Originally we liked the facility, but they did not handle my father properly. We explained everything to them about why it was time for him to be in a facility - anger, hitting, everything. They said that they were trained and could deal with it. There was a bidding war going on to see who could get him. They offered specials etc. but we feel like we were thrown into a boiling pot. There was an incident where he tried to get into bed with another resident thinking that it was his wife. He has Alzheimer's! The staff told him that his wife would be in his room, but when she wasn't there he threw the table and the staff had to call 911. They had said that they would take anyone, but once this happened they said that they could not handle him. The director got fired right after my dad left Arbor Oaks, probably for letting him into the community. They never did an official evaluation, and they didn't document the incident in any of their notes. They dealt with everything very poorly. The staff was distant towards the residents. We liked the building; it was big and beautiful. My friend had his mom there and it worked out well. That's why we ended up choosing it. After he left they distroyed all of his medication but didn't even know which medication they had distroyed. He was only there 11 days and we were charged for the full month. We want a refund.

Poor care at Pine Ridge

My mom was a resident at Pine Ridge for just under 4 months. While her condition declined rapidly during that time it is my belief that lack of attention and poor staffing contributed to the speed of her decline. One of the reasons we chose Pine Ridge for my mother initially was because she had been falling at home. They boasted a motion detector in every room. Within the first 15 days of her stay she fell twice, one of the falls resulted in a shoulder fracture. My mom was continent for the better part of her stay there, she even asked for and used the commode on the day she died. Despite that her caregivers insisted she wear diapers and refused to take her to the bathroom when she asked to be helped. My mom was not very mobile during her stay and eventually ended up slumped over in a wheelchair or sleeping in a recliner in the “safety room” where multiple residents were placed during the day requiring less staff to care for them. I repeatedly ask that she be allowed to lay in bed when she was sleeping, my requests were ignored. While there my mom developed a large pressure sore on her spine that was never reported to the family. She developed open sores on both of her feet that eventually led to larger wounds and necrotic tissue. She had very little if any oral care, her glasses were frequently missing, her hearing aids were often not in her ears at all and if they were they were improperly placed or not charged, she wore the same clothes for up to 4 days At a time and she often had food caked on her clothing and face. She had had a drastic change in her condition on the day she died that I only discovered when I went to visit her, I found her in severe respiratory distress, she passed away 3 hours after I had gotten there to see her. The entire experience at Pine Ridge was stressful, unpleasant and eye opening. My mom received poor care, they were short staffed, communication and follow up with family was lacking. Because of my experience with my mom at Pine Ridge I would not recommend it to anyone.

Think twice

You wrote a review for The Wentworth of Las Vegas 4 days ago 1.0 star rating [date removed] If you plan on moving your loved one to the Wentworth Las Vegas, you might want to think again. My mother has been at the Wentworth Las Vegas for the past 4 years, over that time period we have seen 4 executive mangers come and go. Each time a new one comes on board, they try to make changes to justify their position. They started with wanting to move my mom from her private room to a shared room after many phone calls and meetings they decided to leave her in her room she had been in since day one, then the next one wanted to move her to the memory care unit when her doctor said she only had early stages of dementia and didn't need to be in memory care yet, so after a long battle they dropped that Idea. Then about 8 months ago they got another new manager and she moved my mom to the memory-care side without informing the family, so we couldn't be there, this really messed with my mothers mind and well being. If we could of been there to help with the transition it would of made the world of difference so since then my mom has been very confused always talking about her room upstairs it seem to cause her to start to feel helpless like she did something wrong, this move caused her to be very sad and depressed but by the time we found out it was too late they moved all her stuff and bed so to move her again would only confuse her worse so we decided to keep her and just increase the amount of times we visit her a week and we hired a care taker to come in the middle of the week when we are all working. By the way the person that made that rash decision is no longer with the company. Now we have a new manager and her decision is to EVICT MY 86 year old mother because they said they can't take the risk that she might want to return to her ways of wanting to go for a walk or I should use their terms to wonder outside. She is in the memory care side of the Wentworth they are locked in but they take her into the main lobby once and a while for group functions and she remembers her old ways of going for a walk and wants to go outside, this is very confusing for her but yet they say she is a wonder and a risk to the manager loosing her license so they want to kick her out. What kind of place is this? My mother isn't the only one that wants to go for a walk or the only one suffering from memory loss. Think twice before you decide to trust the Wentworth with your loved one.... My mom keeps asking me what did I do wrong why are they throwing me out. Short answer because you became a problem for them..

A horrible experience!

