Please do not place your loved one in this memory care facility. We would visit my family member multiple times per week and, almost every time, had to find someone to come and clean his room. He would have on dirty clothes and be wet. We spoke with the employees and called supervisors - but were rarely able to speak to anyone - without result. I have never received such poor care and service by a "high end" provider. We removed him after he fell on consecutive days and he passed away 4 days later. It was a horrible experience.

It should never be recommended as a memory care facility!! I had to remove him a few days after admittance because he kept getting out into parking lot and I was asked to spend the night to keep an eye on him and then asked to hire a sitter. All this after having to furnish his room and bathroom!!!! There was no place for him to walk except a small sunny courtyard at Provision. The workers were not attentive to the bed ridden either. Do not recommend this place for memory care!

My experience with provision living was terrible. The nurse came to assess my mom at Porter Hills and said they could take care of her. She was there for a week and they said that they were sending her to the hospital and would not take her back because of falls which they assured me they could handle. But they didn't. The hospital sent her to Saint Ann's where she died. I would not recommend provision living to anyone. It's a beautiful place, but it's deceiving. They fed her in her room and she hardly ate. Medical care was bad. Hospital said Falls were from fluid overload. I'm still angry. When I called to talk to the marketing person, she was no longer there.

My father who had Parkinson’s and dementia was admitted to the memory care unit where he stayed less than 4 weeks. My sisters and I were assured by the staff our father would be assisted with meals and encouraged to eat. However, after a fall at StoryPoint, which was not new to us due to his Parkinson’s, he was admitted to a local hospital and found to be severely dehydrated to the point he had acute kidney injury. He also had a massive weight loss of more than 20 pounds in 26 days! The hospital physician stated our dad had severe malnutrition. The fact that we were not informed at any point of his inability/refusal to eat or drink was very concerning. Our father passed away a few short weeks after leaving Storypoint and I can’t help but wonder if our decision to place him at StoryPoint lead the catalyst that led to his death. Needless to say, I would not recommend anyone place their loved one at this facility. Also, I would rate Zero stars but to submit review I had to choose one.

They are understaffed. Question training to recognize decline in a patient, excessive sleeping, appetite., disinterest in activities.... due to illness, infection? Again, what is the staff's training, when my son in CA recognizes , on the phone ,that something is wrong with his father and no one in person does!

StoryPoint Chesterfield community is terrible..did not refund home care fee and charged a month rent for not even being there. I would give place a negative 10.. and I would not recommend place to nobody!!!

My loved one was previously a resident at Provision Living of Forest Hills and the overall experience was terrible. I was very disappointed with this facility. There was too much staff turnover. The sales person was terminated the day that my loved one moved in. The head nurse said that she was there for the long haul and then quit a short time later. The caregivers need to be trained. They mixed up pills several times, including other residents' pills being left in my loved one's room. Also, my loved one would have to wait an hour or two to get help with the bathroom. The caregivers also were not trained on using a belt for moving a patient. It was a disappointing experience.

While assisting my grandmother at this facility it was horrifying. The staff here were so lazy and couldn’t answer any questions you had regarding times of showers times for activities nothing. Everyday when I would come in everyone would be on there phones in there worker room. You press the emergency button it would take them 30 minutes to get there. As much money as I spend for my family to be here and how worthless the staff and management is here is just awful. I pray for these elders that live here cause they are not getting the care or the love they need in there final years. I hope I can make a difference but I have noticed that every bad review gets taken down.

We didn't leave our mom there very long. Probably less than 3 months - I don't remember the exact dates now. Care was awful. We saw no improvement after talking to the executive director or the head nurse. We had multiple instances where my sister would show up and Mom was filthy. In need of a shower. Under clothes (adult diapers) not changed. Mom ended up in the hospital with a kidney infection and we didn't take her back. The physical facility is very nice. Mom's room was lovely. But the staff was inattentive at best and didn't seem to care at all about the residents.

This facility states they have a memory care unit that meets your loved one where they are and tailors care to their individual needs but it is clear they know nothing about dementia/Alzheimer’s disease and are not interested in caring for those who have difficulty adjusting to their new surroundings and failing health. They want to care for those who are independent and can care for themselves and if your needs are beyond that, your loved one is labeled difficult and the staff will call you multiple times per day to discuss all of the negative characteristics of your family member and ask that YOU come and care for them! If you cannot be with your family member all day they will offer that you can hire a health care companion (approximately $35-37 at your cost)to sit with your loved one. They will not assist in bathing or toileting your loved one and you will ultimately be responsible for bathing them and getting them changed because your loved one “refused” their help.

