Was very disappointed in the care they provided my Mother In Law! Used to be a good facility but now all the good employees are gone. Management (the Director) is deceptive and has lied to us many times and did not provide the information that we requested, in addition told an employee to lie about an innocent that Mom was accused of. The Memory care Nurse mislead us several times regarding mom receiving her meds on a timely basis. Many times we were called during the late evening/early morning for frivolous issues that should have been addressed by the staff. Minimal employees or consistent in the memory care area and are not trained enough to understand the needs or requirements of the residents. Aides have ear buds on, no functional walkie talkies, constantly on their phones and I have observed them sleeping on the job. The food is less than desirable, many residents do not get showered or are they monitored when they are wandering through the facility. Do not go!

No contact with Hixon facility, I called several times for a tour and always had to leave a message and I think they only called back once. I was not impressed with their communication.

The staff is excellent. My rooms are cleaned well weekly.

The day shift at CedarStone did a great job. The evening shift had issues. My loved one was considered a fall risk and on many occasions they placed her walker on the opposite side of the room. This happened on the night she broke her hip. The walker was not available when she used the restroom. I would not recommend CedarStone.

It is a nice place. However, they will keep adding different charges onto the bill. When confronted about things that were added that should not have been, they keep giving you the runaround and don't correct it. I recommend having a lawyer look over the paperwork as they have a lot of small print so that they can add BIG charges ! Buyer BEWARE !

I wouldn't recommend the community to others for their loved ones. The community won't be there to assist your loved one with care. The staff communication with the family could use some improvement as they weren't responsive to our calls, or able to be reached when we had questions or concerns. I feel overall the assessment of my loved one wasn't correctly done. The community is beautiful and a big community for staff to get to all the resident's rooms in a timely manner.

When the place was bought out, by an investment company, all went downhill. The new director they fired, had zero people skills. He didn't even talk to the residents. Missed meetings, lied about taking care of issues, and his boss did nothing.

The StoryPoint Director is all “ smoke and mirrors”. She does not manage her people, and does not follow-thru with residents or family when discussions are had. Care staff is nowhere to be found unless you track them down in their office. Residents sit in the lobby a lot, instead of having fun activities to attend. Care plans are not followed: from the simple reminders to residents, helping residents with tasks, and all care plan items. When these issues are brought up with the director, she says she will make sure the plan items are carried out…. They are not. If a resident needs more than meals… good luck! Care is not given! Do not trust your loved ones to be cared for at StoryPoint.

The Director (name removed) tried, she really has a huge job there. Based on move in day, I'd rate staff at a 1. They're too busy to come in and greet new patients, except for the Director. No sign of (name) beyond the selling day visit. Overall, poor value for the money.

The turned my loved one down for care due to care concerns that they couldn't provide potentially all the necessary care.

They need to improve on staffing and training. We picked this community becasue I thought it would be a good fit but they were not equipped to handle my loved one.

This facility has been terribly managed for the last year (if not longer). Linens and towels were not changed and bathroom not cleaned for 4 months. They didn't inform family members of this situation and expected the residents to advocate for themselves. They also didn't notify family members when a resident tested positive for Covid or when he entered the facility with a large bandage on his head. This is unacceptable. I would not place a family member here.

I was very unhappy with StoryPoint. We had a horrible experience. They stole my wife's clothing. Within 30 days, she wanted out, and they wouldn't let her go. The day before she died, they checked her to see if she was breathing, and they wouldn't give her anything to drink. I was really unhappy with StoryPoInt.

Most of the staff was nice and tried to be helpful. Especially one, she did go above and beyond trying to help us. One suggestion I have for improvement is better handling of a situation upfront before signing a contract of any rules. We had an issue with that and they wouldn't budge.

The nurse from Provision Living came out to assess my mother and said they could take care of her, despite her fall risk. So we moved my mother and her things in. They called me the next week and told me they couldn't take care of her because she was falling too much - despite the fact hat they sold me on the fact that they have cameras and other technology in place to prevent falls. They sent her to the hospital and said they could not take her back. Since then, my calls always goes to a voicemail box that is full, so I can't get through to anybody. They said they would send me a refund check, which they never did. And I am left wondering how often they do that to people. I have nothing good to say about the. It's a pretty place that shows well, but we had at terrible experience.

My loved one was there a short period of time. They allowed my loved one to run out of medication. Also, there was lack of communication, when trying to get in touch with the staff. They were not good with returning calls. I would not recommend to other families. Overall, I felt uneasy with the community.

I would NOT recommend them for memory care. They're fine for people who are in wheelchairs and can't get out, but not for somebody more mobile in need of memory care. I spent a lot of money purchasing furniture and outfitting the room. I ended up having to spend the night there and babysit. I was asked to hire somebody to watch my loved one. They're supposed to be memory care, which is supposed to be secure. But my loved one kept getting out, so I had to remove him.

I would tell others that during the time my mom was here, there was a very large turnover of staff. Staff is obviously not happy about their experience working there. The administrative staff - if you are asking questions because you are genuinely concerned, they pretty much just blow you off. We went in and my mom smelled like pee and she had poop in the closet because she was not being toileting properly. She was physically abused by another resident twice and the staff claimed they did not see this happen either time. The second time my mom was fighting the resident off and they both ended up in the hospital. They told us one story, another story in the nots, and then a 3rd story that they told the hospital. Considering the amount of money that we paid, it was a very disappointing experience as the family.

My mother is a current resident at StoryPoint Portage in Assisted Living. I am very frustrated and disappointed. She does not get the routine care she needs regularly. Not all of the staff are friendly, positive or supportive. They have told me that they do not answer her call button because "she calls too often". She has had several falls. One left her very bruised and with several injuries, but it was never reported by the staff. Communication between staff/shifts has been inadequate.

First, during the application process, it was mentioned that we could get out of our lease if additional care was needed. The paperwork that was signed for this addendum was lost so we are locked into a lease for an apartment that my loved one does not live in. We chose a different memory care facility since the memory care unit at Storypoint seems like an afterthought; the staff sits behind their desks on their phones and do not interact with the residents. We were very displeased with the "nickel and dime approach." We decided to pay for cleaning servics, but i found myself scrubbing the toilet since the service was subpar if done at all. Medication assist added more out of pocket expenses. Also, we had to pay extra, for my loved one to be helped up after they fell, one of the many hidden additional costs that we were unaware of.
