What is happening to a once top-notch facility?

Have had a loved one at Auberge for over a year. The quality of care has steadily declined and the turnover in staff (and often non-replacement) has been astounding. The CNA’s are overworked but mostly wonderful. Some of the nurses are good with the patients. The doctor on staff is great. The facility itself is tired and often hot.

Worst Memory Care in Las Vegas

The turnover is the worst. All residents look abused and our mom ended up with injuries that could of been avoided. The administration cares nothing about the residents and only about money. The food was terrible. Family members aren't welcome like they tell you at first because they don't want you to see how they run the place. Residents aren't shown love and compassion. Worst memory care in Las Vegas. Turn over is everywhere from care staff, maintenance, kitchen so you know it's run horribly. Even the doctor there is not good. He wrapped my loved one's hand so tight the doctor cased a problem then didn't even come see my loved one again. I would tell people to stay clear.

Worst memory care in town

This is a horribly ran memory care where residents have a lack of respect from staff and where safety isn’t a concern. Residents are perpetually black and blue and injured, or in the hospital from lack of supervision as they are always short staffed. The turn over rate with staff is unbelievably high from the kitchen to caregivers. The food is terrible even with it being mostly pre-made and only needing reheating, the chef seems to mess it up. The director acts like a dictator and makes her own policy and rules to suit her own agenda, and promises are never kept. If the president of frontier management knew how the current administration truly treats the residents they would be fired or should be. It was a mistake to have my loved one live there and we are sooo glad we moved out and found a place who really love the residents and respect the family! Save yourself aggravation and go somewhere else!!!

Terrible and unsafe environment

My family member walked into this facility with a walker on [date removed]. Three weeks later she became unable to walk or could only stand with assistance. Her right foot was injured and required an x-ray. She also fell out of bed and her daughter was notified by an attendant. Later the Director made an incident report. The patients there spend an inordinate amount of time in a room where the TV is on, however, many to do have the cognitive ability to engage mentally in a TV show. There is very little emphasis on Individualized Memory Care. The food served is not a balanced diet. The Director is a Licensed Vocational Nurse who is very controlling, and she does not possess the qualifications or knowledge to adequately operate a memory care facility. They offer incentives to bait you in but the care very poor and a high staff turnover. We had to move our family member to keep her safe. I have witnessed patients there with black eyes from falling.

I have nothing good to say. An expensive, awful experience in which my husband got very poor medical care. Wound care awful. He lost another toe. Plus they prevent outside providers saying they are a hospital. But when patients looking for reimbursement from long term care, they say they are assisted living but do Not follow any of those rules. A dangerous place good for walking zombies only. I am aware from some hospice folks also that the place has lost its reputation with the change in ownership.

I want everyone to know that this place has bedbugs they hired people an took them through orientation and said there was a door that had been closed that doesn’t need to be opened at all until said self because they had had bedbugs a week before I was working there for four days, and they had been closing down more rooms because of the problem which they are only heating them and he does not kill all of them. Bedbugs lay eggs and they still will be contagious if all of the residents are not evacuated and the whole building treated I want residence of families to know that this is not OK I got told from another employee that they had them back in December 3 months ago and are still having problems with them. They have the residence running around all the time in and out other residencies homes they say that they keep all the doors locked to prevent that, but never change their clothes or swap them in the morning and put them straight in the dryer to put heat on them to kill the bugs

DO NOT PUT A LOVED ONE HERE!

The place is beautiful and clean, because it is fairly new. The staff is nice but not knowledgeable with illness. They only know how to care for old healthy people. My Mom went in on a temporary basis to heal a wound on her leg. They had no clue what to do, every doctor, therapist is called in on side jobs, they have no doctors on staff on a regular basis, they come one a week. They dropped my mother twice, 1st time leading to a stroke and taking her by ambulance to the hospital, then returned to The Landings, the 2nd time they dropped her when changing her bedding. Her arm was broken in many places, bruises everywhere. They would not change her because her arm was broken. They would not even splint it. They made me go to the store and buy a splint and put in on myself so they could clean her up. My Mother died the next day. This place is the most awful place, do NOT let a loved one go there!!!!

