it was going to be my second choice, it's literally around the corner from my house but then it had some things about it I didn't care for. It's kind of run down, the carpeting on the floor was very stained and the staff there just stood around like they didn't really care about what they were doing and I was going to go back and visit again because I thought it was just a weird down and I was really turned off by what I saw of it. It was dinner time and the staff, when they were feeding the residents really turtned me off by the way they were interacting with them.

Visited here about a year ago...was not impressed with the activity - seemed like Mom wasn't ready for this confining level yet. Did not keep price info. May need to revisit if we cannot figure out a way to afford Summerville Estates

Horizon Bay is an assisted living and memory care facility. We toured this establishment on Wednesday August 14th. Within minutes of our arrival, Janet Aldridge greeted us and we commenced the tour. This place is quite large and has 2 floors. We started on the first floor. There are basically no hallways for residents to get lost or turned around in. All the rooms seem to face a large open area so as soon as a door is opened a person can see where they are with no hallways to look at. It is very well appointed, with nice furniture, airy, pretty quiet and quite clean. The staff on the first floor all seemed to be in good spirits, with lots of smiles. The rooms we saw were fairly spacious and nicely furnished. Residents can decorate however they want. A television is not provided but there are no cost Comcast hookups. Overall the first floor and part of the second were very nice. Also on the second floor is an area (it is called Brookdale) for the residents with memory problems. This area is behind locked doors, and as soon as one enters, the lower quality of the furnishings and general look of this area becomes apparent. It is not nearly as 'palatable' as the first floor. This is not to assume that the care given here is of any less quality as in other areas, but the physical impression of Brookdale leads one to think that. I did not see any prices listed for the assisted living area, but prices in Brookdale are $4600 for a private studio and 3900 for a shared studio. I am not sure what constitutes 'assisted living' but it appears to be what is catered to here. My wife is a heart attack/stroke victim and has lost a lot of control of her body (she is incontinent) and mind. I did not see or hear one mention of the word incontinence...and I forgot to ask. Anyway, I came away with an overall good impression of Horizon Bay, but the Brookdale area was a let-down. Respite is available at a flat rate of $165.00 per day, but the minimum is 14 days, which makes this feature quite expensive, at least for me.

Assisted living only. No skilled beds

They were not prepared for us. Did not know there was a scheduled appointment. Did not meet with director. Does not matter. Not a good fit for us. She said it looked like a funeral home. I say, nice place but I would not want to live there. Even a person w/o short term memory problems could get lost.

Close to family. 3 levels. Neighborhood would need Aid & assist before long.

Actually, after speaking with [name removed] (director) and [name removed] (medical) my rating is now 2-3. My mother has been paying for additional services (almost the most that can be paid) and she has fallen 3 times in less than a 30 day period. She moved in [date removed]. We are now seeking other alternatives for my mom.

Overall, [name removed] the gal who helped us with the whole move in process was very very good. She really worked with us and went above and beyond to make the stay a good one. Unfortunately, one person cannot make the whole facility great. I was not very pleased with the medical staff, [name removed] was great but he did not work every shift. We had some issues with the other staff, they just did not seem to care as much as they should have.

My loved one was able to participate in more activities they enjoyed. My loved one fell during their stay at Ashbrook Village. Ashbrook ended up having to send my loved one to the hospital. The community charged us for two months, even though my loved one wasn't there for that entire period. Ashbrook Village had good food, and nursing aides.

We know our loved one can be difficult at times. Suffering w/dementia, stubbornness, & almost a century in age, makes for a handful of management. Our loved one entered The Haven @ The Summit w/assistance of a walker (due to the long walk). When we picked him up after 17 days in respite, he had to be rolled out in a wheelchair. He could not walk nor stand on his own. The charge nurse did not give a report on his condition upon discharge. She handed us his bag of meds & said goodbye. A custodial worker (w/double-duty responsibilities) found a wheelchair while we packed his belongings. The custodial worker also helped get him into the car. Once home, a male neighbor had to lift him out-of-the-car into his wheelchair which he’d never used. He was bed bound until doctor prescribed weeks of home physical therapy. We also noticed weight loss. After being home for nine-10 weeks, he now walks throughout the house w/out the use of his wheelchair or walker. He regained his weight. Before being admitted to The Haven, we decided to let him go back to stay after this respite period ended. That did not happen! We were not pleased w/his care. The facility (building & grounds) seemed clean, but the memory care was disappointing. They seemed short staffed. Initial meetings & touring were great sale jobs.

