I LIVED AT COUNTRY SIDE LAKES FOR ONE YEAR. I AM A RETIRED TEACHER 35 YEARS AND A USAF VETERAN. IT WAS HORRIBLE, HERE’S WHY – FOR 3 MONTHS RESIDENT UPSTAIRS KEEP TAPPING ON MY CEILING – WOKE ME NEARLY EACH NIGHT, DISRUPTED MY DAYTME. MANAGEMENT SAID THAT THEY WOULD MOVE HIM. THEY NEVER DID, DID THEY REALLY CARE ? FOR $ [price removed] DOLLARS, MONTHLY, THIS WAS UNREAL. WORSE, THE PLACE IS 30 YEARS OLD, BACK THEN, WORKERS USED GLUE TO ATTACH BASEBOARDS AND FLOOR THRESH HOLD PLATES. WELL A THRESHOLD PLATE CAME OFF AND I FEEL. GOT TWO XRAYS, THE MANAGER TOOK ME FOR THEM, NEVER EVEN ASKED HOW I WAS DOING. I GOT A CRACKED RIB FROM THS INCIDENT ! IT OFTEN KEEPS ME FROM GOING TO SLEEP,

NO STAFF - VERY BAD PLACE

Put my mother here to recover from a wound on her leg in [date removed]. In Sept they said my Mom slipped out of bed. They dropped her, causing a Massive Heart Attack, they took her to Glenbrook Hosp who said put her on hospice as she had a DNR, 3 wks later on Tuesday she was doing best I've ever seen her since she was there. That Friday they found her at 5:30 am on the floor. They claim she said I was getting up to go in the bathroom to wash her face. She couldn't speak or walk. They called 911 and they put her back in bed, never noticed her arm was broken. They wouldn't clean her up with a broken arm, I asked The Landing to do it, they said we can't, I asked Hospice nurse to do it, they said they can't. So I asked if I go down the street to Mark Drugs and buy a splint and put it on her, then will you clean her up. They agreed. So I went and bought an arm splint, put it on her. They did then. And I will never forgive or forget what they did to my mother.

RUN AWAY

Most of the staff seems to care about the people in this facility. However, the management is neglectful and uncaring and hard on the staff. The management creates a miserable environment and a high turnover rate. Within 2 weeks of living at this facility, my grandmother fell and broke her hip. I understand that accidents happen and old folks fall however while living here she also received multiple black eyes and severe had bruising on her forehead. I have photos and they would best tell this story but that would be an invasion of my grandmother's privacy and I wouldn't want to do that to her. I have witnessed this facility force opioids on my grandmother despite her refusal verbally and her spitting out the medication. Other times she would be so overly medicated she could not consent to receive the medication and they would just give it to her anyway with a syringe. Multiple staff members told me if they did not do this they would be reprimanded by management. They did not allow my grandmother to maintain any dignity whatsoever. Luckily my grandmother was removed from this facility several months ago has been living just fine for over a year without being overly medicated and with her dignity intact. All of this- despite the Autumn Leaves and hospice claiming she was going to die within a week just before we removed her and took her to the hospital. Methodist hospital diagnosed her with either " being poisoned or being extremely over medicated". I also advise caution when selecting a hospice company I would go with a nonprofit one - do not choose [Name removed]. Do Not put your loved one in Autumn Leaves- do more research - there are other options. Even if you are just trying to find a place to put a difficult relative so you don't have to worry about them anymore - trust me - no one deserves this place. You don't have to choose between an extremely expensive facility or a state-run facility. There are nonprofit facilities which have a more humane approach to caring for the elderly - even those with memory loss. My grandmother is now at a different facility and although the food is terrible- the staff is impeccable the doctors are wonderfully attentive and the management has been fantastic and compassionate. And they are willing to feed her the food that I bring her. Although I don't wish memory loss on anyone, I am grateful that my grandmother doesn't remember her time at Autumn Leaves of Cypresswood. When visiting these kinds of facilities make sure that you stay overnight several times randomly so that you get a full understanding of the environment especially at Autumn Leaves of Cypress Wood. Do not just visit for 15 minutes and think you're going to get the whole picture- there is a lot going on under the surface. If your loved one is not going to harm another person or runaway I do not advise putting them in Autumn Leaves ever. If they just have some memory problems you definitely have other options And honestly even if they are harming themselves or others- just look elsewhere - as I wrote before- NO ONE deserves this place.

