Expensive (Still not the most expensive) and for what?

My mom is a current resident and while they were quick to get her in and sort of help with the move, they really haven't done much since. The nurses do their best, but they appear to be short staffed and have not been paying very close attention to my mom much at all. There have been several administrators who have left since she started, and the communication there is not just bad, it's non-existent. I honestly am not sure what we're paying for beyond rent. A lot of the time the residents have 1-2 people running an activity, but half of the time they're just sitting there. My mom has been wandering more, and the "doctor" met with her but didn't say anything to me. She often goes weeks without showering and brushing her teeth (aka ADLs) and I know that no one is making sure that she eats. They drop off meals, but they're not checking to see if she's eaten. Kind of ridiculous if you ask me, but I have limited options.

My loved one was previously a resident at Symphony of Valley Farms and the experience was not good. Extremely poor housekeeping, poor nursing, staff do not return calls and can never take your calls. Unable to provide protein for vegetarian diet. Constant turn over of staff. They recently lost the director of memory care and recently lost their executive director. Management is extremely poor. Several upset staff and resident families. Lots of falls and no one to answer the phone or let you in after 5 pm. Filling their rooms with high acuity patients. The place was good when we first got there, but went down hill with two different managers. No accountability or quality assurance.

We didn't stay but a couple of months with St Augustine. They turned out to not be a good fit for our mom. It was a beautiful place and had many great qualities but was geared more for those that can somewhat care for themselves. There were a lot of people there that were quite happy. But Mom needed more attention than she was receiving there. Also we had some issues with admin there. I would tell others looking that If the person is able to mobilize themselves they would do ok. Otherwise they will just spend their days alone in their room. Not much motivation given to keep them active, although that's not what they advertise.

Corporate takeover ruins another wonderful care facility

This facility is VERY nice. My problem stems from an apparent corporate takeover that happened just before we decided to move in. The person who gave us the tour, made the promises, and was actually my reason for deciding to move in announced the next day that she was leaving. On the same day, the finance person and the chef also left. My loved one hates the food, which we had sampled before and found it ok. The housekeeping seems very good. I never have a problem speaking to the Director, [name removed], about any concern. She has bent over backwards for my loved one (who can be difficult on a good day). I would suggest hiring a better chef as a start to improving. The residents that have moved from place to place say Symphony at Delray Beach is just as good and bad as any other. That being said, and I know my loved one can not live in her home alone, this is my loved one's best location.

This place looks good. But after your loved one has been there a few months, you’ll see the other side. It’s not pretty. Check with the state ombudsman for residents and ask what residents are actually saying. You’ll find lots of complaints about food and resident care—including residents being forced out because Symphony doesn’t have staff trained to offer the higher levels of care advertised. That’s what happened to us. We moved my mother to another facility, which was traumatic for my mom but necessary. The staff are nice but severely overworked and undertrained. The director of sales and activities director were nice and caring. The executive director, billing director and nursing director left a lot to be desired in terms of skill, and truthfulness. We still haven’t been reimbursed for almost $2k that was paid in advance, and it’s been a year. Overall, I wouldn’t recommend Symphony. There are far better places for the money, with honest directors running them.

My husband was in the Memory Care building for 6 years before he died. The first couple of years were okay. Just before Covid the care began to decline. Admin does not communicate well with staff. They have high turnover and the new caregivers are not well trained in working with dementia residents. Workers spend more time on cell phones than interacting with residents. The building itself is well kept and the food is so so. I would not recommend the Memory Care unit to anyone.

Stay Away During COVID

My ratings are based on my experience of during the COVID lock down. Don't Go there is what I would tell people. I'm assuming things have gotten better since my loved on was there. When COVID hit, they locked my loved one in their apartment for 90 days, and wasn't being woke up or assisted in getting up every day. There were other residents out walking around with and without masks on, even though there were other residents moving around. There were social media posts, but there were times that my loved one was able to see some of the outdoor activities going on and still not be able to participate. This is one of the main reasons I choose to remove my loved one from the community. My loved one's health had declined so much since being in this community, that the other communities weren't wanting to not admit my loved one.

This facility is always short handed. We just figured our that they cut the nursing staff. Families were not told. There has been a stomach virus in memory care, with many residents ill, but doctors were not called and families not told, and now we can't visit. Workers are frustrated because medical care was not provided to residents. Management did not want to call EMS for the dehydrated dementia patients. They finally called for one after she passed out.

Our parents lived at Symphony Square for about six months. For regular assisted living it is fine. The staff are all very friendly and very accommodating, the food is good, the surroundings are beautiful. The activities staff work hard to provide fun and different activities for residents. We DO NOT recommend the "memory care" unit. The design is poor with hallways leading to exit doors that have windows. One windowed door looks out to the main activity area, with lots going on. The other leads to their garden. This is not a good design for a dementia patient as it leads to distress, they want to go through the door but they can't, especially if they can see activity going on. There is one main area where they can be and no place to wander without ending up with a windowed door. The staff are very nice and accommodating, however they are not well trained or supervised. There is always a schedule of activities posted but never actually done. They park the residents around the TV (dementia patients have visual disturbances and often find TV frustrating and confusing as a result) while they talk among themselves or are texting on their cell phones. There have been significant lapses in care. For regular assisted living, no complaints. We WOULD NOT RECOMMEND for DEMENTIA or ALZHEIMER'S CARE. Poor physical design with poorly trained and supervised staff.

Most of the staff is nice but there's a huge turnover, and this facility seems to be short staffed. Dealing with Pharmerica, the drug company that provides medications is a joke most of the time. A person should not have to wait days to get needed medications. Getting back into facility after a hospital stay is ridiculous. Loved one paid for services that were never provided. There are times when food ordered is not food received. Fresh fruit is practically non-existent.

