Big Mistake! We selected this community after coming to the painful realization that we could no longer keep our 91 year old mom at home any more. They will promise you the moon to get you in there. We didn’t need the moon, just a safe, dignified facility where mom would not be afraid. The medication errors run rampant at Clayton Oaks. The medication distribution itself is sketchy at best. The residents are treated with no empathy or dignity. They are left to sit in soiled diapers. This facility has no knowledge of the proper care of Altzheimer and dementia patients. They are more conserned with projecting an opulent impression in the assisted living areas instead of focusing on safe regulated patient care. Luckily we descovered this after 5 months and are moving mom. Unfortunately anytime you relocate an Altzheimers patient there is some degree of decline. A consequence that our mom and family should not have to bear. Heartbreaking.

Don’t let the beautiful facade of the building fool you! For inside it’s walls you will find the rot of greed and the stench of apathetic management from the director’s office. Specifically, [name removed] management of the Med Techs and their extremely high turnover rate for these positions are a life and death issue that he will talk in circles about for hours while our family members suffer immensely. Instead of hiring experienced Med Techs, the person emptying the trash this week may very well be administering life saving medications to to your loved one next week. Of course these inexperienced housekeepers (”med techs”) are going to make serious mistakes that will affect the health your love one. ZERO STARS even though I had to give them one to be able to post this message! BE AWARE!

Do not take your loved ones to Skilled Nursing at The Forum of Tucson. They left my 92 year old father sit in a wheel chair in wet diapers for 5 weeks ... he slept in his clothes ... he did not get the wound care needed to get rid of bed sore nor the physical therapy needed after his fall/broken hip. All they care about is the money .... they actually charged him for a private room for 4 weeks of his stay when he was in a semi private room. Nurses kept telling us he wasn't getting better because of his diet when in fact they were preparing his food (and my father always eats everything he is served). Amazing enough after getting him out of there his wound was totally healed in less than 2 weeks and he is out of the wheelchair and using a walker.

It has been a couple of years but I hate this place for what they did to my husband. I don't know anything about the assisted living and I'm sure that it's a totally different ballgame. In fact, I've heard wonderful things about other Brookdale locations. But my husband was in Memory Care for 3 weeks until he lost 22 pounds (in those 3 weeks) and came down with pneumonia--after which I cared for him at home. He was fairly helpless and they did nothing for him--they didn't help him change his clothes, help him in the bathroom, and worst of all, they didn't help him eat. Their food was terrible and he wouldn't eat it even when I hired my own people to take care of him for part of the day. I think the problem was the upper management--the pay for the employees--and the fact that they refused to hire enough employees.

Administration is not helpful and they have violated my mother's Resident's Rights mutliple times. They will not provide her charts to me despite my written request following their handbook. I was told because the Director is sick despite the Garden House Director, regional nurse and VP were all included on the email. I am her POA. I hired a nursing consultant and provided written authorization for all records to be provided to her and the General Manager refused to provide my mom's charts and is refusing to give them to me. There were unauthorized medication changes and was a struggle to get them back per her doctors order. It took almost a month to get her care plan, despite my multiple requests. This is to be reviewed and signed within 14 days if changes.

Do not put your loved one here. You will regret it! The care is substandard at best. There is a tremendous turnover in staff, utter incompetence and no communication. My father became more ill in his time here. Wouldn't recommend it to my worst enemy. Keep looking for a facility.

I was just in The Forum for 22 days and it was a nightmare. I would NOT recommend the skilled nursing for anyone. I had my knee replaced and a CNA [Removed] me over on my sore knee when she was taking care of the bed pan. I am planning on reporting this rehab center. I was also left on a dirty bed pan for 20 minutes before anyone responded at all. One of the nurses messed up my medication and it could have been a HUGE problem.

The staff is nice and gentle and kind as long as family members are present, but can be very unkind when alone with the resident. I would recommend you look elsewhere if your senior is unable to stand up for themselves. The staff is not above retaliating against the senior, if complaints are filed. The retaliation is verbal and hurtful, not physical.

Skilled Nursing Facilities were poor at best. The ability to reach a patient or the nursing staff was almost impossible. The level of communication is so low--it makes me question the whole facility. Patient personal care was very lacking!! This is suppose to be a rehab unit and very little has occurred--I would strongly consider a different facility.

Christina Millay lies - the employee and resident opinion surveys were not wonderful. She responds that they were positive and that this indicates that this is an indication of heading in the right direction. Wrong - all you have to do is walk into the community and see for yourself. The residents and staff are so unhappy. Way to go Leisure Care - good choice.

Hate it,,,,,,since the change in management everyone hates it. Owners don't care

My mom lived here for 9 years. It used to be great, but since the change in management, the place is terrible. It's cold and unfriendly. Many residents share that feeling. The assisted living is a joke. We paid for showers for years and my mom rarely received them. We paid for med services and had to continuously monitor to make sure that meds were given at the correct time. My mom's doc ordered a stool sample and it took them 6 months to get it. The final straw was when my mom had a bowel accident. My friend was with her and let me know. I called 3 times for a caregiver to help clean my mom and even offered to drive from home and do it myself. I was called and told the caregiver had cleaned my mom. The next morning, my mom was still sitting in her soiled briefs. They left her like that all night. Shame, shame on Leisure Care, who is aware of the problems there and allow them to continue. I moved my mom and 12-15 residents also moved the same month. The head chef subsequently walked off the job and numerous other employees have too. To top it off, nobody from the management team even said goodbye to my mom. They'd take 9+ years of rent, but no goodbye. This place may suffice if you or your loved ones are fully independent, but if they need any assistance, forget it. Run, don't walk, away from this place.

I have heard nothing good about Broadway Proper from any of the residents. People are moving out by the dozen because of the coldness and the lack of follow through of the management team. Leisure Care should be ashamed of themselves for allowing this to happen. I moved my family member out very shortly after moving them in. Work orders don't get complete, assisted living staff doesn't know what they are doing etc.