My father stayed at this place a for a couple of months a few years ago. He has since passed away but this experience haunts me even to this day. This was the first place we found when his Alzheimers came to a place that we were no longer qualified to care for him. We visited him every day and each time we were met with a new negative happening. The clients were left to sit alone, often wet, thirsty and hungry. We were told we could get my dad a cookie or juice whenever we wanted. He always seemed very hungry and thirsty. I often thought, what does he do when we are not here? It only escalated from there, with unexplained bruises, missing things from his room ( I even found his sheets tucked in the crease of the couch one time. The final straw was when we received a call that he had a bad fall. They said it was nothing to be concerned about and that they just wanted to notify us. When we arrived at the place we found my dad badly shaken with a bad black eye and bruising on his body. Everyone had a different story, but they said he was noncompliant with one of the aids and they had to force him into his bathroom and he fell. My dad was always nervous around that male aid and we knew why. I would strongly urge anyone thinking about placing there loved one in this place to keep looking. The staff was shifty, uncaring and dishonest. I will never forget the care my dad didn’t get at this place.

Several times no hot water for days!

Horrible

Unethetical and absolutely no integrity. Management will hide things from you and lie through their teeth! This is why last year many residents were pulled out when new management took over. As well as the original staff leaving! If you love your family members DO NOT TAKE them here! They will not be properly taken care of! Our loved on is at a better facility now! Nothing but a horrible experience! And to have management falsely say they have family in the facility is a disgrace! Frontier management get your act together.

Beware

This place is newer and very clean, however, the care of our family members is what really matters. My father in law has walked out without any staff realizing it, twice, that we know of, within 10 days time. He was missing for hours before anyone contacted us and was found off Sunrise Blvd. the first time and local resident ran into him on the street and realized he had memory issues and called the police. The 2nd time, the police and several family members searched for hours and located him in a vacant field where he laid in the hot sun today for, who knows how long. The Court claims that this is a secure facility and that no one can get out without the alarm going off and staff getting to them before they walk out. Untrue. Unsafe. Unacceptable. I don't recommend this facility.Thank

Poor management

This facility is one of the worst facilities my grandmother has ever lived in. The staff is good and they are doing the best they can however the management is disturbing and [Removed]. Do not put your loved one in this facility. Do your homework first .

Avoid Catalina Springs!

When this facility first opened it showed to be a positive environment for our loved ones. Once the original executive director left, things went downhill fast. They have had tremendous difficulty keeping caregivers and they often run short handed. The sanitation of the resident rooms is poor, caregiver training extremely poor and the nursing staff non-existent. There are very few activities and the daily activity board is a joke. Today the activity director doubles as the front desk secretary (as does the housekeeper and caregivers). The mismanagement over the past 18 months has led to many families moving their loved one out The Executive Director presents her self as uncaring and void of emotion. She is rarely seen on the floors of the facility and when concerns are brought to her attention she often says "Oh?" Most often nothing is done to rectify the problem. I recommend you go elsewhere to place your relative, this facility appears incapable of maintaining a safe and loving environment. It is extremely expensive and offers far less than other facilities. Do your homework and look elsewhere.

Horrible. Stay away!

My father-in-law resided at Washington Gardens Memory Care from January 2015 through October 2016. We chose Washington Gardens because it was new, clean, had large private rooms, and seemed well-designed. Soon after we moved my father-in-law in, the executive director left and the office manager was promoted to that position. Throughout our time at Washington Gardens the office manager, turned new executive director, exhibited incompetence combined with a shocking lack of understanding about dementia, which trickled down to the entire organization. For example, repetitive behaviors are a very common part of dementia; my father-in-law began clapping loudly. Although annoying, it was not harming anyone. Rather than addressing the clapping, Washington Gardens claimed the behavior was unmanageable and threatened to kick him out if we, the family, didn’t figure out a solution. We provided several easy, common sense solutions and the problem was dealt with. Washington Gardens staff should have been able to come up with solutions and manage this on their own. This is only one of many examples of an instance in which administrative staff was incompetent, unhelpful, and threatening. On several occasions when I was visiting I found prescription pills lying on tables, couches, or the floor that patients or the med aides had dropped. This is obviously a significant risk, both to the patients who were clearly not receiving their medication, and to the other residents who could have easily found and taken it. There is no reason for this to ever happen; that I found pills lying around multiple times is inexcusable and extremely dangerous. On one occasion when I was at the facility another resident was wandering the hall covered in feces; it was very obvious by the smell and sight. It was not addressed and she was left to wander. After approximately 10-15 minutes I went to find a staff member to clean her up but could not find anyone. Staff/caregivers not being present on the floor was a recurring problem. When they were present, they at times could be found chatting together away from the residents or looking at their phones. Caregivers made very little effort to engage my father-in-law in activities. He received bare minimum care, and staff did not at all seem to know or like him. This was a stark contrast to his previous facility, where he was clearly well-known and well-liked. As his disease progressed my father-in-law often wet his bed. We brought this up repeatedly with the Washington Gardens executive director, director of the care staff and the caregivers. Over and over again we went to visit and found him sitting in soiled clothing, and his bed made even though it was soaked in urine. Staff did not check his bed in the morning to see if bedding needed to be changed, and put him to bed at night in urine-soaked sheets unless we were there to check. Family is to be notified of health issues, and especially of any need for hospital visits. My father-in-law was sent via ambulance to the emergency room, though we had no notification that there were any significant problems leading up to this. We were not notified until half an hour after he arrived at the hospital. Several hours after arriving at the ER he died. After his sudden death we had to contact Washington Gardens regarding an unresolved financial matter. Despite the executive director's claim that the issue would be resolved within a week or so, more than a month later we had not heard from them. There are good, caring, clean, safe memory care facilities in the Portland area. Washington Gardens is not one of them and should be avoided at all costs.

