Where do I start very pleasant environment. To much rotating staff day shift vs night shift. No one keeps track or relays info to next shift. Not enough dementia patients occupied or active need more PT programs. However, better than some other local communities.

Just talked to my mom again about the long wait time before someone came to assist her to the bathroom. She is on her 4th UTI and has been there less than a year. Many good things about the place, but they should hire separate staff for the dining room instead of using the CNAs on staff because the CNAs are tied up during each meal time, unable/slow to provide assistance if needed.

It's a terrible place. They lost all of my mom's meds & miraculously they reappeared when I threatened to file a complaint against them. Mom went weeks without her meds. The food is shameful. Its an awful place that should be shut down. I did make a complaint with the ombudsman

The facility was good at first, but they need to work on improving the care and attention that they give their residents.

The name of the place I am living now changed ownership and they have raises the rent. it's a profit organization.

We lost money by signing up with Summerfield but then needing to change to a closer facility. They said we signed and we were not refunded thousands of dollars. He never even moved in to the room. I still can't believe it. It was an extremely emotional time and we felt pressure to sign quickly. It may have been in writing, but it still seems highly unethical. They were eager to take my mom's money, but not to give it back. On the positive side, it felt like a clean and pleasant facility, and the staff were friendly.

My husband was in Summerfield five months, less a week. Over that time I had to ask for showers, depends, to find his shoes, and also his walker. I had to take his clothes home and wash as he would not have clean clothes when I would visit him. Some of the staff were so good and others should not be working there. Understaffed and uncaring. During the few months there, my husband fell seven times and was taken to the ER every time but one. The last fall he died in the hospital a week later. I haven't been able to sleep well since his death, as I should have taken him out before I did. No, I can not recommend this place.

I removed my wife from Summerfield of Fresno. They are very badly understaffed. Some excellent caregivers. Some unqualified. They consistently make errors in the giving of important medications.

Overall, I feel my mother is safe there, but I would recommend looking at other facilities even if the price is a little more.

[name removed] has fallen and had to be taken to Emergency room 3 times. Mom has to wash his clothes as his Depends have not been changed. Not getting showers regularly. He does seem to have settled in but concerned he keeps falling. MD requested he uses the walker but is missing at this time.

When we toured the facility we asked if it was a non smoking campus and were told that it was. It’s not and it is causing more health problems for me. Rent was raised 6 weeks after moving in. Due to my limited income it is causing financial strain. I wish they had been truthful and transparent up front. Moving for elderly and ill individuals is extremely difficult. And this is supposed to be a Christian facility? I think not. Most landscaping is done on front of the property. Single level apartments with patios are nice. Kitchen staff and food is great. You get lunch daily.

Sadly, I would say probably do not place your loved one here unless you do memory care. There aren't many activities that are inclusive for people with disabilities. The only happy hour and fun is for an hour on Fridays. There is no safe walking route for the residents except for walking around in the parking lot. I picked this place because it looked clean and bright. I thought it would be better to be honest

Toilet appears to have not been cleaned in the entire time my father has been in the facility and the sheets had not been changed. Floor dirty, sink dirty and other residents constantly opening his door and walking in confused and lost. Father in same clothes everyday I was there. I asked for a cleaning/showering schedule and got nothing. No one will return your calls and impossible to speak to your loved one. They are nice to speak with at the time, but afraid all for show. Saw no stimulation or activities and father seemed ignored due to others needing more attention. Concerned and need to move out soon.

My mother in law has been at this facility for several months. The nutrition is questionable at best. For the fees they charge, you would assume there is a nutritional offering for every meal. My mother in law gets corn dogs as a snack. She deserves salmon and filet and fresh vegetables for these prices. We are terribly disappointed

Personally I would not recommend Pacifica based on our experience. There is no communication, I would make phone calls and no one would return the calls. I would email and no one would return the emails. I did not feel like my concerns were being addressed and they never followed up with any issues I brought to their attention.

They say one thing and do another or nothing.

In the beginning it was fabulous, they took such good care of our loved one, we were so happy to have found this community. However, when new management took over things declined rapidly. The service was terrible, the communication was lacking, it took them awhile to get back to us. They just were not very accommodating at times and seemed to make excuses for things.

I have tried to work with management to address their deficiencies but get a lot of platitudes with little change. The food has become unappetizing and nutritionally questionable. Care staff are responsible for serving meals, laundry, loading dishwashers and general room tidiness. This leaves little time for resident care with their perpetual shortage of staff or failure to schedule adequate staffing. Some of the deficiencies that I have noted are: multiple times leaving an incontinent resident up overnight in their wheelchair, multiple failures to provide a meal when the resident has not been taken to the dining room, allowing crucial medicines to run out, finding my loved one soaked in urine with saturated pads halfway up their back(multiple as well) and failure to inform the family of falls. They received a 2 and not a 1 because most of the staff really do care but are not given the resources to perform at the level that they aspire to. At this time I would suggest looking at a different placement for your loved one.

I have a family member here and we’re always worried about their care. The place seems short-staffed all the time whether it’s day or night. We are really not convinced that much thought or oversight is given to the staffing ratios or the care of the residents. Even if the bare minimum number of staff are showing up for work, how can they possibly be doing their jobs if they are sleeping through their shifts, or lounging around in residents rooms (unbeknownst to the residents who have dementia) or showing up with a scowl on their face? Kokoro claims they have an “open door” policy but who wants to bring things up when nothing good happens when you do? It’s all vague answers, empty apologies and excuses. We are running out of money paying for private caregivers which we have hired because we can’t count on the staff. We have tried working with the staff directly, talking with the director. Nothing is working. We have met some really nice people at Kokoro. The families there are lovely and some of the staff are fabulous. But as much as we'd like to keep our loved one there, we are are constantly thinking that we need to look for other options, A lot of staff have resigned lately, which is another troubling sign. We get it. Imagine witnessing your coworkers not doing their jobs and getting by while others have to pick up the slack and bear the boss’s wrath. Hopefully things will get better but probably not with this management since the current executive director has been there in some role for a long time. I have heard that concerns have been raised to the board of directors but they seem totally handsoff and let the director run the show. It’s hard to know how this place can be improved when no one who is in a position of power cares enough to do anything. Kokoro could have so much going for it if the leadership was stronger and more caring of the elderly. We wished we could write something more positive about it and tell people what an amazing facility it is. But we are really hard pressed right now to justify that it is worth the money.

My husband was at Pacifica for a week for respite care in the memory center. He was okay for 3 days but then wanted out and was violent. His doctor ask what the trigger was and I went to Pacifica three times asking about documentation leading up to the incident. First I was told that they did not document. The next times I was told that they sent documentation to the state but I was not able to get a copy. The cost for the week was $2000