NO LONGER THE SHINY PENNY

All reviews prior to 2020 should be disregarded as they may not reflect the current status and conditions. In the climate of a Covid19 pandemic, one would think an assisted living center would take the position of OVER-communicating to residents and families. However, the Atrium has decided to do the opposite; they do not respond to emails or phone messages from families, residents are left wondering and are worried about their safety. Getting information out of management, including nursing, is like pulling teeth. For example, the latest Covid update on their website - where they state not being aware of any confirmed cases of Covid within their community - is over 6 weeks old. They fail to mention that several of the staff members, including some with resident interaction, have tested positive. My satisfaction level with the Atrium (where I have a parent) has been on the decline long before Covid. With a new management team in place and restructured ownership, it is abundantly clear that the focus is on the bottom-line. Staffing levels do not support the level of care they claim to be able to provide. I receive second-hand complaints that several of the CNA's are well-known for their rude and condescending remarks to residents. Even the food service, for which they once well-regarded, has slipped in to a quality category generally reserved for the incarcerated. I get it that with the pandemic, operations just wont be as smooth as they once were. However, I think the Atrium is being opportunistic to reset a new bar of service and care low enough to blame it on Covid; but they make up for it by charging the same premium rates, possibly hoping to dump some residents who require a higher level of care and backfill with low care residents at the same premium rate. Just my opinion and perspective....

Consistantly short staffed

I don't know when the other reviews were written, but as of July 2019, this facility is extremely short handed, and has been for many months. While there are several kind and hardworking nurses and CNAs, there are not enough if them. Even when fully staffed, the schedule is light. However they are never fully staffed. Management is supposed to fill in for shortages but they do not. There is a big difference between what families are told and who is actually on the premises.

Don't let your loved one experience [Removed] here

My mother has been a resident of St Augustine Plantation for over 3 years and the quality of care has declined to a point that I will no longer recommend the facility. The care-givers and nurses have good hearts but the administration is terrible showing [Removed] and caring that simply can no longer be tolerated. The food served is frozen vegetables and processed entrees like pot pies. Despite my constant requests for fresh fruit, the residents rarely see anything except canned peaches or fruit cocktail. Tonight a resident was crying because despite numerous requests for meat and potatoes for dinner, all he was offered was grilled cheese or cold lunch meat. Families have called and complained and met with management and assured of changes but the situation is not improving. My mother had an unreported leg wound that was [Removed] and became horribly infected - the infection opened a stage 3 wound with infection half an inch away from the bone - an infection that 3 months later we are still fighting. The facility is pretty but until the administration is changed at the top, I urge you to avoid putting anyone you care for in the director's hands.

our family's experience did not meet expectations and would not recommend.

It's a beautiful place and it fooled us at first, but I didn't leave my parents in there a full month and I would never recommend it to anyone. They removed my Mom's thyroid medication from her chart (still not sure how or why that happened because my Mom has Hashimoto's), often took over an hour to respond to the emergency pull, and generally were the opposite of helpful

They could improve on staffing. Communication with the families. Put the dining hall in the middle of the room instead of all the way at the end of the room. Not leave residents soaking wet in their own urine and lying in the floor for hours until the have rhabdo. My favorite thing about the community was when my dad left in an ambulance and never went back.

Misguided lies

Not a well run organization and will be one pricey mistake if you move your loved one in here. The food is horrible the nurses are horrible and the sales pitch is all smoke screens and the executive director is as phony as they come. Medication is either never available or “late” / never received.

Mom not taken care of

Unfortunately, not too long after my mom moved into Symphony she started wandering. Symphony made it clear the only way she could stay there is if she had 24 x 7 Aides. Once that happened, they did not pay attention to her at all. The room was barely cleaned, medication was not always on time. Plus, many of the residents were rude to my mom. Shortly after, I moved my mom out. Bottom line, Symphony is OK For seniors that are self-sufficient. But for seniors that need help, I would look elsewhere.

Review for Symphony of Valley Farms

Had add on's and was fine during the day but not for dementia residents. Hardly any staff after 6 when sundowners occur. Lots of falls, out of milk regularly, not chef prepared meals appears frozen food. Activity director quit and not replaced. No social worker, no Medicare or Medicaid beds to transition to. If a person is alert and has no dementia and only needs minimal assistance, it would be fine. I did have items come up missing. No assist bathing at times.

Symphony at Valley Farms

Do NOT place your mother in this complete private pay facility. There is a high turnover with staff. Understaffed at night. Admitting high acuity residents without proper training to Care for them. Offer add on services but not enough staff to cover what is needed for proper level of care. This is the perfect example of a wolf in sheep's clothing. The activity director quit. No social worker on site. Business is run by an inexperienced team. If you Care about your loved one do not go here if they have any type of cognitive deficits.

Nightmare

At first glance it was beautiful. Then after awhile you get a full understanding it's all about the money. They get involved in family disagreements which creates a larger scale of problems. There is routinely only 1 to 2 persons on night shift if you can find them. They accept high acuity residents without having adequate staff trained to care for them. There are literally Prairie size dustballs under the furniture even after deep cleaning. There's a high level of turnover with staff. There sometimes is not milk for 2 weeks at a time. There isn't enough staff to provide the extra add on assist the resident pays for. The management isn't receptive to any of your complaints and in turn threatens to have you band from a facility that is private pay. Do not fall for the sales pitch if you love your family. This facility is not appropriate for assisted living or memory Care. The director is terrible, the activity director quit, in house select rehab is short staffed. Terrible
