The community is supposed to be doing daily checks; however, they don't seem to be checking on the residents. I don't feel they seem to care about the residents like they are supposed to be. The room and community is very nice. I just feel they didn't fulfill their obligation.

Unsafe, unsanitary. Terrible food. Management will say anything to bring more people into the community. Do not believe ads about Covid safety.

Park Creek seemed to be the best fit for us, our of the list of communities we looked at. We were impressed with the facility and the way that they presented themselves. Our loved ones moved in before COVID, and immediately our experience was impacted with the crisis. There was no visitation, or anything. We have little to no communication with them. Our loved ones declined very quickly. Our mother fell and we were not told in a timely manner, and she ended up going to the hospital. After a second trip to the hospital she passed away. Our father was left alone, and really the only contact he had with us was over the phone. During this time his hearing aids were lost, and his condition was so bad that we had to take him to the hospital. Shortly after arriving he passed away. We do realize COVID played a part, however we felt like the medical support and medical care was not up to standards and was not adequate..

I would want others to know that Evergreen is all about the money and not about the care for your loved ones. They talk a good talk in the beginning, but they do not deliver, we were extremely disappointed and would not recommend.

My loved one was there only for a short time, as this community didn't work out the best for them. Logan Place let me know that there would be no issues caring for my loved one, after assessing my loved one. Within a few days of my loved one was moved up the scale for more care. The community ended up doubling the cost of care for my loved one. There were various situations with my loved one, getting around the community through emergency doors of the community. The community ended up stating they couldn't care for my loved one any longer within one month. I ended up moving them out to another community. We went to pick up my loved one's items, after them moving out. There were feces on my loved one's items, rugs in the bathroom, shoes, and other items. The community placed all of these items in a plastic bag for us to pick up. I am not happy with the way things turned out. The person I initially dealt with was no help, and ended up moving to a smaller community.

I would not send my worst enemy to Park Creek Place. Ever since Enlivant took over, the care and services have steadily deteriorated. They seem to be interested only in profits. Before that, the place was wonderful. I would have highly recommended it. Since March, 2020 my sister's monthly cost went from about $3700 to $8000. They seem to be really taking advantage of the COVID 19 situation, since families cannot get in to actually see what's going on. They either won't give info or provide conflicting information in response to specific questions. My sister was arbitrarily rushed to move from Assisted Living to Memory care in May . Since then she has had about 5-6 unexplained reported falls. We have no idea how many may have not been reported. No , I would definitely not Recommend this place to anyone.

Very disappointed! When we met with their team prior to signing the lease, they talked very highly of themselves and the outstanding care they provide to their residents. However, due to their staffing issues and inability to hire/retain staff, many services that we currently pay for are not being delivered such as daily light housekeeping, weekly deep cleaning, showers and linen changes two times/week, and escorts to daily activities. I can't tell you how many times I've walked into my mom's room and was disgusted at the cleanliness of her bathroom. When we have tried to address this with their Exec Director, [name removed], he gets defensive and blames the "management" for the lack of resources he has to work with.

This community used to be welcoming and five stars all across the board. Unfortunately it would be too generous to give even one star. The new management who have all transferred from bought out company has turned it into business rather than a home. They are concerned about budget rather then the needs of the residents and staff members. The ED walks around unapproachable and will give you the famous line of “I’ll look into that for you” without keeping her word. All complaints are swept under the rug and makes the community picture perfect for corporate. The memory care director has little to no experience and can be see walking the halls on her cell phone while putting her work on her CNAs. Dining services has reached its low point and the food is barely edible. Housekeeping has had a huge turn around rate and my loved ones room is cleaned every other week if we are lucky. When this should be scheduled every week on a specific day. Maintenance request can take days to weeks with several reminders before it gets completed. With the amount of money paid each month with multiple increases over the years the service we receive is horrible and they are always short staffed with the use of agency. The only original direct was of sales and marketing who let go due to personal reasons of the new management. The only approachable and genuine person this community has left. I would not recommend this community for the safety of your loved ones.

My experience with Canfield Place was negative. When my friend had to go to a hospital due to a stroke he was charged for thirty days after he left and could not return. Even though this cost was spelled out in the contract, the staff moved his belongings to another room and rented his room to someone else even though he was paying for the thirty days! They also kept the entire $3000.00 community fee even though he was there a mere 46 days. Total cost about $13,000.00 for 46 days! Nurses aides are not licensed but simply anyone who applies can get hired. Multiple attempts to rectify the situation of cost went unaided even through the Area Agency on Aging. Please beware of this facility.