Not so nice service at a nice looking facility

My family member has been a resident at the Washington Gardens for several months. The facility is clean although they run out of necessary items such as facial tissue, paper towels, or soap. My main complaints are: 1. The facility is grossly understaffed: 2 caregiver for 24 residents. They do not or can not address each resident's needs. Hence, some residents do not get assistance in timely manner. 2. The quality of food: They heat frozen food for the residents and there has been an occasion that the residents were served decaying canned fruits for dessert. They even serve hot dogs for dinner regularly. I like the location since it is close from my residence. However, I am sure not sure whether I made a good choice at this time.

Stay for a while and observe before leave someone here

I wound not take a loved one here again. They are trying to make the most money off each patient. The Doctor and Manager seem to not have the patients best interests in mind. The caregivers and Nurses are caring and did great with the patient but they are overworked. The ratio of caregivers to patient needs to be fixed. Found my dad in a comatose state a few times and wet through a few times. I would not take a loved one here again.

Don't believe the information they give you

Staffing is like a rotating door, Executive director is either incompetent or just doesn't have the training needed for a memory care facility. She is not involved and lacks leadership skills and compassion for the residents and families. Caregivers always seem to be understaffed and expected to do laundry, activities, food service as well as caring for residents. Cleanliness of the facility is hit or miss as they seem to have trouble maintaining a housekeeper. Much of the time residents sit around with nothing to do but wait for the next meal. Other than a glass of orange juice at breakfast, water is the only beverage offered. This was the decision of the executive director. At more than $7,000 a month one would expect a variety of liquids. Breakfast and lunch are well prepared, dinners are less than desirable with repetitive plates served containing tater tots and half of a tuna sandwich. Nurse has also been a position that has had a good deal of trouble with maintaining. The latest flu outbreak is a good example of the lack of supervision and regard found at Catalina Springs. The flu was rampant yet the director failed to notify the health department, failed to put up appropriate warning for visitors on a holiday weekend and failed to notify the health department. Staff, holiday visitors and residents continue to suffer from the flu outbreak because she couldn't (wouldn't) be bothered even after many family members requested she take action. I would never recommend this facility, not until they find staff to manage appropriately.

I wouldn't recommend it. When we first chose them, they did an assessment, and they filled out a care plan, and deemed that she needs to have the highest level of care. I have to be there all the time, she has had all kinds of incidents. They don't have enough staff, it's what the government requires, but it's not enough. If they don't need a lot of help and they are mobile it might be okay, but they don't get any interaction, and there is no stimulation. I was sold a bunch of bull, they tell me that they care and are compassionate. The staff doesn't read charts, just the nurse. She basically gets left in the bed, or sat in her chair or wheelchair. It reminds me of one flew over the cuckoos nest. Since her food has to be pureed, it will sit in a bowl while everyone is eating, and let it sit until someone can get up to feed her, and it's just everything on the plate, put in a blender like pig slop. Go spend a few days, and not when they know you are coming, before you move there. My loved one went in walking and continent and now she is incontinent and can't move.

This facility is very clean and nice. The care however is not for Residents with any degree of behavior issues. Once the dementia/Alzheimer's resident is at the stage of behavior change this place is unable or will not take care if the resident. They should not advertise that they will take care of these residents. They do not implement drs or hospice plans. The management leaves a lot to be desired. They have had 3 directors in 18 months. The current director is rude and send to not care. The nurses prefer to not be bothered. I would not recommend placing my loved one in this facility. We had to move ours for their safety.

