I am appalled at the lack of responsiveness on the part of the staff. We ask for information and rarely get any reply. The place is an apartment that provides food, distributes meds and has opportunities for some residents. If there is a need for medical attention or information on what is available for our loved one, there is no interest in helping us with that basic information, let alone help us understand and figure out how to solve problems within the context of assisted living. I wonder how they keep their accreditation as safety of residents is being compromised.

I would probably move my mom if it wouldn't disrupt her. First they have gone from $[removed] a month to $[removed]. The food is bland and tough and hard for her to chew. They are none responsive with trying to fix it. They are moving employees in and out some are rude and untrained . Bottom line they increased the prices but have not improved service

My husband was in this memory care unit for only 3 weeks. Despite assurances to the contrary, this Memory Care Unit was not equipped to work with a physically fit but challenging non docile 73 year old patient. The truly upsetting issue that I’m dealing with now is that despite demanding I remove him with no notice they are charging me for the balance of the month. They also lost valuable personal property of his and raised his level of care with no notice.

I am going to keep it short and simple well at least try. Huge facility! but behind closed doors a completely different story. Our family was so tired of hearing we are short staff every time I was called due to an incident report being made. It was days my parent called me and said no one came to check on them feed them for breakfast or lunch or dinner just keep giving us medication I immediately reach out to Ceil at Plainfield they apologized even though it happened again and again. All I got was apologies form Ceil at Plainfield, I'm sorry that your parents, didn't eat, get medications, get showers, room or sheets wasn't clean for weeks at a time, I'm sorry for Ceil staff responding to the alert button hour and a half later on a consistent base, I'm sorry for Ceil staff asking for something to drink and taking food and drinks. How many sorry are we going to get until u try to solve some of the problems. my parents are ready to relocate its a very bad look with Ceil at plainfield

The worst facility I have ever dealt with! I wrote a review on Google and they deleted it.. so that just shows you how much stuff they put under the rug! Don't take your loved ones here! The management doesn't decipline there employees and let them do whatever they want.. Anytime you have a concern they have a excuse and try to shut you up from advocating for patients.

The executives here recruited us knowing our mother had dementia and said they could provide everything a memory care center could. We found out immediately that was untrue, they were severely understaffed and in the month our mom lived there, she RARELY saw the same side twice. The staff she were per diem, from an outside agency, and did not know our mom or her needs. Most never even knew she had dementia or the kind of help she needed (assistance with ALL activities of daily living). Response time to emergency alert button was APPALLING (30 minutes to an hour, if AT ALL). She got lost trying to get to the dining room on her own, and staff told us while laughing, as if having dementia is a joke. We visited every day and found her in deplorable conditions each time. Executive Director was never around, was out on family leave, and the head nurse would not or could not understand our concerns. We left when my mom was hospitalized for pneumonia. We received a full refund and an apology.

For the short time our father been there two of their main people have left and many changes have taken place.Residents are not being cared for as promised or made a priority. We were told that they check off whether a resident shows up for a meal and are checked on if they don’t show up. Our dad often misses breakfast and is not checked up on. Our dad has been asked to move from his dining spot when other residents come in and they tell him he is in their seat. Originally they had assigned seating and apparently, now they do not. Management does not follow through on what they say they are going to do. We are very disappointed because they are not providing the care we were promised regarding our dad prior to moving him there.

We placed my mom here because it seemed so wonderful at first. There were a couple of her caregivers that were amazing but the rest of the staff could care less. The emergency pulls in there room, we were told could be pulled and someone would be right there well we had to pull it twice because my mom fell. Nobody came I had to go out to the station where the staff was hanging out and tell them we needed help. The activities director at the time basically did nothing with them. My mom fell in her room several times and she laid on the floor for who knows how long because I found her there. What about the so called sensors in their room that can detect a fall well I don’t really even know if they work or are even real because they did not detect my moms falls. The nurses there were horrible too. My mom passed away within in a few months of being there. I could not care for her at home otherwise I would have taken her out. It’s a beautiful facility but looks are deceiving.

I would not recommend this community to others. I felt very mislead, this community said that they could put her in assisted living and then over to memory care and our loved one would thrive here. That did not happen, They did not take care of our loved ones the way that they said they would. When we would communicate with the staff they would not know what was going with our loved ones.

there were a lot of people who left this facility and I should have moved her out quicker. There is an issue with keeping staff and a high turnover. I felt like my mom was not taken care of correctly, and communication was not good at all. I would not recommend this facility.

