Since 2021 and the Covid pandemic, this facility has be sold TWICE and purchased by large corporations. The quality of living and care has decreased with each sale and the introduction of a new administration. The employees work very hard to provide the quality of life these residents deserve, but turnover is high and the facility is chronically understaffed which leaves residents underserved and at-risk. Those who live in Memory Care, unable to communicate their needs effectively, experience neglect when there are too few (and too many NEW) caregivers to meet their needs. This vulnerable population needs a full staff of caregivers at all times including someone to engage them with activities.

Only had my parents there 6 weeks as their apartment was broken into and had valuables stolen...horrible experience...completely unsafe for the helpless elderly and will never recommend this place due to location...sad but true. The staff are great...the location of the facility is totally unsafe for the elderly

Our loved one was at Broadway Proper (Stellar Living) for 5 years and at first it was good. As time went by he started to have falls, was not participating in any activities, not doing laundry, wearing dirty clothes, not showering for over a month, unable to take his medications, dementia getting worse and he needed more supervision/memory care. He was told that they were helping him to stay independent and make his own decisions so when moving was mentioned he was adamant that he would not move until they told him he had to. We asked [name removed], Director Of Operations at Stellar Living in Utah, to request they tell him he needed to move but they refused and just kept charging more each month to provide more services. There were services they could not provide such as monitoring his water intake as his kidney function was declining. When he started to cause problems, they asked him to move. We regret the time he just sat in his room and could have had a better quality of life.

What you read about is not what you get in memory care here . No activities , laundry lost and occasionally not done . Management is not receptive to family concerns. Once a loved one is in a facility it is hard to move them.

This retirement home is beautiful and the aides and front desk staff are so warm and kind they go above and beyond to care for the residents despite the head nurse’s unprofessional behavior towards staff in mixed company. Unfortunately they are infested with bed bugs! The nurse Sarah doesn’t want staff to know because she doesn’t “want to alarm the residents “ Don’t let the looks fool you!

Our loved one was in the Memory Care unit of The Forum at Tucson for almost two years. We experienced many ups and downs while there, and as their disease progressed so did the stress we all felt. Towards the end of our family member's stay there, no one wanted to even visit. While some of the aides were very kind to us, several others were rude, unprofessional and combative. It would not be uncommon for aides to challenge you, accuse you of causing problems and tell other healthcare professionals not "to listen to the family". However, these same unprofessional behaviors started at the top with the Memory Care Director to the Executive Director. We often felt they must've taken a corporate-wide session in gaslighting. Anytime there was an issue, the family was texted and/or called and expected to drop everything and rush to The Forum to handle the problem. They offered little help and guidance when behavioral and/or health issues came up. It was always the family's problem. There was no team mentality and the family was often left to figure out on their own what to do and how to handle it. Keeping your family member fed and alive seemed to be the extent of their "care". When we moved our family member out, the majority of their clothing was missing with other residents' clothing/undergarments/linens/towels there instead. We spent a small fortune in their two years replacing clothing. Care assessments were conducted without actual interaction with the resident. It was left to the discretion of the Director, who sat at her desk and clicked buttons, as to what level of care, which was often the next level up in price. The final straw was the absolute lack of medication management (which is a monthly fee). Prescriptions would be left to expire and/or tossed away, and the family was never notified. Until a medication was needed and once again, the family was expected to drop everything and get the script renewed, which often meant a visit to their doctor. All of which is hard on the resident and the family. While The Forum at Tucson seems to work for some, it was a stressful experience for our family. In hindsight, we wish we had placed them elsewhere from the start of what is already a very emotionally taxing journey.

I was previously a resident at Villa de San Antonio. The meals and the menu were excellent. The dining room staff were excellent, their efforts were much appreciated in making sure that you didn't dine alone. The housekeeping seemed to always be on the defensive, not friendly at all. I was in Independent Living and I expected to be seen as a full functioning adult and human being...intelligent and able to function independently. I see aging as a natural progression and expect to be treated with mutual respect. I have no idea how they can improve since I didn't like their mindset once I had moved in. I signed a form for no one to enter my room unless I was home, but they came in anyway. I was also required to sign a form that I would pick up after my dog (along with all other pets owners) However, very few picked up after their dog and there was dog waste/elimination everywhere. Additionally, one of the doors was always propped open, there was no key fab access to this side door and the area was not safe for me. I was very scared, always feeling not safe. Sadly, you have to move in before you see all of these things.