The location is near my home, about 5.2 miles away. This review is in regards to the memory care section. The outside looks very good but I was not able to go back and try out a meal prior to placing her in this location. I understand why after seeing the meals. I have been in a couple medicaid only facilities and the meals are better there. I had a major issue getting the medication she is to take straightened out. After taking in a heating pad initially, it was taken away as there was not an order. I did get an order and they have not found the heating pad yet. I will probable take in another one. There is a lack of discipline among the workers as they play their music and they congregate together and not checking on others who are in their room. The place is clean. There is an activities chart on the wall but all the times I have been in there the only activity is music and trying to get the residences to also sing along. The TV is going all the time.

really disappointed in the service provided. Cost was far more than initially quoted.

My husband was moved into Brookdale on Wednesday March 1 2017. On my visit today Saturday I noticed that none of the staff was around on the floor when I was ready to leave. Finally a young lady came out of one of the rooms and let me out and people waiting to get in. There was no supervision on the floor where most of the people were sitting. I wanted to ask a few questions but no one was around after lunch was over.

I went to visit my relative on yesterday took her to appointment.I gathered her laundry together yesterday for her clothes to be washed for today which is Tuesday her wash day.Those dirty clothes are still in her room they didn’t wash them I left word with [name removed] at the desk to tell them to wash her clothes.Last week they let her toilet stay stopped up for 2 days very disgusting.Then the assistant director had the nerve to say you a little frowny today trying to be funny.

Communication was bad once you were a resident and care was no comparison to Weinberg - felt too corporate and only felt comfortable with physical therapy personnel. Too much time spent trying to let up issues. Very much smoke and mirrors. Now wish I had never put my dad at this facility

This was my the place for my mother to stay. She was immobile, wheelchair bound, they should have rejected her application. Fell twice and never contacted me or my stepfather. Did nothing for her swollen feet, only medication

Communication skills need to be improved from staff to families. Better training of staff and less temp service. More staff for residents to prevent long wait times. Having food brought to room needs to be improved. They are last to be fed and sometimes don't get what is on the menu.

Anthem Lakes at this time is currently going through a major transition. From the very beginning I felt like I never dealt with the same person from sales to marketing director to CNA's. Can't seem to keep staff as a rotation and burnout constantly brought in new faces. Not a good sign. Some employees really tried to put their best foot forward while others seemed more concerned with their phones and not even interacting with patients or like it was definitely a bother to do so. Lots of times it was one person taking care of all these patients because someone called out, during the day and at overnight as well. We came almost everyday and it was really frustrating to witness. It looks like a nice place they all do but I really feel like it's smoke and mirrors. For the money hire someone to take care of your love one at home. It's really sad we just want our families to be safe and one great CNA can't handle all of it. Food is not good every now and then might have a decent meal but not really, unless it's a special event than they really do a nice job but not with everyday eating. Cleaning , if your love one had an accident in their bathroom or got sick in the shower, it would stay there until the once a week cleaning person came in to clean which I had witnessed with other patients while visiting with them and just couldn't believe. If your love one can still walk and talk they may have a chance but if they need help it is extremely limited and will suffer for it in the long run.

The facility is very clean and well kept and the front desk and nurses / care givers are very kind. However, we often felt like we were being dismissed when we had questions or concerns. Our father was in memory care and was given the wrong dosage or medication that could have caused his breathing to stop. The doctor was requested to be removed from his care by his family. He often became agitated so he was heavily medicated and slept a lot which was concerning since he did not eat or drink properly due to this. We expressed concerns and no one took time to make sure he was eating. If he didn't touch it, his tray was taken and thrown away, therefore, his kidney function was affected and eventually was sent to the hospital where the doctor determined his kidney functions were low and he was severely dehydrated. This does not happen overnight. From there he was sent hospice and passed away a week later. I do not think his condition would have become so severe if we were notified earlier about his renal function and the extent of what was happening. Afterwards, the sales representative charged us for the entire month even though he had passed away because we were not able to remove his furniture quick enough to suit them. I'm sorry but when my dad is passing away, I will not be there to remove furniture. We provided the paperwork that stated the dates that he was in hospice but still paid the full amount. The whole experience was very upsetting and disappointing and we did not feel like my father mattered to them as long as he was sleeping and quiet.

No place for a non-compliant senior. Although the sales team will try and spin it like they are capable of handling all situations. Not enough of the right type of experienced personnel. Operating on a shoestring budget. Nice building, but wearing quickly. Food is lower institutional quality.
