They had all the highest Care Certifications and at the time, it seemed like the right place. It was horrible. Management cares more about their pockets than the residents. They fired everyone who was caring and kept the worst staff and employees. Prepare yourself to be ignored by nursing and staff. The smell of pee in the memory care unit is unbearable. We complained so many times and nobody did anything about it. There is under staffing and because of it, very little oversight. The only good thing is the food. They have a good dining system, plenty of options, and it's tasty. I do not recommend it. They have a lot of work that needs to be done in order to be a proper home for your loved ones.

My grandmother has been living here since last Spring when they first opened. We have had nothing but trouble from the beginning; inedible food, forgetting to order medicine (they do not allow you to get your prescriptions from anywhere else), forgetting to distribute medicine, a complete lack of response from the staff, to name a few... My grandmother is not mobile and we pay for the additional care that she needs through a plan with Cadence but she is not getting that care. She is currently in the hospital after *another* fall that could've been avoided if the staff had responded in a timely manner. Sitting around for 2 hours waiting for help to get to bed is unacceptable. Sitting on the toilet for an hour waiting for help is unacceptable.

The house keeping needs improvement they aren't doing their best. The memory care side needs a lot more activities and just a lot more help in general so our loved can get the adequate care and attention they deserve.

My family member has been a resident here for over 6 months. Staff turnover is the biggest issue & this includes administration, care givers, food service areas as well as housekeeping services. That should tell you all you need to know. Unfortunately Not sure how different this is vs. other AL communities which is ultimately the saddest statement. Given the cost of care there is an expectation that a certain level of care/service should be provided and that is not seen here. Very difficult to imagine where all the positive comments come from - staff & administrators? Check out how many residents have left before you sign up. Many very skilled staff running out of here....At least 4 chefs in 6 months....etc etc etc

An otherwise beautiful, comfortable and efficient home for my mother is ruined by the food offered in the dining room. I would love to give Harbor Chase in Jasper a 5 star rating but her description of the meals provided are awful!

I have a relative thetr and staff is pretty good. Only problem is if you have medications distributed you may get wrong one or they do not order before last pill is distributed. In addition oifvyou are ambulatory and don’t need a lot of assistance, there isn’t much to do. Only bingo every day .0n weekends they give a scenic ride to beaches and parks but they aren’t allowed off bus! For what they pay per month not worth it

Place is horrible! Placed my father there and everything they promised they did not provide. No follow up with Administration on problems submitted. Doesn't give medication as prescribed. Supposed to check on residents every two hours, it took them hours to notice that my father was not at dinner and when they finally went to "check" on him they found him hurt and in pain on the floor in his room. Requested that he be moved to a double room for months now and it still has not been done! Filed two complaints with the Florida Agency for Health Care Administration. Do you due diligence and do research on these facilitates!

My mother has been a resident at Harborchase Memory Care for several months, but we will be moving her soon. It's a lovely facility, but seems to have many issues with organization & communication. The memory care area seems to be understaffed. There are few activities for higher functioning dementia residents. We have had issues such as medication being found on the floor, assistance with showers not being given regularly, no care plan discussed with or communicated to us. Some of the community chairs smell of urine. The frontline staff are friendly & welcoming, but caregivers seem to need better training for Memory care. Also, if you choose Harborchase for your loved one, be sure to watch your statement. We have found incorrect charges on ours. We cannot recommend Harborchase Memory Care.

all apartments have full sized refrigerators which would not be a benefit to my dad; gives credit for missed meals; snack cart makes rounds 3 times a day

My husband was recently a resident for a year at Harbor Chase in Memory Care and was allowed to elope three times. On one occasion he was gone for several hours and was found by the police miles away. The other two times he was found a mile from the facility. The State, along with the Fire Department recently investigated this facility and found several deficiencies in memory care. The memory care are was never properly secured. Doors and gates were not locked and alarms often didn't work. If the alarm did go off, aides generally ignored it or responded much too late. If your loved one suffering from Alzheimer's and is mobile and high functioning, this is not the place for them.

