Tough moving in...suggest a walk through and still you will have issues. Get as much as you can up front. Book out your 25 hours of move in help for the $2,600 one time lose it moving in fee. Otherwise the people may not be available. The departments seems to offer more to some people than others wonder if she is getting what she is supposed to. They told us to buy our own hand held shower head and they purchased one for someone else before the install. Read the move in packet and get it ahead of time. Do Not Use Price Rite for moving unless you want busted glass, scratches on furniture, sentimental pictures damaged, stuff left behind and a dirty truck from moving a house with lawn equipment the day before. Dead grass dumped out from their truck...when they arrived...found more than expected left as well as one of the movers drinking cup...

My mother is in the memory care. Since her arrival many residents have moved to Autumn Leaves. They do not have many activities for people there to do. They have a large turnover from the care giver to the director leaving (should have happened sooner). There is a lack of leadership in training of the care each resident special needs are. I have been promised many changes with little results. I would recommend a camera to view what all goes on. Meals are not what are listed on website as samples. If your loved one is needing assistance I would recommend checking other places. Look at the number of empty rooms as you visit.

Everyone at the facility is very nice, but the communication to the families is deplorable. There is no communication concerning medication changes, no status updates which is very important for the memory care residents. Trying to get in touch with someone in charge on the weekend is a exercise in frustration. The concierge will tell you outright that she has no idea who is in charge. She then will connect you to numerous extensions to leave messages just in case they are the director in charge. The facility is responsible for the health and well being of our loved one and if nobody knows who is in charge it is impossible for them to act with the speed necessary to address an emergency situation. As nice as the facility is, to receive the necessary care we made the decision to move our loved one.

Not happy with more changes in staff and especially with the nursing. I understand the at Nurse has given her notice and will be leaving. It is unfortunate that this is happening. Over the past couple of months it seems as the community cleanliness, and follow up of concerns has dropped off no response from the Executive Director when concerns are brought to her attention. The new Regional person does not seem engaged with the staff of residents there is no more support......This is sad to see the community take steps backwards especially since I moved my mother here and saw that changes had been made to turn this community around since they opened. My mother has been at 3 other communities. Hopefully they can make the changes needed to pull it back together.

Please do not be deceived. When families go to tour this facility please look beyond the surface beyond the home decorations, list of activities & menu options, Main Street. Ask questions [removed] turnover, ask to see the state inspection ratings. Do the residents seem engaged & happy. Or are they sitting around unattended. There are several rude employees from the top on downward. Very little communication to family members. Its very difficult getting a hold of staff. The rate on this SeniorAdvisor website states average mthly rent is approx $1,700. That is incorrect, it starts around $4,000 on upward. Please correct this information. Senior staff can be quite aloof towards visitors/family etc. They need to be more involved with the hourly workers & residents for better quality of care. There is significant down time & difficulty in finding there Maintenance Man, Al. The head nurse Diana seems disconnected from the residents. There are definitely clicks within the entire staff. However the marketing person Cecilia seems genuine about her concern for residents, I wish all staff with offices and those without offices shared her compassion. I am looking at moving my parent out. Its too bad, we pay alot of money every month for the level of attention we are getting. Perhaps a smaller resident to staff ratio is in order.

Thought this was a great place however over the past 6 months care and other issues have increased. Management makes promises that are not kept-- just lip service and nothing has improved. Odors in the residents' areas. Beds go unmade, trash not picked up, food is not too good and there is a noisy, unruly resident in the dining room that disrupts the meal times daily. Paying for care that is not being provided while staff levels seem to have decreased. Looking elsewhere.

The management is very poor. Being a Executive Director big mistakes were made by this company. You place a Memory care Director their after being open for 2 plus years. If you plan on having the program driven it should have been done from the beginning. Resident's sit around not doing much at all. The Memory care Director is wonderful. The care staff are resistant to assisting her in anyway. The Chef will not even assist. The Maintenance man talks to much and spends most of his time gossiping. The Business office Manager places signs on her door while she puts on her Make up for the day. No discipline at all. No Leadership. This community is beautiful. If I were running this community there would be extremely strong leadership. Must be driven, Happy staff Happy residents.

