I will NOT recommend this place to anyone. Very nice facility, but very poorly run. Honestly buyer beware. They have top down management problems and very poor supervision. Till they are replaced nothing can change. My only advice to others is don't put your loved one in there.

Seagrass Village Gulf Shores, is not as nice as presented. Management is seriously lacking in decision making, and consistently uses "well no one else has had that problem" HA, that's almost funny. The food is not chef inspired, and a lot of times is barely edible. Employees turnover is a serious issue as well as allowing certain residents do what they want to do, even though it's not fair to all residents. Just beware, it's not as nice as advertised

We moved my mother-in-law out after a few months. Memory care left much to be desired. They got her meds wrong, several times, Not many activities she was bored. Landry lost or clean laundry on floor of closet, I went on early to clean her room while she was eating breakfast to find a man sleeping in her bed. We moved her out. Moved her to [community removed] that was a much better place with lot's of activities . Now has more medical issues and was moved to [community removed]. They have been good so far. Medication was given improperly. Several times when her blood pressure was to low. Very difficult too. Find the staff in memory care especially at night. I could not get out after checking on my mother-in-laws heart monitor for over 20 mins. I could not find a single employee on the floor. I found a man in my mother-in-laws bed one morning when I went in to tidy it up while she was eating breakfast. I did get staff to get him out of her bed before she came back. I do not know how many of the staff have been changed. We moved her within 6 months of her being in Westhill. Hopefully they have made changes.

I have to be honest - the initial experience was amazing - [name removed] (sales) was beyond kind & great at sales. The location, Rooms & balcony view was amazing. Beautiful landscape, and facility. Our Dad was happy to go there. And we thought it was the perfect location for him & his cat. However, after time went on, it became a significant challenge. Billing was often incorrect, made excuses for random charges not applicable, extra fees assigned. Shuttle was seldom available, no driver, or broken down & my sister consistently had to take time off work to provide a ride. Medication management charges kept changing & seldom did the clinical staff follow up with his physician. One day he left the building with another resident (we are aware in FL, that the residents legally are allowed to go out on their own being in ASL unless in SNF. They are to sign out & back in when doing so. However, no staff at the front desk means no one had a clue he or the others left or returned. Which he had been gone for several hours & none of the group he went with signed in or out! No one had a clue at meal time they were not back. This item was escalated & no return calls occurred until we escalated to the corporate office & even then dis not get a call back for a week! I've worked in health care for 36 years & the fact of not even knowing if they returned is beyond disturbing. FL law or not. The staff(s) after about two months of his residence, started resigning. The front desk was serving meals, medication management was delayed & at one point they "doubled up on medications" because they did not have enough clinical staff to distribute the medications at their assigned intervals. Honestly, I could go on. This was an experience where the pretty landscape, facility & packaging as they say absolutely did not represent the product. I apologize for the lengthy response but I didn't feel my reply to the survey only & answering with a number response would accurately represent what is really going on at this location.

I have never as much sent a complaint letter in my life or even done a review I understand that some places my not be good fits, and I accept that Our family accepted that after what happened and was ready to move on. But the follow up meeting is what pretty much set things off with misrepresentations, promises that we're not kept , and subsequent radio silence. Improvements needed: -better screening -actually do what you say you will do( if you don't know how to redirect an Alzheimer's patient , don't say you can - return phone calls -when mistakes are made, admit them, and move on , don't misrepresent, or lie about them - follow up on what you say your going to do - there is no reason for someone to be spending hours/days trying to get you to do what you said you would do

These answers will be a mix of what I have experienced living in the cottage and visits to the main building for meals. 1The cottage is nice (they are new) with maintenance issues that are unresolved. The 1 is based on the main building restaurant which is supposed to be Five Stars. Since I am in independent living I am unable to provide a good answer. However, from what I have heard the care is not good. The food is not nutritious for seniors and served cold when it should be hot. They went from a dinner menu to serving soup and some kind of sandwich/wrap for dinner. Most of the meat is tough. Hamburgers are your best choice for meat. They discontinued serving salmon which was pretty good since they do not have the bourbon sauce. Many complaints since I have been here with no improvements. They have placemats that are not replaced instead of having tablecloths which should be replaced every day or disposable placemats. Overall very disappointing since we are paying each month in our rent for quality food. Servers are questionable. Not sure where they are getting some of their staff. The ones I have experienced were good Since this is my first experience in a Senior Living community I cannot provide a good answer. Other residents that I have met and have lived in other Senior Living Communities (Florida) are very disappointed with "Vitality." Residents have moved in and than out since I have been here. Hopefully with the new ownership of Pioneer Senior Living positive changes will be made. Only time will tell!! I am hoping as well as all the residents that everything will have a dramatic improvement. The facility is in a great location and it has the potential to be a wonderful living area for seniors.