Housekeeping is terrible. They lost all my mothers good linens and now she has crap that looks like it came from goodwill. Her clothes go missing all the time too. The staff in memory care act annoyed when residents need extra care and attention. I can't wait to get her moved!

My loved was able to physically walk, unassisted into Jamestown memory care. After 20 days in Jamestown they could not walk, had 18 documented falls, and was medically confined to a wheel chair. After transferring to a new facility there are walking again after 10 days, and has had zero falls. Needless to say I was not happy with the care provided and would not recommend this facility to anyone,.

I was not impressed by the tour provided by the Food Manager. It was quick and it felt like we were in their way.

To make improvements to the community they really just need to clean house - some of the staff members are incredible and others are just awful. The current administrator [name removed] is fantastic. The activities are pretty lame and if they do have activities that day they are usually cancelled. They need to hire new kitchen staff and work on providing meals that cater to those with certain dietary restrictions like diabetes. The meal choices are just not good and the kitchen and is extremely dirty and the refrigerator is not kept cold enough.

We had a miserable experience at Montebello. We have tried to call and talk to anyone interested in resolving our issue, but to no avail. No one including the Executive [name removed] returns phone calls!

I felt like the director was out of touch, even after meeting with him twice, and only cares about the money. The director is very arrogant, uncaring and rude. They allowed our loved one to fall 4 times, requiring them to be hospitalized.

My grandma lived here for 4 years and when they changed names and administration a year ago, it all went down hill. [Name removed] is arrogant and unwilling to admit shortcomings. The facility is beautiful but the staff turnover is incredible! they are not able to keep a nurse on staff and the neglect of my 98 yr old grandma has gotten so so bad that we had no other choice to move her. I have reached out to corporate over and over. I have left reviews and their response is a generic asking for me to reach out and I have SEVERAL times with no response. If you are looking for a move independent 55+ community then this place would be ok. If you need an assisted living place then go somewhere else.

Some residents urinate, defecate, spit in the independent living areas. Cars in parking areas vandalized weekly, food sub standard.

Do not put your loved one in Jamestown in Provo. We had my mother there for less that a month and it was a terrible experience. They don't have the staff to do what they will say they will do. They were extremely rude to my mother and DID NOT inform visitors when most of their residents were in quarantine with covid. Memory care is dirty and the have the residents wear dingy towel like bibs. Our mothers sheets were not changed, her laundry was not done, they said they didn't have the staff to help her shower. The directors are too ill with illnesses of their own to Train or help with any issues (this information came from the director of memory care-[name removed].) Now that we have her moved to another care center, we see how terrible Jamestown Memory Care really is. DO NOT put a loved one there. We wish we could have seen the truth about this place before putting our mother there. I feel badly for the residents that are still there. So sad.

Our family had an in person tour and we were not very impressed at all, there was just a lot of residents wandering around and no staff to really redirect them anywhere. It was during covid and it seemed like they had no limitations on visitors - people coming and going without getting their temperature checked or signing in/out. We just did not get that warm feeling.

WHEN I ASKED SPECIFIC QUESTIONS ABOUT ASSISTENCE WITH DIFFERENT THINGS AND THE COST.... I WAS GIVEN CONTRADICTING 'INFORMATIONN FROM DIFFERENT EMPLOYEES. WHEN I COMPLAIND ABOUT THE LACK OF CONTINUETY, I WAS TOLD, "IM SORRY BUT WE CAN,T ACCEPT YOU.... YOU'LL HAVE TO LOOK ELSWHERE."

It is my grandmother who is currently living in the independent living section. She has been here over a year and I am seeing no improvement in regards to there response to requests made by my mom. They will wait days and even weeks to fulfill requests such as cleaning spots off the floor or replacing fallen blinds from the window. The most recent incident is that my grandmother vomited and had diarrhea all over her bathroom floor. We were promised by lady at the front desk in the morning to have it cleaned by the time we got back from the hospital. We came back tonight and it is still the way it was when the paramedics were here this morning.

Nice building and nice furniture, but staff are rude and snobs. Very unfriendly employees. They will act nice when family is present, but beware of how they interact with their residents when they think no one's watching.
