A Place for Mom has scored Oakmont Management Group with 8.6 out of 10 using our proprietary review score based on 1,229 reviews in the last 2 years. Over all time, Oakmont Management Group has 4,223 reviews with an average "overall experience" of 4.3 out of 5.
Ivy Park is a beautiful community. It was an impressive tour and I really liked [name removed] the executive director. You can tell she is passionate about her community. Unfortunately, this community is out of our price range. It would be helpful if communities could be more transparent about pricing before coming in for a tour. As of [date removed] , the fee for the most basic 306 sq ft studio was $[removed] /month, community fee is $[removed] , med fees vary but we were quoted $[removed] (it's $[removed] per point), they didn't give me a quote for care fees, and the estimated price increase per year I believe was 7%-10%. If you can afford it, this would be an absolutely lovely community for a loved one.

Overall experience has been positive. Food was better when my mom moved in but there was a change in c h ef so food not as good. However, there is a good selection of food. Help is generally good but a little inconsistent. Place is very clean. Activities are generally good but not that much participation in activities. On boarding pretty weak. People are nice and helpful but a little clicky. Apartment size is good.

The staff is fabulous. There are so many people working there that really care for everyone. From the Front Desk folk who know the names and quirks of all the residents to the caregivers who are very supportive to the technical staff working in the custodial and kitchen etc. I would give them a 5 but we had a bit of a kerfuffle when my husbands iPad and Kindle (loaned to him by a friend) ended up in his wastebasket and subsequently thrown out. Sometimes there's an issue with the WiFi connection but as I understand it, it affects a certain portion of the neighborhood each time so it's a Comcast issue but annoying nonetheless. The thermoses I'm very happy with the employees and the environment. There's something happening every day activity wise and even the USPS delivery person knows the residents. I would like to add that the Evergreen section of the facility is very well run. That section is their memory care wing and it's very secure and there's a ton of staff in attendance. You need to be let in and out of that section of the building so it's very safe. My husband isn't a part of that area but we did have to use their elevators to get to the main lobby when the main elevator was being worked on. The main part of the building contains five floors of little "apartments." Each room has its own fridge, microwave, spacious closet and bathroom. Housekeeping is once a week and there's a laundry room on each floor if you want to do your own laundry instead of using the housekeeping laundry services. (I thought that last paragraph might be important since each facility is different and people might want to know when looking for a place)

The location and the apartments here and exercise programs for older people is great. The food is a bit problematic having a medical problem. They're trying to accommodate me. The employees here are very very nice and it's a beautiful location close to stores easy driving and many people have been here for years and years up to 25 years. Also, if a single person, without relatives, has to investigate, be sure to get all promises in writing. It took me a while to get everything up to par. Especially the food if they have restrictions because of a medical problem.

First, the facility is great. Small, only around 50 residents (not full yet) on 2 floors. Well appointed. Bright colors. Extra wide hallways. Beautiful outdoor patio on the 2nd floor. Residents can go in on own, but exit through the dining/activity area and it is enclosed in glass so residents can be observed. With only 25 or so rooms on each floor, the design is simple, no long walks for residents. Rooms are very nice, Mom has a single. Pretty spacious. Large picture window for natural light & ability to open window (with a latch so not wide enough for someone to try and get out) to let in fresh air. Access to, included in rent, cable TV, if you want a TV. Refrigerator for, in Mom's case, water for visitors, her favorite drinks which the caregivers give her. Bathroom two parts, like an apartment. Spacious closet opposite the vanity. Amenities cabinet next to vanity. Large space for toilet and walk in/roll in shower. The food is excellent. A chef is in charge of meal preparation & very nice. Use fresh ingredients. They accommodate for whatever issues a resident might have. So all that would be a 4.5. The second part is more difficult. When Mom moved in, it was [community removed]. Everyone was very nice but they were struggling. Announced in December they were purchased by Oakmont with transition in January to take over [date removed]. They were very gracious with legacy residents. MS Care levels until resident leaves or passes away. If level of care changes, use MS level. Honored rent agreement as agreed to with MS. Many MS employees left or were asked to leave (surmising here) so we had acting staff in key positions. Good news. Oakmont used Oakmont "roving" managers so at least we were dealing with those that understood Oakmont processes. From February to recently, lost Exec Director, 2 Wellness Directors and 1 Activity Directors which was very difficult and disruptive. We now have what appears to be an Exec Director who is a good manager, responsive, understands issues (per first Townhall) and should be around. Just hired an Activities Director so far seems very good. This was so important in the beginning. My Mom, who we thought wouldn't participate, did! Oakmont activities are quite impressive (as were MS's). They kept the residents engaged. Oakmont appears to try and match activities to residents. Do they like large, small, groups or alone activities? New Wellness Director hasn't started, so can't evaluate. The LVN's, med techs, caregivers are getting better Oakmont has done an excellent job of recruiting good people who seem to be staying. Wasn't the case before. So difficult for residents and families. One important thing....is all the staff from management to caregivers, support staff (maintenance, concierge) are so nice, helpful and very caring. I believe the residents are well cared for. Also, they believe in getting people up and out of their rooms unless ill. This is great. We were worried my Mom wouldn't want to get up or do things. Different people may have different opinions based on what their family member requires. My Mom can communicate. She has stroke related cognitive issues. So confused, but recognizes family, feeds herself, can take all her oral medications & communicate her needs. I am very optimistic for the future. Seems to be order where previously it was somewhat chaotic. Of course much of it due to changing management and lack of caregivers. This seems to be solved.

