My parents move was not anticipated, and they were not prepared for the changes in their life any more than their family. The entire staff, and I do mean entire, has made the transition to the assisted living community as pleasant and comfortable as possible. The comfor level of their care to us, and to them, is very high. It would not to be possible for us to provide the quality or amount of attention to the daily activities of their life that are now necessary. Heritage woods, and Mercedes who is our adviso from A place for Mom, have made their lifestyle possible, and given us the peace of mind knowing they are safe. And they feel safe. They have been there since Dec. 10th, 2013. In the 4 months they have been at Heritage woods, they have met new faces, learned to be independent again, gaining confidence, feel safe in knowing medical needs are taken care of, and Mom doesn't have to cook or clean, do laundry, or be afraid she can not take care of my Dad, and Dad cannot any longer care for Mom. The acceptance is not easy, but we know they are where they belong, and thankfully, after 65 years, they are together.

I feel that the marketing staff at Heritage Woods went out of their way to accommodate my aunt at a time in her life when she needed a place to live when her health was failing and was unable to safely live alone. Being 3 hours away from her, I am satisfied she has the quality care that she needs and the privacy that she desires.

I cannot say enough about the immediate positive impact The Fountains has had on my Mother-In-Law, therefore us! Paula Hauck is a gift to Watermark. Her patient, caring manner made us feel positive about this community as our choice. She is an angel. She was not pushy, was very informative and extremely kind. Every visit, from the first tour all the way through preparing my MIL's apartment, I never once heard anything negative from the residents or workers. Everyone seemed so happy there. (I thought, "Could this really be? I know it's not for show because I witnessed just regular interactions while I was alone there.) Then the staff - Dave Fennell, amazing, Tracy - Nurse, fabulous - and the rest of the staff is terrific. My Mother-In-Law was having a very hard time due to her dementia. The first couple days were a little rough. She was scared, wasn't going to eat, didn't want to see anyone, was not happy because people kept making her go downstairs for meals, hated getting up early, would never find her way around - but after that, every day was an improvement. On the 8th day, she kept talking about how good the food was, where she liked to sit in the dining room, the people she met! On the 9th day, she said how she set her alarm so she wouldn't miss anyone at breakfast! I noticed on her calendar, on the square for December 27th, she had written "Beautiful, sunny day". And now she is going to events, won at bingo and wants to learn to crochet! OMG! How wonderful!!!! A Place For Mom guided us through this whole process. And The Fountains of Crystal Lake has been wonderful beyond our expectations! They have given us peace of mind. If my Mother-In-Law had any idea of how this all occurred, I know she would say Thank You for a happy rest of her life.

Due to an inevitabale reduction in my mothers' savings, we needed to seek a community that would take her, and convert her to Medicaid at the necessary time. She had been in private care for 6 years after having lived with me for 4. We worked with "A Place for Mom", who narrowed our search to about 6 providers and we narrowed it further to preferred locations for our family. Of the 3 locations we interviewed, Heritage Woods was immediately encouraging, and reassuring that she would live in the kind of environment we wanted for her. The professionalism and warmth of all staff members we met and worked with, affirmed this. Their help in getting her Medicaid approval, reduced what could have been a nightmare, to a reasonable job. She felt at home as soon as she moved in, and is quite content there. We continue to be impressed with Heritage Woods and the warm personal care of the staff and administration.

Janice is absolutely wonderful! She is going to meet my mom at Bethany Rehab to get to know her better. She is also going to set up a day for the bus from Grand Victorian to pick my mom up and bring her there to have lunch and check out some of the residents and activities. They also have a special running on moving in(it ends the end of the year), but there is no doubt that my mom would love it there.

My mom seems to be enjoying her self. Oak Hill has a really good, young staff. Her room is really nice; It is brand new. She seems pleased. I read the calendar every month and it seems like there is a variety of activities available, and she participates in most of them. I've eaten there a few times, and the food has always been good.

Lovely location with very friendly people. Was more of what my mother was looking for.

I would recommend Heritage Woods of Manteno. It is extremely clean. It is bright and cheerful. They have lots of activities, and fun parties. The food is excellent. My mom lived there for 2 years. The staff is very helpful and caring It is a good value as well.

