My parents moved here in 2018 - they moved from Florida into the Independent Living portion of Liberty Heights. They liked the attached garage and the view. At first the food was mediocre, but palatable. Then the "chef/cook" left, then things started going downhill. I spent an entire evening in the ER with my dad after he choked on a piece of beef that was so tough he could even chew it. I couldn't even cut mine, so I was shocked that he even took a bite. Then the kitchen manager changed and things got so bad that residents started taking complaints to corporate - to no avail. Unfortunately, the management had also changed since they first moved there, and he was less than receptive; in fact he moved his office to make himself less available. I have to buy groceries for my mom just so that she can get some nutritious, let alone edible food. The facility is dirty and worn, yet they raise the rent every January - but just got a notice that rents would go up in October this year (probably again in January.) This is NOT the place for anyone looking to enjoy their senior years. In two years my parents have paid enough to have bought a condo, or for me to have built an ancillary dwelling for them. At 88 years old, the thought of another move is too much for them - so they will continue to flush hard earned money down the toilet and still have to buy groceries and cook. In essence, they are paying $6,000 a month for a condo with an attached garage and nothing else - oh, wait, they push a button every morning so staff knows they are still upright. As far as "gated community," in the winter, the gates never close. I have left there at all hours of the night, only to have the gates wide opened. Go Elsewhere if you value your retirement and your portfolio.

Addington Place kicked my father out (sneakily sent him to a hospital and then refused to let him back in) *without notice* within one year of his residency. They kept an absurd amount of his money even though he wasn't even there for the majority of the month, and they kept the television that I bought for him when he moved in. Furthermore, they gouged him another $500 due to *their* accounting error, and are *still* ducking calls and not replying to emails. I explained that this $500 charge would cause him to become unqualified for Medicaid and put him at risk, but they don't care. We're trying to save his life, and Addington cares more about $500. His new facility still has not received his belongings that Addington claimed they mailed. I asked for an itemized inventory list and tracking number, but they refuse to provide any information. Please beware. Find any other place but Addington. They are a fake smile mask on a wicked, subversive monster.

I did call your facility and now my husband died, no one was there for him that morning in memory care. Every time my family and I came there was always cleaning person around but you had to look for nurses, Do not place your loved one in memory care here, the place focuses on the assisted not the memory care. Always a big change in staff. The.cleaning lady did more for my sweet husband. Soooo disappointed and family is still upset.

Terrible food rundown facility short on staff my mom has fallen three times due to lack of care look elsewhere for loved one

If you have a conscience, DO NOT send your loved one to this place. We just moved my husband’s grandmother out of here. This place is filthy and understaffed with indifferent employees. The poor people who live here just wander around aimlessly while being ignored by staff. While at this place, I came across a confused older woman who couludn’t find her room. Unable to find any staff to assist, I walked her around the unit until we found her room. Upon entering, I noticed there was an incredible amount of dirt on the floor, which looked like it hadn’t been swept or mopped in a month. After tucking her into bed, I was able to track down a staff member and I asked if there were any custodial staff who could clean her room. She responded with “I’m just an agency nurse how should I know?” So I asked her who would. I was directed to the Administrative Coordinator who informed me that there was no custodial service on the weekends. Mind you, the common areas were filthy and had trash on the floor, the window ledges were layered with dirt, and the carpets looked like they hadn’t been vacuumed since 2006. The elderly are high risk when it comes to infections, and living in the unhygienic conditions here certainly increases the chance of falling ill. These people cannot care for themselves, it is Park Creek Place’s responsibility to provide a comfortable and safe living environment for them and they are clearly not doing so. The residents here should be cared for with warmth and love, and it is heartbreaking to see that this facility does the exact opposite. Shame on you, Park Creek Place. This is the type of facility that you want to avoid at all costs.

I absolutely think it was the worse decision I have ever made. I would not recommend this place to anyone. My father was there for not even two weeks. He fell three times and within two weeks he was gone. One of the three falls broke his hip. And his heart could not stand the surgery. We contacted them a couple different times stating he was not in the correct level of care. But they insisted he was. Well I guess we should have followed our gut feeling.

My father was a recent resident of Walden Place in Cortland, NY. We were pleased with his care during his stay there. The problem arose when my father fell and broke his hip. He required surgery and was in intensive care for a week. During that week it became clear that he would not qualify to return to Walden Place. We gave notice and removed his belongings on November 9, 2018. We were told by one staff member that we would be required to continue to pay for his room through December 9, 2018 even though for medical reasons he would not be allowed to return. When we questioned this policy another staff member told us we would not be required to continue to pay if the resident had vacated due to medical issues. My father has subsequently passed away and the family is being pressured into continued payment through 12/9/18. We were never told that a resident would need to schedule a fall, hip surgery and subsequent death to assure that adequate notice was given when vacating a room.

my mother resided here. Initially, the staff were professional and competent - do whatever to get the contract signed. [Name removed] and her staff quickly turned to corporate turn-coats. they've had more turn-over than a Taco Bell. They do not manage to resident's acuity level - they manage to profit margin. There's no care or concern for resident's well-being here. They would bring mother to the dining room in the morning with her pants soiled and leave her in the hallway because there was no staff 'available' to feed her. Smoke and mirrors, promises unkept.

I do not know how this facility can even be rated period!!!! The care is horrible an the personnel that work there I would not even trust my dog to. They are rude, uncaring,and very unprofessional. They don't care about anything but sitting on their butts behind the desk an chatting. Don't disturb them god forbid you will get your head torn off. Please if you care about your love one go somewhere else!!!!!!