There have been issues with medication, food, and being understaffed. There have been unreturned phone calls. The management within the community is trying but they don't get support from upper management. Unfortunately, they were purchased by a corporation that just doesn't care. The community is not as clean as it should be in the memory care unit. Fresh paint, and clean furniture would be nice. Most of the rooms don't have wheelchair accessibilityin one of the memory care areas.

After months of working with their sales team, making plans with our mother and getting ready to move her in, they pulled the rug out from under us. Don't believe a word they say. Get everything in writing. They are all about the almighty dollar.

Don't judge a book by its cover...

My sister-in-law has lived there for nearly 4 months, and we’re in the process of finding her a new home. She's morbidly obese and in a wheelchair, and she needs help going downstairs to eat, dressing, cleaning her room, showering, and changing her diapers (she's also incontinent). She also needs skilled nursing to treat her lymphedema and cellulitis. That's a lot but this facility promises they can do it all - for a price, of course. At first, everything was great, but it quickly took a 180-degree turn. And I can't count the times I've complained to management - 2 executive directors, their head of skilled nursing, and even one of their Regional VPs - all for naught. So the facility is short-staffed... that's not my sister-in-law's fault. She isn't getting any price break for services not rendered. I'm giving this facility 1 star only because there is no option for a "0". I strongly don't recommend it to anyone. And FYI – the last 3 reviews are from people who only did a “tour”.

I went two weeks, I think for tour staff members on phone while residents siting on dinning room almost falling out of wheel chair, and activities director wasn't no where to be found so idk why they lie on pages, they put themselves on page the clean ect, activities only 4 people participate lier, hall ways smell bad pee all over musty..

If I could rate 0 stars, I would.

I came to visit my mother @6pm and she was very very soaked I asked one of the carestaff for help. She told me not right now & gave me a really bad attitude and looked like she didn’t even know what to do the facility had an uncomfortable smell not overwhelming but there. Absolutely not. Take your loved one ANYWHERE ELSE PLEASE.

had to leave, no sleep noisy got hurt on bad floor

I LIVED AT COUNTRY SIDE LAKES FOR ONE YEAR. I AM A RETIRED TEACHER 35 YEARS AND A USAF VETERAN. IT WAS HORRIBLE, HERE’S WHY – FOR 3 MONTHS RESIDENT UPSTAIRS KEEP TAPPING ON MY CEILING – WOKE ME NEARLY EACH NIGHT, DISRUPTED MY DAYTME. MANAGEMENT SAID THAT THEY WOULD MOVE HIM. THEY NEVER DID, DID THEY REALLY CARE ? FOR $ [price removed] DOLLARS, MONTHLY, THIS WAS UNREAL. WORSE, THE PLACE IS 30 YEARS OLD, BACK THEN, WORKERS USED GLUE TO ATTACH BASEBOARDS AND FLOOR THRESH HOLD PLATES. WELL A THRESHOLD PLATE CAME OFF AND I FEEL. GOT TWO XRAYS, THE MANAGER TOOK ME FOR THEM, NEVER EVEN ASKED HOW I WAS DOING. I GOT A CRACKED RIB FROM THS INCIDENT ! IT OFTEN KEEPS ME FROM GOING TO SLEEP,

NO STAFF - VERY BAD PLACE

Put my mother here to recover from a wound on her leg in [date removed]. In Sept they said my Mom slipped out of bed. They dropped her, causing a Massive Heart Attack, they took her to Glenbrook Hosp who said put her on hospice as she had a DNR, 3 wks later on Tuesday she was doing best I've ever seen her since she was there. That Friday they found her at 5:30 am on the floor. They claim she said I was getting up to go in the bathroom to wash her face. She couldn't speak or walk. They called 911 and they put her back in bed, never noticed her arm was broken. They wouldn't clean her up with a broken arm, I asked The Landing to do it, they said we can't, I asked Hospice nurse to do it, they said they can't. So I asked if I go down the street to Mark Drugs and buy a splint and put it on her, then will you clean her up. They agreed. So I went and bought an arm splint, put it on her. They did then. And I will never forgive or forget what they did to my mother.