Not a Positive Experience

Originally we liked the facility, but they did not handle my father properly. We explained everything to them about why it was time for him to be in a facility - anger, hitting, everything. They said that they were trained and could deal with it. There was a bidding war going on to see who could get him. They offered specials etc. but we feel like we were thrown into a boiling pot. There was an incident where he tried to get into bed with another resident thinking that it was his wife. He has Alzheimer's! The staff told him that his wife would be in his room, but when she wasn't there he threw the table and the staff had to call 911. They had said that they would take anyone, but once this happened they said that they could not handle him. The director got fired right after my dad left Arbor Oaks, probably for letting him into the community. They never did an official evaluation, and they didn't document the incident in any of their notes. They dealt with everything very poorly. The staff was distant towards the residents. We liked the building; it was big and beautiful. My friend had his mom there and it worked out well. That's why we ended up choosing it. After he left they distroyed all of his medication but didn't even know which medication they had distroyed. He was only there 11 days and we were charged for the full month. We want a refund.

Poor care at Pine Ridge

My mom was a resident at Pine Ridge for just under 4 months. While her condition declined rapidly during that time it is my belief that lack of attention and poor staffing contributed to the speed of her decline. One of the reasons we chose Pine Ridge for my mother initially was because she had been falling at home. They boasted a motion detector in every room. Within the first 15 days of her stay she fell twice, one of the falls resulted in a shoulder fracture. My mom was continent for the better part of her stay there, she even asked for and used the commode on the day she died. Despite that her caregivers insisted she wear diapers and refused to take her to the bathroom when she asked to be helped. My mom was not very mobile during her stay and eventually ended up slumped over in a wheelchair or sleeping in a recliner in the “safety room” where multiple residents were placed during the day requiring less staff to care for them. I repeatedly ask that she be allowed to lay in bed when she was sleeping, my requests were ignored. While there my mom developed a large pressure sore on her spine that was never reported to the family. She developed open sores on both of her feet that eventually led to larger wounds and necrotic tissue. She had very little if any oral care, her glasses were frequently missing, her hearing aids were often not in her ears at all and if they were they were improperly placed or not charged, she wore the same clothes for up to 4 days At a time and she often had food caked on her clothing and face. She had had a drastic change in her condition on the day she died that I only discovered when I went to visit her, I found her in severe respiratory distress, she passed away 3 hours after I had gotten there to see her. The entire experience at Pine Ridge was stressful, unpleasant and eye opening. My mom received poor care, they were short staffed, communication and follow up with family was lacking. Because of my experience with my mom at Pine Ridge I would not recommend it to anyone.

A horrible experience!

My father stayed at this place a for a couple of months a few years ago. He has since passed away but this experience haunts me even to this day. This was the first place we found when his Alzheimers came to a place that we were no longer qualified to care for him. At first this place looked nice and big and the intake person made us feel like this would be a good fit for him. Nothing could be further from the truth. We visited him every day and each time we were met with a new negative happening. The clients were left to sit alone, often wet, thirsty and hungry. We were told we could get my dad a cookie or juice whenever we wanted. He always seemed very hungry and thirsty. I often thought, what does he do when we are not here? It only escalated from there, with unexplained bruises, missing things from his room ( I even found his sheets tucked in the crease of the couch one time. The final straw was when we received a call that he had a bad fall. They said it was nothing to be concerned about and that they just wanted to notify us. When we arrived at the place we found my dad badly shaken with a bad black eye and bruising on his body. Everyone had a different story, but they said he was noncompliant with one of the aids and they had to force him into his bathroom and he fell. My dad was always nervous around that male aid and we knew why. I would strongly urge anyone thinking about placing there loved one in this place to keep looking. The staff was shifty, uncaring and dishonest. I will never forget the care my dad didn’t get at this place.

Several times no hot water for days!

