Due to being short handed, many of the activities planned can't be completed. The facility is a beautiful place and makes a great first impression; I was just disappointed that promises made to get someone to move in, wasn't true. The person in charge of the memory care unit is very nice, I have no complaints with her. However, the staff is another issue. They are forever short staffed; an issue I specifically asked about prior to moving in.

She hates it there so it hasn't worked out. They keep raising the rates, the food is pretty bad, and they nickel and dime her for each service e.g. putting her sheets on her bed, bringing her meal to her room, etc. particularly since they charge so much. Approx 1/3 of the occupants have left due to huge price increases in the past couple years. the economy is a problem but they're really taking advantage of elderly people.

While our experience during the first few months was generally positive, we became increasingly disappointed after our loved one transitioned from assisted living to the memory care unit. Communication and attention to care significantly declined. For example, we signed paperwork in November to begin podiatry care, but by March, it became clear that he had never been seen. His toenails were severely overgrown, and no one had followed up with us regarding the delay. There was also a fall incident where protocol required calling an ambulance and sending him to the hospital. Unfortunately, we were not notified until an hour after he had already been transported—and to the wrong hospital—despite having a preferred facility clearly documented in his file. Later, when our father was placed on hospice care, the staff failed to inform the hospice nurse that his diabetic test strips had not arrived from the pharmacy and that he had run out of insulin. As a type 2 diabetic, he went four days

The residents are nice. They can improve on repairs, advocating for their residents when loss happens, they need more qualified maintenance people and the required tools, needs to make residents needs a priority not the convenience of the staff. Also need to expand transportation services in terms of hours. Hire a bus driver and not make activities person do 2 or 3 jobs.

Food isn't very good needs better nutrition and organization. Dinner time tonight was 5:00 we were served at almost 6:00 construction going on at all hours. For the amount I'm paying I think it would be a lot better

My elderly sister was left with wet and soiled diapers until family came to change them. Staff rarely came to her room. Meals were not given if she could not get to the dining area. She became very sad and homesick. We are looking for a better place.

It wasn't a good experience for our family. They do not have very good communication and the staff was short handed.

They lied about everything. This facility is the absolute worst. The CEO doesn't care about the residents and the sales lied about everything they have and offer. You don't want to be a part of this company. They are terrible people that run this place.

It looked like it would work for my sister. Unfortunately, it really did not provide enough assisted living support. Plus, the only elevator kept breaking. I've moved her to a new facility

They offer dementia care, however, I question their staff training in regards to managing patients' behavior. My mother would be allowed to walk back to her room from common areas such as the dining room unaccompanied. When she would wander into other patients' rooms, staff would then have to try to retrieve her. Some of the staff's tone and approach were not friendly, and my mother would become aggressive. They then said that she was not fit to stay there. They sent her for a psych evaluation and determined that while she was gone. They didn't adjust her meds or her care plan before making their decision.

Cleaning services and laundry are not being well managed at all. I would not recommend at this time. There is also no security in new wing completed.

Big turn over and not enough staff. At the moment we would not recommend StoryPoint Shaker Heights to other families.

Huge turnover in staff. Not a good sign!! You are only as good as your people. When you have good people you take care of them, because they take of you!! If you are really concerned, find out why the big turnover in employees.

Care services - when we needed more assistance, it was a 1 - lucky we had outside options. Never again saw the person that promised us a rose garden! The need for additional support got expensive fast!! The studio apartment was small, we were on the wait list for a one bedroom but after a while my dad didn't want to make the move again, the facility was located in Plymouth, Michigan. Dog noises were a problem, and smells were also an issue.

My aunt (95 yo) paid for care from about [date removed] through the end of [date removed] She moved in on [date removed] and she passed away on [date removed]. The community fee was $[fee removed]($[fee removed]higher than what we originally talked about). She fell a couple times. The first fall took her to the ER with a broken leg and subsequent surgery. Then rehab for about 6 weeks. All the time she wasn't at story point, she was being charged for "care" . We told them what she would eat, which was soup, ice cream, Coke. They said she wouldn't eat when they continued to give her sandwiches and other full meals. They went through a box of laundry detergent much quicker than I thought it should have been. They said she had diarrhea all the time and they had to wash her clothes. Well who's giving her all the medications to give her diarrhea?? Her rent increased by $[fee removed] per month within 2 months of moving in. When she came back to story point from rehab her care cost had increased from $[fee removed]to $[fee removed].

Food is very poor. It was down to 48° last night. 52 now and we have no heat in my apartment. This is absolutely no place for Mom. I am supposed to use more than 200 characters. But I do not need that many to express my disappointment in living here.

The memory care floor always smells like urine, the floor in her room isn't cleaned well or often, her bathroom isn't cleaned properly, they never dust. The food is truly awful. The executive director isn't accessible or engaged in the daily operations. The receptionists are wonderful, kind and compassionate; as is the maintenance supervisor, he's been so good to us.

If you want adequate care for your loved one while in residence at Provision Living, hire an agency caregiver to be with him/her during waking hours to ensure meds are administered on time and to ensure adequate hydration. Too many residents to too few staff.

CAUTION: Danbury Tallmadge is currently ([date removed]) offering a ticket to win a gift basket for every positive review posted. It doesn't sound ethical to me, but I suppose it's probably legal, so that's good enough for them. I have had a loved one here for over three years. At first the care was quite good and everything went very well. Staff seemed happy and liked the workload and management. Since then StoryPoint, an investor group, has taken over and things are nowhere near as well run. While there are still some committed staff that have Resident Care as their first concern, much of the staff and management are no longer interested, or maybe just not capable of providing changes to ensure basic services are provided. Repeated requests to direct care staff have them agreeing that there's problems, like lost laundry, but they have no idea how to fix it. Management promises over and over to fix issues, but they persist.

Physically it is a nice facility and a nice location. The staff were pleasant but overworked, there was a lot of turnover in the two months my parents lived there. The administrative staff were not competent - they could not provide information about when a resident was last bathed, they changed their pricing policy within the first month without any written notice and felt it was appropriate to do business this way. Their billing statements were difficult to comprehend and my father and I would get different statements for the same services. It was not a good experience.