HarborChase of Plainfield was not a good fit for my husband. He needed memory care but because of Covid they placed him in assisted living. After 2 days I was told he needed to move to HarborChase of Naperville. Their memory care didn't work out either. Because of behavioral issues he was sent to Silver Oaks. Medically he needed more care and when we moved him even though he had not occupied a room at HarborChase for a whole month we did not get any of that fee refunded.

My parents live in 5-star Yonkers for over 5 years. In the past year there is a bug problem, particularly flies in the hallways and residences. The problem is apparently worse for apartments over the dining room/kitchen. It is a awful to see my parents with 6-7 large flies a day swooping in their faces, in their hair and on their food. I contacted management months ago and got no response from [name removed], the director. The place looks nice but beware - would you rent from an apartment with a bug problem unchecked? Something just isn't right in this property lately.

We notified the receptionist and a facilities person that the door to my Mom's apartment was hard to open and asked that they adjust her door (and other doors in the facility) to accommodate the abilities of the residents. The doors weren't fixed. My 90 year old mother fell backwards when her hand slipped off the door handle because she had to put her weight into opening the door. She was on the floor, used the alarm to summon help - no one came. She dragged herself to the telephone and called the front desk. Eventually, help arrived. Apparently another resident incident was occurring at the same time and that accounted for the delay. The facility RN cleared her to do her normal activities because my Mom said she didn't hurt too bad. My Mom clearly has osteoporosis as most 90-year old women would be expected to have. No one called me or any family members to tell us what had happened. Mom had a hair appointment at the downstairs beauty shop and she walked downstairs to the appointment. After sitting in the beauty shop chair, she started having pain. She asked an attendant to get her a wheel chair and take her back to her apartment. The attendant assured her she could walk back on her own and left her to do so. By the time she got back to her apartment, she was in deep pain and called my sister to take her to the ER, where we learned her hip was broken. She went into surgery the next day and 5 days later passed away, my brother and I were with her. Cause of death was surgical complications secondary to broken hip and osteoporosis. After she passed, the doors were fixed. The facility director assured me they were looking in to why it took so long for help to arrive to her apartment. The RN supervisor apologized for missing the, to me obvious, probability of injury after she fell, everyone was sorry they had not contacted family members (my sister lives 5 minutes away) or gotten a better medical evaluation, and that they allowed my Mother to walk after her fall. I believe weight bearing activity after her fall was a big factor in her injury. The attendant who did not get her a wheel chair was identified and counseled. I believe the series of mis-steps at Fieldstone killed my Mother. My mother was a beautiful soul. She was mentally acute and relatively independent at the time of her death. She came from a family with considerable longevity, and I thought I had years with her still. I will forever regret allowing her to move to Fieldstone (she did not want to move in with me or other family members). It's been over 1-1/2 years since she died. My grief is still acute; I will miss her forever. If you love your parent, I do not recommend moving them to Fieldstone.

Due to time constraints we weren’t able to visit all the places that I had booked tours with. So to save time we drove by a couple of the communities and from the outside my father described it as ‘hotel living’ and he wasn’t interested in seeing it. It may have been lovely and perfect for someone else but my father was looking for more outdoor space.

We had a terrible experience with Bella Vista. It started with constantly increasing prices, what we ended up paying was a thousand dollars higher than the initial figure they gave us. While we were here they did not impress me with the service at all. The sheets they put on our bed had stains on them, and they would not switch them out when I asked. I also thought it was ridiculous that they only had one phone available for all of the residents in the entire community to use. They also never sent anyone to clean the room in the 4 weeks that we were there. I was not impressed with the food, and when I stopped by the kitchen to see if I could get water that was not from a tap the kitchen was not clean or well maintained. I would avoid this place at all costs, I did not enjoy my stay here.

There was no greeting, introduction, or tour of the community upon moving in. We were not welcomed warmly. Communication among the staff leaves a lot to be desired. The residents are not always greeted in the dining room and has sat for half an hour without being acknowledged. The apartment is very nice, but there is no communication, and when we ask for help they are not very accommodating. They had a meet and greet for new tenants, but she felt like she didn't know anyone and she felt out of place and nobody cames to extend invitations and say hey come on down we'd love to meet you and get to know you. They don't seem to check on her, or make any effort to see that she gets integrated into the community. The staff should be more accomodating of new tenants. People we are being referred to bring questions or concerns to the attention of, are not on-site, they are in another location, and aren't there on a regular basis. I am very disappointed.