Unethetical and absolutely no integrity. Management will hide things from you and lie through their teeth! This is why last year many residents were pulled out when new management took over. As well as the original staff leaving! If you love your family members DO NOT TAKE them here! They will not be properly taken care of! Our loved on is at a better facility now! Nothing but a horrible experience! And to have management falsely say they have family in the facility is a disgrace! Frontier management get your act together.

Avoid Catalina Springs!

When this facility first opened it showed to be a positive environment for our loved ones. Once the original executive director left, things went downhill fast. They have had tremendous difficulty keeping caregivers and they often run short handed. The sanitation of the resident rooms is poor, caregiver training extremely poor and the nursing staff non-existent. There are very few activities and the daily activity board is a joke. Today the activity director doubles as the front desk secretary (as does the housekeeper and caregivers). The mismanagement over the past 18 months has led to many families moving their loved one out The Executive Director presents her self as uncaring and void of emotion. She is rarely seen on the floors of the facility and when concerns are brought to her attention she often says "Oh?" Most often nothing is done to rectify the problem. I recommend you go elsewhere to place your relative, this facility appears incapable of maintaining a safe and loving environment. It is extremely expensive and offers far less than other facilities. Do your homework and look elsewhere.

Horrible. Stay away!

My father-in-law resided at Washington Gardens Memory Care from January 2015 through October 2016. We chose Washington Gardens because it was new, clean, had large private rooms, and seemed well-designed. Soon after we moved my father-in-law in, the executive director left and the office manager was promoted to that position. Throughout our time at Washington Gardens the office manager, turned new executive director, exhibited incompetence combined with a shocking lack of understanding about dementia, which trickled down to the entire organization. For example, repetitive behaviors are a very common part of dementia; my father-in-law began clapping loudly. Although annoying, it was not harming anyone. Rather than addressing the clapping, Washington Gardens claimed the behavior was unmanageable and threatened to kick him out if we, the family, didn’t figure out a solution. We provided several easy, common sense solutions and the problem was dealt with. Washington Gardens staff should have been able to come up with solutions and manage this on their own. This is only one of many examples of an instance in which administrative staff was incompetent, unhelpful, and threatening. On several occasions when I was visiting I found prescription pills lying on tables, couches, or the floor that patients or the med aides had dropped. This is obviously a significant risk, both to the patients who were clearly not receiving their medication, and to the other residents who could have easily found and taken it. There is no reason for this to ever happen; that I found pills lying around multiple times is inexcusable and extremely dangerous. On one occasion when I was at the facility another resident was wandering the hall covered in feces; it was very obvious by the smell and sight. It was not addressed and she was left to wander. After approximately 10-15 minutes I went to find a staff member to clean her up but could not find anyone. Staff/caregivers not being present on the floor was a recurring problem. When they were present, they at times could be found chatting together away from the residents or looking at their phones. Caregivers made very little effort to engage my father-in-law in activities. He received bare minimum care, and staff did not at all seem to know or like him. This was a stark contrast to his previous facility, where he was clearly well-known and well-liked. As his disease progressed my father-in-law often wet his bed. We brought this up repeatedly with the Washington Gardens executive director, director of the care staff and the caregivers. Over and over again we went to visit and found him sitting in soiled clothing, and his bed made even though it was soaked in urine. Staff did not check his bed in the morning to see if bedding needed to be changed, and put him to bed at night in urine-soaked sheets unless we were there to check. Family is to be notified of health issues, and especially of any need for hospital visits. My father-in-law was sent via ambulance to the emergency room, though we had no notification that there were any significant problems leading up to this. We were not notified until half an hour after he arrived at the hospital. Several hours after arriving at the ER he died. After his sudden death we had to contact Washington Gardens regarding an unresolved financial matter. Despite the executive director's claim that the issue would be resolved within a week or so, more than a month later we had not heard from them. There are good, caring, clean, safe memory care facilities in the Portland area. Washington Gardens is not one of them and should be avoided at all costs.

Some of the most unpleasant administrative staff I have ever encountered! Care staff are overworked and residents suffer. In this day and age when customer service is critical to maintaining customers, I don't know how they stay in business!