Some of the most unpleasant administrative staff I have ever encountered! Care staff are overworked and residents suffer. In this day and age when customer service is critical to maintaining customers, I don't know how they stay in business!

Terrible experience from beginning. Initially was told that dad would be placed in a private or double room until his behaviors could be accessed and matched with another roommate. However the first night he was there, I was called and told his roommate didn't appreciate him being there. He was yelled and cursed at by the roommate so they wanted to transfer to another room across the hall with another new roommate. I was stunned after being told he would be in a private room initially. Asked them to place him in private room at a cost of $10,000/month so that no one would yell/curse at him anymore!!!!!!! We ended up paying the $10,000/month while dad's initial roommate is still in double room by himself and paying the double room rate, much less that the private room rate we are paying????? In addition, while medical staff tried to balance his meds making him dizzy and unable to walk steadily, he started falling down. Dad had never fallen down prior to coming to Silverado. Discovered the first fall myself. Dad had bruises on face and bottom, and swollen knee, however staff had no clue that he had fallen despite the many visual indications. Even though dad fell many more times during period of trial and error on his meds, nothing was done to help prevent him from falling, or adjust meds to not make him so dizzy where he was unable to walk. Then the big fall came that was severe enough for us to take him to ER. CAT scan revealed that his neck was fractured and had a pneumonia infection too which causes more confusion in dementia patients. Staff at Silverado had no clue that he even had pneumonia even though he had been examined by doctor 2 days before. We noticed that he was coming down with something the day before his fall due to his coughing and chest pain. Staff is not trained enough to recognize when patients need assistance. Now with 12 - 14 days left to live, even though didn't have much quality of life before the fall, he now has absolutely none and fell that they didn't do enough to keep my father safe. I wish I would have kept him at home now.

Not Good

I will never use this facility again. After Mom stayed there a few days and was returned to our home, she arrived in poor condition. She had half of her undergarments on and she was soiled. There was dried vomit on her face. Another resident's narcotic prescriptions were given to us. Her medications were missing, so I had to drive back there to pick them up. One of them was lost. It took two days at home for her to recover. In spite of a verbal agreement from the director that Mom's clothing would be washed, it was not. The soiled clothing was in plastic bags in her suitcase. Our CNA visited the director's office to address concerns, but he went into his office and closed the door.

Pine Ridge - No Thanks

The building was nice, now it smells, carpet is dirty. One housekeeper doesn't have to do any work because she is a friend of the VP of the company.The laundry person is the laziest of the whole bunch, but if the manager tries to get them to actually work, then they claim it is a hostile work environment. What a bunch of BS! Nurses and caregivers spend most of their day outside smoking while the residents lay in their wet beds.

Arbor Jokes

I brought my mom here and would come visit everyday so I soon got attached to other residents as well and saw horrible care given to them. I saw residents crying that were being ignored and residents they never even got out of bed. The only time I witnessed good care was on swing shift, Mickey was an amazing med aide and for that I think her. The only caregivers I felt cared were Sydney , Janet, Brittney, and Vicki and for that i also thank them. I felt as if my mom was very unsafe unless she was in there care. I moved her to a new facility and she's doing much better than she was when she was at Arbor Oaks, and she still asks about those 5 caregivers who I sadly heard resigned not long after mom left. I would not recommend this facility to anyone!

New (horrible) managment

Used to be a great place. Entire staff I dealt with moving mom in (director, nurse, and the tour guide) are gone, along with several care staff I felt comfortable with. Not only did nobody bother to contact me but the people that replaced them seem cold and uncaring. Activities and food are still acceptable although the frequency of entertainment has decreased significantly during th past month. I see horrible caregivers and nurses (mom was once left for almost 2 days without being checked on) The staff that took over from the locked side were correcting things but it does not look hopeful now. A nurse named Carmen and another named Shelley have gone above and beyond to try to help my mom but given the way staff is treated I don't expect them to stay long. If you would like your parents to live in a safe and caring place I suggest you seek other options. I am looking for alternate options for my mom as are many other families.