If you have a conscience, DO NOT send your loved one to this place. We just moved my husband’s grandmother out of here. This place is filthy and understaffed with indifferent employees. The poor people who live here just wander around aimlessly while being ignored by staff. While at this place, I came across a confused older woman who couludn’t find her room. Unable to find any staff to assist, I walked her around the unit until we found her room. Upon entering, I noticed there was an incredible amount of dirt on the floor, which looked like it hadn’t been swept or mopped in a month. After tucking her into bed, I was able to track down a staff member and I asked if there were any custodial staff who could clean her room. She responded with “I’m just an agency nurse how should I know?” So I asked her who would. I was directed to the Administrative Coordinator who informed me that there was no custodial service on the weekends. Mind you, the common areas were filthy and had trash on the floor, the window ledges were layered with dirt, and the carpets looked like they hadn’t been vacuumed since 2006. The elderly are high risk when it comes to infections, and living in the unhygienic conditions here certainly increases the chance of falling ill. These people cannot care for themselves, it is Park Creek Place’s responsibility to provide a comfortable and safe living environment for them and they are clearly not doing so. The residents here should be cared for with warmth and love, and it is heartbreaking to see that this facility does the exact opposite. Shame on you, Park Creek Place. This is the type of facility that you want to avoid at all costs.

Horrible!! They want to rent apartments! There is no standard of care or cleanliness!! Once you’re in, there is absolutely no response or accountability! My husband was rarely bathed, remained in soiled undergarments and there was usually dried feces all over his sofa, carpet and bathroom! The smell was always horrific and after sharing pictures constantly never an improvement! They don’t care! I would find piles of soiled undergarments on the floor that he changed himself! He was never shaved nor had his nails trimmed! I have many horrifying pictures!

For approximately six months my brother was at Wellsprings rehab. He was a Viet Nam Marine Core Veteran. His care was substandard at best. He was suffering from a bed sore, urinary tract infections and complications from CLL. It took months to get him the proper mattress for the bed sore! Aides and nurses took over an hour to respond to his calls for help. When questioned it was always the same answer.. "we are short staffed". The aides were rude and nasty to both my brother and to his visitors. He was in a rehab but never received any rehab at all. When asked when he would receive it the answer was ..we don't know when the therapist is coming". My brother died in December 2018. I would not send my worst enemy to this place!

If I could give no stars I would, this place is horrible. I feel bad I put my dad here. The care is horrible, the dining room is horrible. Everything about this place is horrible. If you are looking for a place for a loved one stay away!

We walked in and patients were all in the lobby at 10:15 waiting for lunch. There seemed to be no life in any of them, they all seemed to be medicated. We declined the tour and left.

There is no where near adequate staffing, and the staff that is provided is not trained properly (especially to handle the memory care residents). I witnessed staff in the memory care yelling at residents, and residents were constantly falling and walking around in soiled clothing and briefs. My grandmother will not even eat the food here because it is so unappetizing; it is either under or over cooked. I have witnessed my grandfather being given another reisdent’s medications. Sometimes call lights go completely unanswered. The aids and cleaning staff are constantly complaining to my grandmother about the poor staffing and how they are forced to come into work while sick. The building is new and pretty, yes, but do not let this obscure your judgement, as the care is simply very poor. Additionally, nearly every month we have had to argue the billing because we were overcharged.

I was unimpressed during my tour of the memory care that I was interested in for my mother. There was no activities and people were just sitting around unattended. I chose another facility where the staff was more involved with memory patients.

The lobby is beautiful showcasing crystal chandeliers and a baby grand piano; however the caregivers and nursing staff are not so friendly and there is a high turnover with all. I had to move my father out of the memory care unit because his arms were bruised, feet were swollen from being on the floor all day and night and he needed to have his feet elevated at least a few hours a day or during the evening but they said, He refused. He has dementia so he needed someone to build rapport and trust. Too busy there to build rapport. I found my father soaked in urine and soiled clothing daily until I changed him. Only one caregiver was able to do so but he was very nervous there. I had to move him out as I could not continue to worry about my father having to feel nervous or agitated because the caregivers could not spend time building rapport and making him feel comfortable. Blisters from urine burning is skin, nurse did not have a thermometer to take his temp so I bought one. The monitor system they had on the wall in his room was not working, but it hung on the wall with a light on as if it were set up so when my dad fell out of bed due to a new drug that made him dizzy, he laid on the floor for over an hour without assistance. Very disturbing and stressful for me and my husband as well as my father. I moved him out and his feet are no longer swollen and the blisters have healed. The caregivers are not trained in dementia either. They are mean spirited and not the caring type that you would want to take care of your parent. It was just a paycheck to them. It was also hard to move him in, then have to move him out a year later. Be careful.