They do not deserve a "1". How about "0"? When Dad was going to be discharged from [community name removed], I toured Harbor Chase and decided to reserve a unit for him in the memory care area. Unfortunately, on the day that Dad was going to be released from [community name removed], he had abnormal lab results and was unresponsive so he was admitted to Advent hospital in Altamonte. He declined rapidly and when he was released from the hospital, he was admitted to Advent Hospice in Altamonte. Dad never set foot inside Harbor Chase. They never fed him a meal, helped him get dressed, helped him bathe or even said hello to him as they never met him. Due to Dad's declining medical condition Harbor Chase was unable to provide the care that he needed. When this happened with [removed], they refunded 100% of the deposit money which we had paid to reserve a unit. Harbor Chase refunded nothing. They kept $[removed]and did absolutely nothing. Someone in the finance department was supposed to call me and give me the decision of the leadership team, but no one ever called. You would think that if someone just gave you $[removed] for doing nothing that you would at least be able to pick up the phone and call that person. I could not be more disgusted by the management at Harbor Chase. To prey on seniors in desperate situations and abscond with their retirement money without doing anything to earn it, is deplorable. If they had deducted the cost to send a nurse to evaluate Dad and an administrative fee for processing the application, that would've been perfectly reasonable - but to keep $[removed] without providing a single service? It is hard to imagine that businesses operate this way in 2026. I could not recommend Harbor Chase to anyone. My advice would be to stay far away.

The patient staff ratio is too high. They don't follow through with treatment plan unless you stay on them. Memory care patients are left in stimulated for the majority of the time

What they could improve on would be the staff training.

The facility is beautiful, the food is delicious and the people are nice but the care giving was horrendous.

Stay away from this place. They are becoming unlicensed. As per their spokesperson, [name removed], their sales person, the recent crime with robbery was a fictional delusion of a resident, but an arrest was made, now she said they aren’t losing their license. Not only is there a security issue, but there is also an integrity and license issue. Run for the hills!!!

The facility smells of urine, the staff is all very pleasant My loved ones room is not kept very clean, the food looks and smells terrible My loved one pretty much stays in the room, no staff member encourages my loved one to go join in any activity At the time we needed placement, Harbor Chase seemed to be the best option, the salesman [name removed] was just that, a salesman and not interested at all in my loved one as a person, just another sale All of the other employees I had contact with were always very pleasant

We chose Harbor Chase based on our limited knowledge of facilities despite doing extensive research. We were moving my father to a facility for the first time. He has Parkinson’s and his dementia had worsened quickly. We had many concerns and each one was addressed by [name removed], the sales agent. This was within 3 miles of his home making visiting easy. It was a small facility so we figured he would get more care. They also touted an ECC license. We were pressured into signing before the end of the year to guarantee his room as there were only two left. When we were finalizing the room she told us we got the last room. On moving day we were told there were only 12 of the 20 rooms filled. Such liars! The MC staff was nice but not enough to adequately care for the 12 people. Two at most and going to 1! Zero activities. Encouraged naps and watching Disney movies. Memory care needs stimulation! We saw a quick decline in my dad. Nothing was as promised! We moved him out after 3 weeks.

It was newish. They need to improve on many levels for memory care. They allow bullying from staff to family members, short staffed for qualified people, did not take the concern of our father being physically harmed by staff

It's the worst place I've ever seen, but it's not the only one all of them are trash. They don't give the attention to the patient. They don't clean him properly. They don't pay any attention to whatever they call or need so really I'd rather die than go to any of those Care facilities. It is much better if you can avoid it never go there. It doesn't matter how much you pay they're all bad.

They don’t have enough staff and let the receptionist go at 8:00pm. The door says to press it and it will open, so [name removed] did it, got out and walked a mile before they found him and called me at 9:15pm to say they found him. They had no activity for him and he spent to much time in the room. They don’t know how to redirect people with active minds. They are only ok with people sleeping in wheelchairs! We moved him to [community removed] which is safe and can challenge him and others who are higher functioning!