My mother was at this facility for a short time in 2023-2024. It was a disaster! I do not recommend this facility. Do your research. Review the State of Indiana Social Services records and then make your own judgement.

Harbor Chase did not follow through on their promise to care for my dad who was in hospice care. In fact, the young aides (some teenagers) informed me that they were not trained to care for my dad. As a result I had to stay with him all night to make sure that he was properly attended to. It was frustrating to watch the slow response time time when my dad needed help. At times no one showed up for 30 minutes to an hour. By the time they did show up I had already helped my dad to the bathroom and back. The one time that I did leave late in the night, I returned the next day to find that he was not served breakfast. Please do not refer anyone in dire need of immediate care to this facility. I had to leave with my dad in less than a week there. Management was so appalled at their own inept staff that they gave me a full refund. They were not giving my dad his meds. I would ask the nurse every evening where all his meds were. The day he died I was heading up with the list to discuss. One med, he should have been getting every 12 hours in the VA. Told me that was very important that he got it every 12 hours. That med he was getting at 11 am and then 5 pm , so he would go way too long without that med. I talked to the nurse about that nightly but that didn't change. The first morning he woke up. I happened to get to his apartment at 8 am and he was yelling in his bed help me!! We were told that they would be checking on him continuously but obviously they did not.

They are not professional! They weren't helpful. The food was awful. Staff unhappy . The staff were also uncaring and not available for any questions.

Good food and nice decor do not care for the elderly. The right care does. This community lacks this. If an assisted living community highlights their care with a restaurant and hospitality, that tells you all you need to know.

I would not recommend moving to Sunrise for Memory Care. There staff are not properly trained to work with residents who have Alzheimer's/dementia. They also do not clean the rooms regularly and my parent's lived in filthy deplorable conditions. I would advise those looking for Memory Care to find a place where they solely focus on memory care. Also management is unresponsive to requests or questions. They also raise rates without prior notification or an explanation of why.

My parents have been very disappointed with their stay at Independence Village of Ames. They always seem to be short staffed. There have been time where they have pulled the assistance cord, and no one shows up for 30 minutes so they call 911. The ambulance arrives before staff. They are supposed to have assistance with showers twice a week, but sometimes it's two weeks between showers. The food according to them is terrible most days. They can't seem to keep a "chef." Them saying chef prepared meals is a joke. They now prepare most of their own meals themselves. There's also supposed to be daily cleaning, and monthly deep cleaning, but really it's just once a week cleaning, and no deep cleaning. I'm guessing my review doesn't even get posted, but if it does beware of this place.

This place has had some truly exceptional people working here but unfortunately, they have all left. They treat their staff quite terribly. They bend over backwards to get people to move in and after they do, they no longer care about your concerns. They just seem to be interested in making money. Even when you decide to move out, they don't seem to care because they have already collected a very hefty, non-refundable community fee in addition to your rent. The employees who have left seemed to be tricked into working there just as much as everyone tricked into living there. They outright lie to people to get them in the door. It really is a shame. It seemed like a beautiful place that would add to the beauty of Shaker but it isn't what it seemed. I feel badly for the people still living and working there, although I think it's an entirely new group of employees. They'll surely leave too and then who will take care of all those lovely people?

We feel that we did not get what we had paid for. If it wasn't for the hospice team coming into help it would have been a very bad experience for our loved one.

Be careful!! Our Mom moved into Springvale in September. Less than 3 months later we received a notice of rent increase- of nearly 15%! This left barely a week to make a decision because we had to give a months notice, and knowing that moving would have to be over the holidays. After agonizing for the week, the stress making her ill, my mom decided to move. Springvale is already the most expensive facility in the area, and the extra $ considering the level of service was not worth it. After I sent in our 30 day notice to move her before [date removed], management then tried negotiating with my mom, not once but multiple times dropping the price trying to get her stay, causing added stress. (She is 93 years old) Keep in mind, when you move in, you pay several thousand dollars in NON-REFUNDABLE fees.. so in essence, besides the rent, threw away thousands of extra dollars to stay there a few months. To try and raise the rent by that much so soon after moving in feels manipulative and is un
