this new state I can't take care of her safely. She "fell" twice on the same day i brought her home. We immediately had to shower her upon arriving home and it took two people to get her off the shower floor (we used a shower chair and have been for some time without incident). This was the tipping point for my family because it was increasingly becoming difficult to care for her. So, now with the added stress of having to transfer her, we decided to place her full-time outside our home in a personal care home where she is receiving care in a safe environment. I will never recommend The Heritage to anyone, anytime...not even for a day stay. Unfortunately, this is our 4th poor experience with respite stays, but thankfully our last.

This is not a good place. We were forced to leave after 30 days in the best interest of my dad due to substandard care and lack of compassion from some caregivers. I do not recommend. They do not know what's going on in their own facility.

Our family had a deeply troubling experience at Parkside. The facility was consistently understaffed, call lights went unanswered for long periods, and basic care needs were not met despite repeated requests. Cleanliness, supplies, and communication were all serious issues, and the support promised during admission was not delivered. Although some staff were kind, the overall environment was unsafe and unreliable. I strongly urge others to look elsewhere and avoid making the same mistake we did.

I toured The Heritage at Eldridge Parkway and left feeling very uneasy. The building looks nice when you walk in, but the atmosphere didn’t feel warm. Staff seemed busy and not really focused on the residents. When I asked questions, I couldn’t get clear answers, which made me feel like communication would be a problem if my loved one lived here. It just didn’t give me the peace of mind I was hoping for.

toured this community for my loved one and was disappointed. There was a strong odor in several areas, and I didn’t see much for residents to do. Whenever someone from outside came in, staff seemed to focus on them instead of the residents. Management didn’t feel warm or welcoming, and after my visit, I understand why there haven’t been any reviews since [date removed].

Current facility and owners are operating under Heritage ([date removed] to current) NOT Autumn Leaves(pre [date removed]!) So only positive reviews are from YEARS AGO! I could go on and on about everything i saw and how many executive directors have come and gone not to mention staff and nursing directors. The owners work out of Florida and ignored seriously egregious maintenance failures. Anytime looking at a facility ask how long the Executive Director or other core mgmt staff have been in place; that says everything.

horrible owners and mgmt on site that changes nonstop. Lack of Maintenance, washers/dryers and bathing facilities are horrendous. A place for mom is nothing more than a kickback site that presents all facilities throughout rose colored glasses. #shame

[name removed] (Med Tech) has on multiple occasions stonewalled relay doctor information for my grandmother that is living here. Rather than communication and providing solutions, this sole individual has both disrespected my family and created an environment where my grandmothers health is at risk.

First of all, Mom wants to die at her house, but the other siblings want us to put her here, so we did. It was fine, until they didn't want to listen to the POA request, the ones that really care and visit her everyday, even at her house before she got sick. Nurses and others are gossipers here and taking sides. Specially the RN [name removed]/ [name removed]/ [name removed], she's the LIAR, GOSSIPER, MANIPULATOR, FAKE of this facility, she told Moms sitter not to call my husband (POA) when mom had seizures. The only people that you can trust is the cook, [name removed] is so nice and caring, she cut moms food and very gentle to people and the black american CNAs (they're the nicest people that u can ask here). KC and that little nurse were caring too. I didnt like how they handle the family of the patient, they're taking sides. They didn't even know who's the good and bad guys in real life, they didnt know the real stories, they shouldn't take sides, and just be in the middle or be with the patient's side. I call and leave many messages to check or ask questions about my mom and they never answer the phone nor return calls. This a red flag flying high to me. They may not know my name but they don't have every family members name if they don't answer calls. Highly inappropriate for a place to care for a loved when there's no communication. I would not recommend for anyone who cares and love your family. I pray my mom is not being mistreated and God is protecting her.