We chose it because it's in town and we can easily both get over and see mom, first and foremost was location, secondly, they do take medicaid if and when she runs into that problem. My sister had her mother in law there and they felt fairly satisfied with it. It's a sad situation, you can never feel 100% about doing this, it's not the facility's fault, its the discomfort with having to place someone there and the person you've placed not beign happy there but she really couldn't stay alone anymore. I actually think what's happened has probably been really good, everybody was really nice, the food is great, I loved the food, there's always a choice (she says its not like home cooking), she get's a little exercise, she's always been a good walker and they did say they'd try to find her a walking partner but that never happened; that may be a difficult thing for them to find there.

My mother wasn't happy with the food there but I think that was just her. It was a very nice facility. I never had any issues there at all, we just relocated. Thier staff was very nice.

Facility combines compassion, competence and safety. My mom receives well balanced meals, timely medications and care as needed. We are fortunate to have located this fine facility.

My son, daughter and I visited and were extremely impressed with staff. I have scheduled three days to be with them to discover even more about them. They gave me information about VA benefits and I have attended a meeting with my son and daughter.

I though this place would have been really good. We really thought about going here. Their activity package is a five plus! They're wonderful about providing things to do. They have all kinds of outings, a bowling league. They actually have a travelling bowling league. They go to concerts. Pretty much everything happening in Rockford, they have a little trip to. Their calender was just packed. It's more independent living for healthy, active seniors.

We chose this community because it was recommended by Lauren at A Place for Mom. It has lots of activities for the residents. The community is very clean. The staff was very friendly and helpful. Overall it is just a great community that we are pleased with.

First impressions--very nice. Director called everyone by first names. Everyone friendly. Nice apartments. Dad can't stay there is he falls. Waiting list.

We chose this community over the 4 others we looked at because of previous experience with it when my mother was living here years ago and because it was the best value for the price. My mother had just had such a good experience here we decided to use The Fountains again, but this time for my mother in-law. The rooms are more spacious then other comparable communities especially for the price. They have plenty of activities but it is up to the residents to participate and get involved. The community is surrounded by trees and they get really thick around back. With it being surrounded by trees it really secludes the community in a very good way because it keeps it nice and quiet. The food is pretty good at least that is what I am told, which is a very good thing. You really get what you pay for and even a little more. We would recommend The Fountains to anyone looking for a community.

Heritage Woods was recommended to us as having a good program for its residents. There is a memory care unit that elligible residents can transfer to if needed. When we toured Heritage Woods the staff introduced us to other residents all of whom seemed to enjoy it there. The introductions were spontaneious and not one resident had anything bad to say. We felt comfortable putting my mother there.

We chose West Park Place because when we visited they gave wonderful personal attention. On the tour the staff addressed residents by their names, they weren't just using a generic "hi." They knew about their residents. The residents were active, friendly, and out & about. We were pleased with the four other properties we toured but at some we didn't see people out & about, it may have been the time of day, but the activeness of West Park Place is really what drew us there. The staff is excellent. We live a ways away but they allowed us to stay and help our loved one get acclimated. We are very pleased, there isn't anything we are not pleased about. It is too early for us to be able to rate the activities, it seems they have a wide variety but our loved one has not participated in them yet.

The staff tried everything to help my love one adapt. We are satisfied with our experience at Brookstone Estates. The location was a deciding factor in picked Brookstone because there was not much else for the elderly in the area. We have a unique situation so it is hard to rate the value, care services, and activities, however, the survey does not allow for those categories to remain blank. I would recommend this place to others.

The primary reason for choosing the Village at Westerville was the fact they allowed my mom's little dog. The Westerville Retirement Center is exceptionally well appointed, very clean, and my mom who is in Independent Living likes that it does not have the feel of a nursing home. The staff is very welcoming and she has a nice apartment with bedroom and living room. Her meals are provided, they offer a great variety of food. My mom had been losing weight because she had become disinterested in cooking for just one. She picked this place because there are people like her there and she can join them for conversation, meals, and activities. Transportation is also available to different destinations and shopping centers. My mom pickwas the one who picked this place it met all of her requirements, most importantly allowing her dog to live with her.