RUN AWAY

Most of the staff seems to care about the people in this facility. However, the management is neglectful and uncaring and hard on the staff. The management creates a miserable environment and a high turnover rate. Within 2 weeks of living at this facility, my grandmother fell and broke her hip. I understand that accidents happen and old folks fall however while living here she also received multiple black eyes and severe had bruising on her forehead. I have photos and they would best tell this story but that would be an invasion of my grandmother's privacy and I wouldn't want to do that to her. I have witnessed this facility force opioids on my grandmother despite her refusal verbally and her spitting out the medication. Other times she would be so overly medicated she could not consent to receive the medication and they would just give it to her anyway with a syringe. Multiple staff members told me if they did not do this they would be reprimanded by management. They did not allow my grandmother to maintain any dignity whatsoever. Luckily my grandmother was removed from this facility several months ago has been living just fine for over a year without being overly medicated and with her dignity intact. All of this- despite the Autumn Leaves and hospice claiming she was going to die within a week just before we removed her and took her to the hospital. Methodist hospital diagnosed her with either " being poisoned or being extremely over medicated". I also advise caution when selecting a hospice company I would go with a nonprofit one - do not choose [Name removed]. Do Not put your loved one in Autumn Leaves- do more research - there are other options. Even if you are just trying to find a place to put a difficult relative so you don't have to worry about them anymore - trust me - no one deserves this place. You don't have to choose between an extremely expensive facility or a state-run facility. There are nonprofit facilities which have a more humane approach to caring for the elderly - even those with memory loss. My grandmother is now at a different facility and although the food is terrible- the staff is impeccable the doctors are wonderfully attentive and the management has been fantastic and compassionate. And they are willing to feed her the food that I bring her. Although I don't wish memory loss on anyone, I am grateful that my grandmother doesn't remember her time at Autumn Leaves of Cypresswood. When visiting these kinds of facilities make sure that you stay overnight several times randomly so that you get a full understanding of the environment especially at Autumn Leaves of Cypress Wood. Do not just visit for 15 minutes and think you're going to get the whole picture- there is a lot going on under the surface. If your loved one is not going to harm another person or runaway I do not advise putting them in Autumn Leaves ever. If they just have some memory problems you definitely have other options And honestly even if they are harming themselves or others- just look elsewhere - as I wrote before- NO ONE deserves this place.

Nice - until you request your deposit back

Our family toured the Wentworth and had decided to place our loved one in the memory care unit. [name removed] was welcoming during the tour, and after. always greeted us with "Hello Friend." We gave a deposit, paperwork, a doctor's physical and then, later on, found a better fit in a family style Alzheimer's Association approved group home. I emailed her to let her know we were going a different route. Instead of emailing me back, I had to call, and was curtly told, "Your paperwork is in a folder here for you" I did not get transferred to [name removed], as I asked. Then, when finding that the doctor's physical was NOT in the folder, I was, again, curtly, directed to [name removed], the executive director, who told me I could not have it as my father was not a resident! I am the one that supplied it, and I could not have it back! I insisted I was shown her 'shredding it." The staff is nice until you take away the $1500 deposit and the future $4300 a month for a shared room, and then, they won't return an email or greet you. Glad we chose a different location.

Not so nice service at a nice looking facility

My family member has been a resident at the Washington Gardens for several months. The facility is clean although they run out of necessary items such as facial tissue, paper towels, or soap. My main complaints are: 1. The facility is grossly understaffed: 2 caregiver for 24 residents. They do not or can not address each resident's needs. Hence, some residents do not get assistance in timely manner. 2. The quality of food: They heat frozen food for the residents and there has been an occasion that the residents were served decaying canned fruits for dessert. They even serve hot dogs for dinner regularly. I like the location since it is close from my residence. However, I am sure not sure whether I made a good choice at this time.

Not a good facility if your loved one is combative

My Father was here for only two weeks unfortunately. When they first assessed him they knew he was combative and they still said it was okay and they could handle him. They do charge a non-refundable community fee of $3,000. After a little bit, I started to think that the assessment wasn’t real or detailed at all. When he became combative, they ended up bringing him to the hospital and said that they couldn’t handle him anymore and he was not allowed to come back. I feel like they could have done this in a more professional manner. I would not recommend this community for people who are combative. However, it is a great facility for residents who just have dementia. It is a locked down unit and they have a little outdoor are that is also locked down. The staff is very attentive here.