Ivy Park offers independent and assisted living services. They also have a memory care unit. This was ideal in the event my uncles care requirements change in the future. The staff is friendly and efficient. The facilities were clean and well maintained. We were able to find a unit that fit my uncle's desire for space and comfort. He took up residence last month and is enjoying his stay.

Overall = 4.5 Staff = 1,000,000/5!! Everyone I interact with is amazing. Very impressed with their care team. From the janitor to the Director - they are all exceptional and really care about their roles in these elders lives! Facility = 4/5 (This may have a lot to do with change of ownership - doesn't make sense to update something if someone else is going to come in and change it anyway). It is in need of an update. It is always kept very clean. But some of the furniture and fixtures could use some updates. I'm interested to see what the new owners will do! Food = 4.5/5 I think this is just how it goes for the style of food they have to make. Hard to accommodate so many different diet restrictions, I'm sure! They do offer a large variety for any meal. So if the main option isn't appealing, there are always multiple other choices to make. Meal time seems to be everybody's most anticipated daily activity. It truly is the only guaranteed unique thing each day. I think most don't pay attention to the menu ahead of time and enjoy the surprise of what's being served that day. I think there should be more activity centered around food prep and menu planning. A lot of these residents are really bored and would probably enjoy washing fruits and vegetables. Sorting different sizes. Sorting dishes and silverware, the possibilities are endless! But that requires staff, which requires funding, which is unknown until the new owners step in. Activities = 4/5 Activities are only as successful as the participants are willing to make it. It really does depend on the group demeanor at any given time. Certain residents can certainly raise or lower the enjoyability of things. They do offer consistent activities. I wish there was more focus on physical movement beyond the daily exercise class. I feel like it should be incorporated in everything, throughout the day. Just for general core strength and mobility improvement. But again, that requires staffing, which requires funding. Room size = 5/5 I think the rooms are a perfect size! You really can personalize however you choose. And I think it is awesome that everyday, everyone is up and dressed and beds made. I really think that is key in keeping people's moods uplifted and helping keeping them engaged day to day. Most people spend a lot of time outside of their rooms and interacting with their neighbors. Side note- there's a lot of great outdoor spaces that aren't used as much as they could be. But they do a great job of having beautiful landscaping and bird feeders throughout the property. There's always something blooming, or a cute creature foraging. They also have a perfect location in Livermore. Extremely close to multiple medical facilities and services. There's even a grocery store right next door.

Overall the staff was great, sensitive to my mom's needs and to my expectations. If there was an issue it was resolved quickly. Dining experience was good, food was good; however, the serving staff could have been more attentive to all the residents in the main dining area. The Nursing teams were excellent and very kind to my mom. Planned Social Activities were consistent, fun, and there was always something to do for the residents. The management team was professional and met with me daily & monthly to review my mom's progress. The community was beautiful, clean, & had all the amenities my mom wanted, including a pool, Sunday Church services and outdoor gardens. I would recommend The Oakmont of Chino Hills to anyone.
