The number of caregivers need to be increased. While visiting, there would be 15 minutes or longer when no caregiver was in the common area. When someone needed help, I, as a visitor, would help them. .[name removed] were served cold. Salad would be set on there table with no utensils, no napkins, no drink. My mom enjoyed coffee with each meal. She would not be given coffee unless she asked. Then it may or may not be given to her. In Memory Care, there is no routine except for meals. No music time, no exercise time, no activity time. If there was a craft, it was sporadic. Mom wasn't taken to the Assisted Living side for trivia or other activities she enjoys. She would sit in the dining area, common area, all day with nothing to do. The Executive Nurse was rude and would discuss my mom in front of other residents and visitors. Executive Director doesn't respond to emails for days. The trash in unit is not emptied daily. It accumulates for days which begins to stink. Need for training

The care that we received here was ok at first, and as the months wore on it became worsened. We paid additional fees for additional support and assistance that were not provided, or were provided very half heartedly. Through many specific, documented interactions, and promises made and broken, we lost faith with CedarStone and found different care. Outside of care, there were times over the winter where there was not sufficient heat to support the residents. Supplemental heat was provided, but this was a concern for us going into the upcoming cold season, as a rather new facility would hopefully not have these ongoing issues.

Lakeview Terrace is always short-staffed. There are employees smoking pot before their shift in the parking lot.

I would not recommend that place for someone with even mild behavioral issues as most memory care patients have, they were not well trained or experienced. The charge nurse pretty much said my mom was disrupting there routine and wanted to put her on an antipsychotic drug. A few days later mom ended up in ER unresponsive very close to death and placed on hospice. They refused to have her back in the facility despite the fact that she was totally unresponsive, she died a few hours later in the ER. This was [date removed]. So no I would not recommend the memory care unit to anyone.

Unprofessional, disgusting and staff lie directly to your face! Gourmet meals? Yeah, try disgusting slop! Not worth the outrageous pricing! Price Gouging to the max! Would never recommend to anyone! You should be ashamed of yourselves!

The sales team is really good, but the follow through with what is promised lacks integrity. The onboarding and evaluation of our memory care patient was haphazard at best. No real time was taken to determine what our patient needed. Verbal recommendations from family were ignored. We would hate for anyone who is dealing with a loved one who is suffering from any disease needing memory care to go through this experience.

This has been a horrible experience. They are in the midst of a reorg and I would not be surprised if it is closed like the IV in Zionsville. Head of nursing incompetent and doesn't care. New director trying but not making an impact. Most food terrible and the chef actually came out and told my dad that he needs to shut up and eat the cold food. His room wasn't cleaned for 8 weeks and new maintenance guy let trash build up in the trash rooms for weeks without emptying so the stench was palpable.

This facility has major management issues and appears far more concerned with making money than they do with providing appropriate care. If you have any other options, send your loved one somewhere else. They may live longer because of it.

Please do not place your loved one in this memory care facility. We would visit my family member multiple times per week and, almost every time, had to find someone to come and clean his room. He would have on dirty clothes and be wet. We spoke with the employees and called supervisors - but were rarely able to speak to anyone - without result. I have never received such poor care and service by a "high end" provider. We removed him after he fell on consecutive days and he passed away 4 days later. It was a horrible experience.

It should never be recommended as a memory care facility!! I had to remove him a few days after admittance because he kept getting out into parking lot and I was asked to spend the night to keep an eye on him and then asked to hire a sitter. All this after having to furnish his room and bathroom!!!! There was no place for him to walk except a small sunny courtyard at Provision. The workers were not attentive to the bed ridden either. Do not recommend this place for memory care!

My experience with provision living was terrible. The nurse came to assess my mom at Porter Hills and said they could take care of her. She was there for a week and they said that they were sending her to the hospital and would not take her back because of falls which they assured me they could handle. But they didn't. The hospital sent her to Saint Ann's where she died. I would not recommend provision living to anyone. It's a beautiful place, but it's deceiving. They fed her in her room and she hardly ate. Medical care was bad. Hospital said Falls were from fluid overload. I'm still angry. When I called to talk to the marketing person, she was no longer there.