Please do not place your loved ones at this facility if you expect for them to have competent care. The care provided is terrible, if you can even find a caretaker to help anyone. They are severely understaffed. The workers share this staffing information along with their other disgruntled complaints freely with anyone who visits the facility.

The memory care facility is a waste of money! DO NOT PUT YOUR LOVED ON HERE! For over $6000/mo the care you receive is terrible. There are a few caring employees but in general the care is not what you would expect for the cost. The care is what you expect from a Medicaid facility not one that costs $6000. The cleanliness of the rooms was poor (dirty, not cleaned up after accidents or partially cleaned, mold in cabinets), The hygiene of those in their care was subpar (nail trimming, hand washing, teeth brushing were rare if ever) and had to requests to such services to be provided. There is no responsibility for lost items such as hearing aids, eye glasses or shoes. I understand clothing can get lost and things can wander but for them to be lost forever and no compensation for their replacement when they were lost on their watch? Staff spend more time on their phones than tending to those in their care. Residents are left unsupervised and spend most of their time sitting in the main room watching TV. Management of the unit is overwhelmed and not effective so conditions for the residents will not improve. New administrator of Evergreen is a snake and I don’t say that lightly. You will never get a straight answer or a response to your questions. The sales staff are there to sell you on the facility and say what you want to hear but they misrepresent and lie. We were told they cater to the residents tastes, and make you think they are going to be accommodating, but that is all smoke and mirrors. As always, get everything in writing that is stated to you. We were told that my parent would not have to leave as one of our main concerns was how he would handle having to move to a Medicaid facility once the money ran out and we were assured that would not be the case and that they would be able to stay there. That got us in the door and they got all the money only to have to do what we wanted to avoid and move them to a Medicaid facility. Now, that said, the independent living is quality as long as you can afford it. You will be better off going to a continuum care facility.

My uncle currently resides in memory care at Sheridan Tyler Creek. He has Alzheimer's disease and has been there for seventeen months. He came to the facility because he couldn't live alone anymore & had fallen. Corporate has decided to evict him because he doesn't sleep at night. They want us to hire a 1:1 caregiver & pay for it from 9pm till 8am.. HE ALWAYS HAS PAID HIS BILL ON TIME! Only 1 tech for 18 patients on a memory care unit at night! Be careful before choosing this facility!!

Wellsprings is a beautiful looking place. However, the staff including everyone from the Administrator down to the aides and the therapist are unprofessional caretakers and horrible communicators. Do NOT, I repeat Do Not put your loved one in this facility. There was one nurse who was very professional and kind. All of the aides acted like they hated their job and did not want to be there and the Therapist always acted like they were in a hurry. They preferred to keep the residents in bed because they did not want to be bothered. This place is a joke. An aide put my physically disabled mother on the toilet and told her she had to leave and someone else would come back and take care of her. No one ever came and she fell off of the toilet. She pulled the emergency button but no one ever came. She was on the floor for over 1/2 hour before someone heard her call for help. The aide was written up and made it a point to tell my mother about it. I have spoken to other people that had loved ones in this facility and everyone says the same thing...Wellsprings is not the place that you take your loved one to for rehab or to stay permanently. I have reported them to the State and I will continue to tell people how awful this place is.

This place is a piece of [profanity removed]. There are numerous negative things that I can say about this place but...let me just keep it simple enough to say that they have very very poor management and only care about the money. The ED that's there now ([name removed]) is a [profanity removed]. I overheard some things about staff that should be kept between management but the staff, residents, and families knew everything. They will discuss your finances and personal information with other staff, residents, and families. The Wellness Nurse ([name removed]) doesn't care about the residents, she acts as if she doesn't even want to touch them. She doesn't direct her staff so basically the doesn't get done. I've heard a family say that their loved one hasn't had a shower in n over a month and the room hasn't been cleaned good in a very long time. I never came back after this incident. And to think, I was gonna put my dad here. [Profanity removed], ruuuuuuun.

Since the previous director left, several staff and families have also moved out. Costs are too high and the new director doesn’t seem to be as engaged. The food used to be excellent, but has really been unsatisfactory lately. My loved one lives in the personal care neighborhood; however, the residents seem to know everything going on... including that a memory care resident was missing and the police got involved. He was 4 miles away. Residents really shouldn’t be privy to private hippa info. They also heard the director never came in. Too many great staff members have left due to the lack of leadership both locally and from the corporate office in Tampa. Too expensive for what we are receiving

At first I was interested, very impressed. Once I spent time I saw start Qwen the woman can not related to the storage key. She fabricated a story of notes passed to the residents, which never happened. She should have just said she didn't have large gloves for the nurse. Memory care ran out of all sizes of the gloves. The Nurse direct or did not have psych consults to fill out so the residents just [Removed] one to one. I reported it to the police first hand. Thanks again for the help and support. I would not put my mother in memory care, it is too big and too far from the nurse. Hope this helps.

The concierge is in attentive with Apple iPhone buds, then the memory care doesn't use gloves , the activities are great. I believe the upper management stays in the office and is not active with the people. Front desk security at night is nice. The value is too high and you have to pay 3000.00 for carpet when you move out. Not a great value at all. The food can be good or bad depends on the meal. This is what I have observed. Some moved out already after not staying very long, a declined couple. I noticed some are on the wrong unit. Thanks

I had a nightmare experience with this home care agency. I was not expressed with the quality of care givers they sent me. They did not wanna work. When I asked one of the care givers to tidy up the restroom a little bit, she rose to her feet, and called the agency. They then called me to inform me that she is not allowed to do that. Which I think is ridiculous. Some of my appliances were broken by the caregivers. I had a horrible experience with this home care agency.
