There’s a few things at Legend at Silver Creek that should be fixed that I noticed plenty of time when I come to visit family. I feel as though that the higher ups are not appreciated of their team when it comes to med tech and the caregivers. they do a lot for family from caring to answering questions when the higher-ups can’t even give a call back. These are some things that should change when it comes up to the higher-ups and show improvement to your staff and not belittle them or talk to them like their kids if it wasn’t for caregivers or med tech this facility wouldn’t be able to stay open!! I would say upper management was rude and argumentative. The memory care side was dirty n staffing was slim. Food was poor At meals many residents ate in soiled underwear or urinated when sitting at the table with no one there to clean it up or change the residents clothes. I can say the activity director was good n kept many residents busy. Its a large memory care unit and while staffing was poor on days, nights at times had maybe 2-3 staff caring for 54 residents in attendance

The overall experience was terrible, we never got a chance to test to care services because we had to leave due to the bad experience. The community however was very clean. The sales guy was a 5 for friendliness, and the receptionist was a 4 but the next 2 in line receive a 1. Based on our experience I could not recommend this facility

My parent has been at Stone Creek memory care since April and it was awful moving them in, due the traumatization and terrified state. They finally became adjusted and stopped trying to escape. Now due to the terrible service from the employees who never seem to want to help, if they can be found not on their phones, the laundry staff who have to be asked to do the laundry and then place the laundry in a ball in the closet, or the admin team who fails to file the insurance claims on a timely basis we have having to put my parent through another stressful situation of moving them to another facility. I would never recommend placing a loved one in this memory care facility.

they barely acknowledged that my husband was there when we got there! Their head nurse left and a new one took her place... she had an entire month to contact me to see what meds my husband was on.. never called I didn't even meet her until the day I arrived...we were sitting in there and barely anyone apple to us...the head nurse spoke to me once and left for the day and didn't even bother to come talk to me about the meds...the aides didn't even know who he was left alone didn't not even that he was coming. No one took them under their wing to acclimate him to the new environment. the aides couldn't handle him so they ended up calling the cops and the stupid nurses want to act like doctors saying he was sciczo... so much more my review will be detailed!! Place charged me money to hold a room for nothing I took him out of there the very next day!!! He was only there for one day... one day!! They robbed me of [fee removed] dollars to hold a room which they didn't need to cause they were not even close to full... hate that place!

My mom was only there for a little over 2 weeks. She fell one week and they never told us. We noticed bruising on her hand. That same week she fell and broke her shoulder and hip. So needless to say after her surgery and hospitalization we took her out of atria. The place is beautiful. The majority of the aides were nice. However, they are not well equipped to deal with dementia patients who are further along in their disease, like my mother. She's now at a memory care and skilled facility.

The memory care always had dirty floors and the food got worse it was cold and the turn around of staff was not good. They had no one to do activities with the people so I moved him and they were going to increase the monthly rate and not give anything better This is not a place I would recommend to anyone.

My uncle is there. I have gone into his room blazing hot. No working air conditioning. Chemical smell was terrible from the AC burnout. Water on the floor from leaking shower along with mold. He was in filthy clothes. He has fallen due to improper cleaning causing slick floors. They left him all night bleeding with skin tears. They are very defensive when you bring anything to their attention.

Do not take your loved ones here if you have any care for them at all. They will sell you but in the end your loved one will be miserable. Horrible staff who really just don't care. Management is incapable of providing any service or care. Do not recommend.

Our mother received horrible care at PH. Med management was nonexistent, housekeeping was terrible, On site manager, [name removed], is a joke. NOTHING is ever done with complaints. Most are too fragile to complain, which works in their benefit. Food was always cold, was too done (chicken tenders were always so hard my mom couldn’t eat them due to her dentures. Office staff very rude, especially the lady that sits at the desk. Most are chatting and on phones so no wonder care is horrendous. I just don’t have ONE good thing to say. Legal action is being taken. It was that bad. Don’t put your loved one here.

For the last 2.5 weeks of her 6 week stay I was on them to get my Mom a simple lab test and antibiotics for an infection. I was told by the nurse that she was struggling to get the sample, then was told they sent the sample in with no name on it. I never saw the results. In my opinion, as a result of not treating the infection, mom fell, broke her hip, had a concussion and the infection was confirmed. She is still in rehab 6 weeks later. The ICU nurse and Trauma Surgeon both reported to me that it was over an hour before Meadowview staff found her and called paramedics. Now Meadowview is charging me for the month she was in the hospital & rehab. According to their pharmacy's reports I received later, her medications were incorrect and I wasn't able to get any answers on what she had been receiving. Outwardly they appeared to care. Now my Momma can't even come for dinner and it isn't likely to be able to again.