My father who had Parkinson’s and dementia was admitted to the memory care unit where he stayed less than 4 weeks. My sisters and I were assured by the staff our father would be assisted with meals and encouraged to eat. However, after a fall at StoryPoint, which was not new to us due to his Parkinson’s, he was admitted to a local hospital and found to be severely dehydrated to the point he had acute kidney injury. He also had a massive weight loss of more than 20 pounds in 26 days! The hospital physician stated our dad had severe malnutrition. The fact that we were not informed at any point of his inability/refusal to eat or drink was very concerning. Our father passed away a few short weeks after leaving Storypoint and I can’t help but wonder if our decision to place him at StoryPoint lead the catalyst that led to his death. Needless to say, I would not recommend anyone place their loved one at this facility. Also, I would rate Zero stars but to submit review I had to choose one.

They are understaffed. Question training to recognize decline in a patient, excessive sleeping, appetite., disinterest in activities.... due to illness, infection? Again, what is the staff's training, when my son in CA recognizes , on the phone ,that something is wrong with his father and no one in person does!

StoryPoint Chesterfield community is terrible..did not refund home care fee and charged a month rent for not even being there. I would give place a negative 10.. and I would not recommend place to nobody!!!

My loved one was previously a resident at Provision Living of Forest Hills and the overall experience was terrible. I was very disappointed with this facility. There was too much staff turnover. The sales person was terminated the day that my loved one moved in. The head nurse said that she was there for the long haul and then quit a short time later. The caregivers need to be trained. They mixed up pills several times, including other residents' pills being left in my loved one's room. Also, my loved one would have to wait an hour or two to get help with the bathroom. The caregivers also were not trained on using a belt for moving a patient. It was a disappointing experience.

While assisting my grandmother at this facility it was horrifying. The staff here were so lazy and couldn’t answer any questions you had regarding times of showers times for activities nothing. Everyday when I would come in everyone would be on there phones in there worker room. You press the emergency button it would take them 30 minutes to get there. As much money as I spend for my family to be here and how worthless the staff and management is here is just awful. I pray for these elders that live here cause they are not getting the care or the love they need in there final years. I hope I can make a difference but I have noticed that every bad review gets taken down.

We didn't leave our mom there very long. Probably less than 3 months - I don't remember the exact dates now. Care was awful. We saw no improvement after talking to the executive director or the head nurse. We had multiple instances where my sister would show up and Mom was filthy. In need of a shower. Under clothes (adult diapers) not changed. Mom ended up in the hospital with a kidney infection and we didn't take her back. The physical facility is very nice. Mom's room was lovely. But the staff was inattentive at best and didn't seem to care at all about the residents.

This facility states they have a memory care unit that meets your loved one where they are and tailors care to their individual needs but it is clear they know nothing about dementia/Alzheimer’s disease and are not interested in caring for those who have difficulty adjusting to their new surroundings and failing health. They want to care for those who are independent and can care for themselves and if your needs are beyond that, your loved one is labeled difficult and the staff will call you multiple times per day to discuss all of the negative characteristics of your family member and ask that YOU come and care for them! If you cannot be with your family member all day they will offer that you can hire a health care companion (approximately $35-37 at your cost)to sit with your loved one. They will not assist in bathing or toileting your loved one and you will ultimately be responsible for bathing them and getting them changed because your loved one “refused” their help.

My mother was at this facility for a short time in 2023-2024. It was a disaster! I do not recommend this facility. Do your research. Review the State of Indiana Social Services records and then make your own judgement.

Harbor Chase did not follow through on their promise to care for my dad who was in hospice care. In fact, the young aides (some teenagers) informed me that they were not trained to care for my dad. As a result I had to stay with him all night to make sure that he was properly attended to. It was frustrating to watch the slow response time time when my dad needed help. At times no one showed up for 30 minutes to an hour. By the time they did show up I had already helped my dad to the bathroom and back. The one time that I did leave late in the night, I returned the next day to find that he was not served breakfast. Please do not refer anyone in dire need of immediate care to this facility. I had to leave with my dad in less than a week there. Management was so appalled at their own inept staff that they gave me a full refund.
