This facility is pretty good! I'm not sure about activities, and I think the food is pretty good. If I had one comment, it's that mom is in the TV room almost every time I come. I don't see her doing many...

This was a good facility for dementia care. The bedrooms are spacious and the staff is involved in the overall care, not just mentally, but physically as well.

The staff we dealt with while we were there were friendly. They tried to keep the residents engaged during the day with activities. Anytime I visited, there always seemed to be an activity going on. She seemed more comfortable there, she was starting to form a little group of friends. They have been very fair with their pricing. They have worked with us and have been accommodating.

The memory care is not as well kept as the assisted living side. The activities are an insult to their intelligence. I am finding that I have to go and get her and provide her enrichment, some of the residents are so bored that they want to come with us when I take my her out. The highlight is that she has social interaction with the other residents. In memory care everything is on a set schedule, she feels she has been downgraded. When they go to memory care they just lose so many ammenities, wi-fi, all day coffee and cookies, those things. Lightbulbs need to be replaced, there is nothing that tells you when things happen, who the staff are, when mealtimes bathtimes, or medication times happen. The staff is lovely the residents are lovely. They are understaffed. I feel like I am part of the family. The communication among the staff is lacking sometimes. I would reccommend this complex to others just because of the staff and how genuinely nice they are, things aren't perfect everything is so individual. It's a mixed review, there are definitely things that need to be improved but they care and are genuinely nice. I give them a C+.

We toured this location when we were looking into options for our loved one. It looked like a nice place, it was clean and well kept, and the staff and residents all looked friendly and happy. This was definitely an option we were considering, although we ended up choosing one more conveniently located for us.

The Autumn Leaves staff give dignity to those who need extensive care. It brings joy to my families hearts to see the care given to my mother the staff. It's rare that I give these sorts of reviews, but I have been very impressed by this community.

My father-in-law was there. He liked it quite a bit. It's very nice. They only thing I think they could improve is their food. It's really repetitive and cafeteria-like. It's just not the greatest. I think more options would be nice.

My mother is in this community. The staff is very friendly and helpful; they pay attention to the people. The apartments/room are nice looking and clean. Laundry is done in room with a stackable washer and dryer so we don’t have to worry about mom losing things. The grounds are beautiful with rocking chairs on the front porch, with pretty plants blooming all around. The patio also has chairs with umbrellas. Also church services are available. Mom likes the food, they have many options and you can order breakfast for any meal of the day. Fruit and salad options are always available. They have options and that’s a good thing. Mom is visually impaired and it would be nice to have more activities for people who are less vision dependant. The pricing is reasonable.

Drove by facility and it is beautiful from outside. Very large, new facility. I've been told that these sorts of facilities charge for every little thing...each time patient gets up and goes to bathroom, etc. This charge is added onto the general monthly bill. This would be something to figure out if determined interested.

I really like their administrative staff and the caretakers, they were all very sweet and tried to make my dads stay best as possible and listened to our needs and concerns. My dad is more advanced in his Alzheimer's and they were not equipped to handle his care. I think facilities need to say what they are able to specialize in. This facility is more equipped to handle individuals who have earlier stages of Alzheimer's.

I really liked this community when we toured it, in fact, it was my first choice. The community was very clean and modern looking. The staff was wonderful very professional and nice. However, there were just a couple of small things that I didn't think would be a good fit for my dad. First, when we were walking around there were no places to sit in the hallway in case a resident needed to sit for a moment or two. Then in the dining hall, they had assigned seating for dinner. I didn't think dad would like that very much.

We have been associated with this place since 10/12/16. [Removed] is still out on it overall. Heath people are good and caring, but management and activities people are weak and need to get better soon. Need to get more value for what we are shelling out in $$$............

We are early on in the game and overall it is good, but activities have been lacking. Healthcare people have been very good and concerned about mama's well being. Hoping it gets better and she is comfortable.

My tour of the community went well. I was able to get a lot of informative feedback about what is offered. I noticed there are many resident living in this facility. They were very accommodating with allowing me to visit them the day I called. After visiting I felt this was not a right fit for my dad.

The rest of these ratings come from my dad's perspective of course. He's not been too pleased with the staff. He feels that the nurses are a little pushy as they wake him up at 7 AM to give him a pill and then again at 9 AM for the rest of his pills. He states the dining room is difficult to find a seat due to the people being very cliquey and not wanting to give up their spot. He says the food is not very edible. He was unhappy with the cleanliness of his room for the first three weeks as no one bothered to clean his room until I made a visit to specifically discuss this issue with the director. The size of the rooms are nice and is a good price. They come with a lot of features, including a refrigerator, microwave, and stove. It's been easy to get in touch with the director or nurse to discuss any situation with my dad. I think most of the issues with my dad has simply been that he's an old grouch and really didn't want to leave his home three years ago. He has gotten a little more active with the social activities which they have quite a few of.

The admin staff is amazing. The care team was very nice. I was disappointed that they didn't actually have staff actually consistently check on my Grandfather. I know it's not a nursing home but I feel like he got almost no care after the first week of his respite stay. I also think they should clean the rooms more than once a week.

In July when [name removed] was being placed in Parkside, they were planning on separating men/women floor. This left them with short staffing, which they now have improved and lots of hiring has taken place. I am happy about that. Meals look appetizing and are presented with appeal. The activities were little to none on the men's floor. I have often spoke of this. Most of the 14 men on this wing have little desire to do anything except watch tv, although they now have cornhole boards up there. They have hired a new activity director who is very good and hope the 3rd floor will be included in some of the new activities. I take [name removed] to the 1st floor when they have music. Have had music on third floor once.
The move in process was easy. The staff was willing to help. Staff is friendly. The one major complaint is communication with the family. My brother has gone to the hospital twice and I was never notified by any staff member.