My loved one was not dressed most of the time, not checked on regularly, they would not use their military pharmacy and ordered medications from THEIR pharmacy (at an outrageously expensive rate), and I didn't believe that they were administering her medications as prescribed. Also , someone was supposed to be there 24/7 to answer inquiries but it was almost impossible to get anyone to answer the phone after and I would have to leave a voicemail for a return call, which was not returned, usually until the afternoon

Those who are there review your bills very closely. Residents as well as staff that have been there for years are extremely upset with the way this facility is run. My loved one passed away [date removed]. Creekside was a terrible experience. They don't know how to handle dementia patients. They let him do whatever he wanted wouldn't get him to shower. Wouldn't get him to eat after five days. I had to pick him up and take him to the VA hospital.
virtually everyone was competent and professional, the facility was beautiful and clean, the food was extraordinarily good for a facility, they had multiple activities every day... They also have polished marble floors in the bathroom... even in memory care. In addition, there were no covers or precautions to prevent residents from turning the water on by themselves. After a month of improvement and doing well, my husband fell 4 times in 9 days. His last fall, he never recovered from, and after a hospital stay that required a second brain surgery he came home under hospice care, never walked again and passed four months later. I complained about the floor multiple times and nothing was done, the staff even complained when I brought in a non-slip rug and often removed it... I visited several times before moving him there. The residents were clean and engaged. I thought I was doing what was best for him. If someone didn't have mobility or memory/fall risk issues it may be fine. However; I still believe the bathroom floors are a critical design flaw and safety issue for anyone. The staff knew it was an issue, I was there one evening as they mopped the floor and was warned to be careful and not go into the bathroom because it was slippery... and I am able bodied. His room had premium vinyl flooring... only the bathroom was marble.

This place has been a disaster for my parents since we moved them in 3 months ago. There is no food service director, no memory care director, and the executive director is a constant turn over. The place was built to look nice, however, everything seems broken. No attention to detail. No one seems to care about how the residents are. I have not heard one positive experience from any of the assisted living or independent living residents. Their words: "I've just given up".

My mother was there for two months. She hated it there. I hated it as well. People sitting in wheelchairs and urinating and no one noticing it. A woman walking down the hall after having soiled herself, crying and no one noticed. One of the servers at meals practically threw the plates on the table. No respect at all for the patients. We moved mom out of there. She lived with family until she passed away last [date removed]. She was happy. For the price of Beacon, I found it unacceptable. No telling how mom was treated when I wasn't there. I do not recommend.

We did not have a good experience with the Colonnade at Carrollwood. My father's admittance into the facility hinged on satisfactory healing of some pressure wounds he had developed on his heels during the course of his rehab stay. In spite of the fact that the Colonnade's representative came to his room at the rehab, placed her own two eyes on the wounds, photographed them and sent them to the corporate office for review, and then told us that he was qualified to be admitted into the Colonnade, they kicked us out 3 days into his stay there because their nurse on staff decided that the wounds were, in fact, stage four and therefore in violation of Florida law for that facility. We literally had to vacate the premises within an hour of receiving this news. It was demoralizing, frustrating, and completely inexcusable, in my opinion. A real money grab that took advantage of a family's desire to provide their loved one with more comfortable accommodations. I would not recommend this facility to anyone. To add insult to injury, the staff nurse, who made this startling assessment after having ripped the bandages off of his heels, left the injured heels dangling off the edge of his chair without any new dressings. They sat there, exposed to air and elements, for several minutes, if not an hour, while people in the facility scrambled to find something to cover them with after we complained. I think they might've sent him off to the hospital without so much as a Band-Aid had we not spoken up and demanded that someone do something to cover the wounds. Just egregious. I stand by my comments about the management, but I do think that the staff and the facility itself are probably better than our experience could reflect.

The first night he slid out of his chair. I rang the bell and no one came. I went to the desk there were four women looking at the monitor flashing Emergency! I told them he was on the floor . No one moved; they told me to call the fire department. He was 145 pounds so it wasn't that he was heavy. The firemen even called the director saying what was wrong with their people. Next morning (I had stayed the night), at 9:30 we woke up. No one came to his room to care for him or give his insulin and breakfast. I rang the bell this time someone came. I reminded her that he needed care. She said someone called in and she was the only care giver and he would have to wait until she did seven other people. I called an ambulance and got him out of there; I could not leave him. Hospital checked him and said he could go back. Told the nurse what happened and he was not going back there. The nurse was lovely and understood She found him another rehab. He stayed there until there from last February to his passing through [date removed]. I would not recommend Village Veranda for a dog let alone my loved one. It was just a bad place with horrible attitudes from the workers. They get new directors instead of new people. They are afraid to confront these workers. They have many complaints very similar to ours, but they change nothing! Please do not send anyone there. I am sorry but I needed to be honest hopefully it will help another family.

When I first brought my mom who was suffering from Alzheimer's and dementia to Creekside our biggest concern was her safety as she would escape my house while I was at work I was unable to be with her 24/7. Noted she was as fully able to care for herself when she first went to Creekside The first couple months things seemed to be going great until They failed to give her life saving medication as prescribed by her doctor. The first time she spent a week in the hospital as a result of not receiving her meds. We trusted the mistake would be fixed only for it to happen again. She spent another week in the hospital the 2nd time and needed a rehab. She never recovered and unfortunately passed away on Oct 4th from the lack of care she received at Creekside.

Disrespectful toward residents from Care partners , staffing with one nurse to 60 residents. Management does not answer phone when notified of staffing needs or resident care at times. . No RN on staff. They don't respond to calls for help from neck worn transmitter. Executive director is as useless as a box of rocks. Housekeeping is hit or miss, went a month without any housekeeping front the canopy staff in early 2023, high turnover of all staff. They are on their fifth (useless) executive director in two years (no kidding). Two executive directors have been fired in the past year ([dates removed]) one for stealing funding from residents. Please feel free to inquire about this statement with their sales staff. Sadly, I made a mistake placing my loved one in this facility, I thought they would be better, or at least adequate. Unfortunately, new and shiny doesn't mean "good". Don't believe the sales pitch from SRI (lack of) management - the canopy at Walden Woods. STAY AWAY!