My parents have been living here since the place first opened. When my wife and I first visited, we were extremely excited about the possibilities. It was unlike any facility we had visited or ones that my parents had stayed in before. We signed up and moved my parents here several months ago. Like with any new facility, there are growing pains. One of the main reasons we selected this facility was because of the food. Their kitchen is littered with state of the art appliances and the idea of a home cooked meal was music to our ears. We visited for lunch before moving in and they served a tenderloin that was out of this world. This was a great relief as my dads biggest complaint in the two facilities he was in prior was the food. Well, that was short lived. The chef either quit or got fired and the food service from that point on suffered greatly. My dad started to complain about the food and the lack of it. For dinner, for example, they were serving hot dogs, grilled cheese sandwiches, or a bowl of soup. My dad complained regularly. We talked with the management staff on many occasions, even spoke personally with the chef explaining that my dad does not like the meals he is being served. Although the staff acknowledged our complaint and assured us that my dad would no longer be served these types of meals, all fell on deaf ears. During one discussion, the management staff promised to keep a log of the food my dad was being served and track how many meals he was refusing to eat. Promises, promises. When asked to see the log, one was not being kept. Since he moved in, my dad has lost 14lbs. In summary, this place has a long way to go to measure up to the standards by which they market. At over 6,000 per month, one should expect a level of care commensurate with the cost. They are terribly understaffed and often do not meet the needs of the residents in a timely manner. We are now looking into moving my parents to another facility.

We were really disappointed because this one was one of the newest and they had lots of plumbing problems. We visited 2 or 3 times and they never got resolved. The food was really bad, like cafeteria food. I had a fish with a lemon sauce, the fish was definitely overcooked and it was very rubbery; they tried to hide it with a bunch of sauce and that didn't work because it was too lemony. My son had a hot dog and that was also overcooked- I don't understand how you do that- but when you grill hotdogs they have grill marks, these weren't grilled but they were burnt and rubbery... overall it was all burnt and it had no taste, except for my fish that was covered in sauce

We went ahead and put her in the memory care unit. We received three phone calls in the spam of a couple of months letting us know my mother had gotten out of their secure memory care unit. Also, they weren't able to give my mother her medication. They said she refused to take it. My mother's room in the memory care unit was always a complete mess. We caught them in multiple lies. My mother was not cleaned properly. I would never recommend this place to anyone.

I recently toured Landon Ridge for my mom. From the exterior and interior, the community looks nice at first glance, but it is very poorly maintained. The overwhelming smell of urine upon entering made a strong negative impression. The tour was confusing, with staff seeming disorganized and unclear about details, and communication before and during the process was poor. Overall, despite appearances, I cannot recommend this facility based on our visit.

Food is inconsistent, tableware is not clean. Staff was not timely in responsiveness to issues. Medicines not dispensed properly. And the aides are not well trained. My mom fell twice while directly under their care, unacceptable in my opinion. Cannot recommend for assisted living at all. My parents just moved to Palms of Largo, 100% better so far.

Since Buffalo Creek has been purchased by Sagora quality has declined. They are having staffing issues. They have an activities calendar but it is not accurate. We have since moved after only 6 weeks very disappointed. Nothing was as sold as. Dining was not good, staff did not pay attention. No follow through with things that were asked for . Laundry, cleaning ect. After I gave notice they didn't cared or gave concern.

We moved my aunt in because we were told that it was the best place in town. It is also close to our new house. Within a month, my aunt had gained 20 lbs, she was swollen from head to toe and having trouble breathing. Because we were new in town (in fact we hadn't completely moved to San Angelo) I asked the charge nurse for a referral to a doctor and asked her to make the arrangements for her medical care. They put her (age 103, with poor hearing and vision) on the bus and took her to the dr. There was no one accompanying her, only the bus driver. I wasn't told of when her dr visits were or the results of the exam, either from the dr or Brayden Park. She was taken to the dr like this more than once in the first 2 months she was there. She tried to tell the nurse that she was having trouble breathing, but I was told later that the nurse thought she was just complaining. (That nurse was fired several months later). (My husband was very ill and he was admitted to the hospital before we completed our move to San Angelo). After the move was completed, I was able to take Aunt [name removed] to the dr, she was immediately sent to the ER, with a heart rate of 170, and congestive heart failure. After she was discharged from the hospital, she was returned to Brayden Park, where she within a month- six weeks she was back in the hospital again for the same thing. When she was discharged that time, a space at the Baptist Skilled Nursing facility was available, so she was moved there. She never returned to Brayden Park. I felt like the administrators talked a good game, but I felt that there was a lack of follow through. I felt that the aides were very caring and concerned about their patients. I tried to ask for a low salt diet for her, but was told "they don't cook like that." Otherwise, the food was fine, everyone got to order their meal from a menu, and the food was at least reasonably warm when it was served. We ate several meals there with her. The place was kept clean, but there were problems with the maintenance of the building; the elevator door didn't shout properly and shook in transit, the front automatic handicap doors were broken for several months, and it took them awhile to finish roofing part of the building. All of these problems were observed last fall and winter. I haven’t been over there since she died in January.
