My parents have lived here for 6 months they promise and tell you what you need to hear and once you get moved in you get to see all the flaws they can't keep good employees because they over work them there isn't a nurse their everyday and the weekends they don't have enough employees to take care of the residents . I know all this because i am involved with my parents care i am there every day. I have brought all these complaints or concerns to the director and she just [language removed] and agrees but does nothing. I am looking for a better place for my parents i did a better job my self when they lived in their own home. I feel sorry for the residents that have no one to speak up for them.

The staff, except for a few, at CSL seem to genuinely care about the residents and their benefit. However, the staff needs to leave their cellphones at home or in the main office and pay more attention to the residents. There is too much noise and chatter coming from the nursing staff and medical management could be improved. Several times the staff failed to give my wife her prescribed antibiotics and heart medication. The food and presentation is quite good given the amount of different diets and preferences, They have gone through 3-4 activities persons in the last 7 months which takes away from the socialization and participation of the residents,

Some signs of improvement of this home for the aged in the past few weeks. More staff visible, working and checking on residents. Beds are being made and hydration stations are cold & filled. Bringing residents out of their rooms to socialize or just be together in the beautiful, numerous sitting areas is still lacking. Doing a review on this site enables real change to occur and we thank this site & others for their service. This is such an important issue because many are afraid or unable to speak themselves. Keep checking this facility especially on weekends and speak truth to the issues so we can hold the owner of this community accountable.

I'd like to give a good rating as I do have hope for this place but because of the transition they are going through everything is quite a mess. People are not getting what they are promised. The management is not at it's best, that's for sure. I think there is a great potential but having seen first hand the moving in process for a loved one was seriously overlooked. Once they got the lease signed, it was brush off city with no follow up.

If your loved one falls or has reoccurring UTI's do not send them here. The staff in blue are very caring but are understaffed and there is a high turnover. Some of the directors don't follow through and seem to just be there for a paycheck, not for the care of your loved one.

My dad did not participate in the activities, but the activities person did include him. Overall, I was highly disappointed in the responsiveness of the business and health staff to questions and concerns about my father's treatment. Even at the end after my father passed, there was a bill for [fee removed] we could not get satisfaction through a conversation. My father was transferred to a skilled nursing facility in which he was provided with better care. Commonwealth was a private institution... But the medicare facility took much more interest in the families.

Remember this was 1.5 years ago. Their own staff was unhappy with how the supervisors ran the operation. The pharmacy management was particularly bad.

Positives: Beautiful view of the lake, bright open dining room, near the turnpike, nice spots to sit outside, overall look is clean and pleasing, some caring staff members, food is pretty decent. Negatives: High turnover rate (including admin), staff do not receive sufficient training (as said by the director herself), lack of responsibility. Staff breaks expensive medical equipment and is not held accountable. My father has had meals skipped because they forgot him. He's also not received medication. Emergency button had been taken away because he needed to be changed and had been sitting for over two hours. Promises are made by admin with no follow through. Liquor has been stolen from room. Legal, medical decisions have been made without going through the proper channels and without family consent.

My mother went to this facility when she became significantly debilitated after a hospitalization. She and I are both arms. She required assistance with all ADLs, which we were assured would be provided by the staff and had planned to phase her home caregivers out after a couple of weeks. We ended up keeping them on and extending their hours to ten per day. The aides complained often about the poor care. Against her wishes, my mother was put in bed shortly after 8:00 pm most nights. When staff arrived at 8:00 am she was still in bed most mornings. PRN medications for pain were often not provided when she requested them. There were other problems with medications as well. We did have fantastic results with the PT and OT staff, who helped her regain quite a bit of her mobility. She will be moving to a very nice but less expensive Independent living community with a live-in aide, where we will not have to pay for the assisted living fee and where we can handle her medications.

Prices went up, meals are awful and safety of residents is so low on their radar. We actually just took my Aunt out of Commonwealth and she is thriving already. You shouldn't have to pay extra to have care takers come in to care for loved ones. I would NEVER recommend this facility. We came so close to contacting an Ombudsman to look into the care but family members were afraid of retaliation.

I sought this facility out as a secure living environment for my mother. My mother is and was a difficult patient. She is struggling with early onset dementia and psychiatric care. After a psychiatric stay at a mental health hospital, she was not welcome back. Exceptional cleanliness and a beautiful facility. The special dietary needs (gluten and nut free) of my mother were able to be be met! However, they were unable to accommodate "a difficult patient" whose medical needs were not only early onset dementia care-- she also has psychiatric issues. Upon discharge from a psychiatric hospital mom was not welcome back there. "We are unable to accommodate her level of care." I needed a safe, secure facility for mom to live in while we navigated her medical and psychiatric needs. Commonwealth did their best, yet were unable to help us.

The only draw back was the healthcare, my father was assessed as a level 5 (highest) which we were promised he'd get extra care which cost more. However, it was no different from level 1 and the nursing staff was terrible!! Inattentive, didn't take extra time for his needs, treated him more like a hassle than a person. Needless to say, my father went into the hospital twice in two weeks and decided he'd rather come home to die than go back.

Overall the experience was not pleasurable for our family. My mother-in-law never liked the place and did not feel safe there, however, there were two family members that did not help the situation and fed her fears and dislikes. I felt American House had potential and we could have possibly worked out the issues but it never happened. I did have concerns about the care and I felt a lot of the issues came from the top down which in any business unless that changes the business will always have issues. Communication was horrible.

I've actually waited a couple of years to write this so that my emotions about the passing of my family member (which was natural) would not interfere with my assessment of The Ballentine. So at this point, I can clearly share how much I regret choosing this place. It began with the small discrepancy between what I was told in our initial visit and how my family member's first day unfolded, followed immediately by a lot of clothing and personal possessions disappearing within 24 hours, and also a second time. There were a lot of other things also, and whenever I tried to address them, the double-talk was so evident. I felt I needed to be present to make sure had what she needed, fortunately, I could be there regularly. I am appreciative however of some of the staff who were really good.