Legend Assisted Living is a good place to live if you don’t need assistance. The cleaning crew is very efficient and friendly. They do care about your loved one. The dining room is OK, and the food is decent. You can even find good administrative staff to help you during your stay. The problem starts when your loved one needs assistance. The caregiver team is comprised of incompetent personnel with zero empathy. They have no idea how to treat older people. I don’t know what the internal problem is, but the result is clear: your loved one will NOT receive the care they need and deserve. Countless times we approached management and were left with the impression that they were going to do something to improve the situation. They never did. We paid extra to increase the level of care to match his condition, but care didn’t improve. Basic care, like hygiene, went undone. More than once they forgot to take him to the dining room to eat. When you walk through the door it's beautiful but the care your family member will receive does not match. Legends is a big corporation and it's about the bottom line. They try and cut corners everywhere but reel families in by stating otherwise. Corporate does not respond to families. They wait you out until it's too late hoping you will go away. We have discussed this with other families and no one gets a response from corporate. I hate giving negative reviews but I have photos of residents asleep in their plates in the dining room because no aid would wheel them back after dinner. And there was no one left in the dining room. Another family wheeled this individual back to their room. It's very sad. We are taking care of both moms. And now I would like to give you a positive on a facility that I would recommend. We have a mom at Baptist Village and it may not be the bright shining Penny because of its age but it is such a caring facility from the top down. My mom has blossomed there because of the genuine loving care the residents receive. We have also been there for a little over a year.

I have nothing positive to say about this place. They do not follow state regulations and are always understaffed. Communication from nursing staff with family is non existent and when questioned about things you are gaslit. They have let my family member suffer in isolation and caused them to miss MANY meals since they wouldn’t bring them down for meals which was in the care plan. Moved her out when management tried to deny all allegations. My cameras don’t lie. Please do not put your loved one here.

Mother (90 years of age) had fallen in her apartment and according to staff she was not found for approximately 5 hours. Never noticed her empty chair at lunch, etc. Due to fall, restricted to her recliner, developed swelling in ankles. Facility doctor increased the furosemide but nurses did not monitor possible improvement and continued to administer increased dosage after condition improved. Result was phone call to me to inform mother was not responsive and should go to ER. ER doctor immediately identified severe dehydration. This threatened her life. When nurse at Sunrise was told what they had not done, her response was we "should have caught that". Check class action in 9th Circuit of Appeals regarding Sunrise staffing issues.

AWFUL PLACE. Gone to hell in the last 2 years. They seem to have moved several from Assisted Living to Memory Care,($[removed]more a month). The staff is under qualified and very minimal at that. My mother-in-law laid in bed almost 24 hours without food or water, [date removed]. Eyes were matted shut, lips and tongue were crusted/dry. She says NO and they walk out, no encouragement or thought to helping her. When she would not wake up on Saturday, I ask who was in charge they said the Med Tech, she herself did not know what to do, finally I said take her to hospital. They also had no knowledge of the DNR she had in place, thankful she was not dead. They have a man in memory care that walks around and goes to other residents doors and literally walks in. I mentioned this to them because my MIL was nervous of him. They told me he was harmless, but to keep her door locked at all times, of course family cannot have a key to her room in memor

Absolutely horrible experience for my uncle. He was there 3 weeks and in the er 7 times. He had two injuries we were never notified about. He had a Catheter and they did not keep him clean or his bag emptied without us asking them to empty it. No communication returned by director or the head nurse. I had to clean his room, change his sheets and do his laundry. It took us asking them to bath him. It was 9 days until he was bathed. I would not put a dog there. Short staff. The director is awful. Misleading as to what their care involves. Just want the rooms filled but no care involved.

My loved one was a resident at Sunrise of Upper St. Clair and the experience was terrible. Basically, they didn't get her out of bed for weeks, never used the Hoyer lift, and the care was lacking. Then they sent us a $[removed] bill. I would definitely not recommend this facility to others.

Regrettably, some employees resort to finding places to rest while on duty. The retention of tenants is notably low, with an average residency duration of only four months. Despite the facility's claims to provide engaging activities for residents, the lack of adequately staffed personnel hinders the promised outings. Consequently, the available options for residents are limited, primarily centered around consuming subpar cafeteria food. Furthermore, incidents of individuals who are meant to be under supervision displaying inappropriate behavior, such as yelling at staff and engaging in unsanitary acts like urinating in common areas, have been reported. Given these concerns, it is strongly advised to ex

1. 1. It was not the right place for mom and they are only setup to manage a particular resident. We are moving out this week. 2. 2 sticky floors, urine smell and worse on furniture and in common areas. Dirty pullups left in garbage can days in room 3. 3. There are several people that made this place tolerable and made us rethink moving out sooner. Unfortunately, when they are not working or making facility decisions, it's not enough. 4. 2. This isn't a 1 only because of the few who were gems and obviously found their calling to care for this population 5. 1. We have been there long enough to see the REALLY bad meals and for whatever reason small portions or lack of offering food to the ones that are not all that bad. To compare, we had 5 days in hospital and the food was more appealing there! They serve high-sugar juice and sweets at every meal and only caffeine tea or coffee -it's not at all geared towards health 6. 2. They don't try to engage with everyone or have the staff to support doing this and instead activities are a task to accomplish... much like everything else 7. 1. you are willing to pay for peace of mind that your lo is cared for and cherished - when that stops happening, there is negative value
